Noninvasive Cardiac Function Monitoring [Volume Clamp with Physiological Calibration]

초록

평가 배경 혈역학적 감시란 혈역학적 불안정성을 보이는 중증 환자에서 진단과 치료를 목적으로 혈역학 지표들을 지속적으로 확인하는 것을 말한다. 기본적으로는 혈압, 심박수, 호흡수 등 생체징후의 감시를 포함하며, 침습적 방법으로는 혈관 내 도관을 삽입하여 동맥압, 중심정맥압, 심박출량 등을 측정하는 방법이 사용되고 있다. 최근에는 기술의 발전에 따라, 보다 간편하고 안전하게 적용할 수 있는 비침습적인 심기능 측정기술이 개발되면서, 임상 현장에서의 활용이 점차 점차 확대되고 있다. 비침습적 심기능 측정법 [볼륨 클램프 방식 및 생리적 보정법]은 손가락에 커프를 감은 후 손가락과 심장의 높이 차이 및 생리적 특성을 보정하여 상완동맥혈압 파형으로 재현·산출된 혈압 및 심박출량 수치를 모니터링하는 검사로 2017년 신의료기술평가를 거쳐, 2020년에 선별급여(80%)로 등재되어 사용하고 있다. 동 기술은 2025년 선별급여 적합성 평가시기를 고려하여 내부 모니터링을 통해 재평가 주제로 발굴하였다. 다양한 대체 의료기술이 존재하는 현 시점에서 비교기술 대비 동 기술의 안전성과 효과성에 대한 근거를 확인하고자 2024년 제8차 의료기술재평가위원회(2024.8.9.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 심기능 측정이 지속적으로 필요한 성인 환자에서 볼륨 클램프 방식 및 생리적 보정법을 이용한 비침습적 심기능 측정법이 혈압과 심박출량을 측정하고 감시하기 위한 목적으로 사용 시 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 비침습적 심기능 측정 [볼륨 클램프 방식 및 생리적 보정법]에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “비침습적 심기능 측정 통합 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 순환기내과 3인, 호흡기내과(중환자의학) 2인, 흉부외과 2인, 마취통증의학과 2인, 근거기반의학 전문가 1인 등 총 10인으로 구성하였다. 본 평가의 대상질환은 지속적으로 비침습적 심기능 측정이 필요한 성인 환자로 하였으며, 혈압과 심박출량 감시 목적에 따라 참고표준검사는 혈압은 침습적 동맥압 혈압 측정(Invasive arterial blood pressure, IAP), 심박출량은 폐동맥 카테터에 의한 심박출량 검사(Pulmonary artery catheter-cardiac output, PAC-CO)로 정의하였으며, 비교검사의 경우 혈압은 비침습적인 혈압 측정법(arm Non-invasive blood pressure, arm NIBP), 심박출량은 (열희석법 보정 후) 동맥압에 기초한 심기능 측정, 비침습적 심기능 측정(바이오리엑턴스법, 바이오임피던스법, 이산화탄소 부분 재호흡법, 맥파전송시간 이용법), 비침습적 심기능 측정을 수행하지 않은 경우로 설정하였다. 결과변수로는 안전성과 효과성을 확인하였으며, 안전성은 검사와 관련된 이상반응 및 부작용을 지표로, 효과성은 측정정확성(참고표준검사/비교검사와의 일치도, 상관성), 의료결과에의 영향 지표로 평가하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문 데이터베이스(KMbase), 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2024. 10. 25.). 최종 선택된 연구의 비뚤림위험 평가는 연구유형에 따라 진단법 평가연구는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2), 무작위배정 임상시험 연구는 Cochrane의 Risk of Bias (RoB), 비무작위 비교연구는 Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS Ver.2)를 사용하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하였고, 질적 분석을 수행하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과, 최종 선택연구는 총 56편(연구대상자 3,592명, 연령 20-70대)으로 혈압 감시에 관한 연구 48편(진단적 코호트 연구 43편, 무작위배정 임상시험 연구 4편, 비무작위 비교연구 1편), 심박출량 감시에 관한 연구 9편(모두 진단적 코호트 연구)이었다. 연구대상자는 혈압 감시 관련 연구에서는 신경외과, 정형외과, 산과 수술 등을 포함한 비-심장수술 환자가 23편으로 다수였으며, 그 밖에도 심장수술(9편), 중증·중환자실·응급실 환자 8편, 마취 환자 2편, 혈액 투석, 뇌졸중 환자가 포함되었고, 심박출량 감시 관련 연구에서는 대부분 심장수술 환자(7편)였고, 그 외 심부전 및 중증 환자가 포함되었다. 또한, 수술 전·후, 수액주입 전·후, 중환자실 등 중재가 적용된 임상적 상황이 이질한 특성을 보였다. 혈압 관련 연구의 경우 진단법 평가연구에 대한 비뚤림위험 평가결과, 환자선택 영역에서 비뚤림위험 ‘불확실’이 74.4%이었고, 비교검사 영역에서는 ‘불확실’이 16.3%이었다. 무작위배정 임상시험 연구는 선택비뚤림 영역에서 ‘불확실’이 25%, 결과평가에 대한 눈가림 항목에서 ‘불확실’이 75%, 기타 비뚤림 영역에서 제조자 지원을 받은 연구에서 ‘높음’이 75%, 비무작위 비교연구는 교란변수 영역에서 ‘불확실’이 100%이었다. 심박출량 관련 연구에서는 진단법 평가연구에 대한 비뚤림위험 평가결과, 환자선택 영역에서 비뚤림위험 ‘불확실’이 77.8%이었다. 안전성 안전성은 총 9편(혈압 6편, 심박출량 3편)의 연구에서 다루었으며, 모든 연구에서 중재 검사와 관련된 부작용 및 이상반응은 발생하지 않은 것으로 보고하였다. 효과성 효과성은 측정 정확성(참고표준검사/비교검사와의 일치도 및 상관성), 의료결과에의 영향을 지표로 설정하여, 혈압과 심박출량 감시 목적별로 구분하여 평가하였다. 1) 혈압 측정 정확성은 참고표준검사인 침습적 동맥압 측정(IAP)과 비교검사인 비침습적 혈압측정법(arm NIBP)과의 일치도, 상관성으로 평가하였다. 일치도는 수축기 혈압, 이완기 혈압, 평균동맥압 각각의 혈압별로 두 검사방법의 측정값 간의 평균 차이와 표준편차(bias±Standard Deviation (SD)), 일치한계(Limit of Agreement, LOA), 백분율 오차(Percent Error, PE), 일치허용성 지수(Agreement Tolerability, ATI), 오차그리드 분석(검사방법 간의 차이를 임상적 위험수준으로 분류), 추세능(Trending Ability) 일치율의 다양한 지표로 제시하였다. 참고표준검사와의 일치도는 총 33편에서 보고하였다. 참고표준검사를 기준으로 중재검사와 비교검사 모두 보고한 연구는 6편으로, 참고표준검사와의 혈압 평균차이(bias) (절대값)의 경우, 수축기 혈압(5편)은 중재검사 4.3-16mmHg, 비교검사(arm NIBP) 3.5-13mmHg, 이완기 혈압(5편)은 중재검사 0-9.7mmHg, 비교검사 2-5.9mmHg, 평균동맥압(6편)은 중재검사 1-7.2mmHg, 비교검사 2-13mmHg의 범위로 보고하였다. 백분율 오차(PE)는 수축기 혈압(2편)이 중재검사 23-25%, 비교검사 22-37%. 이완기 혈압(2편)은 중재검사 26-33%, 비교검사 33-35%, 평균동맥압(3편)은 중재검사 22-27%, 비교검사 30-32%이었다. 오차그리드 분석결과, 두 검사방법 간의 차이의 임상적 위험 수준 A 비율(위험 없음)은 수축기 혈압(2편) 중재검사는 89.5-92.1%, 비교검사 85.1-92.9%, 평균동맥압(2편)은 중재검사 77.1-86.7%, 비교검사 69.5-75.4%이었다. 추세능의 일치율(3편)은 수축기 혈압이 중재검사 74-90%, 비교검사 59.8-85%, 이완기 혈압은 중재검사 77-86%, 비교검사는 60.4-89%, 평균동맥압은 중재검사 78.3-91%, 비교검사 67-90%로 보고하였다. 중재검사와 참고표준검사를 비교한 연구는 27편이었으며, 두 검사방법 간 혈압 평균차이(절대값)는 수축기 혈압(25편) 0.5-18.1mmHg, 이완기 혈압(25편) 1-24.7mmHg, 평균동맥압(27편) 0-23mmHg의 범주로 보고하였다. PE(12편)는 수축기 혈압 16.2-43.8%, 이완기 혈압 19.0-53.8%, 평균동맥압 12.0-45.1%이었다. 오차그리드 분석결과, A 비율(5편)은 수축기 혈압 76.5-96.7%, 평균동맥압 50.4-91.4%, 추세능의 일치율(6편)은 수축기 혈압 84-100%, 이완기 혈압 77.9-100%, 평균동맥압 85.5-97.3%의 범주로 보고하였다. 비교검사(arm NIBP)와의 일치도는 총 9편에서 보고하였다. 두 검사법 간의 평균차이(절대값)는 수축기 혈압(9편) 0.1-15.3mmHg, 이완기 혈압(7편) 0.45-9.4mmHg, 평균동맥압(5편) 0.68-6mmHg이었고, PE는 수축기 혈압(2편) 29-40.3%, 이완기 혈압(1편) 33%, 평균동맥압(1편) 26%, 추세능의 일치율은 수축기 혈압(3편) 60.7-81.5%, 이완기 혈압(1편) 54.3%, 평균동맥압(1편) 59.1%로 보고하였다. 참고표준검사와의 상관성은 총 17편에서 상관계수(r), 결정계수(R2)의 지표로 보고하였다. 수축기 혈압은 r=0.32-0.96, R2=0.35-0.78, 이완기 혈압은 r=0.28-0.93, R2=0.61-0.74, 평균동맥압은 r=0.53-0.96, R2=0.50-0.88이었다. 비교검사와의 상관성은 총 6편에서 보고하였다. 수축기 혈압은 r=-0.44-0.83, R2=0.3, 이완기 혈압은 r=-0.3-0.7, R2=0.4, 평균동맥압은 r=0.51-0.67, R2=0.44이었다. 의료결과에의 영향은 총 7편에서 보고하였다. 중재검사와 비교검사(arm NIBP)를 적용한 두 군 간 의료결과(저혈압 사건, 수술 중후 오심·구토, 출혈, 재원기간 등)를 보고하였다. 저혈압 에피소드 관련하여 중재군에서 더 적게 발생하거나 군 간의 차이가 없었고, 그 외 고혈압 감지 시간(2편), 수술 중 오심·구토(2편) 및 재원기간(1편)은 두 군 간 유의한 차이가 없었다. 2) 심박출량 측정 정확성은 참고표준검사인 폐동맥 카테터에 의한 심박출량 검사(PAC-CO)와의 일치도, 상관성을 평가하였다. 일치도는 두 검사방법의 측정값 간의 평균 차이와 표준편차(bias±SD), 일치한계(LOA), 백분율 오차(PE), 추세능(극좌표 도표, 4분면 도표분석) 일치율(concordant rate)의 다양한 지표로 제시하였다. 참고표준검사와의 일치도는 총 9편에서 보고하였다. 두 검사방법 간의 평균차이(절대값)는 0-0.58 L/min, 백분율 오차(PE)는 26.4-58%, 추세능의 일치율은 51.1-100%로 보고하였다. 참고표준검사와의 상관성은 총 6편에서 보고하였다. 상관계수(r)(4편) 0.54-0.89, 결정계수(R2)(2편) 0.52, 0.83이다. 의료결과에의 영향을 보고한 문헌은 없었다. 결론 및 제언 의료기술재평가 소위원회에서는 비침습적 심기능 측정 [볼륨 클램프 방식 및 생리적 보정법]의 안전성과 효과성 결과를 현재 확인된 근거를 바탕으로 다음과 같이 제언하였다. 동 기술은 검사 관련 부작용이 발생하지 않았고, 체외에서 이루어지는 비침습적인 모니터링 검사로 환자에게 직접적인 위해를 가하지 않아 안전한 기술이라는 의견이었다. 효과성은 혈압 및 심박출량 감시 목적별로 구분하였다. 1) 혈압은 참고표준검사와의 차이가 Association for the Advancement of Medical Instrumentation (AAMI) 지침에 따른 ±5mmHg 기준을 벗어난 연구들이 다수 확인되었으나, 이미 임상에서 보편적으로 사용되고 있는 비침습적 혈압측정과 비교할 때 유사한 경향을 보이고 있어 수용가능하다는 의견이었다. 또한, 비교검사보다 백분율 오차는 낮고 추세능력의 일치율은 높은 것으로 나타나 지속적으로 혈압을 비침습적으로 감시하는데 이용해 볼 수 있다는 의견이었다. 다만, 다양한 임상적 상황에 따라 측정 결과 해석 시 주의가 필요하다는 의견이었다. 2) 심박출량은 대부분의 연구에서 참고표준검사와의 차이가 일치한계 범위 내에 포함되어 있어 수용가능한 수준이긴 하지만, 백분율 오차를 보고한 연구 중 상당수에서 다수 연구들에서 정의한 허용가능한 범위인 30%를 초과한 경우가 많아 정확성에 대한 우려가 제기되었다. 다만, 침습적 방법으로 심박출량 추이를 지속적으로 확인하기 어려운 의료환경이나 환자에게는 비침습적 방법의 안전성, 편의성, 지속적인 측정가능성을 고려할 때, 심박출량 추이를 지속적으로 감시하는데 임상적으로 활용할 수 있다는 의견이었다. 또한, 심박출량의 정확성은 혈역학적 변동성의 영향을 크게 받을 수 있어 환자의 상태나 측정 환경 등에 따라 측정 결과 해석 시 주의가 필요하다는 의견이었다. 이에, 소위원회에서는 침습적인 기존의 검사법을 적용하기 어려운 성인 환자에서 혈압과 심박출량 추이를 지속적으로 감시하기 위한 비침습적인 동 검사의 효과성은 임상적으로 수용가능하다고 판단하였다. 2025년 제3차 의료기술재평가위원회(2025. 3. 14.)는 ‘비침습적 심기능 측정 [볼륨 클램프 방식 및 생리적 보정법]’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 지속적으로 혈압, 심박출량 측정 및 감시를 위해 침습적인 기존 검사법을 적용하기 어려운 성인 환자에서 혈압과 심박출량 추이를 지속적으로 감시하기 위한 목적으로 볼륨 클램프 방식 및 생리적 보정법을 이용한 비침습적 심기능 측정법의 사용을 ‘약하게 권고함’으로 심의하였다.


Background Hemodynamic monitoring is the continuous assessment of cardiovascular parameters, such as blood pressure (BP), heart rate, and respiratory gas analysis, in critically ill patients for diagnosis and therapeutics. Invasive monitoring techniques involve using intravascular catheters to measure arterial pressure, central venous pressure, and cardiac output. With technological advances, easier and safer noninvasive cardiac function monitoring methods are gaining traction in clinical practice. “Noninvasive cardiac function monitoring [Volume Clamp with Physiological Calibration]” is a technique that monitors BP and cardiac output via a finger cuff, which is calibrated for the height difference between the heart and the finger, and individual physiological characteristics to produce a brachial artery pressure waveform. This technique, assessed in the 2017 New Health Technology Assessment, has been listed as a selective benefit item (80% coinsurance) since 2020. Consequent to the selective benefit eligibility review scheduled for 2025, internal monitoring deemed the technique eligible for reassessment to evaluate its current clinical safety and effectiveness compared with those of alternative technologies. The reassessment was conducted after the eighth 2024 Health Technology Reassessment Committee (August 9, 2024) had reviewed and approved the reassessment plan and constitution of the Subcommittee. Objective To provide scientific evidence on the clinical safety and effectiveness of “Noninvasive Cardiac Function Monitoring [Volume Clamp with Physiological Calibration],” a technique for measuring and monitoring BP and cardiac output in adult patients requiring continuous cardiac function assessment, and to determine the strength of recommendation for its continued use. Method Safety and effectiveness were assessed using a systematic literature review. All assessment methods were deliberated and finalized, accordant with the objective of the assessment, by the “Joint Subcommittee on Noninvasive Cardiac Output Monitoring” (hereinafter, the “Subcommittee”). The Subcommittee comprised 10 members: three cardiologists, two critical care pulmonologists, two thoracic surgeons, two anesthesiologists, and one evidence-based medicine expert. The target population was adult patients requiring continuous noninvasive cardiac function monitoring. The reference standards were invasive arterial blood pressure (IAP) and pulmonary artery catheter (PAC-CO)-based techniques for BP and cardiac output monitoring, respectively. Comparators included arm cuff-based noninvasive BP (arm NIBP) for BP, and arterial pressure-based cardiac output monitoring (with or without thermodilution calibration), bioreactance, bioimpedance, partial CO2 rebreathing, pulse wave transit time, and no cardiac output monitoring for cardiac output monitoring. Outcome measures included safety (technique-related adverse events and complications) and effectiveness (measurement accuracy based on agreement and correlation with the reference standard or comparators, and impact on clinical outcomes). Three international (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews – Cochrane Central Register of Controlled Trials) and three Korean (KoreaMed, KMbase, and RISS) databases were searched (Final search date: October 25, 2024). Risk of bias was assessed using the following tools: Quality Assessment of Diagnostic Accuracy Studies-2 for diagnostic studies, Cochrane Risk of Bias for randomized controlled trials (RCTs), and Risk of Bias Assessment Tool for Nonrandomized Studies 2.0 for nonrandomized comparative studies (NRS). Data were extracted using a predefined form, and qualitative analysis was conducted. All processes were independently conducted by two reviewers, and any discrepancies were resolved consensually. The Health Technology Reassessment Committee proffered the final recommendation following a review based on the findings of the Subcommittee. Results Of the 56 selected studies (n = 3,592; participant age range: 20–79 years), 48 assessed BP monitoring (43 diagnostic cohort studies, four RCTs, and one NRS), and nine assessed cardiac output monitoring (all diagnostic cohort studies). For BP monitoring, patients undergoing non-cardiac surgery, including neuro, orthopedic, and obstetric surgeries, were the most common study population (23 studies). Other study populations included patients undergoing cardiac surgery (nine studies), those critically ill or in the intensive care unit (ICU) or emergency department settings (eight studies), those undergoing anesthesia (two studies), and those with stroke or undergoing hemodialysis. For cardiac output monitoring, most studies involved patients undergoing cardiac surgery (seven studies), with the remainder involving patients with heart failure or critical illness. The index technique was used several clinical settings, including pre- and postoperative periods, during fluid therapy, and in the ICU. Approximately 74.4% and 16.3% of diagnostic cohort studies assessing BP monitoring had an “unclear” risk of bias for the patient and comparator domains, respectively. Among the four RCTs, 25% and 75% had an “unclear” risk of bias for random sequence generation and blinding of outcome assessors, respectively, and 75% had a “high” risk of bias due to other sources (e.g., manufacturer sponsorship). The single NRS had an “unclear” risk of bias for confounding (100%). In the cardiac output monitoring studies, 77.8% had an “unclear” risk of bias for the patient selection domain. Safety Safety was reported in nine studies (six for BP and three for cardiac output monitoring). No technique-related adverse events or complications were reported in any of the included studies. Effectiveness 1) BP Measurement accuracy was evaluated in terms of agreement and correlation with the reference standard (IAP) and the comparator (arm NIBP). Agreement was assessed for systolic BP (SBP), diastolic BP (DBP), and mean arterial pressure (MAP) using various indicators, including the mean difference and standard deviation (bias ± SD) between both techniques, limit of agreement (LOA), percentage error (PE), agreement tolerability index, error grid analysis (classification of clinical risk levels based on between-technique differences), and trending ability (agreement rate). Agreement with IAP was reported in 33 studies. Of these, six studies reported values for both the index technique and arm NIBP relative to IAP. The mean bias (absolute value) for the index technique and arm NIBP was as follows: for SBP (five studies), 4.3–16 and 3.5–13 mmHg; for DBP (five studies), 0–9.7 and 2–5.9 mmHg; and for MAP (six studies), 1–7.2 and 2–13 mmHg, respectively. The PE for the index technique and arm NIBP was as follows: for SBP (two studies), 23–25% and 22–37%; for DBP (two studies), 26–33% and 33–35%; and for MAP (three studies), 22–27% and 30–32%, respectively. Error grid analysis revealed that the proportion of readings classified as risk level “A” (no clinical risk) for the index technique and arm NIBP was 89.5–92.1% and 85.1–92.9%, respectively, for SBP (two studies), and 77.1–86.7% and 69.5–75.4%, respectively, for MAP (two studies). Trending ability for the index technique and arm NIBP was reported in three studies: for SBP, 74–90% and 59.8–85%, respectively; for DBP, 77–86% and 60.4–89%, respectively; and for MAP, 78.3–91% and 67–90%, respectively. In total, 27 studies compared the index technique and IAP. Absolute bias was 0.5–18.1 mmHg for SBP (25 studies), 1–24.7 mmHg for DBP (25 studies), and 0–23 mmHg for MAP (27 studies). The PE (12 studies) was 16.2–43.8% for SBP, 19.0–53.8% for DBP, and 12.0–45.1% for MAP. Error grid analysis (five studies) revealed risk level “A” proportions of 76.5–96.7% for SBP and 50.4–91.4% for MAP. Trending ability agreement rates (six studies) were 84–100% for SBP, 77.9–100% for DBP, and 85.5–97.3% for MAP. Agreement with arm NIBP was reported in nine studies. Absolute bias was 0.1–15.3 mmHg for SBP (nine studies), 0.45–9.4 mmHg for DBP (seven studies), and 0.68–6 mmHg for MAP (five studies). The PE was 29–40.3% for SBP (two studies), 33% for DBP (one study), and 26% for MAP (one study). Trending ability agreement rates were 60.7–81.5% for SBP (three studies), 54.3% for DBP (one study), and 59.1% for MAP (one study). Correlation with IAP was reported in 17 studies using correlation coefficients (r) and coefficients of determination (R2): for SBP, r = 0.32–0.96 and R² = 0.35–0.78; for DBP, r = 0.28–0.93 and R² = 0.61–0.74; and for MAP, r = 0.53–0.96 and R² = 0.50–0.88. Correlation with arm NIBP was reported in six studies: for SBP, r = –0.44–0.83 and R² = 0.3; for DBP, r = –0.3–0.7 and R² = 0.4; and for MAP, r = 0.51–0.67 and R² = 0.44. Impact on clinical outcomes was reported in seven studies. Outcomes such as hypotensive events, nausea and vomiting, bleeding, and length of stay were compared between the index technique and arm NIBP groups. Fewer or comparable hypotensive events were reported in the index technique than in the arm NIBP group. No significant between-group differences were observed in time to detect hypertension (two studies), intraoperative nausea and vomiting (two studies), or length of hospital stay (one study). 2) Cardiac output Measurement accuracy was assessed in terms of agreement and correlation with the reference standard (PAC-CO). Agreement was evaluated using the mean difference and standard deviation (bias ± SD) between both techniques, LOA, PE, and trending ability concordance rate (CR) (polar and four-quadrant plot analyses). Agreement with PAC-CO was reported in nine studies. The absolute mean difference was 0–0.58 L/min, PE was 26.4–58%, and trending ability CR was 51.1–100%. Correlation with PAC-CO was reported in six studies: r = 0.54–0.89 (four studies) and R² = 0.52–0.83 (two studies). None of the studies reported the effect on clinical outcomes. Conclusion and Strength of Recommendation Based on the current evidence, the Subcommittee reviewed the clinical safety and effectiveness of “Noninvasive Cardiac Function Monitoring [Volume Clamp and Physiological Calibration]” and reported its findings. Since the technique is noninvasive, being performed externally and does not directly harm the patient, the Subcommittee deemed it clinically safe. The assessment of effectiveness was divided by monitoring purpose: BP and cardiac output. 1) BP: Despite many of the studies demonstrating differences from the reference standard that exceeded the ±5 mmHg threshold set by the Association for the Advancement of Medical Instrumentation, the Subcommittee noted that the results exhibited trends similar to other NIBP monitoring techniques already widely used in clinical practice and considered the findings acceptable. Additionally, the PE was lower and trending ability CR was higher than those in the comparator, suggesting potential clinical utility for continuous NIBP monitoring. However, the Subcommittee advised cautious interpretation depending on the clinical situation. 2) Cardiac Output: In most studies, the difference from the reference standard was within the LOA, indicating an acceptable level of accuracy. However, in several studies, the reported PE exceeded the commonly accepted threshold of 30%, oppugning measurement accuracy. The Subcommittee further noted that cardiac output monitoring accuracy can significantly be affected by hemodynamic variability; hence, it advised noting the patient’s condition and the measurement environment and cautiously interpreting the results. Nevertheless, because of its safety, convenience and ability to allow continuous assessments, the Subcommittee deemed the clinical effectiveness of the technique applicable for continuous monitoring of BP and cardiac output trends, in healthcare environments or adult patients for whom conventional invasive monitoring techniques are contraindicated. The third 2025 Health Technology Reassessment Committee (March 14, 2025 reviewed the technique, based on the clinical safety and effectiveness and other relevant factors, and concluded the following: In the current Korean clinical setting, “Noninvasive Cardiac Function Monitoring [Volume Clamp and Physiological Calibration]” is “weakly recommended” for continuous monitoring of BP and cardiac output trends in adult patients for whom invasive monitoring is difficult.
