초록
평가배경 ‘발음 및 발성검사’는 성대결절, 폴립, 성대마비, 후두악성종양 등 음성장애를 유발하는 모든 질환자를 대상으로 음성장애의 유무 및 정도를 평가하고, 수술 후 추적관찰 및 수술·음성치료 효과 분석을 목적으로 비급여로 등재된 기술이다. 동 기술은 유관기관 수요조사를 통해 발굴되어 2025년 제4차 의료기술재평가위원회(2025.4.11.)에서 평가계획서 및 소위원회 구성안을 심의받은 후 재평가가 수행되었다. 평가목적 본 평가의 목적은 발성장애 환자에서 발음 및 발성검사의 임상적 유용성에 대한 근거를 검토하여 관련 정보를 제공하기 위함이다. 평가방법 본 평가는 발음 및 발성검사의 임상적 유용성을 확인하기 위해 “발음 및 발성검사 공동소위원회(이하 ‘소위원회’)”의 논의를 거쳐 평가방법을 확정하였다. 소위원회는 의료기술재평가위원회에서 심의한 평가계획에 따라 이비인후과 2인, 재활의학과 2인, 근거기반의학 1인을 포함한 총 5인으로 구성하였다. 발음 및 발성검사는 소리를 분석하는 기능 검사로 해부학적 병변 및 움직임 등을 확인하는 영상 검사를 비교검사로 설정하기는 어렵다고 판단하였다. 이에 본 평가에서는 체계적 문헌고찰을 통해 비교검사와의 효과성을 비교·분석하기보다는 의학교과서 및 국내외 임상진료지침 검토를 통해 임상적 유용성을 확인하고자 하였다. 또한, 검사항목이 다섯 가지(공기역학검사, 음성음향검사, 청각심리검사, 전기성문파검사, 비음측정검사)로 구성된 점을 고려하여 검사 사용 현황을 파악하기 위해 한 개의 데이터베이스를 토대로 간략 문헌검색을 수행하였다. 본 평가에서는 음성장애와 발성장애가 혼용되어 사용되고 있으나, 검사명을 고려하여 ‘발성장애’로 제시하였다. 평가 핵심질문은 ‘발음 및 발성검사는 발성장애 환자를 진단 및 평가하기 위해 임상적으로 효과적인가?’로 설정하였다. 교과서는 이비인후과, 재활의학 분야를 검토하였고, 임상진료지침은 국내외 데이터베이스 검색 후 소위원회 논의를 통해 포함 여부를 확정하였다. 문헌고찰은 핵심질문을 토대로 Medline 데이터베이스를 이용하여 수행하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종 심의하였다. 평가결과 발성장애를 일으키는 모든 질환에서 발음 및 발성검사의 유용성을 확인하기 위하여 교과서, 임상진료지침 및 관련 일차 연구를 검토하였다. 교과서 검토 국내 이비인후과학 및 후두음성언어의학 교과서를 검토한 결과, 청각심리검사, 공기역학검사, 음성음향검사, 전기성문파검사 등 다양한 음성검사에 대해 검사 원리, 주요 특징 및 측정치 지표의 범위가 비교적 상세히 기술되어 있었다. 국외 이비인후과 교과서에서도 청지각적 음성평가, 공기역학검사, 음성음향검사, 전기성문파검사 및 비음측정검사 등 여러 음성검사의 주요 특징과 임상적 활용에 대한 내용을 확인할 수 있었다. 임상진료지침 검토 총 11편의 임상진료지침을 검토한 결과, 대부분의 지침에서 음성 평가의 필요성을 제시하고 있었다. 질환별로 살펴보면, 발성성장애 환자를 대상으로 한 임상진료지침 5편 중 4편은 진단 및 치료 과정에서 음성 평가가 필요하다고 권고하고 있었으며, 특히 일본 및 유럽 지침에서 Grade Roughness Breathiness Asthenic Strained (GRBAS) 척도, 음성음향검사 및 공기역학적 검사를 구체적으로 제시하고 있었다. 나머지 1편의 지침은 경련성 발성장애 환자에서 보톨리눔 독소 치료 후 음성음향검사, 청각심리검사 및 환자 자가보고 도구(Voice Handicap Index, VHI) 등을 활용하여 치료 효과를 평가한 연구 사례를 제시하고 있었다. 갑상선 질환 환자를 대상으로 한 3편의 임상진료지침 중 2편은 갑상선 수술 전·후 음성 평가를 권고하였으며, 나머지 1편은 숙련된 외과의가 갑상선 절제술을 시행할 것을 권고하면서 초기 청력 평가와 함께 최대발성시간, 청각심리검사(GRBAS) 등의 음성 검사의 필요성을 언급하고 있었다. 두경부암 환자를 대상으로 한 임상진료지침에서는 두경부암 생존자에 대한 전반적인 언어평가의 필요성을 제시하였고, 후두암 환자 대상 지침에서는 치료 전 전문가에 의한 음성 평가, 비디오 스트로보스코피 및 음성 관련 삶의 질 평가를 권고하였다. 국내 편측 성대마비 환자 대상 임상 진료지침에서는 치료 전·후 청각심리검사, 음성음향검사, 공기역학검사 및 환자 자가보고 도구를 포함한 음성 평가를 강하게 권고하고 있었다. 일차연구 검토 발성장애 대상으로 발음 및 발성검사의 사용현황을 파악하고자 문헌검색을 통해 총 147편의 연구를 선택하였다. 연구대상자의 분포는 근긴장성, 기능성 및 경련성 발성장애를 포함한 발성장애 환자 대상 연구가 56편(38.1%), 편측 성대마비, 성대 결절 및 성대 용종 등 성대 병변 관련 연구가 47편(32.0%), 후두암, 성문암 등 암환자 대상 연구가 16편(10.9%), 갑상선 수술 환자 10편(6.8%), 후두염 등 후두 병변 관련 연구가 6편(4.1%)이었고, 이 외에 파킨슨병, 인후두 역류장애, 편도 및 아데노이드 수술, 근위축성 측삭경화증 또는 다발성경화증으로 인한 발성장애 등에 대한 연구가 확인되었다. 발음 및 발성검사의 종류는 음성음향검사가 122편(83.0%)으로 가장 많았고, 청각심리검사 98편(66.7%), 공기역학검사 19편(12.9%), 전기성문파검사 10편(6.8%), 비음측정검사 1편(0.7%)의 분포를 보였다. 청각심리검사 중 GRBAS 도구를 사용한 연구가 80편(54.4%)이었고, 음성음향검사의 경우 주요 장치는 다차원 음성프로그램(Multi-Dimensional Voice Program, MDVP)이 58편(39.5%)으로 가장 많았고, 그 다음으로 음성분석 프로그램인 Praat가 38편(25.9%)이었다. 공기역학검사는 주로 음성공기역학시스템(Phonatory Aerodynamic System, PAS)(13편, 8.8%)을 사용하고 있었다. 또한, 대부분의 연구는 발음 및 발성검사와 함께 환자 자가보고 도구(VHI 등), 후두스트로보스코피와 같은 영상검사 등을 함께 수행하고 있었다. 발음 및 발성검사의 사용 목적은 진단 목적의 연구가 15편, 질병과의 연관성을 보고한 연구 35편, 치료 효과 평가 및 비교 결과를 제시한 연구가 40편, 치료 전·후 모니터링 연구가 57편이었으며, 치료 효과를 비교하거나 질병 경과를 관찰한 연구가 더 많았다. 주요 검사지표로 음성음향검사의 경우 기본 주파수(fundamental frequency, F0), 지터, 쉬머, 켑스트럼 최고치 돌출값(cepstral peak prominence, CPP) 등이 있었고, 공기역학검사는 최대발성시간(maximum phonation time, MPT)을 비롯하여 발성역치압(phonation threshold pressure, PTP), 평균호기류율(mean flow rate, MFR) 등, 비음측정검사는 비강음 점수(nasalance score) 등의 객관적인 지표들을 다양하게 활용하였다. 결론 및 제언 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같이 결과 및 의견을 제시하였다. 소위원회는 국내외 교과서에서 발성장애 평가를 위해 활용되는 다섯 가지 검사의 원리, 주요 특징, 측정치 지표의 범위 및 임상적 활용이 비교적 상세히 제시되어 있음을 확인하였다. 또한 갑상선 환자, 두경부암 및 후두암 환자, 음성장애 환자를 대상으로 한 총 11편의 임상진료지침을 검토한 결과, 이 중 2편에서는 갑상선 수술 전·후 다차원적 음성평가를 권고하고 있었다. 미국, 일본, 한국 및 유럽에서 발간된 5편의 임상진료지침에서는 음성평가의 필요성을 제시하면서 음향학적 평가 및 공기역학적 평가 등에 대한 구체적인 음성평가 방법을 제시하고 있었다. 또한, 관련 임상 연구를 검토한 결과, 발음 및 발성검사는 다양한 질환에서 발성장애를 평가하거나 수술 또는 음성치료 전·후의 음성 상태 변화를 확인하기 위해 후두경 검사 및 음성 관련 자가보고 도구 등과 병행하여 활용되고 있었다. 이에 소위원회는 발음 및 발성검사가 후두점막질환, 후두암, 갑상선 수술 환자, 식도·흉부 수술 환자 등 다양한 환자에서 음성 상태를 평가하고 치료 후 음성 변화를 관찰하는 데 이미 임상적으로 유용한 검사이며, 대부분의 검사가 환자에게 위험성이 없는 비침습적인 검사라는 의견이었다. 다만, 국내 임상 상황에서 식약처 허가 사항 및 건강보험심사평가원 고시항목 등을 종합적으로 고려하였을 때, 발음 및 발성검사가 여러 개의 개별 검사로 구성되어 행위 정의가 모호한 측면이 있으므로 이를 명확히 할 필요가 있으며, 의료기관마다 장비와 소프트웨어가 다양하여 검사 결과의 참고치와 해석 기준에 대한 표준화가 필요하다는 의견을 제시하였다. 2025년 제3차 재평가전문위원회*(2025.11.14.)는 소위원회 검토 결과를 바탕으로 “발음 및 발성검사-발성장애”에 대해 심의하였으며, 소위원회의 검토 의견을 원안대로 의결하였다.
Background The Pronunciation and Phonation Test includes auditory perceptual testing, voice-acoustic testing, aerodynamic testing, electroglottography, and nasal resonance measurement, targeting all patients with conditions causing voice disorders, such as vocal nodules, polyps, vocal paralysis, and laryngeal malignancies. This test is a non-reimbursed health technology for evaluating voice disorders, postoperative follow-up, and the effectiveness of surgical treatment and voice therapy. This health technology was identified as a reassessment item in a demand survey conducted by the Health Technology Reassessment (HTR) Program. This health technology was selected for reassessment at the 4th Health Technology Reassessment Committee (April 11, 2025), and the reassessment was completed accordingly. Objective The objective of this assessment is to evaluate the evidence regarding the clinical utility of the Pronunciation and Phonation Test in patients with phonation disorders. Methods All assessment methods were finalized by the "Subcommittee on the Pronunciation and Phonation Test" ("Subcommittee") after deliberation and aligned with the assessment objectives. The Subcommittee comprised five experts: two otolaryngologists, two rehabilitation medicine specialists, and one evidence-based medicine specialist. In this evaluation, clinical utility was verified through a review of medical textbooks and clinical practice guidelines. Additionally, a brief literature search was conducted using a single database to identify current usage status. Medical textbooks in otolaryngology and rehabilitation medicine were reviewed. Clinical practice guidelines were identified through domestic and international database searches, after which their eligibility was determined through discussion within the Subcommittee. The literature review was performed using the Ovid-MEDLINE database. Based on the Subcommittee's review, the Health Technology Reassessment Committee reviewed and approved. Results Textbooks on otolaryngology and laryngopharynxonology described in relatively detail the principles, measurement parameters, and clinical applications of voice assessment, including auditory-perceptual evaluation, aerodynamic assessment, acoustic voice analysis, electroglottography (EGG), and nasometer. A total of 11 clinical practice guidelines were reviewed. Of the five clinical practice guidelines for patients with voice disorders, four recommended voice assessment during the diagnostic and treatment process; in particular, Japanese and European guidelines presented the Grade Roughness Breathiness Asthenic Strained (GRBAS) scale, voice acoustics, and aerodynamic testing. Of the three clinical practice guidelines for thyroid disease, two recommended voice evaluation before and after thyroid surgery. Clinical practice guidelines for head and neck cancer suggested the necessity of a comprehensive language assessment for head and neck cancer survivors. Guidelines for laryngeal cancer recommended voice evaluation by a specialist, videostroboscopy, and voice-related quality of life before voice therapy. The Korean clinical practice guidelines for unilateral vocal cord paralysis strongly recommended voice assessment before and after treatment, including auditory-perceptual evaluation, acoustic voice analysis, aerodynamic assessment, and patient-report outcome measures such as the Voice Handicap Index (VHI). A literature search identified 147 studies evaluating the clinical use of the Pronunciation and Phonation Test in patients with phonation disorders. The study populations included: Phonation disorders such as muscle tension dysphonia, functional dysphonia, and spasmodic dysphonia (56 studies; 38.1%), Vocal fold lesions, including unilateral vocal fold paralysis, vocal nodules, and vocal polyps (47 studies; 32.0%), Patients with laryngeal or glottic cancer (16 studies; 10.9%), Patients undergoing thyroid surgery (10 studies; 6.8%), Laryngeal disorders such as laryngitis (6 studies; 4.1%). Among the voice assessment modalities, acoustic voice analysis was the most frequently used, appearing in 122 studies (83.0%), followed by auditory-perceptual evaluation in 98 studies (66.7%), aerodynamic assessment in 19 studies (12.9%), electroglottography in 10 studies (6.8%), and nasometry in one study (0.7%). The main measurement tools used were the GRBAS in 80 studies (54.4%), the Multi-Dimensional Voice Program (MDVP) in 58 studies (39.5%), the voice analysis program Praat in 38 studies (25.9%), and the Phonatory Aerodynamic System (PAS) in 13 studies (8.8%). Additionally, most studies performed patient self-report tools (such as VHI) and imaging tests like laryngostroboscopy. The main measurement indicators included fundamental frequency (F0), jitter, shimmer, cepstral peak prominence (CPP), maximum phonation time (MPT), phonation threshold pressure (PTP), mean flow rate (MFR), and nasalance score. The purpose of use for the Pronunciation and Phonation test included diagnosis (15 studies), disease association (35 studies), evaluation and comparison of treatment effects (40 studies), and pre- and post-treatment monitoring (57 studies). Conclusion and Recommendations Based on a literature review of textbooks, clinical practice guidelines, and clinical studies, the Pronunciation and Phonation test is used to evaluate voice disorders or monitor voice status before and after surgery or voice therapy, and many countries have indicated the necessity of voice evaluation. The Subcommittee concluded that the Pronunciation and Phonation Test is clinically useful for evaluating voice function and monitoring treatment-related changes in patients with benign laryngeal mucosal diseases, laryngeal cancer, thyroid surgery, and esophageal or thoracic surgery, and that these tests are non-invasive procedures and therefore pose minimal risk to patients. However, the Subcommittee identified several issues requiring further consideration in the Korean healthcare setting. Because the Pronunciation and Phonation Test consists of multiple individual assessment components, the definition of the procedure remains ambiguous and should be more clearly specified. In addition, considerable variation exists among medical institutions with respect to testing equipment and software platforms. Therefore, establishing standardized reference values, measurement protocols, and interpretation criteria is necessary to improve the consistency and comparability of test results. The 3rd Health Technology Reassessment Expert Committee* (November 14, 2025) reviewed the Subcommittee's assessment findings. The Expert Committee endorsed the Subcommittee's conclusions without modification. *With the amendment to the "Rules on the Evaluation of New Health Technology" (Ministry of Health and Welfare Decree No. 1098, effective September 7, 2025), a new Health Technology Reassessment Expert Committee has been formed and has been operating since September 2025.
