Glaucoma Subconjunctival Stent Insertion (XEN)

초록

평가배경 ‘녹내장 결막하 스텐트 삽입술(Surgery for glaucoma-stent insertion-subconjunctival)’은 약물치료로 안압이 조절되지 않는 녹내장 환자를 대상으로 안압을 조절하기 위해 결막하 공간에 스텐트를 삽입하여 방수액을 배출시키는 방법을 사용하는 최소침습녹내장 수술이다. 2018년 제8차 신의료기술평가위원회에서 약물치료로 안압이 조절되지 않는 녹내장 환자에서 안전하고 효과적인 의료기술로 인정받아 현재 선별급여(본인 부담률 50%)로 등재된 기술이다(보건복지부 고시 제2018-220호(2018.10.2)). 동 기술은 의료기술재평가사업의 수요조사를 통해 재평가 안건으로 발굴되었다. 다양한 대체 의료기술이 존재하는 현 시점에서 비교기술 대비 동 기술의 효과성에 대한 근거를 확인하고자 2024년 제3차 의료기술재평가위원회(2024.3.8.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 약물치료로 안압이 조절되지 않는 녹내장 환자에서 안압을 조절하기 위해 결막하 공간에 스텐트를 사용하는 것이 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 권고등급을 결정하기 위함이다. 평가방법 녹내장 환자에서 결막하 스텐트 삽입술에 대한 안전성 및 효과성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “녹내장 결막하 스텐트 삽입술 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 안과 3인, 근거기반의학 2인, 총 5인으로 구성하였다. 본 평가의 대상자는 녹내장 및 고안압증 환자, 비교시술은 기존 녹내장 수술 및 다른 최소침습 녹내장 수술로 정의하였다. 결과변수로는 안전성과 효과성을 확인하였으며, 안전성은 저안압, 일시적인 안압상승, 안내염, 맥락막출혈, XEN 장치 관련 부작용, 여과포누출, 여과포복원술, 추가 녹내장 수술 및 시술, 주사침복원술 등을 결과지표로, 효과성은 평균 안압, 수술성공률, 항녹내장 약물 사용수를 결과지표로 평가하였다. 연구유형은 비교연구 이상의 연구만 평가에 포함하였다. 연구는 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews-Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS)) 데이터베이스에서 검색하였다(최종검색일 2024. 4. 29.). 최종 선택연구의 비뚤림위험 평가는 무작위배정 임상시험 연구(Randomized Clinical Trial, RCT)인 경우 Cochrane의 Risk of Bias (RoB)를, 비무작위 연구는 Risk of Bias Assessment for Nonrandomized Studies (RoBANS 2.0)를 사용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거수준을 평가하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하고, 양적 분석이 가능한 경우 메타분석을 하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과, 최종 선택연구는 총 24편으로 무작위배정 임상시험 연구 1편, 비무작위 연구 22편, 경제성 연구 1편이었다. 기존 신의료기술평가의 선택연구 중 코호트 연구 1편만이 선택배제 기준을 충족하여 본 평가에 포함되었다. 연구대상자는 녹내장 및 고안압증 환자이며 비교기술로는 섬유주절제술 17편, 슈렘관 스텐트 삽입술 2편, 비관통여과술 3편, 방수유출관삽입술 1편이 평가에 포함되었다. 연구대상자수는 안(eye) 단위로 보고하여 이를 중심으로 평가하였다. 비뚤림위험 평가결과는 무작위배정 임상시험 연구 1편에서 무작위배정순서 은폐방법, 눈가림 등의 언급이 없어 비뚤림위험이 불확실하였고, 제조사 지원으로 인한 비뚤림위험이 높았다. 비무작위 연구 22편은 대상군 비교가능성 영역에서 대부분 군 간 특성이 유사하다고 보고하여 비뚤림위험 ‘낮음’으로 평가하였고, 일부 연구에서 군 간 이질성이 존재하여 비뚤림위험 ‘높음’으로 평가하였다. 교란변수 영역은 대상자의 매칭이나 통계적 보정에 대한 언급이 없어 비뚤림위험 ‘높음’이 50%, 눈가림 영역은 언급이 없거나 후향적 자료로 인한 한계점을 언급하였으나 눈가림의 영향을 판단하기 어렵다고 보아 모든 연구에서 ‘불확실’로 평가하였다. 결과평가 영역은 사전에 결과변수를 정의하지 않고 결과에서 측정값을 보고한 경우 ‘불확실(41%)’, 선택적 결과보고 영역은 안전성 지표(합병증 등)를 보고하지 않은 경우 ‘불확실(23%)’로 평가하였다. 안전성 안전성은 핵심적인 지표로 저안압, 저안압황반병증, 안압상승, 안내염, XEN 장치 관련 부작용, 맥락막출혈, 여과포누출, 여과포섬유화, 여과포복원술, 추가 녹내장 수술 및 시술을 포함하였고, 중요하지만 핵심적이지 않은 지표로 주사침복원술, 전방출혈을 포함하여 평가하였다. 저안압을 보고한 연구는 9편으로 섬유주절제술과 비교한 1편의 RCT 연구는 섬유주절제술에서 저안압증 발생이 높았고, 비무작위 연구 6편을 메타분석한 결과는 군 간 유의한 차이가 없었다(Odds ratio (OR)=1.16, 95% Confidence interval (CI) 0.40~3.34, I2=50%). 비관통여과술 및 방수유출관삽입술과 비교한 연구는 각각 1편으로 결막하 스텐트 삽입술에서 저안압증이 유의하게 낮게 발생하였다. 저안압황반병증을 보고한 연구는 섬유주절제술과 비교한 연구 1편, 비관통여과술과 비교한 연구 1편으로 군 간 유의한 차이는 없었다. 안압상승을 보고한 연구는 3편으로 섬유주절제술과 비교한 1편의 RCT 연구는 결막하 스텐트 삽입술에서 10mmHg 이상 안압상승이 더 많이 발생하였다. 슈렘관 스텐트 삽입술 및 비관통여과술과 비교한 각각 1편의 연구에서 결막하 스텐트 삽입술이 다소 높게 발생하였으나 두 군 사이 유의한 차이는 없었다. 안내염은 섬유주절제술과 비교한 연구 2편 중 1편은 섬유주절제술에서 1건 발생하였고, 다른 1편의 연구는 두 군 모두 발생하지 않았다. 감염(infection)은 안내염이나 여과포 관련 감염을 포함하여 섬유주절제술과 비교한 2편의 연구에서 두 군 모두 발생하지 않았다. 염증(inflammation)은 섬유주절제술과 비교한 2편의 연구에서 결막하 스텐트 삽입술은 0~2.1%, 섬유주절제술은 1.1~3.9% 발생하였으나 군 간 유의한 차이는 없었다. XEN 장치와 관련된 부작용을 보고한 연구는 8편으로 압출(extrusion), 노출(exposure), 재배치(reposition), 제거(removal) 등이 있었으며 1.1~14.3%의 분포를 보였다. 맥락막 출혈을 보고한 연구는 없었다. 여과포누출을 보고한 연구는 4편으로 섬유주절제술과 비교한 1편의 RCT 연구는 섬유주절제술에서만 15.9% 발생하였고, 2편의 비무작위 연구를 메타분석한 결과는 결막하 스텐트 삽입술에서 유의하게 낮았다(OR=0.13, 95% CI 0.11~0.16, I2=0%). 비관통여과술과 비교한 1편의 연구에서는 초기 발생은 결막하 스텐트 삽입술에서 유의하게 낮았으나 후기 발생은 두 군 사이 유의한 차이가 없었다. 여과포섬유화는 섬유주절제술과 비교한 연구 3편 중 1편의 RCT 연구에서 결막하 스텐트 삽입술에서만 4.2% 발생하였고, 2편의 비무작위 연구를 메타분석한 결과는 두 군 간 유의한 차이가 없었다(OR=5.86, 95% CI 0.42~81.29, I2=0%). 여과포복원술을 보고한 연구는 7편으로 섬유주절제술과 비교한 1편의 RCT 연구는 결막하 스텐트 삽입술에서 5.3%, 섬유주절제술에서 4.5% 발생하였고, 4편의 비무작위 연구를 메타분석한 결과는 두 군 사이 유의한 차이가 없었다(OR=0.59, 95% CI 0.20~1.74, I2=13%). 비관통여과술 및 방수유출관삽입술과 비교한 각각 1편의 연구는 두 군 사이 유의한 차이가 없었다. 추가 녹내장 수술 및 시술을 보고한 연구는 10편으로 섬유주절제술과 비교한 1편의 RCT 연구는 결막하 스텐트 삽입술이 7.4%로 섬유주절제술에 비해 높게 발생하였고, 비무작위 연구 6편을 메타분석한 결과는 결막하 스텐트 삽입술이 섬유주절제술에 비해 추가 녹내장 수술 및 시술이 더 발생하였다(OR=2.57, 95% CI 1.38~4.76, I2=0%). 비관통여과술과 비교한 3편의 연구는 결막하 스텐트 삽입술에서 추가 녹내장 수술 및 시술이 더 발생하였다(OR=2.01, 95% CI 1.14~3.56, I2=0%). 주사침복원술을 보고한 연구는 14편으로 섬유주절제술과 비교한 1편의 RCT 연구에서 결막하 스텐트 삽입술이 23.2%, 섬유주절제술이 18.2% 발생하였고, 10편의 비무작위 연구를 메타분석한 결과는 결막하 스텐트 삽입술이 섬유주절제술에 비해 주사침복원술이 더 발생하였으나 이질성이 높아 결과해석에 주의가 필요하였다(OR=2.70, 95% CI 1.19~6.13, I2=81%). 비관통여과술 및 방수유출관삽입술과 비교한 각각 1편의 연구에서 두 군 사이 유의한 차이는 없었다. 전방출혈을 보고한 연구는 16편으로 섬유주절제술과 비교한 1편의 RCT 연구는 결막하 스텐트 삽입술에서 1.1~6.3%, 섬유주절제술에서 4.5~6.8% 발생하였고, 비무작위 연구 11편을 메타분석한 결과는 군 간 유의한 차이가 없었으나 이질성이 높아 결과해석에 주의가 필요하였다(OR=0.78, 95% CI 0.27~2.30, I2=67%). 슈렘관 스텐트 삽입술과 방수유출관삽입술과 비교한 연구는 각각 1편으로 군 간 유의한 차이는 없었다. 비관통여과술과 비교한 2편의 연구는 결막하 스텐트 삽입술에서 전방출혈이 유의하게 높게 발생하였다. 이 외에 얕은/편평한 전방, 맥락막박리는 비교시술과 유의한 차이가 없었으며, 섬유주절제술과 비교한 1~2편의 연구는 결막하 스텐트 삽입술에서 맥락막삼출, 결막버튼홀, 녹내장 진행 및 악화, 시력감소, 망막박리 등이 더 적게 발생하였다. 효과성 효과성은 핵심적인 지표로 평균 안압, 수술성공률(완전 및 부분성공률), 항녹내장 약물 사용수를 포함하여 평가하였다. 평균 안압을 보고한 연구는 22편으로 섬유주절제술과 비교한 1편의 RCT 연구에서 결막하 스텐트 삽입술의 평균 안압이 섬유주절제술에 비해 높았고 비무작위 연구 8편을 메타분석한 결과, 결막하 스텐트 삽입술의 평균 안압이 더 높았으나 이질성이 높아 결과해석에 주의가 필요하였다(Mean difference (MD)=1.84mmHg, 95% CI 0.89~2.78, I2=59%). 슈렘관 스텐트 삽입술과 비교한 2편과 방수유출관삽입술과 비교한 1편의 연구는 군 간 유의한 차이가 없었다. 비관통여과술과 비교한 3편 중 2편의 연구는 결막하 스텐트 삽입술에서 평균 안압이 높았고, 1편의 연구는 결막하 스텐트 삽입술에서 안압감소량이 더 크게 나타난 경향이 있었다. 완전성공률은 추가 약물 사용 없이 목표 안압을 유지하는 경우이거나 수술 전에 비해 20% 이상 안압 감소가 있는 경우로 정의하였으며, 여러 목표 안압을 제시한 경우 18mmHg 값을 기준으로 메타분석을 수행하였다. 완전성공률을 보고한 연구는 18편으로 섬유주절제술과 비교한 1편의 RCT 연구에서 결막하 스텐트 삽입술의 완전성공률이 44.2%로 섬유주절제술 59.1%에 비해 낮았고, 비무작위 연구 10편을 메타분석한 결과는 결막하 스텐트 삽입술의 완전성공률이 더 낮았으나 이질성이 높아 결과해석에 주의가 필요하였다(OR=0.49, 95% CI 0.30~0.80, I2=53%). 슈렘관 스텐트 삽입술과 비교한 1편은 결막하 스텐트 삽입술의 완전성공률이 높았고, 비관통여과술과 비교한 3편은 두 군 간 유의한 차이가 없었다. 방수유출관삽입술과 비교한 1편의 연구는 결막하 스텐트 삽입술의 완전성공률이 더 낮은 경향이 있었다. 부분성공률은 추가적인 약물이나 치료가 발생하거나 발생하지 않은 경우를 모두 포함하면서 목표 안압을 유지하는 경우 및 수술 전에 비해 20% 이상 안압 감소가 있는 경우로 정의하였으며, 여러 목표 안압을 제시한 경우 18mmHg 값을 기준으로 메타분석을 수행하였다. 부분성공률을 보고한 연구는 16편으로 섬유주절제술과 비교한 1편의 RCT 연구에서 결막하 스텐트 삽입술의 부분성공률이 62.1%로 섬유주절제술 72.7%에 비해 낮았고, 비무작위 연구 8편을 메타분석한 결과, 결막하 스텐트 삽입술의 부분성공률이 더 낮은 경향이 있었으나 통계적 유의성은 없었고 이질성이 높아 결과해석에 주의가 필요하였다(OR=0.53, 95% CI 0.25~1.17, I2=65%). 슈렘관 스텐트 삽입술과 비교한 1편, 비관통여과술과 비교한 3편, 방수유출관삽입술과 비교한 1편의 연구에서는 두 군 간 유의한 차이가 없었다. 항녹내장 약물 사용수를 보고한 연구는 21편으로 섬유주절제술과 비교한 1편의 RCT 연구에서 결막하 스텐트 삽입술의 항녹내장 약물 사용수가 섬유주절제술에 비해 다소 많았고, 비무작위 연구 10편을 메타분석한 결과는 두 군 간 유의한 차이가 없었으나 이질성이 높아 결과해석에 주의가 필요하였다(MD=0.07개, 95% CI -0.06~0.21, I2=59%). 슈렘관 스텐트 삽입술과 비교한 2편과 비관통여과술과 비교한 3편의 연구는 두 군 간 유의한 차이가 없었다. 방수유출관삽입술과 비교한 1편의 연구는 결막하 스텐트 삽입술에서 항녹내장 약물 사용수가 많은 경향이 있었다. 경제성 동 기술과 관련하여 비용-효과성을 보고한 연구는 없었고, 재정 부담을 보고한 1편의 연구가 있었다. 결막하 스텐트 삽입술을 포함한 경우와 그렇지 않은 경우에 대해 각 시나리오별 녹내장 환자의 중증도에 따라 분류하였으며, 백내장이 있는 경증 녹내장 환자 3~5%, 백내장이 있는 중등도 녹내장 환자 30~40%, 백내장이 없는 녹내장 환자 30~40% 비율로 가정하여 비용을 분석한 결과, 결막하 스텐트 삽입술을 포함한 시나리오는 그렇지 않은 경우에 비해 1년간 환자당 평균 비용이 465.24유로, 2~3년 사이 618.82유로를 절감하는 효과가 있었다. 결론 및 권고결정 의료기술재평가 소위원회에서는 현재 확인된 근거를 바탕으로 녹내장 및 고안압증 환자에서 결막하 스텐트 삽입술의 안전성과 효과성, 경제성 결과를 다음과 같이 제시하였다. 체계적 문헌고찰 결과, 결막하 스텐트 삽입술의 핵심적인 지표들에 대한 근거 수준이 ‘매우 낮음’ 또는 ‘낮음’이었지만, 맥락막 출혈, 안내염 등의 심각한 합병증이 발생하지 않았고 여과포누출 발생이 적어 안전한 기술로 판단하였다. 효과성 측면에서는 기존 섬유주절제술에 비해 안압을 낮추는 효과가 다소 떨어지나 수술 후 평균 안압이 12~20mmHg 정도이고 항녹내장 약물 사용량도 감소하였으며 실제 임상 현장에서 약물을 사용하면서 안압이 20% 이상 감소하거나 목표안압을 유지하는 지표인 부분성공률이 섬유주절제술과 유의한 차이가 없다는 점에서 약물치료로 안압 조절이 되지 않는 녹내장 및 고안압증 환자를 대상으로 안압을 조절하는데 효과가 있는 기술로 평가하였다. 또한, 임상적으로 섬유주절제술에 비해 결막하 스텐트 삽입술 후 환자가 경험하는 통증이 적고, 일상회복이 빠르며 수술시간이 짧은 점, 기존 섬유주절제술은 술자에 따라 수술의 효과에 상당한 영향을 끼칠 수 있으나 결막하 스텐트 삽입술은 비교적 수술이 용이하며 동일한 제품을 사용함으로써 술기가 표준화되어 술자에 따른 수술효과 차이가 크지 않은 이점이 있는 기술이라는 소위원회의 의견이 제시되었다. 다른 녹내장 수술인 슈렘관 스텐트 삽입술(iStent), 비관통여과술, 방수유출관삽입술(Ex-PRESS)과 비교시 유사한 수준의 안전성과 효과성을 보이지만 연구 수가 부족하여 근거가 불충분할 수 있다고 평가하였으며, 1편의 연구에서 재정부담을 보고하였으나 국내 임상상황에서 경제성을 평가하기는 어렵다고 판단하였다. 2024년 제10차 의료기술재평가위원회(2024. 10. 11.)는 ‘녹내장 결막하 스텐트 삽입술’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 결막하 스텐트 삽입술은 현재 (선별)급여 내 다른 수술과 비교 시 치료대안 중 하나로 수술의 표준화와 술자에 따른 편차 감소, 이로 인한 수술 접근성 향상 등의 이점이 있으나, 기존 섬유주절제술 및 다른 비교기술에 비해 효과성이 우월하지 않고 근거수준이 높지 않은 점의 고려가 필요하다고 판단하였다. 이에 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 등을 종합적으로 고려하였을 때, 국내 임상상황에서 약물치료로 안압 조절이 되지 않는 녹내장 및 고안압증 환자를 대상으로 안압을 조절하기 위해 결막하 스텐트 삽입술의 사용을 ‘약하게 권고함’으로 결정하였다.


Background The XEN gel stent is an ab interno surgical device designed to lower intraocular pressure (IOP) by facilitating the drainage of aqueous humor from the anterior chamber into the subconjunctival space. In 2018, during the 8th New Health Technology Assessment Committee meeting, subconjunctival stent insertion using the XEN device was recognized as a safe and effective treatment option for glaucoma patients who have difficulty using anti-glaucoma medications. Currently, this procedure is classified as a selective benefit, with patients responsible for 50% of the treatment cost (Ministry of Health and Welfare Notice No. 2018-220, October 2, 2018). This technique was identified for reevaluation through a demand survey conducted as part of the Health Technology Reassessment (HTR) Program. In consideration of the availability of alternative health technologies, the reassessment aimed to evaluate the evidence supporting the clinical effectiveness of this procedure in comparison with existing alternatives. This health technology was selected as a health technology reassessment agenda during the 3rd Health Technology Reassessment Committee in 2024 (March 8, 2024) and performed accordingly. Objective The objective of this assessment is to evaluate the clinical safety and effectiveness of XEN for reducing IOP in patients with glaucoma, and to determine an appropriate recommendation grade for its use. Method A systematic literature review was conducted to evaluate the safety and effectiveness of XEN in patients with glaucoma. All assessment methods were finalized by the "Subcommittee on Glaucoma Subconjunctival Stent Insertion (XEN)" (“Subcommittee”) after deliberation aligned with the assessment objectives. The Subcommittee comprised five experts: three in ophthalmology and two in evidence-based medicine. The key question in the assessment was, “Is XEN clinically safe and effective in patients with glaucoma?”. Safety indicators included hypotony, transient elevation of IOP, endophthalmitis, choroidal hemorrhage, XEN device-related complications, bleb leakage, bleb revision, additional glaucoma surgeries or procedures, and needling. Effectiveness indicators included the mean IOP, complete or partial success rate, and the number of anti-glaucoma medications required. Moreover, economic indicators included cost-effectiveness or cost-savings. For the systematic review, three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews–Cochrane Central Register of Controlled Trials) and three domestic databases (KoreaMed, KMbase, and RISS) were searched, with the final search conducted on April 29, 2024. The risk of bias in the final selected studies was assessed using the Cochrane Risk of Bias (RoB) tool for randomized controlled trials (RCTs) and the Risk of Bias Assessment Tool for Non-randomized Studies (RoBANS 2.0). The level of evidence for the systematic review was evaluated using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach. Data were extracted using a predefined data extraction form, and meta-analysis was conducted when quantitative synthesis was feasible. All data were extracted at the study level. Any discrepancies were resolved through discussion with a third reviewer to reach a consensus. Based on the Subcommittee's review, the final recommendation grade was determined by the Health Technology Reassessment Committee. Results The systematic literature review identified a total of 24 studies, including one RCT, 22 non-randomized studies (NRS), and one economic evaluation. Among the studies included in previous new health technology assessment, only one cohort study met the inclusion criteria for this evaluation. In the single RCT, the risk of bias was unclear due to the lack of information on random sequence generation, allocation concealment, and blinding. Additionally, a high risk of bias was identified due to manufacturer sponsorship. Among 22 NRS, most were assessed as having a low risk of bias in the domain of comparability of the target group, as they reported similar baseline characteristics across groups. In the confounders domain, 50% of the studies were rated as high risk of bias due to the absence matching or statistical adjustment for confounding variables. For the blinding domain, all studies were rated as unclear because blinding was either not mentioned or limited by the retrospective nature of the data, making the influence of blinding difficult to assess. In the outcome assessment domain, 41% of the studies were rated as unclear due to a lack of predefined outcome measures. In the domain of selective outcome reporting, 23% of the studies were rated as unclear due to the omission of safety indicators, such as complications. Safety Nine studies reported hypotony. In one RCT comparing XEN with trabeculectomy, the incidence of hypotony was higher in the trabeculectomy. A meta-analysis of six NRS found no significant difference between the groups (odds ratio [OR]=1.16, 95% confidence interval [CI]: 0.40–3.34; I²=50%). One study compared XEN to non-penetrating deep sclerectomy (NPDS), and another to EX-PRESS; in both studies, the incidence of hypotony was significantly lower with XEN. Three studies reported IOP elevation. In one RCT comparing XEN with trabeculectomy, XEN had a higher incidence of IOP elevation ≥10 mmHg. Two other studies comparing XEN with iStent and NPDS also showed slightly higher incidence in the XEN group, though these differences were not statistically significant. In studies comparing XEN with trabeculectomy, endophthalmitis, infection, and inflammation were either absent or more frequent in the trabeculectomy, with no statistically significant differences between groups. Eight studies reported XEN device-related complications, such as extrusion, exposure, repositioning, and removal, with a prevalence ranging from 1.1% to 14.3%. No studies reported choroidal hemorrhage. Four studies reported bleb leaks. In one RCT, trabeculectomy resulted in a 15.9% bleb leaks. A meta-analysis of two NRS showed significantly fewer leaks with XEN (OR=0.13, 95% CI 0.11–0.16, I²=0%). Compared to NPDS, XEN was associated with fewer early bleb leaks, with no significant difference in late leaks. Three studies reported bleb fibrosis between XEN and trabeculectomy. In one RCT, bleb fibrosis occurred in 4.2% of the XEN group. A meta-analysis of two NRS found no significant difference between the two groups (OR=5.86, 95% CI 0.42–81.29, I²=0%). Seven studies reported bleb revision. In one RCT comparing XEN with trabeculectomy, bleb revision rates were 5.3% and 4.5%, respectively. A meta-analysis of four NRS showed no significant difference between the two groups (OR=0.59, 95% CI 0.20–1.74, I²=13%). No significant differences were in two other studies comparing XEN with NPDS and EX-PRESS. Ten studies reported additional glaucoma surgeries or procedures. In one RCT, the XEN group showed a higher incidence (7.4%) compared to trabeculectomy. A meta-analysis of six NRS also found significantly higher rates with XEN (OR=2.57, 95% CI 1.38–4.76, I²=0%). Similarly, three studies comparing XEN with NPDS reported more frequent additional glaucoma surgeries in the XEN group (OR=2.01, 95% CI 1.14–3.56, I²=0%). Fourteen studies reported needling. In one RCT comparing XEN with trabeculectomy, needling rates were 23.2% and 18.2%, respectively. A meta-analysis of ten NRS found more needle revisions with XEN than with trabeculectomy (OR=2.70, 95% CI 1.19–6.13, I²=81%), though high heterogeneity limits interpretation. Two additional studies comparing XEN with NPDS and EX-PRESS showed no significant differences. Effectiveness A total of 22 studies reported mean IOP. In one RCT comparing XEN with trabeculectomy, the mean IOP was higher in the XEN group. A meta-analysis of eight NRS showed higher mean IOP with XEN (mean difference [MD]=1.84 mmHg, 95% CI 0.89–2.78, I²=59%), though high heterogeneity limits interpretation. Two studies comparing XEN with iStent and one comparing it with EX-PRESS found no significant differences. Among three studies comparing XEN with NPDS, two reported higher mean IOP in the XEN group, while one showed a greater IOP reduction with XEN. Complete success was defined as achieving target IOP without additional medications or a reduction of >20% IOP from baseline. For studies reporting multiple target pressures, 18 mmHg was used as the threshold in the meta-analysis. Eighteen studies reported complete success rates. In one RCT comparing XEN with trabeculectomy, the complete success rate was lower with XEN (44.2%) than with trabeculectomy (59.1%). A meta-analysis of ten NRS found a lower complete success rate with XEN (OR=0.49, 95% CI 0.30–0.80, I²=53%), though high heterogeneity limits interpretation. One study comparing XEN with iStent reported a higher success rate with XEN. Three studies comparing XEN with NPDS found no significant difference. One study comparing XEN with EX-PRESS showed a slightly lower complete success rate with XEN. Qualified success was defined as achieving target IOP or achieving a >20% reduction from baseline, regardless of additional medications or treatments. When multiple target pressures were reported, 18 mmHg was used as the threshold in the meta-analysis. Sixteen studies reported qualified success rate. In one RCT comparing XEN with trabeculectomy, the qualified success rate was lower in the XEN group (62.1%) than in the trabeculectomy group (72.7%). A meta-analysis of eight NRS suggested a trend toward lower success rate with XEN, though it was not statistically significant and heterogeneity was high (OR=0.53, 95% CI 0.25–1.17, I²=65%). Studies comparing XEN and iStent, NPDS, and EX-PRESS reported no significant differences. Twenty-one studies reported the use of anti-glaucoma medications. In one RCT comparing XEN with trabeculectomy, the number of anti-glaucoma medications was slightly higher in the XEN group. A meta-analysis of ten NRS found no significant difference in medication (MD=0.07 medications, 95% CI -0.06 to 0.21, I²=59%), though high heterogeneity limits interpretation. Two studies comparing XEN with iStent and three comparing it with NPDS reported no significant differences. One study comparing XEN with EX-PRESS showed higher medication use in the XEN group. Cost-effectiveness One study assessed the budget impact of XEN through a cost analysis stratified by glaucoma severity. Assuming distributions of 3–5% for mild glaucoma with cataracts and 30–40% for moderate glaucoma with or without cataracts, XEN resulted in cost savings of €465.24 per patient at one year and €618.82 over two to three years, compared to scenarios without XEN. Conclusion and Recommendation Despite the overall “low” to “very low” certainty of evidence, XEN was considered safe, with no serious complications reported and a low incidence of bleb leakage. Although slightly less effective than trabeculectomy in lowering IOP, XEN achieved a postoperative mean of 12–20 mmHg and reduced the need for medications. Its comparable qualified success rate supports its efficacy in patients with uncontrolled glaucoma or ocular hypertension. The Subcommittee noted that XEN involved less postoperative pain, faster recovery, and shorter surgical time than trabeculectomy. Unlike trabeculectomy, which is highly dependent on surgical skill, XEN offers a standardized technique with reduced variability. Compared to other procedures—such as iStent, NPDS, and Ex-PRESS—XEN showed comparable safety and effectiveness. However, the Subcommittee acknowledged that the limited number of studies may result in insufficient evidence. Although one study addressed the budget impact of XEN, evaluating its cost-effectiveness within the context of domestic clinical practice remains challenging. The 10th Health Technology Reassessment Committee reviewed the findings based on the Subcommittee’s assessment on October 11, 2024. The Health Technology Reassessment Committee concluded that although subconjunctival stent insertion (XEN) offers advantages such as procedural standardization, reduced surgeon variability, and improved accessibility, it does not demonstrate superior effectiveness over conventional trabeculectomy or other glaucoma surgeries. The Committee also noted the need to account for the limited quality of supporting evidence. The Committee determined a weak recommendation for the use of XEN to control IOP in patients with glaucoma or ocular hypertension uncontrolled by medications, based on clinical settings and values in Korea (recommendation level: weak recommendation).
