정량적 감각기능 검사-전류인지 역치

초록
평가배경 정량적 감각기능 검사(quantitative sensory test, QST)는 말초신경염에 의한 신경감각 손상의 유무와 심한 정도를 상하지 말단 부위에서 진동감각, 온도감각, 특정 주파수 전류 등 자극에 대해 감각을 인지할 수 있는 자극강도를 정량적으로 측정하여 감각기능의 이상을 진단, 치료에 대한 반응을 관찰, 추적하는 목적으로 사용되는 검사이다. 이 중 전류인지역치는 특정 주파수(5, 250, 2,000 Hz)의 전류를 사용하여 각각의 인지 역치를 측정하는 검사이며, 신의료기술평가제도 도입 이전인 2002년 100분의 100 본인부담 항목으로 등재된 후, 2006년 비급여로 전환된 기술이다(보건복지부 고시 제2005-89호, 2005.12.22.). 동 기술은 수요조사(유관기관)를 통해 비급여 보고제도 항목으로서 대국민 정보제공의 필요성이 확인됨에 따라, 관련 선정 절차를 거쳐 최종 재평가 대상으로 선정되었다. 이에 동 기술의 임상적 효과성에 대한 정보를 제공하고자, 2025년 제8차 의료기술재평가위원회(2025.8.8.)에서 평가계획서 및 소위원회 구성안을 심의한 후 재평가를 수행하였다. 평가목적 본 평가의 목적은 ‘정량적 감각기능 검사-전류인지역치’에 대한 사용대상과 평가도구에 대한 근거를 검토하여 임상 현장에서의 적절한 사용을 지원하기 위한 정보를 제공하기 위함이다. 평가방법 정량적 감각기능 검사는 다양한 병인을 가진 이질적인 질환에서 사용되고, 중재 및 비교검사가 단계적, 상호보완적으로 활용되는 특성을 고려하여, 본 평가에서는 효과성 비교 중심의 체계적 문헌고찰 대신 임상적 맥락을 반영하기 위해 교과서 및 임상진료지침을 검토하였다. 모든 평가방법은 평가목적을 고려하여 “정량적 감각기능 검사 공동 소위원회(이하 ‘소위원회’)”의 논의를 거쳐 확정하였다. 소위원회는 신경과 2인, 내분비내과 2인, 재활의학과 1인, 총 5인의 전문가로 구성하였다. 본 평가의 핵심질문은 “정량적 감각기능 검사-전류인지역치는 말초신경병증에서 감각신경 기능을 평가하는 데 임상적으로 효과적인가?”로 설정하였다. 대상환자와 관련하여 현행 복지부 고시에서 ‘말초신경염’으로 규정되어 있으나 현재 임상에서 말초신경염과 말초신경병증을 혼용해 사용하고, 출판된 문헌에서도 대부분 말초신경병증으로 기술하고 있어 소위원회 논의를 거쳐 본 평가의 대상환자는 ‘말초신경병증’으로 설정하였다. 비교검사와 관련하여 당뇨병 말초신경병증의 진단은 현재 건강보험요양급여목록에 등재되지 않은 당뇨병 신경병증 선별검사(Michigan neuropathy screening instrument, MNSI) 자가 설문과 모노필라멘트 검사로 1차 선별을 시행한 후, 이상 소견 시 등재 급여인 신경전도검사를 표준검사로 활용하고 있다. 다만 신경전도검사는 주로 Aβ 신경섬유를 평가하여 모든 신경 손상을 단독으로 확인하기 어려우므로 중재 및 비교검사는 상호보완적으로 임상에서 활용되고 있다는 소위원회 의견에 따라 비교검사는 설정하지 않았다. 의료결과는 임상적 유용성을 확인하였다. 교과서는 신경학, 신경근육질환, 당뇨병학, 재활의학 분야를 검토하였다. 임상진료지침은 국내 임상진료지침 정보센터(Korean Medical Guideline Information Center), 국제 진료지침 네트워크(Guideline International Network), Trip Medical Database 등에서 ‘neuropathy’, ‘quantitative sensory’, ‘QST’, ‘vibration’, ‘thermal’, ‘current/current perception’, ‘electrical’, ‘pain’, ‘threshold’ 등 주요어를 조합하여 검색하였고, 검색된 임상진료지침을 활용하여 국내외 의료기술평가기관 및 전문학회에서 발간한 임상진료지침을 수기 검색하였다(2025.9.10). 이후 소위원회에서 교과서 및 임상진료지침의 적절성을 확인하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종 심의하였다. 평가결과 교과서 검토 결과 교과서 4편을 검토한 결과, 정량적 감각기능 검사는 신경전도검사로 평가하기 어려운 가느다란 유수초 또는 무수초 신경섬유(Aδ, C)를 포함한 소섬유의 기능을 객관적으로 정량화하는 검사로 제시하였다. 또한 당뇨병 신경병증이나 소섬유신경병 등에서 무증상 단계의 신경 손상 조기 발견에 유용하며, 신경전도검사의 한계를 보완하는 보조적 검사로 활용된다고 하였다. 검사 항목에는 진동감각역치(대신경섬유 평가), 온도역치(소신경섬유 평가), 전류감각역치(무수·소·대신경섬유 구분 평가) 평가가 포함되며, 신경 손상의 범위와 정도, 기능 변화를 정량적으로 평가할 수 있다. 해당 교과서들에서 전류인지역치 검사는 당뇨병 말초신경병증 초기 단계부터 진단을 할 수 있는 검사 방법으로 무수신경섬유, 소신경섬유, 큰신경섬유에 해당하는 전류인 5, 250, 2,000 Hz를 이용하여 감각과민과 지각 감퇴를 진단한다고 제시하였다. 임상진료지침 검토 결과 국내외 임상진료지침 14편을 검토한 결과, 정량적 감각기능 검사는 당뇨병 (말초)신경병증과 소섬유신경병증을 포함한 말초신경병증 환자의 감각기능 및 신경 손상 정도를 평가하기 위한 보조적 진단검사로 제시하였다. 정량적 감각기능 검사는 진동, 온도, 전류인지, 통증인지 자극에 대한 감각역치를 정량화하여 감각저하나 통각 감소 등과 같은 음성 감각 이상뿐 아니라 이질통(allodynia) 및 과민통(hyperalgesia)과 같은 양성 감각 이상을 객관적으로 평가할 수 있는 검사로 제시하였다. 다만 진동, 온도, 전류인지, 통증인지 자극과 관련된 신경섬유 유형(Aβ, Aδ, C 등)의 구체적 구분 기준은 대부분의 임상진료지침에서 명확히 제시하지 않았다. 다수의 임상진료지침에서 정량적 감각기능 검사는 검사자의 숙련도나 피검자의 주관적 반응에 따라 결과가 영향을 받을 수 있어, 단독 진단검사로 활용하기에는 한계가 있다고 지적하였다. 이에 정량적 감각기능 검사는 임상 소견과 함께 신경전도검사, 피부생검, 자율신경기능 검사 등과 병행하여 해석하는 보조적 검사로 활용할 것을 제안하였다. 또한 일부 임상진료지침에서는 정량적 감각기능 검사를 통해 감각 역치를 정량화하여 환자의 감각기능 전반의 양상과 패턴을 체계적으로 기록한 감각 프로파일을 문서화할 수 있으며, 이를 기반으로 치료 반응 및 예후 평가에 활용할 수 있다고 제시하였다. 그러나 해당 임상진료지침들에서 정량적 감각기능 검사-전류인지역치에 대하여 구체적으로 언급한 내용은 확인할 수 없었다. 결론 및 제언 의료기술재평가 소위원회에서는 현재의 평가 결과에 근거하여 다음과 같이 결과 및 의견을 제시하였다. 정량적 감각기능 검사는 교과서 및 임상진료지침에서 말초신경병증 환자의 감각기능 이상을 확인하는 검사로 제시되고 있다. 특히, 사람마다 통증을 느끼는 정도가 다르고 신경전도검사로 확인하기 어려운 작은 유수초 및 무수초 신경섬유를 측정할 수 있다는 점에서 임상적으로 보완적 가치가 있다. 그러나 환자의 주관적 증상을 객관적 수치로 측정할 수 있다는 이점에도 불구하고 개인별 통증의 역치 차이, 장비 간 측정값의 비일관성, 개별 검사들의 낮은 민감도 및 특이도, 그리고 검사자 숙련도에 따른 결과 변동 등 여러 제한점이 있다고 보았다. 이에 소위원회는 정량적 감각기능 검사가 단독으로 사용되기보다 말초신경병증 환자에서 감각신경 이상을 확인하기 위한 상호보완적 검사로 활용할 수 있다는 의견이었다. 또한, 해당 검사방법의 표준화를 위해 연령 및 성별, 인종 등 인구집단 특이적 참고치 마련과 질환별 검사의 신뢰도 및 타당도에 대한 추가연구가 필요하다고 제언하였다. 정량적 감각기능 검사 중 전류인지역치는 특정 주파수별 전류 자극에 대한 감각역치를 측정하는 검사로, 환자마다 느끼는 주관적인 증상을 객관적 측정치로 제시할 수 있으나, 단독 검사로 활용하기에는 한계가 있고 현재 해당 의료기기를 생산하지 않아 향후 사용량이 감소할 것이라는 의견이었다. 2026년 제1차 재평가전문위원회*(2026.1.9.)는 소위원회 검토 결과를 바탕으로 ‘정량적 감각기능 검사-전류인지역치’에 대해 심의하였으며, 소위원회의 결론을 원안대로 의결하였다.
Background of Assessment The quantitative sensory test (QST) is a test used to diagnose abnormalities in sensory function, observe treatment response, and follow up on the presence and severity of sensory nerve damage caused by peripheral neuropathy by quantitatively measuring the stimulus intensity at which a subject can perceive stimuli such as vibration sense, thermal sense, and electrical current of specific frequencies at the distal upper and lower extremities. Among these, the current perception threshold (CPT) test measures the perception threshold for electrical current at specific frequencies (5, 250, and 2,000 Hz). Prior to the introduction of the New Health Technology Assessment system, this technology was listed in 2002 as a 100% patient-copayment item, and was subsequently converted to a non-reimbursed item in 2006 (Ministry of Health and Welfare Notification No. 2005-89, December 22, 2005). Through a demand survey (of related institutions), the need to provide information to the public regarding this technology as an item under the non-reimbursement reporting system was confirmed, and it was selected as a final subject for reassessment through the relevant selection procedure. Accordingly, in order to provide information on the clinical effectiveness of this technology, the reassessment was conducted after the assessment plan and subcommittee composition were reviewed at the 8th Health Technology Reassessment Committee meeting of 2025 (August 8, 2025). Purpose of Assessment The purpose of this assessment is to review the evidence regarding the target population and assessment tools for the 'Quantitative Sensory Test – Current Perception Threshold' and to provide information to support its appropriate use in clinical practice. Assessment Methods Considering that quantitative sensory tests are used across heterogeneous diseases with diverse etiologies, and that interventional and comparator tests are used in a stepwise and complementary manner, this assessment reviewed textbooks and clinical practice guidelines to reflect clinical context, rather than conducting a systematic literature review centered on comparative effectiveness. All assessment methods were finalized through discussions of the "Quantitative Sensory Test Joint Subcommittee" (hereinafter the "Subcommittee"), considering the purpose of the assessment. The Subcommittee was composed of five experts: two in neurology, two in endocrinology, and one in rehabilitation medicine. The key question of this assessment was set as: "Is the Quantitative Sensory Test – Current Perception Threshold clinically effective for assessing sensory nerve function in peripheral neuropathy?" Regarding the target patient population, although the current Ministry of Health and Welfare notification specifies "peripheral neuritis," the terms peripheral neuritis and peripheral neuropathy are currently used interchangeably in clinical practice, and most published literature uses the term peripheral neuropathy. Accordingly, through Subcommittee discussion, the target patient population for this assessment was set as "peripheral neuropathy." Regarding comparator tests, the diagnosis of diabetic peripheral neuropathy currently involves primary screening using the Michigan Neuropathy Screening Instrument (MNSI) self-questionnaire and monofilament testing, which are not listed on the National Health Insurance benefit list, followed by nerve conduction studies—a reimbursed, listed test—as the standard test when abnormal findings are found. However, since nerve conduction studies primarily assess Aβ nerve fibers and are difficult to use alone to confirm all types of nerve damage, the Subcommittee's opinion was that interventional and comparator tests are used complementarily in clinical practice, and therefore no comparator test was set. Health outcomes were assessed in terms of clinical utility. Textbooks in the fields of neurology, neuromuscular disease, diabetology, and rehabilitation medicine were reviewed. Clinical practice guidelines were searched using combinations of key terms such as 'neuropathy,' 'quantitative sensory,' 'QST,' 'vibration,' 'thermal,' 'current/current perception,' 'electrical,' 'pain,' and 'threshold' in the Korean Medical Guideline Information Center, the Guideline International Network, and the Trip Medical Database, among others; clinical practice guidelines published by domestic and international health technology assessment agencies and professional societies were then manually searched using the retrieved guidelines (as of September 10, 2025). The Subcommittee subsequently confirmed the appropriateness of the textbooks and clinical practice guidelines. This assessment was finally deliberated by the Reassessment Expert Committee based on the Subcommittee's review results. Assessment Results Textbook Review Results A review of 4 textbooks indicated that the quantitative sensory test objectively quantifies the function of small nerve fibers, including thinly myelinated or unmyelinated fibers (Aδ, C), which are difficult to assess with nerve conduction studies. It was also noted that the test is useful for the early detection of nerve damage at the asymptomatic stage in conditions such as diabetic neuropathy and small fiber neuropathy, and is used as an adjunctive test to complement the limitations of nerve conduction studies. Test items include the vibration perception threshold (assessing large nerve fibers), thermal threshold (assessing small nerve fibers), and current perception threshold (differentiating unmyelinated, small, and large nerve fibers), allowing quantitative assessment of the extent and severity of nerve damage and changes in function. These textbooks indicated that the current perception threshold test may help detect sensory abnormalities associated with early diabetic peripheral neuropathy by measuring responses to electrical currents at 5, 250, and 2,000 Hz, which are conventionally considered to predominantly reflect C-, Aδ-, and Aβ-fiber function, respectively. Clinical Practice Guideline Review Results A review of 14 domestic and international clinical practice guidelines indicated that the quantitative sensory test is presented as an adjunctive diagnostic test for assessing sensory function and the extent of nerve damage in patients with peripheral neuropathy, including diabetic (peripheral) neuropathy and small fiber neuropathy. The quantitative sensory test was presented as a test that can objectively assess not only negative sensory abnormalities, such as hypoesthesia and reduced pain perception, but also positive sensory abnormalities, such as allodynia and hyperalgesia, by quantifying sensory thresholds for vibration, thermal, current perception, and pain perception stimuli. However, most clinical practice guidelines did not clearly present specific criteria for distinguishing the types of nerve fibers (Aβ, Aδ, C, etc.) associated with vibration, thermal, current perception, and pain perception stimuli. Many clinical practice guidelines pointed out that, because results of the quantitative sensory test can be influenced by the examiner's proficiency and the subject's subjective responses, there are limitations to its use as a standalone diagnostic test. Accordingly, it was suggested that the quantitative sensory test be used as an adjunctive test, interpreted together with clinical findings and other tests such as nerve conduction studies, skin biopsy, and autonomic function tests. In addition, some clinical practice guidelines presented that the quantitative sensory test can quantify sensory thresholds to systematically document a sensory profile, recording the overall pattern of a patient's sensory function, which can then be used to assess treatment response and prognosis. However, none of these clinical practice guidelines were found to specifically address the Quantitative Sensory Test – Current Perception Threshold. Conclusion and Recommendations Based on the current assessment results, the Health Technology Reassessment Subcommittee presented the following results and opinions. The quantitative sensory test is presented in textbooks and clinical practice guidelines as a test for identifying sensory function abnormalities in patients with peripheral neuropathy. In particular, given that pain perception varies among individuals and that the test can measure small myelinated and unmyelinated nerve fibers that are difficult to assess with nerve conduction studies, it has clinically complementary value. However, despite the advantage of being able to measure a patient's subjective symptoms as objective numerical values, several limitations were identified, including individual differences in pain threshold, inconsistency in measurements across devices, the low sensitivity and specificity of individual tests, and variation in results depending on the examiner's proficiency. Accordingly, the Subcommittee's opinion was that the quantitative sensory test can be used as a complementary test to identify sensory nerve abnormalities in patients with peripheral neuropathy, rather than being used alone. In addition, the Subcommittee recommended that, in order to standardize this test method, further research is needed to establish population-specific reference values by age, sex, and race, and to determine the reliability and validity of the test for each disease. Among quantitative sensory tests, the current perception threshold test measures sensory thresholds for electrical current stimuli at specific frequencies, and can present a patient's subjective symptoms as an objective measurement. However, the opinion was that it has limitations when used as a standalone test, and that its future use is expected to decline, as the relevant medical device is no longer being manufactured. The 1st Reassessment Expert Committee of 2026* (January 9, 2026) deliberated on the 'Quantitative Sensory Test – Current Perception Threshold' based on the Subcommittee's review results, and resolved to adopt the Subcommittee's conclusion as originally proposed.
