다주파수 바이오임피던스분석법을 이용한 말기 신부전 환자의 체수분 상태 측정

Body Fluid Status Measurement Using Multi-Frequency Bioimpedance Analysis in Patients with End-Stage Renal Disease

초록

평가배경 다주파수 바이오임피던스 분석법을 이용한 말기 신부전 환자의 체수분 상태 측정은 투석 중인 말기 신부전 환자를 대상으로 체수분 평가 결과를 체액관리에 활용하여, 건체중을 효과적으로 조정하고 의료결과를 개선하기 위한 기술이다. 다주파수 바이오임피던스 분석법을 이용한 체수분 상태 측정은 2013년 신의료기술평가를 통해 신의료기술로 인정받았으며, 이후 2019년부터는 「가1 진찰료」 또는 「가2 입원료」의 소정 점수에 포함되어 별도의 수가 없이 활용되어 왔다. 최근 본 기술에 대한 별도 수가 신설 필요성이 제기됨에 따라 연구주제 수요조사를 거쳐 재평가 안건으로 제안되었고, 우선순위 심의를 통해 최종 재평가 대상으로 선정되었다. 이에 따라 동 기술의 임상적 효과성을 현 시점에서 확인하고자, 2024년 제4차 의료기술재평가위원회(2024.4.12.)에서 평가계획서 및 소위원회 구성안을 심의한 후 재평가를 수행하였다. 평가 목적 본 평가의 목적은 다주파수 바이오임피던스 분석법을 활용한 체수분 측정이 투석을 받는 말기 신부전 환자의 체액관리와 의료결과 개선에 임상적으로 효과적인가에 대한 근거를 제공하여 의료기술재평가 권고등급을 결정하기 위함이다. 평가 방법 다주파수 바이오임피던스 분석법을 이용한 체수분 상태 측정이 투석 중인 말기 신부전 환자의 체액관리와 의료결과 개선에 임상적으로 효과적인지 확인하기 위해 체계적 문헌고찰을 수행하였다. 이를 위해 ‘다주파수 바이오임피던스 분석법을 활용한 상지 림프부종 고위험 환자의 체수분 상태 측정’ 평가와 함께 공동 소위원회를 구성하였으며, 신장내과 전문의 2인, 유방외과 전문의 2인, 일반외과 전문의 1인, 재활의학과 전문의 1인, 산부인과 전문의 1인, 근거기반의학 전문가 1인 등 총 8인의 위원으로 구성하였다. 평가방법 및 세부 절차는 평가목적에 따라 ‘다주파수 바이오임피던스 분석법을 이용한 체수분 상태 측정 소위원회(이하 ‘소위원회’)’의 논의를 거쳐 결정하였다. 본 평가의 대상은 투석을 시행받고 있는 말기 신부전 환자로 설정하였다. 중재군은 다주파수 바이오임피던스 분석법을 이용한 체액 관리 그룹으로, 비교군은 임상의의 신체검진 등 기존의 임상평가에 기반한 체액 관리 그룹으로 정의하였다. 의료결과 평가 시, 다주파수 바이오임피던스 분석법은 비침습적 검사로 인체에 별도의 위해를 가하지 않는 기술이므로 안전성 평가는 별도로 수행하지 않았다. 효과성 평가는 해당 기술을 활용한 체액 관리가 환자의 건체중 및 의료결과에 미치는 영향을 중심으로 이루어졌다. 일차지표로 건체중, 임상사건 발생, 사망/생존을 설정하였고, 이차지표로는 심장 기능 지표, 수분 상태, 잔존 신장기능, 동맥경직도, 혈압약 사용량 변화, 수분조절과 관련된 혈액지표(N-말단 프로-B형 나트륨이뇨펩타이드(N-terminal pro-B-type natriuretic peptide, NT-proBNP), Troponin), 삶의 질 등을 포함하였다. 또한 해당 기술의 비용효과성이나 비용절감 효과를 다룬 경제성 연구도 평가 범위에 포함하였다. 체계적 문헌고찰은 위의 핵심질문을 중심으로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials)와 국내 데이터베이스 3개(KoreaMed, 의학논문데이터베이스, 한국교육학술정보원)를 활용하여 연구를 검색하였다. 연구의 선택 및 배제기준 적용과 비뚤림위험 평가 과정은 2명의 평가자가 독립적으로 수행하였으며, 의견이 불일치할 경우 제3자와 논의를 거쳐 최종 합의하였다. 선정된 연구의 비뚤림위험 평가는 선택연구가 모두 무작위배정 비교임상시험(randomized controlled trials, RCT)으로 확인되어 risk of bias (RoB) 도구를 이용하여 수행하였으며, 경제성 연구는 별도의 비뚤림위험 평가를 실시하지 않았다. 자료분석은 양적 분석이 가능한 경우 메타분석을 시행하고, 양적 분석이 어렵다고 판단되는 경우 질적 분석으로 대체하였다. 본 평가의 결과는 소위원회 검토를 거쳐 의료기술재평가위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다. 평가 결과 최종적으로 선정된 연구는 총 21편이었으며, 이 중 효과성 평가 연구가 19편(RCT 19편, 연구대상자 총 3,240명), 경제성 연구가 2편 포함되었다. 효과성 평가 대상 연구 중 혈액투석 환자를 포함한 연구가 11편, 복막투석 환자를 포함한 연구가 8편이었다. 중재군(intervention, 이하 ‘I’)은 다주파수 바이오임피던스 분석법을 이용한 체액 관리 그룹으로, 사용된 장비는 Body Composition Monitor(BCM)가 16편으로 가장 많았으며, InBody 720을 사용한 연구가 2편, Bodystat을 사용한 연구가 1편이었다. 비교군(control, 이하 ‘C’)은 기존 임상 평가(임상의의 신체검진과 같은 전통적 평가 방법)에 따라 체액 관리를 시행한 그룹으로 정의하였다. 비뚤림위험 평가는 무작위배정 방법에서 ‘불확실’한 연구가 많았는데, 특히 무작위 배정 순서의 은폐(73.7%), 연구진 및 평가자의 눈가림(89.5%) 항목에서 ‘불확실’로 평가된 비율이 높았다. 또한 높은 환자 탈락률(42.1%), 선택적 보고(5.3%), 제조사 지원을 받은 연구(36.8%) 등이 확인되어 비뚤림위험 ‘높음’으로 평가되었다. 전반적으로 본 평가에 포함된 근거 수준은 ‘낮음’에서 ‘매우 낮음’ 정도로 판단하였다. 효과성 건체중을 평가한 연구는 총 3편(I: 385명/C: 374명)이었다. 이 중 1편에서는 건체중을 1.1kg 이내로 유지한 환자의 비율이 중재군에서 비교군보다 통계적으로 유의하게 높았다. 또 다른 1편에서는 임상적으로 사용 중인 건체중과 다주파수 바이오임피던스 분석법으로 측정한 건체중 간에 매우 높은 상관관계(r=0.99)가 나타났으며, 12개월 추적 시 중재군이 비교군보다 건체중을 덜 과대평가하는 경향이 관찰되었다. 나머지 1편에서는 중재군에서 목표 체중(target weight)과 정상 수분상태에서의 체중(normally hydrated weight) 간 차이가 더 작게 나타났다. 임상사건 발생을 평가한 연구는 14편(I: 1,365명/C: 1,321명)이었다. 이 중 정량합성이 가능한 연구를 대상으로 메타분석을 시행한 결과, 심혈관 사건 발생(7편, I: 875명/C: 849명)은 통합 상대위험비(risk ratio, 이하 ‘RR’) 0.76 (95% 신뢰구간(confidence interval, 이하 ‘CI’) 0.56, 1.05, I2=10%)으로 통계적으로 유의한 차이는 없었다. 다만, 추적기간을 구분한 하위군 분석에서는 1년 이내 추적관찰한 연구에서 중재군의 심혈관 사건 발생 위험이 비교군에 비해 유의하게 낮았으나(통합 RR 0.59, 95% CI 0.39, 0.92, I2=0%), 1년을 초과한 연구에서는 유의한 차이가 없었다. 저혈압 발생(3편, I:317명/C: 312명)은 통합 RR 1.2 (95% CI 0.75, 1.92, I2=0%), 입원 발생(7편, I: 747명/C: 724명)은 통합 RR 1.02 (95% CI 0.92, 1.12, I2=0%)로 두 지표 모두 군 간 유의한 차이가 없었다. 추가로 시행한 저혈압 및 입원 발생의 추적기간별 하위군 분석에서도 군 간 유의한 차이가 관찰되지 않았다. 사망 및 생존과 관련된 연구는 총 12편(I: 1,290명/C: 1,246명)이었다. 사망 위험비(hazard ratio, 이하 ‘HR’)를 보고한 연구는 4편(I: 552명/C: 556명)으로, 이 중 2편에서는 군 간 유의한 차이가 없었으며, 1편에서는 중재군에서 사망 위험이 유의하게 낮았다. 나머지 1편에서는 모든 원인의 사망에 대한 HR이 1년 시점에서 군 간 유의한 차이가 없었으나, 3년 시점에서는 중재군에서 유의하게 낮게 나타났다. 생존 위험비를 보고한 연구는 2편(I: 356명/C: 329명)으로, 이 중 1편은 두 군 간 유의한 차이가 없었으며, 다른 1편은 3년 전체 생존율에 대한 HR이 중재군에서 유의하게 낮은 것으로 나타났으나, 1년 생존율에서는 차이가 없었다. 사망 발생을 보고한 연구는 10편(I: 1,120명/C: 1,078명)이었으며, 메타분석 결과 통합 상대 위험비(risk ratio, 이하 ‘RR’)는 0.66 (95% CI 0.47, 0.94, I2=23%)으로 중재군에서 사망 위험이 유의하게 낮은 것으로 나타났다. 다만 하위군 분석에서는 추적 기간과 사용 장비에 따라 통계적으로 유의한 결과와 유의하지 않은 결과가 혼재되어 있었다. 심장기능지표는 6편(I: 328명/C: 321명)에서 평가하였으며, 좌심실 질량, 좌심실박출률, 이완기 초기 승모판 유속 대 초기 승모륜 유속 비율 및 좌심방 용적지수를 보고하였다. 각 지표는 연구에 따라 추적관찰 시점에서 기저치 대비 정상 범위 내로 유지되거나, 비정상 상태에서 유지되거나, 혹은 개선 또는 악화되는 등 다양한 양상을 보였으며, 이러한 경향은 두 군에서 모두 혼재되어 나타났다. 수분 상태는 총 15편(I: 1,015명/C: 988명)에서 평가하였으며, 과수분 상태(overhydration, 이하 ‘OH’)(11편), 세포 외 수분 대비 총 체수분 비율 (4편), 하대정맥 직경 측정 (2편)을 보고하였다. 각 지표 및 연구별로 초기 측정값과 비교하여 추적관찰 시점에서 정상 범위를 유지하거나 체수분 과잉상태가 지속되거나, 개선 혹은 악화되는 다양한 결과가 관찰되었으며, 이러한 경향은 두 군에서 모두 혼재되어 나타났다. 잔존 신장 기능을 평가한 연구는 11편(I: 842명/C: 833명)이었다. 이 중 소변량을 보고한 연구는 총 7편(I: 402명 / C: 403명)이었으며, 두 군 모두 추적관찰 시점에서 초기 대비 소변량이 감소하는 경향을 보였다. 소변량 감소에 대한 메타분석 결과(변화량 분석, 7편, I: 402명/C: 403명)는 통합 평균차가 66.25mL(95% CI -29.75mL, 162.26mL, I2=63%)로 두 군 간 유의한 차이는 없었다. 그 외 크레아티닌 청소율(1편), 사구체 여과율(3편), 잔뇨량(1편)의 경우에도 초기 대비 추적관찰 시점의 변화량은 중재군과 비교군 간 유사한 수준으로 나타났다. 또한, 잔존 신장 기능이 30% 초과로 감소한 환자의 비율(1편), 200mL 이상의 이뇨가 유지된 환자의 비율(1편), 무뇨증 환자 비율(3편) 및 위험비(1편)는 모두 두 군 간에 차이가 없었다. 동맥경직도 변화를 평가한 연구는 5편(I: 288명/C: 288명)이었다. 이들 연구에서 맥파속도의 변화를 분석한 결과, 중재군에서는 5편 중 4편에서 초기 대비 추적관찰 시점에서 유의한 개선을 보였고, 나머지 1편에서는 유의한 변화가 없었다. 반면 비교군에서는 1편에서만 유의한 개선이 보고되었으며, 3편에서는 유의한 악화가 관찰되었고, 나머지 1편에서는 유의한 변화가 나타나지 않았다. 혈압약 사용량 변화를 평가한 연구는 총 6편(I: 274명/C: 260명)이었으며, 이 중 1편에서만 중재군에서 혈압약 사용 개수가 유의하게 감소하였다. 나머지 5편에서는 추적관찰 시점에서 혈압약 사용 개수가 초기 대비 두 군 간 비슷한 수준으로 나타났으며, 통계적으로 유의한 차이가 없었거나 차이에 대한 통계적 유의성을 명시적으로 보고하지 않았다. 혈액검사 중 NT-proBNP의 변화를 보고한 연구는 4편(I: 196명/C: 184명)이었다. 이 중 1편에서는 중재군과 비교군 모두 추적관찰 시점에서 초기 대비 NT-proBNP 수치가 증가하였으나, 군 간 차이에 대한 통계적 유의성은 보고되지 않았다. 나머지 연구 중 2편은 두 군 모두에서 유의한 변화가 관찰되지 않았으며, 1편에서는 두 군 모두에서 NT-proBNP가 유의하게 개선된 것으로 나타났다. Troponin의 변화를 보고한 연구는 총 2편(I: 107명/C: 108명)이었으며, 두 연구 모두 중재군과 비교군에서 유의한 변화는 없었다. 삶의 질 변화를 보고한 연구는 2편(I: 247명/C: 242명)이었으며, 각 군의 초기 대비 추적관찰 시점에서 뚜렷한 변화 없이 비슷한 수준을 유지하였고, 군 간 비교에서도 유의한 차이는 관찰되지 않았다. 경제성 경제성 결과지표를 보고한 연구는 총 2편으로, 모두 영국에서 수행된 연구였다. 두 연구 모두 만성 신장질환 환자를 대상으로 마콥모델을 적용하여 점증적 비용효과비를 보고하였다. 경제성 분석 결과, 다주파수 바이오임피던스 분석법을 이용한 체액 관리는 투석 환자에서 일정 수준의 비용효과성을 가질 가능성이 있으나, 분석 시 투석 비용 포함 여부에 따라 결과가 달라질 수 있다고 제시하였다. 또한, 비용효과성 결과가 특정 장비에 국한되어 일반화 가능성이 제한적이고, 중간 지표인 체수분 상태나 맥파속도와 임상적 최종 결과(사망률, 심혈관사건) 간의 인과관계가 명확히 입증되지 않아 결과 해석에 불확실성이 존재한다고 보고하였다. 결론 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같은 의견을 제시하였다. 체계적 문헌고찰 결과, 효과성 측면에서 투석 중인 말기 신부전 환자를 대상으로 다주파수 바이오임피던스 분석법을 이용한 체액관리 그룹(중재군)과 임상 평가 기반 체액 관리 그룹(비교군)을 비교했을 때, 임상사건(심혈관 사건, 저혈압, 입원)이나 잔존 신장 기능에서는 유의한 차이를 확인하지 못하였다. 다만, 중재군이 비교군에 비해 건체중의 과대평가가 적었고, 사망 위험 감소와 관련된 유의한 결과가 확인되었으며, 동맥경직도에서 더 개선된 결과가 관찰되었다. 이를 바탕으로 다주파수 바이오임피던스 분석법을 활용한 체수분 평가 기반 체액 관리가 투석 중인 말기 신부전 환자의 의료 결과 개선에 도움이 될 수 있다고 판단하였다. 또한, 전체 선택 연구 19편 대부분에서 특정 장비를 사용하였으며, 장비별로 사용되는 지표나 프로토콜이 상이하여, 본 평가결과를 다주파수 바이오임피던스 분석법을 사용하는 모든 장비에 일반화해서 적용하기 어렵다는 의견이 있었다. 경제성 연구 검토 결과, 다주파수 바이오임피던스 분석법은 투석 중인 말기 신부전 환자의 체액 관리에 비용 효율적인 접근법으로 활용될 가능성이 있음을 확인하였으나, 연구 수가 적고 선행 연구의 임상 상황이 국내 임상 환경과 차이가 있어 경제성을 평가하기는 어렵다고 판단하였다. 2024년 제12차 의료기술재평가위원회(2024.12.13.)는 소위원회 결론 및 분과의견을 검토하여 다음과 같이 심의 의결하고 의료기술재평가 권고등급을 결정하였다. 의료기술재평가위원회는 말기 신부전 환자에서 다주파수 바이오임피던스 분석법을 이용한 체수분 상태 측정이 의료결과, 특히 사망 위험 감소와 관련된 효과에 대해 추적기간 및 장비에 따라 통계적 유의성이 달라지는 것을 확인하였다. 또한 환자 탈락률이 높은 연구들이 다수 포함되어 연구결과에 영향을 미쳤을 가능성이 있으며, 전반적인 근거 수준은 낮은 것으로 평가하였다. 다만, 환자의 체수분상태를 측정하는데 있어 별다른 대안이 없는 현재 상황에서, 다주파수 바이오임피던스 분석법은 객관화된 수치로 체수분 상태를 확인하여 건체중 조절 등의 임상적 환자관리에 도움이 될 수 있다고 판단하였다. 의료기술재평가위원회는 임상적 효과성의 근거 등을 종합적으로 고려하였을 때, 국내 임상상황에서 투석 중인 말기 신부전 환자를 대상으로 다주파수 바이오임피던스 분석법의 사용을 ‘약하게 권고함’으로 심의하였다. 안건명은 기술명에 대상환자를 추가하여 ‘다주파수 바이오임피던스 분석법을 이용한 말기 신부전 환자의 체수분 상태 측정’으로 변경하였다.


Background Body fluid status measurement using multi-frequency bioimpedance analysis (BIA) is a technique aimed at improving fluid management and clinical outcomes in patients with end-stage renal disease (ESRD) who are undergoing dialysis, by using body fluid assessment results to guide dry weight adjustment. This technology was designated as a new health technology in 2013 and has been implemented without a separate fee since 2019, as part of the reimbursed items under “Ga-1 Outpatient Visit Fee” or “Ga-2 Inpatient Fee.” Recently, the need for charging a separate fee for this technology was raised; thus, the technology was selected for reassessment following research topic demand surveys and priority deliberations. The reassessment plan and subcommittee composition for examining the evidence for its effectiveness were reviewed and approved at the 4th 2024 Health Technology Reassessment Committee meeting (April 12, 2024). Objectives The objectives of this assessment were to determine whether body fluid status measurement using multi-frequency BIA is clinically effective for fluid management and improvement of clinical outcomes in patients with ESRD who are undergoing dialysis and to evaluate the strength of recommendation for its clinical use. Methods A systematic literature review was conducted to evaluate whether body fluid status measurement using multi-frequency BIA is clinically effective for fluid management and improvement of clinical outcomes in patients with ESRD who are undergoing dialysis. The subcommittee was convened jointly with the subcommittee for the assessment of body fluid status measurement using multi-frequency BIA in patients at high risk for upper limb lymphedema. The subcommittee consisted of eight members: two nephrologists, two breast surgeons, one general surgeon, one rehabilitation medicine specialist, one obstetrician/gynecologist, and one evidence-based medicine expert. The assessment methods and detailed procedures were finalized through discussions of the "Subcommittee for Body Fluid Status Measurement Using Multi-Frequency Bioimpedance Analysis" (hereinafter referred to as the “subcommittee”). The target population was defined as patients with ESRD who were undergoing dialysis. The intervention group was defined as patients in whom fluid management was based on multi-frequency BIA, while the control group was defined as patients in whom fluid management was based on traditional clinical assessments, such as physical examination by clinicians. Since multi-frequency BIA is a noninvasive test that poses no direct harm to patients, no separate safety assessment was conducted. The effectiveness assessment focused on the impacts of fluid management based on this technology on dry weight and on the clinical outcomes of such management. Primary outcomes included dry weight, clinical events, and mortality/survival. Secondary outcomes included cardiac function parameters, hydration status, residual renal function, arterial stiffness, changes in antihypertensive medication use, blood biomarkers related to fluid management (N-terminal pro-B-type natriuretic peptide [NT-proBNP], troponin), and quality of life. Economic evaluations related to cost-effectiveness or cost savings associated with the technology were also included in the assessment. Literature searches were conducted in three international databases (Ovid-MEDLINE, Ovid-EMBASE, and Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, KMbase, and RISS). Study selection, application of inclusion and exclusion criteria, and risk-of-bias assessments were independently performed by two reviewers, and any disagreements were resolved through discussion with a third party. All studies selected for effectiveness assessment were randomized controlled trials (RCTs); therefore, the risk of bias was assessed using the Cochrane Risk-of-Bias tool. No separate risk-of-bias assessment was conducted for economic evaluations. A meta-analysis was performed when quantitative analysis was feasible; where this was not feasible, a qualitative synthesis was conducted. The final strength of recommendation was determined by the Health Technology Reassessment Committee, based on the subcommittee’s findings. Results Twenty-one studies were evaluated: 19 studies were selected for effectiveness assessment (RCTs; total n = 3,240) and two studies were included for economic assessments. Of the effectiveness studies, 11 included patients undergoing hemodialysis, while eight included patients undergoing peritoneal dialysis. The intervention (I) group consisted of patients managed with multi-frequency BIA. A body composition monitor was used in 16 studies, the InBody 720 in two studies, and a Bodystat in one study. The control (C) group was defined as patients who were managed using traditional clinical assessments, such as physical examination. In the risk-of-bias assessment, many studies were rated as “uncertain” in terms of randomization, particularly regarding allocation concealment (73.7%) and blinding of personnel and outcome assessors (89.5%). High patient dropout rates (42.1%), selective reporting (5.3%), and manufacturer-sponsored studies (36.8%) were also identified, leading to an overall high risk of bias. Taken together, the level of evidence for the included studies was “low” to “very low.” Effectiveness Three studies (I: 385 patients; C: 374 patients) assessed dry weight. In one study, the proportion of patients maintaining a dry weight within 1.1 kg was significantly higher in the intervention than in the control group. Another study reported a very strong correlation (r = 0.99) between the clinically assessed dry weight and that measured by multi-frequency BIA, with the intervention group showing less overestimation of dry weight than did the control group at the 12-month follow-up. In the remaining study, the difference between the target weight and the normally hydrated weight was smaller in the intervention than in the control group. Fourteen studies (I: 1,365 patients; C: 1,321 patients) evaluated clinical events. A meta-analysis was performed on studies eligible for quantitative synthesis. For cardiovascular events (seven studies; I: 875 patients; C: 849 patients), the pooled risk ratio (RR) was 0.76 (95% confidence interval [CI]: 0.56–1.05; I² = 10%), with no statistically significant difference. However, subgroup analysis by follow-up period showed a significantly lower risk of cardiovascular events in the intervention group within 1 year (pooled RR 0.59; 95% CI: 0.39–0.92; I² = 0%), while no significant difference was observed in studies with a follow-up of longer than 1 year. For hypotension events (three studies; I: 317 patients; C: 312 patients), the pooled RR was 1.2 (95% CI: 0.75–1.92; I² = 0%). For hospitalizations (seven studies; I: 747 patients; C: 724 patients), the pooled RR was 1.02 (95% CI: 0.92–1.12; I² = 0%). No statistically significant differences were observed between the groups for either of these outcomes. Additionally, no significant differences were found between the groups in terms of hypotension events or hospitalization rates in additional subgroup analyses conducted based on follow-up period. Twelve studies (I: 1,290 patients; C: 1,246 patients) reported on mortality and survival outcomes. Four studies (I: 552 patients; C: 556 patients) presented hazard ratios (HRs) for mortality. Two of these found no significant difference between groups, while one study revealed a significantly lower mortality risk in the intervention than in the control group. In the remaining study, the HR for all-cause mortality did not differ significantly between the groups at 1 year but was significantly lower in the intervention than in the control group at 3 years. Two studies (I: 356 patients; C: 329 patients) reported the HR for survival. One study found no significant difference, while the other reported a significantly lower HR for the 3-year overall survival in the intervention than in the control group, with no difference in the 1-year survival. Ten studies (I: 1,120 patients; C: 1,078 patients) reported on mortality events. A meta-analysis yielded a pooled RR of 0.66 (95% CI: 0.47–0.94; I² = 23%), indicating a significantly lower mortality risk in the intervention than in the control group. However, subgroup analyses showed mixed results, depending on follow-up duration and the device used. Six studies (I: 328 patients; C: 321 patients) evaluated cardiac function parameters, including left ventricular mass, ejection fraction, the early mitral inflow velocity to early mitral annular velocity (E/E’) ratio, and the left atrial volume index. Across studies, some parameters remained within the normal range, some worsened or improved, and trends varied within and between groups at follow-up. Fifteen studies (I: 1,015 patients; C: 988 patients) assessed hydration status. Overhydration (OH) was reported in 11 studies; the extracellular water to total body water ratio in four studies; and the inferior vena cava diameter in two studies. Results varied across studies, with some showing stable measurements at follow-up, while others showed persistent, improved, or worsened OH in both groups. Residual renal function was evaluated in 11 studies (I: 842 patients; C: 833 patients). Seven of these studies (I: 402 patients; C: 403 patients) reported on urine output, showing a decreasing trend in both groups at follow-up. A meta-analysis of changes in urine output (volume of change; seven studies; I: 402 patients; C: 403 patients) showed a pooled mean difference of 66.25 mL (95% CI: –29.75 to 162.26 mL; I² = 63%), with no significant difference between groups. Changes in other parameters, including creatinine clearance (one study), glomerular filtration rate (three studies), and residual urine volume (one study), as compared to the baseline values, were similar in the intervention and control groups. The proportions of patients with residual renal function that declined by > 30% (one study), patients who maintained urine output ≥ 200 mL (one study), and those with anuria (three studies), as well as the associated risk ratios (one study) also did not differ significantly between the two groups. Five studies (I: 288 patients; C: 288 patients) evaluated changes in arterial stiffness. In the intervention group, four of the five studies reported significant improvement in pulse wave velocity at follow-up, while the remaining study showed no significant change. In terms of the control group, only one study reported improvement, three reported significant exacerbation, and one showed no significant change in this parameter. Six studies (I: 274 patients; C: 260 patients) assessed changes in antihypertensive medication use. Only one study reported a significant reduction in the number of antihypertensive agents used in the intervention group. The remaining five studies reported no significant change or did not clearly report statistical significance, with both groups showing similar levels of medication use at follow-up. Changes in NT-proBNP levels were reported in four studies (I: 96 patients; C: 184 patients). In one study, both groups demonstrated increased NT-proBNP levels at follow-up, but the statistical significance of the difference between the groups was not reported. Of the three remaining studies, two reported no significant changes in either group, while one study reported significant improvements in NT-proBNP levels in both groups. Changes in troponin levels were reported in two studies (I: 107 patients; C: 108 patients), but no significant changes were observed in either group. Two studies (I: 247 patients; C: 242 patients) assessed the patients’ quality of life. Both groups maintained similar quality-of-life levels at follow-up as compared to baseline, and no significant differences were observed between the groups. Cost-effectiveness Two studies reported on economic outcomes, and both were conducted in the United Kingdom. Both studies applied Markov models in studies of patients with chronic kidney disease and reported incremental cost-effectiveness ratios. The economic analyses suggested that fluid management based on multi-frequency BIA may be cost-effective in patients on dialysis; however, the results varied depending on whether dialysis costs were included in the analysis. In addition, both studies noted limitations in generalizability, as the cost-effectiveness outcomes were specific to certain devices, and causal relationships between intermediate markers (e.g., hydration status, pulse wave velocity) and the final clinical outcomes (e.g., mortality, cardiovascular events) had not been fully established, thereby introducing uncertainty in the interpretation of the results. Conclusion Based on the current assessment results, the subcommittee presented the following opinions: Regarding effectiveness, the systematic literature review findings revealed no significant differences between the intervention (patients receiving fluid management based on multi-frequency BIA) and control groups (patients receiving fluid management based on clinical assessments) in terms of clinical events (cardiovascular events, hypotension, and hospitalizations) or residual renal function. However, the intervention group had less frequent overestimation of dry weight, significantly lower mortality risk, and greater improvement in arterial stiffness than did the control group. Based on these findings, the subcommittee determined that fluid management guided by body fluid assessment using multi-frequency BIA may help to improve clinical outcomes in patients with ESRD who are undergoing dialysis. Nevertheless, the results of this assessment cannot be generalized to all multi-frequency BIA devices, as most of the 19 included studies used specific devices and utilized different indices and protocols. Regarding economic evaluations, multi-frequency BIA may represent a cost-effective approach to fluid management in patients with ESRD who are undergoing dialysis; however, due to the small number of studies and differences between study settings and current Korean clinical practice, the subcommittee stated that cost-effectiveness cannot be determined at this time. At the 12th 2024 Health Technology Reassessment Committee meeting (December 13, 2024), the committee reviewed the subcommittee’s conclusions and panel opinions and determined the final strength of the recommendation. The Health Technology Reassessment Committee confirmed that the effectiveness of body fluid status measurement using multi-frequency BIA (particularly in terms of reducing mortality risk) varies, depending on the follow-up period and the device used in patients with ESRD. The committee also noted that many of the included studies had high dropout rates, which may have affected study outcomes, and that the overall level of evidence was deemed to be low. However, in the current clinical environment where no clear alternatives exist for objectively assessing hydration status, multi-frequency BIA may assist with clinical management, such as dry weight adjustment, by providing objective measurements of body fluid status. Considering the totality of evidence on its clinical effectiveness, the Health Technology Reassessment Committee determined that the use of multi-frequency BIA in patients with ESRD who are undergoing dialysis should be “weakly recommended” in the Korean clinical setting. The agenda title was modified to specify the target population as “Body Fluid Status Measurement Using Multi-Frequency Bioimpedance Analysis in Patients with End-Stage Renal Disease.”
