약물유도 수면상기도내시경검사
초록
‘약물유도 수면상기도내시경검사’는 폐쇄성 수면무호흡증 환자를 대상으로 약물을 이용하여 수면을 유도한 상태에서 상기도의 폐쇄 정도를 확인하는 검사이다. 폐쇄성 수면무호흡증은 수면 중 상기도의 폐쇄 또는 허탈로 인하여 무호흡이 유발되는 질환이다. 약물유도 수면상기도내시경검사는 2012년 신의료기술평가를 통해 안전성 및 유효성이 인정된 이후 비급여로 등재된 기술이다. 해당 의료기술은 비급여 관리 강화를 목적으로 유관기관으로부터 재평가를 요청받은 주제로, 다양한 대체 의료기술이 존재하는 현시점에서 비교기술 대비 동 기술의 효과성에 대한 근거를 확인하고자, 2024년 제2차 의료기술재평가위원회(2024.2.16.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 폐쇄성 수면무호흡증 환자에서 약물유도 수면상기도내시경검사가 상기도 폐쇄 부위 확인을 통한 치료방침 결정에 도움을 주는 검사로서 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 권고등급을 결정하기 위함이다. 평가방법 폐쇄성 수면무호흡증 환자에서 약물유도 수면상기도내시경검사에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “약물유도 수면상기도내시경검사 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 이비인후과 2인, 호흡기내과 1인, 구강내과 1인, 마취통증의학과 1인, 근거기반의학 1인, 총 6인으로 구성하였다. 본 평가의 대상자는 폐쇄성 수면무호흡증 환자이며, 비교군은 중재검사인 약물유도 수면상기도내시경검사를 시행하지 않은 경우(non-Drug induced sleep endoscopy, 이하 non DISE)로 정의하였고, 중재검사 이외 다른 검사(이학적 검사, 뮬러법(Muller maneuver, MM)를 동반한 연성내시경검사)는 양군에 모두 동일하게 적용한 연구를 포함하여 약물유도 수면상기도내시경검사를 추가했을 때의 효과를 보고자 하였다. 결과지표 변수로는 안전성과 효과성을 확인하였으며, 안전성은 검사 관련 부작용 및 이상반응을 지표로 설정하였고, 효과성은 일차지표를 ‘치료(수술) 후 수면무호흡증 개선 지표’와 ‘치료(수술)성공률’, 이차지표를 ‘치료부위변화’, ‘치료방법결정변화’로 설정하여 평가하였다. 연구유형은 비교연구 이상의 연구만 평가에 포함하기로 하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controolled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2024. 4. 8.). 최종 선택된 연구의 비뚤림위험 평가는 무작위배정 임상시험 연구의 경우 Cochrane의 Risk of Bias (RoB)를, 비무작위 연구는 Risk of Bias Assessment for Nonrandomized studies (RoBANS Ver.2)를 사용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거수준을 평가하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하였고, 양적 분석이 가능한 경우 메타분석을 수행하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과 최종 선택한 연구는 총 14편(대상자수 1,454명, 중재군 736명, 비교군 756명, 남성 72.7%, 여성 27.3%)으로 무작위배정 임상시험 연구(Randomized controlled trials, RCT) 3편, 비무작위 연구(Non randomized studies, NRS) 11편이었다. 연구대상자는 모든 연구에서 폐쇄성수면무호흡증 환자 중 양압기를 처방하지 않거나, 양압기 치료에 실패하여 타 치료(수술, 구강내장치 등)를 시행한 환자였다. 비뚤림위험은 RCT 연구에서는 배정순서 은폐, 눈가림 영역에서 불확실(66.7%), 높음(33.3%)으로 평가하였고, NRS 연구에서는 비뚤림위험 수준이 대체로 낮음이긴 하나 교란변수, 평가자의 눈가림 영역에서 불확실(81.8%)로 평가하였다. 안전성 약물유도 수면상기도내시경검사의 안전성은 2편의 연구를 대상으로 검사 관련 부작용 및 이상반응을 확인하였다. 검토 결과, 2편 모두 폐쇄성 수면무호흡증 환자에서 약물유도 수면상기도내시경검사와 관련된 부작용이나 이상반응은 발생하지 않았다고 보고하였다. 효과성 약물유도 수면상기도내시경검사의 효과성은 14편의 연구를 대상으로 치료(수술) 후 수면무호흡증 개선 지표, 치료(수술)성공률, 치료부위 변화, 치료방법결정 변화를 결과지표로 평가하였다. 치료 후 수면무호흡증 개선지표로 크게 ① 무호흡-저호흡지수(Apnea Hypopnea Index, AHI) 및 호흡장애지수(Respiratory Disturbance Index, RDI) ② 최저 산소포화도(LOS(Lowest Oxygen Saturation), ODI(oxygen desaturation index), CT90%(Cumulative Time with Oxygen Saturation Below 90%)를 보았다. 무호흡-저호흡지수 및 호흡장애지수를 보고한 총 11편(RCT 3 편, NRS 8편) 중 정량적 합성이 가능한 연구를 메타분석한 결과, DISE군이 DISE 미시행군(non DISE)보다 호흡지수가 낮아진 경향을 보였으나, 통계적으로 유의한 차이는 없었다(RCT: MD –1.79 (95% CI –4.88. 1.30), I2=26%, NRS: SMD–-0.25(95% CI –0.55, 0.04), I2=79%)). 3가지 기준(① non DISE의 세부비교별, ② 표준 내시경검사자세(supine) ③ 치료방법(수술, 구강내장치))으로 하위군을 분석한 결과, 이학적 검사만 단독으로 시행하는 것보다 DISE를 추가로 진행하였을 때, 기존 가이드라인 및 교과서에서 권장하는 ‘앙와위 자세(supine)’에서 시행하였을 때, 구강내장치 치료대상자에서 유의한 호흡지수 개선효과를 확인하였다. 최저 산소포화도를 보고한 총 5편(RCT 3편, NRS 2편) 중 정량적 합성이 가능한 연구를 메타분석한 결과, 모두 통계적으로 유의한 차이는 없었으나 이질성이 높아 해석에 주의가 필요하였다(RCT: MD 0.78(95% CI –0.91, 2.48, I2=22%, NRS: 2.35(95% CI –7.30, 2.25, I2=95%)). 치료(수술)성공률은 문헌에서 ‘수술 후 무호흡-저호흡지수(AHI)값이 시간당 20회 미만으로 감소하거나 수술 전 AHI에 비해 50% 개선된 경우’로 공통적으로 정의하고 있었으며, 총 9편(RCT 2편, NRS 7편)에서 보고하였다. 메타분석한 결과 모두 통계적으로 유의한 차이는 없었으나 이질성이 높아 해석에 주의가 필요하였다(RCT: RR 1.17(95% CI 0.84, 1.64, I2=69), NRS: OR 1.19(95% CI 0.59, 2.39, I2=76)). 이차지표인 ‘치료(수술)부위 변화’, ‘치료방법결정 변화’를 다룬 연구는 없었다. 결론 및 권고결정 의료기술재평가 소위원회에서는 폐쇄성 수면무호흡증 환자에서 약물유도 수면상기도내시경검사의 안전성 및 효과성 결과를 현재 확인된 근거를 바탕으로 다음과 같이 제시하였다. 체계적 문헌고찰 결과, 약물유도 수면상기도내시경검사 과정에서 보고된 부작용 및 이상반응이 없으므로 동 기술의 안전성은 기존 진정제를 사용하는 내시경검사와 유사한 것으로 평가하였다. 다만, 수면 중 무호흡이 발생하는 환자인 점을 고려하여 의료진의 관리감독 하에 장비가 잘 갖춰진 의료환경에서 시행해야 한다고 판단하였다. 효과성의 경우 전체 분석 결과에서 동 검사가 각성검사(이학적 검사, 뮬러법(Muller maneuver, MM)을 동반한 연성내시경검사)와 비교 시 치료 후 수면무호흡증 개선지표, 수술성공률에 있어 유의한 차이는 없었고, 수면무호흡증 개선지표 중 호흡지수와 관련한 하위군 분석 일부(세부비교별, 내시경자세(supine) 기준, 치료방법별)에서만 유의한 개선효과를 확인하였으며, 근거수준은 ‘낮음’으로 평가하였다. 다만, 동 검사가 임상에서 대부분 양압기를 처방하지 않거나, 양압기에 실패한 환자를 대상으로 시행되고 있고 ‘비양압기 치료(non PAP treatment)’ 표준화가 확립되어 있지 않은 점, 수면 상태에서 역동적으로 폐쇄 부위 및 정도를 확인할 수 있는 치료 대안이 없다는 임상적 상황을 고려했을 때, 동 검사가 양압기를 처방하지 않는 폐쇄성 수면무호흡증 환자에게 도움을 줄 수 있는 기술이라고 제언하였다. 2024년 제9차 의료기술재평가위원회(2024.9.6.)는 ‘약물유도 수면상기도내시경검사’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 등을 종합적으로 고려하였을 때, 국내 임상상황에서 양압기 치료를 처방하지 않거나, 양압기 치료에 실패하여 외과적 수술이 필요한 폐쇄성 수면무호흡증 환자에 한해 약물유도 수면상기도내시경검사를 상기도 폐쇄부위 확인을 위한 목적으로 사용하는 것에 대해 ‘약하게 권고함’으로 결정하였다.
""Drug-induced sleep endoscopy" is a test used to determine the degree of upper airway obstruction in patients with obstructive sleep apnea, with sleep being induced through medication. Obstructive sleep apnea is a condition characterized by intermittent apnea caused by obstruction or collapse of the upper airway during sleep. The utilization of drug-induced sleep endoscopy has been deemed a non-covered technology since the safety and effectiveness of this procedure were formally acknowledged in a 2012 New Technology Assessment. This health technology has been requested for reassessment by relevant organizations to strengthen non-reimbursement management. To validate the efficacy of the technology compared to alternative health technologies currently available, the Second Health Technology Reassessment Committee convened on February 16, 2024, to deliberate on the reassessment plan and the composition of the subcommittee, followed by the reassessment. Objectives The aim of this assessment was to provide evidence regarding the clinical safety and effectiveness of drug-induced sleep endoscopy in patients diagnosed with obstructive sleep apnea. The goal was to evaluate its use as a test that aids in treatment decision-making by identifying sites of upper airway obstruction and to determine the level of recommendation for its use. Method A systematic literature review was conducted to assess the safety and effectiveness of drug-induced sleep endoscopy in patients with obstructive sleep apnea. All methods were discussed and finalized by the "Subcommittee for Drug-Induced Sleep Endoscopy" (hereinafter referred to as the "subcommittee"), with consideration of the purpose of the assessment. The subcommittee consisted of six experts: two specialists in otolaryngology, one in respiratory medicine, one in oral medicine, one in anesthesiology and pain medicine, and one in evidence-based medicine. The subjects of this assessment were patients with obstructive sleep apnea, and the control group consisted of those who did not undergo the interventional test (non-drug-induced sleep endoscopy, non-DISE). In addition to the intervention test, studies that included other tests (such as physical examination, flexible endoscopy with the Muller maneuver (MM)) in both arms were included to evaluate the effect of adding drug-induced sleep endoscopy. Outcome variables were identified as safety and effectiveness. Safety was assessed by examining test-related adverse events and adverse reactions as indicators, while effectiveness was evaluated using "post-treatment (surgery) sleep apnea improvement indicators" and "treatment (surgery) success rates" as primary indicators, and "change in treatment site" and "change in treatment decision" as secondary indicators. Only comparative studies were included in the assessment. The systematic review was conducted by searching three international databases (Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews-Cochrane Central Register of Controlled Trials) and three domestic databases (KoreaMed, Korean Medical Article Database (KMbase), and Research Information Service of Korea (RISS)) based on the key questions (last searched on April 8, 2024). Cochrane's Risk of Bias (RoB) for randomized clinical trials and Risk of Bias Assessment for Nonrandomized Studies (RoBANS Ver.2) for nonrandomized studies were used to assess the risk of bias in the final selected studies. For the systematic literature review, the level of evidence was assessed using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) method. The data were extracted using a predetermined format. Meta-analysis was performed if quantitative analysis was feasible. All processes were conducted independently by two assessors, and in the event of disagreement, a consensus was reached between the assessors. The Health Technology Reassessment Committee conducted a final review of this assessment based on the findings of the subcommittee and determined the recommended rating. Results The systematic review identified 14 studies (1,454 subjects, with 736 in the intervention group and 756 in the control group, consisting of 72.7% males and 27.3% females). The studies included three randomized controlled trials (RCTs) and 11 non-randomized studies (NRS). All studies included patients with obstructive sleep apnea who either were not prescribed a positive airway pressure device or had undergone other treatments (such as surgery or intraoral devices) after unsuccessful positive airway pressure therapy. The risk of bias was assessed as uncertain (66.7%) and high (33.3%) in the domains of allocation concealment and blinding in the RCT study and as uncertain (81.8%) in the domains of confounders and rater blinding in the NRS study, although the risk of bias was generally low. Safety The safety of drug-induced sleep endoscopy was examined in two studies, which identified test-related adverse events and adverse reactions. Both studies reported no adverse events or adverse reactions associated with drug-induced sleep endoscopy in patients with obstructive sleep apnea. Effectiveness The effectiveness of drug-induced sleep endoscopy was the focus of 14 studies, which used various outcome measures to assess sleep apnea improvement after treatment (surgery), treatment success rate, change in treatment site, and change in treatment decision. Post-treatment sleep apnea improvement was primarily measured by: (1) apnea hypopnea index (AHI) and respiratory disturbance index (RDI), and (2) lowest oxygen saturation (LOS), oxygen desaturation index (ODI), and cumulative time with oxygen saturation below 90% (CT90%). A meta-analysis of 11 studies (three randomized controlled trials and eight non-randomized studies) that reported AHI and RDI and allowed for quantitative synthesis found that the DISE group exhibited a tendency to have a lower respiratory index than the non-DISE group; however, the difference was not statistically significant (RCT: MD –1.79 [95% CI –4.88, 1.30], I²=26%, NRS: SMD –-0.25 [95% CI –0.55, 0.04], I²=79%). Following a thorough analysis of the subgroups under three distinct criteria (1) non-DISE comparison, (2) standard endoscopy position (supine), and (3) treatment method (surgery, intraoral device)), the incorporation of DISE into the physical examination resulted in a substantial enhancement of the respiratory index in patients treated with an intraoral device when the supine position was adopted, as recommended by prevailing guidelines and textbooks. A meta-analysis of five studies (three RCTs and two NRSs) that reported LOS, all of which allowed for quantitative synthesis, found no statistically significant differences. However, given the high degree of heterogeneity among the studies, their interpretation should be done with caution (RCT: MD 0.78 [95% CI –0.91, 2.48], I²=22%, NRS: 2.35 [95% CI –7.30, 2.25], I²=95%). Treatment success, as defined in the extant literature, was commonly reported as a decrease in the postoperative AHI values to less than 20 apneas per hour or a 50% improvement over preoperative AHI. This outcome was reported in nine studies (two RCTs and seven NRS). None of the meta-analyses demonstrated statistically significant differences; however, the results were marked by substantial heterogeneity, necessitating cautious interpretation (RCT: RR 1.17 [95% CI 0.84, 1.64], I²=69), NRS: OR 1.19 [95% CI 0.59, 2.39], I²=76%). No studies addressed the secondary outcomes of “change in site of treatment (surgery)” or “change in treatment decision.” Conclusions and Recommendations The Health Technology Reassessment Subcommittee presented the following safety and effectiveness findings for drug-induced sleep endoscopy in patients with obstructive sleep apnea based on currently available evidence. A systematic review has revealed an absence of adverse events or adverse reactions reported during drug-induced sleep endoscopy, indicating that the safety of the technique is comparable to that of conventional sedation endoscopy. However, given that apnea occurs during sleep, it is recommended that it be performed in a well-equipped clinical setting under the supervision of healthcare providers. For effectiveness, the overall analysis demonstrated no statistically significant differences in post-treatment sleep apnea improvement indicators or surgical success rates when compared with testing while awake (physical examination, flexible endoscopy with the Muller maneuver (MM)). However, certain subgroup analyses (subcomparison, supine position, and treatment) exhibited significant improvements in respiratory index, although these findings were assessed as low-quality evidence. Given that the test is predominantly administered in clinical practice to patients who are not prescribed a positive airway pressure device or who have failed a positive airway pressure device, and considering that there is no standardization of "non-PAP treatment," and no treatment alternatives that can dynamically identify the site and extent of obstruction during sleep, the subcommittee suggests that the test is a technology that could benefit patients with obstructive sleep apnea who are not prescribed a positive airway pressure device. On September 6, 2024, the Ninth Health Technology Reassessment Committee deliberated on “drug-induced sleep endoscopy” as follows: Considering the evidence of clinical safety and effectiveness, the Health Technology Reassessment Committee has determined that the utilization of drug-induced sleep endoscopy for the purpose of identifying upper airway obstruction is “weakly recommended” for patients with obstructive sleep apnea who are not prescribed positive airway pressure therapy or who require surgical intervention following failure of positive airway pressure therapy in Korean clinical practice.
