초록

평가배경 ‘비스쿰 알붐(Viscum album)’, ‘이뮤노시아닌(Immunocyanin)’, ‘싸이모신 알파 1(Thymosin α1)’은 암 환자 및 면역기능 저하자를 대상으로 종양 치료, 항암 보조요법, 암 재발 예방, 면역기능 보조요법 등의 목적으로 사용되고 있는 비급여 주사제이다. ‘비급여 상세내역조사’ 자료에 따르면, 해당 약제들은 종양용약 중에서도 진료비 규모가 큰 항목에 속한다. 이들 기술은 ‘건강보험 비급여 관리강화 종합대책’ 수립(’20.12.31.)에 따라, 적정한 의료서비스 제공및 합리적 의료이용을 지원하기 위해 유관기관으로부터 의료기술재평가를 요청받았다. 2024년 제4차 의료기술재평가위원회(2024.4.12.)에서 의료기술재평가 계획서 및 소위원회 구성안을 심의하고, 임상적 안전성과 유효성에 대한 전문적이고 심층적인 검토를 통해 재평가를 수행하였다. 본 평가의 목적은 비스쿰 알붐(Viscum album), 이뮤노시아닌(Immunocyanin) 및 싸이모신 알파 1(Thymosin α1)의 임상적 안전성과 유효성에 대한 객관적인 근거를 도출하고, 해당 기술에 대한 의료기술재평가 권고등급을 제시하는 데 있다. 본 보고서에는 그 중 싸이모신 알파 1의 안전성 및 유효성 평가 내용이 포함되어 있다. 평가방법 싸이모신 알파 1의 안전성 및 유효성을 평가하기 위해 체계적 문헌고찰(Systematic Review)을 수행하였다. 세부 평가 방법 및 결과는 「비스쿰 알붐ㆍ이뮤노시아닌ㆍ싸이모신 알파 1 약물의 안전성 및 유효성 평가 통합 소위원회」(이하 ‘소위원회’)의 검토와 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 심의한 평가계획에 따라 혈액종양내과 2인, 가정의학과 2인, 근거기반의학 전문가 2인으로 구성되어 총 6인이 참여하였다. 싸이모신 알파 1은 국가별로 사용 현황 및 허가 적응증에 차이가 있다. 소위원회는 국내 허가 적응증이 ‘백신 접종 시 보조요법’에 해당하며, 실제 임상에서는 암 환자를 대상으로 종양 치료 보조제로도 빈번히 사용되고 있다는 점을 고려하여, 평가 대상자를 ‘백신 투여 환자’ 및 ‘암 환자’로 설정하였다. 안전성 평가는 심각한 이상반응 및 부작용, 일반적인 이상반응 및 부작용을 주요 지표로 설정하였다. 유효성 평가는 두 가지로 구분하여 진행하였다. 백신 보조요법의 경우 항체반응률과 면역반응을, 항암 관련 유효성은 암 환자에서 싸이모신 알파 1을 추가 투여했을 때의 생존율, 재발률, 질병 진행, 항암 반응률, 삶의 질을 결과 지표로 설정하였다. 해당 약물이 주로 암 환자에게 사용된다는 점을 고려하여, 본 평가는 선택 비뚤림 및 교란 요인 등을 최소화할 수 있는 연구설계인 무작위배정 비교임상시험(Randomized Controlled Trials, RCTs) 연구로 한정하여 문헌을 선정하였다. 문헌 검색은 핵심질문을 기반으로 국외 3개 데이터베이스(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials)와 국내 3개 데이터베이스(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS))에서 수행되었다(최종검색일 2024. 7. 25.). 최종 선정된 연구에 대해서는 ‘비뚤림 위험(Risk of Bias, RoB)’ 평가 도구를 활용하여 평가를 수행하였고, 자료 추출은 사전에 정의한 양식에 따라 시행하였다. 양적 분석이 가능한 경우에는 메타분석을 실시하였다. 모든 과정은 2인의 평가자가 독립적으로 수행하였으며, 의견 불일치 시 합의를 통해 최종 결과를 도출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거 수준을 평가하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회의 심의를 거쳐 의료기술재평가 권고등급이 결정되었다. 평가결과 체계적 문헌고찰 결과 최종 선정된 연구는 무작위배정 비교임상시험(RCT) 총 7편이었다. 이 중 백신 보조요법 관련 연구는 2편(총 206명: 싸이모신 알파 1 중재군 127명, 비교군 79명), 암 환자 대상 연구는 5편(총 1,003명: 싸이모신 알파 1 중재군 641명, 비교군 361명)이 포함되었다. 백신 보조요법 연구는 각각 1989년과 2012년에 발표된 연구로, 연구대상자가 66세 이상의 고령자 및 신장 투석 중인 말기 신부전(end-stage renal disease, ESRD) 환자였다. 암 환자 대상 연구의 연도별 분포는 2000년 이전 1편, 2000~2010년 사이 3편, 2010년 이후 1편(2023년)이었다. 대상자 암종은 간세포암 관련 연구 2편, 비소세포폐암, 전이성 흑색종, 대장암 관련 연구가 각각 1편씩이었다. 비뚤림위험(Risk of Bias) 평가는 각 항목별 기준에 따라 수행하였다. 먼저, 무작위배정 순서 생성과 순서 은폐 방법에 대한 기술이 없는 경우에는 관련 정보의 부족으로 인해 비뚤림위험을 ‘불확실’로 평가하였다. 눈가림 항목의 경우, 생존율이나 무질병 생존율 등 객관적인 지표를 결과로 제시한 연구는 눈가림의 영향을 받지 않는다고 판단하여 비뚤림위험을 ‘낮음’으로 평가하였다. 반면, 삶의 질과 같은 주관적 지표를 활용한 연구는 ‘불확실’로 평가하였다. 불충분한 결과 자료 항목에서는 추적 관찰 손실이 20% 이상 발생한 경우 ‘불확실’로 평가하였다. 선택적 보고에 대해서는 안전성 또는 유효성 지표 중 하나만 보고한 경우 ‘불확실’로 평가하였다. 기타 비뚤림위험 요소와 관련해서는, 연구 수행에 있어 약물을 제조사로부터 제공받았거나 연구비를 지원받은 경우는 비뚤림위험 ‘높음’으로 평가하였다. 안전성 안전성은 백신 보조요법 연구와 암 환자 대상 연구를 구분하지 않고 통합하여 평가하였다. 이상반응은 심각한 이상반응과 일반적인 이상반응으로 나누어 분석하였다. 심각한 이상반응은 총 3편의 연구에서 보고되었으며, 싸이모신 알파 1 중재군의 발생률은 2~50%, 비교군은 11~64%로 나타났다. 그러나 이들 모두 통계적 유의성에 대한 분석 결과는 제시하지 않았다. 사망 및 이상반응으로 인한 투약 중단은 각각 2편의 연구에서 보고되었으며, 사망률은 싸이모신 알파 1군에서 3.3~21.0%, 비교군에서 8.6~45.0% 발생하였다. 일반적인 이상반응은 4편의 연구에서 보고되었다. 이 중 3편에서는 싸이모신 알파 1 중재군의 이상반응 발생률이 3.5~93.0%, 비교군은 9.5~91%로 나타났고, 나머지 1편에서는 중재군 41~66%, 비교군 62%로 치료 관련 이상반응이 보고되었다. 전반적으로 싸이모신 알파 1군이 비교군에 비해 이상반응 발생률이 낮거나 유사한 경향을 보였다. 그 외 Maio 등(2010)에서 독감 유사 증상, 오한, 투여 부위의 국소 부종 및 통증, 피로 등의 발생이 두 군 간 유의한 차이가 없다고 보고하였고, Gish 등(2009)은 투여 부위의 국소 열감과 경증의 근육 손실이 있었으나 대부분 경미하고 일시적인 이상반응으로 나타났다고 보고하였다. 유효성 1. 백신 보조요법 관련 유효성 싸이모신 알파 1의 백신 보조요법 유효성을 평가한 연구는 2편이었다. Carraro 등(2012)은 싸이모신 알파 1을 3.2 mg 및 6.4 mg 용량으로 백신과 병용 투여한 결과를 보고하였다. 3.2 mg 투여군에서는 초기 시점(21일, 42일)에서 비교군보다 항체 역가가 높았으나, 후기 시점(84일, 168일)에서는 항체 증가율이 비교군보다 낮아지는 경향을 보였다. 6.4 mg 투여군은 모든 시점에서 비교군보가 높은 역가를 보였으나, 두 경우 모두 통계적 유의성은 보고되지 않았다. Gravenstein 등(1989)은 싸이모신 알파 1 투여 후 6주 시점에서 중재군의 항체 역가가 비교군보다 높게 나타나는 경향을 보였다고 보고하였다. 2. 항암 보조요법 관련 유효성 항암치료에 싸이모신 알파 1을 추가 투여한 암 환자를 대상으로 한 연구는 총 5편으로, 주요 평가지표는 생존율, 생존기간, 재발 및 질병 진행, 항암반응률이었다. 생존율은 3편의 연구에서 보고되었다. Schulof 등(1985)은 싸이모신 알파 1 중재군의 1년 생존율이 비교군보다 유의하게 높았다고 보고하였다(p=0.009). Gish 등(2009)은 싸이모신 알파 1 중재군의 1년 생존율이 66%, 비교군은 61%였으나 통계적 유의성은 보고되지 않았다. Maio 등(2010) 연구에서는 생존율과 관련하여 두 군 간 유의한 차이가 없었으며 위험비(Hazard Ratio, HR) 또한 통계적으로 유의하지 않았다. 생존기간은 3편의 연구에서 보고되었다. Gish 등(2009)과 Maio 등(2010)은 군 간 중앙 생존기간에서 유의한 차이를 보고하지 않았으며, Shuqun 등(2004) 연구에서는 싸이모신 알파 1 중재군의 중앙 생존기간이 10개월, 비교군은 7개월로 나타났으나 통계적 유의성은 제시되지 않았다. 재발 및 질병 진행과 관련된 평가는 5편의 연구에서 보고되었다. Schulof 등(1985)은 싸이모신 알파 1군의 무재발 생존율이 비교군보다 유의하게 높았다고 보고하였다(p = 0.04). Maio 등(2010) 연구에서는 싸이모신 알파 1의 용량 및 병용치료에 따라 비교군 대비 무진행 생존율 결과가 일관되게 나타나지 않았다. 또한, Shuqun 등(2004)은 무재발 생존기간, Maio 등(2010)은 무진행 생존기간, Niu 등(2023)은 무질병 생존기간을 각각 보고하였으나, 군 간 유의한 차이가 일관되게 나타나지는 않았다. 항암반응률은 2편에서 평가되었다. 완전관해 또는 부분관해를 보인 환자의 비율은 두 군 간 유의한 차이가 없었으며, 메타분석 결과 상대위험비(Risk Raio, RR)가 1.7(95% CI 0.85, 3.38, I2 = 0%)로 나타났다. 결론 및 의료기술재평가 권고결정 의료기술재평가 소위원회는 싸이모신 알파 1의 국가별 품목허가 현황 및 적응증에 차이가 있음을 확인하였다. 국내에서는 해당 약물이 백신 접종 시 보조요법으로 허가되어 있으며, 실제 임상에서는 기타 종양치료제로 분류되어 암 환자에게도 빈번이 사용되고 있는 점을 고려하여, 본 평가는 싸이모신 알파 1의 백신 보조요법 및 항암 보조요법으로서의 안전성과 유효성을 중심으로 수행하였다. 체계적 문헌고찰 결과, 싸이모신 알파 1 중재군은 비교군에 비해 전체 심각한 이상반응, 약물 관련 심각한 이상반응, 일반적인 이상반응, 약물 관련 이상반응 및 투약 중단에 따른 이상반응 발생률이 전반적으로 유사한 수준으로 나타났다. 일부 연구(2편)에서는 설사, 두통, 오한, 독감 유사 증상, 주사 부위 통증 등의 이상반응이 보고되었으나, 대부분 경증이며 일시적이었다. 유효성은 백신 보조요법에서는 면역반응(항체 역가), 암 환자 대상 항암 보조요법에서는 생존율, 재발 및 질병 진행, 항암반응률 등을 중심으로 평가하였다. 백신 관련 2편의 연구에서 싸이모신 알파 1군은비교군보다 항체 역가가 높게 나타나는 경향을 보였으나, 연구 수가 적어 근거의 제한이 있었다. 항암 보조요법과 관련해서는, 생존율과 이환율에 대한 연구 결과가 일관되지 않았으며, 항암 반응률은 싸이모신 알파 1군이 비교군보다 높은 경향을 보였으나, 이 역시 2편의 연구만 포함되어 근거가 제한적이었다. 소위원회는 싸이모신 알파 1이 기존 치료와 유사한 수준의 이상반응을 보여 전반적으로 안전하다고 평가하였다. 그러나 유효성에 있어서는, 백신 보조요법으로 사용 시 항체 역가 향상에 긍정적인 경향이 있더라도 연구 수가 적고 최신 근거가 충분하지 않아 신중한 해석이 필요하다고 판단하였다. 암 환자 대상 항암 보조요법에서도 생존율, 재발 및 질병 진행, 항암반응률 등에서 일관된 이득을 입증하기에는 근거가 부족하다는 의견을 제시하였다. 또한, 소위원회는 싸이모신 알파 1에 대한 기존 연구들의 암종(간암, 대장암, 흑색종, 폐암 등)이 상이하고, 종양의 병기 및 환자 특성이 이질하며, 연구의 대부분(5편 중 4편)이 2010년 이전에 설계되어 암 치료는 표준치료가 빠르게 바뀌고 있다는 점에서 최신 치료 환경을 반영하지 못하고 있는 점을 지적하였다. 이 외에도, 다수의 연구에서 무작위배정 방법, 순서은폐 방법 등을 명확히 보고하지 않는 등 비뚤림위험이 존재하였다. 이에 따라 전체적으로 근거수준은 ‘낮음’ 또는 ‘매우 낮음’으로 평가되었다. 2024년 제12차 의료기술재평가위원회(2024.12.13.)에서는 의료기술재평가사업 관리지침 제4조제10항에 근거하여 소위원회의 검토 결과를 바탕으로 싸이모신 알파 1에 대한 최종 심의를 진행하였다. 위원회는 싸이모신 알파 1이 면역저하자에게 백신 접종 시 보조요법으로 사용되거나, 암 환자에게 통상적인 항암치료에 추가로 사용될 경우 안전성은 기존 치료와 유사한 수준으로 판단하였다. 그러나 암종별로 환자의 특성이 이질하고 임상 결과가 일관되지 않은 점, 연구의 비뚤림위험에 대한 우려, 최신 연구 부족 등으로 인해 유효성에 대한 연구의 근거수준은 낮다고 평가하였다. 이에 따라, 의료기술재평가위원회는 임상적 안전성과 유효성 등에 대한 근거를 종합적으로 고려하였을 때, 국내 임상 상황에서 면역저하자에게 백신 접종 시 싸이모신 알파 1을 보조적으로 사용하거나, 암 환자에게 종양 치료 및 재발 예방 목적으로 기존 통상적인 암 치료에 싸이모신 알파 1을 추가 투여하는 것을 ‘권고하지 않음’으로 결정하였다.


Background Viscum album, Immunocyanin, and Thymosin α1 are non-reimbursable injectable drugs used in patients with cancer or impaired immune function for purposes such as tumor treatment, adjuvant cancer therapy, prevention of cancer recurrence, and immune support therapy. According to data from the “Detailed Survey of Non-reimbursable Items,” these drugs rank among the highest in medical expenditure in the category of antineoplastic agents. In accordance with the “Comprehensive Plan to Strengthen the Management of Non-Reimbursable Services in the National Health Insurance” (December 31, 2020), these drug-based techniques were referred for health technology reassessment by relevant institutions to promote appropriate healthcare delivery and rational healthcare utilization. The reassessment protocol and subcommittee composition were reviewed at the 4th 2024 Health Technology Reassessment Committee meeting (April 12, 2024), and the reassessment was conducted through a professional and in-depth review of clinical safety and effectiveness. The objective of this reassessment was to provide objective evidence on the clinical safety and effectiveness of Viscum album, Immunocyanin, and Thymosin α1 and to determine the strength of recommendation for each technology. This report presents the assessment results for Thymosin α1. Method A systematic literature review was conducted to evaluate the safety and effectiveness of Thymosin α1. The detailed methods and results were finalized through review and discussion by the “Joint Subcommittee on the Safety and Effectiveness Assessment of Viscum album, Immunocyanin, and Thymosin α1” (hereafter referred to as the “Subcommittee”). The Subcommittee comprised six members: two hematology-oncology specialists, two family medicine specialists, and two evidence-based medicine experts, in accordance with the reassessment plan approved by the Health Technology Reassessment Committee. Thymosin α1 use patterns and approved indications vary across countries. The Subcommittee noted that, in Korea, the approved indication is “adjuvant therapy for vaccination,” but in actual clinical practice, it is also frequently used as an adjuvant in cancer treatment. Accordingly, the target population for this assessment was defined as “patients receiving vaccination” and “patients with cancer.” Safety was assessed based on severe adverse events (AEs) and complications, as well as general AEs and complications. Effectiveness was evaluated in two parts. For adjuvant therapy with vaccination, the outcome measures included antibody response rate and immune response. For cancer-related effectiveness, the outcome measures included survival rate, recurrence rate, disease progression, antitumor response rate, and quality of life in patients receiving additional Thymosin α1. Considering that the agent is primarily used in patients with cancer, the literature review was restricted to randomized controlled trials (RCTs) to minimize selection bias and confounding. The literature search was conducted using key questions across three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews – Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, KMbase, and RISS), with the final search performed on July 25, 2024. Risk of bias was assessed for each included study using the Risk of Bias (RoB) tool, and data were extracted with a predefined form. A meta-analysis was performed when quantitative synthesis was feasible. All processes were independently conducted by two reviewers, and any discrepancies were resolved by consensus. The level of evidence was evaluated using the GRADE (Grading of Recommendations, Assessment, Development and Evaluation) approach. The final strength of recommendation was determined through review by the Health Technology Reassessment Committee, based on the Subcommittee’s findings. Results The systematic literature review identified a total of seven RCTs. Of these, two studies on adjuvant therapy with vaccination (n = 206; Thymosin α1 intervention group, 127; control group, 79) and five studies in patients with cancer (n = 1,003; Thymosin α1 intervention group, 641; control group, 361) were included. The two vaccination adjuvant therapy studies, published in 1989 and 2012, enrolled elderly participants aged 66 years or older and patients with end-stage renal disease (ESRD) undergoing hemodialysis. Among the five studies conducted in patients with cancer, one was published before 2000, three between 2000 and 2010, and one after 2010 (2023). The cancer types studied included two trials in hepatocellular carcinoma and one trial each in non-small cell lung cancer, metastatic melanoma, and colorectal cancer. Risk of bias was assessed by domain. Studies that did not describe random sequence generation or allocation concealment were rated “unclear” for these domains due to insufficient information. For blinding, studies reporting objective outcomes such as survival rate and disease-free survival were considered unaffected by blinding and were therefore rated “low” risk, whereas those reporting subjective outcomes such as quality of life were rated “unclear.” For incomplete outcome data, studies with >20% loss to follow-up were rated “unclear.” For selective reporting, studies that reported only safety or only effectiveness outcomes were rated “unclear.” For other sources of bias, studies that received the study drug or funding from the manufacturer were rated “high.” Safety Safety was assessed by combining data from the vaccination adjuvant therapy and cancer studies. AEs were categorized as severe or general and analyzed accordingly. Severe AEs were reported in three studies, with incidence rates of 2–50% in the Thymosin α1 group and 11–64% in the control group. None of these studies reported statistical analyses of significance. Deaths and treatment discontinuation due to AEs were each reported in two studies. Mortality rates ranged from 3.3–21.0% in the Thymosin α1 group and 8.6%–45.0% in the control group. General AEs were reported in four studies. In three of these, AE incidence ranged from 3.5%–93.0% in the Thymosin α1 group and 9.5–91% in the control group. In the remaining study, treatment-related AEs were reported in 41–66% of the intervention group and 62% of the control group. Overall, the incidence of AEs in the Thymosin α1 group was generally lower than or comparable to that in the control group. In addition, Maio et al. (2010) reported no significant differences between groups in influenza-like symptoms, chills, localized swelling and pain at the injection site, or fatigue. Gish et al. (2009) reported localized warmth at the injection site and mild muscle loss, noting that most AEs were mild and transient. Effectiveness 1. Effectiveness as adjuvant therapy with vaccination Two studies evaluated the effectiveness of Thymosin α1 as an adjuvant to vaccination. Carraro et al. (2012) reported the results of co-administration of Thymosin α1 at doses of 3.2 mg and 6.4 mg with vaccination. In the 3.2 mg group, antibody titers were higher than those in the control group at early time points (days 21 and 42) but tended to be lower at later timepoints (days 84 and 168). In the 6.4 mg group, antibody titers were higher than those in the control group at all time points. However, statistical significance was not reported for either dose group. Gravenstein et al. (1989) reported that antibody titers at week 6 tended to be higher in the Thymosin α1 group than in the control group. 2. Effectiveness as cancer adjuvant therapy Five studies evaluated Thymosin α1 in addition to anticancer therapy in cancer patients. Key outcome measures included survival rate, survival duration, recurrence or disease progression, and antitumor response rate. Survival rate was reported in three studies. Schulof et al. (1985) reported that the 1-year survival rate was significantly higher in the Thymosin α1 group than in the control group (p = 0.009). Gish et al. (2009) reported 1-year survival rates of 66% in the Thymosin α1 group and 61% in the control group, with no statistical significance. Maio et al. (2010) also found no significant difference in survival rate between groups, and the hazard ratio (HR) was not statistically significant. Survival duration was reported in three studies. Gish et al. (2009) and Maio et al. (2010) reported no significant difference in median survival between groups. Shuqun et al. (2004) reported a median survival of 10 months in the Thymosin α1 group compared with 7 months in the control group, although statistical significance was not reported. Recurrence or disease progression was reported in five studies. Schulof et al. (1985) reported a significantly higher recurrence-free survival rate in the Thymosin α1 group compared with that in the control group (p = 0.04). In the study by Maio et al. (2010), progression-free survival varied depending on the dose of Thymosin α1 and type of combination therapy used. Shuqun et al. (2004) reported recurrence-free survival duration, Maio et al. (2010) reported progression-free survival duration, and Niu et al. (2023) reported disease-free survival duration; however, no consistent statistically significant differences between groups were observed. Antitumor response rate was evaluated in two studies. The proportion of patients achieving complete or partial response did not differ significantly between groups. A meta-analysis showed a risk ratio (RR) of 1.7 (95% CI: 0.85–3.38, I² = 0%). Conclusion and Strength of Recommendation The Subcommittee confirmed that the approved indications and regulatory status of Thymosin α1 vary by country. In Korea, the agent is approved as “adjuvant therapy for vaccination” and is classified as an “other antineoplastic agent,” with frequent use as an adjuvant in tumor treatment in clinical practice. Accordingly, this assessment focused on the safety and effectiveness of Thymosin α1 as both vaccination adjuvant therapy and cancer adjuvant therapy. The systematic literature review found that the incidence of overall severe AEs, drug-related severe AEs, general AEs, drug-related AEs, and treatment discontinuation due to AEs was generally comparable between the Thymosin α1 and control groups. In two studies, AEs such as diarrhea, headache, chills, influenza-like symptoms, and injection-site pain were reported; however, most were mild and transient. Effectiveness was assessed in terms of immune response (antibody titers) for vaccination adjuvant therapy, and survival rate, recurrence or disease progression, and antitumor response rate for cancer adjuvant therapy. In the two vaccination-related studies, the Thymosin α1 group tended to have higher antibody titers than the control group; however, the small number of studies limited the strength of the evidence. For cancer adjuvant therapy, results for survival and morbidity outcomes were inconsistent, and although the antitumor response rate tended to be higher in the Thymosin α1 group than in the control group, this finding was based on only two studies and thus provided limited evidence. The Subcommittee determined that Thymosin α1 demonstrated a safety profile comparable to existing therapies. However, regarding effectiveness, while vaccination adjuvant use showed a favorable trend toward improved antibody titers, the small number of studies and lack of recent evidence warrant cautious interpretation. For cancer adjuvant therapy, the Subcommittee concluded that evidence was insufficient to demonstrate consistent benefits in survival, recurrence or disease progression, or antitumor response rate. The Subcommittee also noted that existing studies on Thymosin α1 included heterogeneous cancer types (e.g., hepatocellular carcinoma, colorectal cancer, melanoma, lung cancer), varied disease stages, and diverse patient characteristics. In addition, most studies (four of five) in cancer patients were conducted before 2010, and given the rapid evolution of standard cancer treatments, these findings may not reflect the current treatment environment. Additional concerns included lack of clear reporting of randomization and allocation concealment methods in many studies, contributing to a risk of bias. Overall, the evidence level was rated as “low” or “very low.” At the 12th 2024 Health Technology Reassessment Committee meeting (December 13, 2024), the Committee reviewed the Subcommittee’s findings in accordance with Article 4(10) of the Health Technology Reassessment Project Management Guidelines. The Committee concluded that the safety of Thymosin α1, whether used as adjuvant therapy for vaccination in immunocompromised patients or in addition to standard anticancer therapy in patients with cancer, is comparable to that of existing therapies. However, the level of evidence for effectiveness was rated as low due to heterogeneity in patient characteristics by cancer type, inconsistency in clinical outcomes, concerns about risk of bias, and the lack of recent studies. Accordingly, the Health Technology Reassessment Committee determined that the use of Thymosin α1 as adjuvant therapy for vaccination in immunocompromised patients or as an adjunct to standard cancer treatment for treatment of tumors or prevention of recurrence in cancer patients is “not recommended.”
