초록

평가배경 근골격계 종양의 고주파열치료술(radiofrequency ablation, RFA)은 CT 유도하(연부조직의 경우 초음파유도)에 전극이 부착된 바늘을 근골격계 종양 내에 삽입하여 고주파열로 종양 부위를 선택적으로 괴사시키는 시술이다. 근골격계 종양(musculoskeletal tumor)은 크게 골종양과 연부조직 종양으로 나눌 수 있으며, 양성종양은 성장속도가 느리고 발생부위에 국소적으로 머물러 있으나 악성종양은 성장속도가 빠르고 혈관이나 림프관을 타고 주변 조직을 침범하여 전이를 일으키는 특성이 있다. 근골격계 종양의 고주파열치료술은 신의료기술평가 제도 시행 전 도입된 등재비급여 기술이다(보건복지부고시 제2002-28호, 2002. 3. 29.). 동 기술은 내부 모니터링을 통해 재평가 안건으로 발굴되었다. 다양한 대체 의료기술이 존재하는 현 시점에서 비교기술 대비 동 기술의 효과성에 대한 근거를 확인하고자, 2024년 제6차 의료기술재평가위원회(2024. 6. 14.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 근골격계 종양 환자에서 고주파열치료술을 종양제거 및 증상완화 목적으로 사용하는 것은 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 근골격계 종양 환자에서 고주파열치료술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 ‘근골격계 종양의 고주파열치료술 소위원회(이하 ‘소위원회’라 한다)’의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 정형외과 2인, 영상의학과 2인, 근거기반의학과 1인, 총 5인으로 구성하였다. 본 평가의 대상자는 양성 근골격계 환자와 악성 근골격계 환자로 구분하였고, 비교시술은 수술치료, 냉동제거술, 방사선요법으로 정의하였다. 결과변수로는 안전성과 효과성을 확인하였으며, 안전성은 부작용 및 이상반응을 결과지표로, 효과성은 통증지표, 치료성공률, 종양학적 지표, 기능지표, 수술시간 및 재원기간, 삶의 질 및 만족도를 결과지표로 평가하였다. 연구유형은 비교연구 이상의 연구만 평가에 포함하기로 하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-Embase, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMBASE), 한국교육학술정보원(RISS)) 데이터베이스에서 검색하였다(최종검색일 2024. 8. 13.). 최종 선택된 연구의 비뚤림위험 평가는 무작위배정 임상시험 연구는 Cochrane의 Risk of Bias (RoB)를, 비무작위 연구는 risk of bias assessment for nonrandomized studies (RoBANS ver 2.0)를 사용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거수준을 평가하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 표로 제시하고, 양적 분석이 가능한 경우 메타분석을 하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과 최종 선택된 연구는 총 23편(대상자 수 1,442명)으로 무작위배정 임상시험 연구 1편, 비무작위 연구 22편이었다. 연구대상자는 양성 근골격계 종양 9편, 악성 근골격계 종양 14편이었다. 대조군은 양성 근골격계 종양의 경우 수술과의 비교 8편, 수술과 고주파열치료술 병용군과 미병용군 비교 1편이었으며, 악성 근골격계 종양의 경우 수술과 고주파열치료술 병용군과 미병용군 비교 7편, 냉동제거술과의 비교 5편, 방사선요법과의 비교 2편이었다. 비뚤림위험은 RCT와 NRS 연구 모두 중등도인 것으로 판단하였다. 안전성 근골격계 종양의 고주파열치료술의 안전성은 부작용 및 이상반응으로 평가하였다. 양성 근골격계 종양 대상 고주파열치료술과 수술치료의 단독비교 8편에서 통증을 포함한 모든 안전성 지표에서 중재군과 대조군 간 유의한 차이가 없거나, 일시적이거나 경미한 부작용이 양 군에서 유사한 수준으로 발생하였다. 양 군에 수술치료를 시행하고 한 군에만 고주파열치료술을 병용하여 비교한 연구 1편에서는 양 군 모두에서 신경 또는 방사선 관련 부작용이 발생하지 않았다고 보고하였다. 악성 근골격계 종양 대상 고주파열치료술과 냉동제거술 단독으로 혹은 양 군에 수술치료를 병용하여 비교한 연구 5편 중 1편에서 중재군이 대조군보다 경미한 합병증의 발생률이 유의하게 더 높았으며, 나머지 연구에서는 일시적이거나 경미한 부작용이 양 군에서 유사한 수준으로 발생하였다. 양 군에 수술치료를 시행하고 각각 고주파열치료술과 방사선요법을 병용하여 비교한 연구 1편에서는 모든 지표에서 중재군과 대조군 간 유의한 차이가 없었으며, 양 군에 수술치료를 시행하고 한 군에만 고주파열치료술을 병용하여 비교한 연구 5편에서도 두 군 간 유의한 차이가 없거나, 일시적이거나 경미한 부작용이 양 군에서 유사한 수준으로 발생하였다. 효과성 근골격계 종양의 고주파열치료술의 효과성은 통증지표, 치료성공률, 종양학적 지표, 기능지표, 수술시간 및 재원기간, 삶의 질 및 만족도로 평가하였다. 양성 근골격계 종양에서 고주파열치료술과 수술치료의 단독비교의 경우, 통증지표는 3편 중 1편에서 중재군이 대조군보다 유의하게 좋았고 2편에서는 중재군에서 더 좋은 경향성을 보였으나 통계적 유의성은 보고되지 않았다. 치료성공률은 4편 중 3편에서는 중재군과 대조군 간 유의한 차이가 없었고, 1편에서는 두 군 간 유사한 수준이었으며 통계적 유의성이 보고되지 않았다. 재발률을 보고한 6편을 합성하였을 때 중재군이 대조군보다 유의하게 높았으며, 무질병생존율은 1편에서 중재군이 대조군보다 유의하게 낮았다. 기능지표는 1편에서 중재군과 대조군 간 유의한 차이가 없었으며, 재원기간은 3편 중 1편에서 중재군이 대조군보다 유의하게 짧았고 2편은 중재군에서 더 짧은 경향성을 보였으나 통계적 유의성이 보고되지 않았으며, 수술시간은 2편 중 1편에서 중재군이 대조군보다 유의하게 짧았고 1편은 중재군에서 더 짧은 경향을 보였으나 통계적 유의성이 보고되지 않았다. 만족도는 ‘매우 만족’의 비율이 중재군에서 높았으나 통계적 유의성은 보고되지 않았다. 양 군에 수술치료를 시행하고 한 군에만 고주파열치료술을 병용하여 비교한 연구 1편에서는 통증과 기능지표가 중재군이 대조군보다 유의하게 좋았으며, 나머지 지표들은 보고되지 않았다. 악성 근골격계 종양에서 고주파열치료술과 냉동제거술 단독으로 혹은 양 군에 수술치료를 병용하여 비교한 연구의 경우, 통증지표는 4편 중 2편에서 중재군이 대조군보다 유의하게 높았고 2편에서는 대조군에서 더 좋은 경향성을 보였으나 통계적 유의성이 보고되지 않았으며, 치료성공률, 종양학적 지표, 기능지표는 보고되지 않았다. 수술시간은 1편에서 중재군과 대조군 간 유의한 차이가 없었고, 재원기간은 1편에서 중재군이 대조군보다 유의하게 길었다. 삶의 질은 대조군에서 더 높았으나 통계적 유의성은 보고되지 않았다. 양 군에 수술치료를 시행하고 각각 고주파열치료술과 방사선요법을 병용하여 비교한 연구의 경우, 통증지표는 2편 중 1편에서는 중재군이, 다른 1편에서는 대조군이 유의하게 좋아 상반된 결과를 보였고, 기능지표는 1편에서 중재군이 대조군보다 유의하게 좋았으며, 이외의 지표들은 보고되지 않았다. 양 군에 수술치료를 시행하고 한 군에만 고주파열치료술을 병용하여 비교한 연구의 경우, 통증지표는 4편을 합성하였을 때 1개월과 6개월 시점 모두 중재군이 대조군보다 유의하게 좋았다. 치료성공률은 2편에서 중재군과 대조군 모두 100%로 보고하였고, 재발률은 3편을 합성하였을 때 중재군이 대조군보다 유의하게 낮았다. 기능지표는 6편 중 3편에서는 중재군이 대조군보다 유의하게 좋았지만 3편에서는 두 군 간 유의한 차이가 없었으며, 수술시간은 3편을 합성하였을 때 중재군과 대조군 간 유의한 차이가 없었고, 삶의 질은 1편에서 두 군 간 유의한 차이가 없었다. 결론 및 권고결정 의료기술재평가 소위원회에서는 근골격계 종양의 고주파열치료술의 안전성과 효과성 결과를 현재 확인된 근거를 바탕으로 다음과 같이 제시하였다. 체계적 문헌고찰 결과, 양성 근골격계 종양에서 고주파열치료술은 시술 후 통증, 신경손상 발생 등이 일부 연구에서 보고되고 있으나, 이는 시술 특성에 따른 일시적인 부작용이며 최소침습적인 시술로서 소위원회는 동 기술을 안전한 의료기술로 판단하였다. 동 기술은 수술치료보다 재발률은 더 높으나 통증지표, 수술시간이나 재원기간이 감소한 결과가 있었고, 수술치료 시 고주파열치료술을 병용하는 것이 병용하지 않을 때보다 통증과 기능지표에서 유의한 개선을 나타낸 결과가 있어, 양성 근골격계 종양 환자에서 하나의 치료대안으로 사용할 수 있는 효과적인 기술이라고 평가하였다. 악성 근골격계 종양에서 고주파열치료술은 시술 후 통증, 신경손상 발생 등이 일부 연구에서 보고되고 있으나, 이는 시술 특성에 따른 일시적인 부작용이며 최소침습적인 시술로서 소위원회는 동 기술을 안전한 의료기술로 판단하였다. 동 기술은 통증지표 등 일부 지표에서 냉동제거술보다 개선효과가 낮거나, 방사선요법과는 유사한 효과가 나타난 결과가 있었고, 수술치료와 고주파열치료술을 병용하는 것이 병용하지 않을 때보다 통증지표, 재발률, 기능지표에서 개선효과가 높게 나타난 결과가 있어, 악성 근골격계 종양 환자에서 하나의 치료대안으로 사용할 수 있는 효과적인 기술이라고 평가하였다. 또한, 소위원회 검토 시 국내 임상상황에서 고주파열치료술이 냉동제거술에 비해 장비에 대한 접근성 및 시술의 편의성이 높고, 유골골종의 경우에는 수술치료보다 고주파열치료술을 일차치료로 시행하는 추세라는 의견이 있었다. 2025년 제2차 의료기술재평가위원회(2025. 2. 14.)는 ‘근골격계 종양의 고주파열치료술’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 양성 근골격계 종양 환자의 종양제거 및 증상완화 목적으로 고주파열치료술의 사용을 ‘약하게 권고함’으로 결정하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 악성 근골격계 종양 환자의 종양제거 및 증상완화 목적으로 고주파열치료술의 사용을 ‘약하게 권고함’으로 결정하였다. ※ 동 시술은 수술치료가 어렵거나, 환자가 수술치료를 원치 않는 경우 등에 한하여 전문의의 판단하에 제한적 사용을 권고함


Background Radiofrequency ablation (RFA) is used to induce selective tumor tissue necrosis in musculoskeletal tumors. For this technique, radiofrequency thermal energy is delivered to tumor tissues via a needle with an electrode, which is inserted into the tumor under computed tomography guidance (or ultrasound guidance for soft tissue). Musculoskeletal tumors are broadly categorized into bone and soft tissue tumors. Benign tumors typically grow slowly and remain localized to the site of origin, whereas malignant tumors are characterized by rapid growth and a tendency to invade surrounding tissues or metastasize via blood or lymphatic vessels. RFA for musculoskeletal tumors was listed as a non-reimbursable technology before the implementation of the New Health Technology Assessment (nHTA) system (Ministry of Health and Welfare Bulletin No. 2002-28, March 29, 2002). The need for reassessment of this technology was identified through internal monitoring. Given the current availability of various alternative medical technologies, this reassessment was conducted to establish evidence regarding the effectiveness of RFA versus alternative technologies. The reassessment protocol was reviewed and approved, and a subcommittee was formed at the 6th 2024 Health Technology Reassessment Committee meeting (June 14, 2024). Objective The objective of this reassessment was to evaluate the clinical safety and effectiveness of RFA for tumor removal and symptom relief in patients with musculoskeletal tumors and to determine the appropriate strength of recommendation for the use of this technology. Methods A systematic literature review was conducted to assess the safety and effectiveness of RFA in patients with musculoskeletal tumors. All assessment methods were finalized through discussions by the Subcommittee on Radiofrequency Ablation for Musculoskeletal Tumors (hereinafter referred to as “Subcommittee”). The Subcommittee consisted of five members: two orthopedic surgeons, two radiologists, and one evidence-based medicine expert, as designated by the Health Technology Reassessment Committee. The target population included patients with either benign or malignant musculoskeletal tumors. Comparator treatments were defined as surgical treatment, cryoablation, and radiotherapy. Outcome measures included safety and effectiveness. Safety was assessed based on adverse events and complications. Effectiveness was assessed based on pain indicators, treatment success rate, oncological outcomes, functional outcomes, operation time, length of hospital stay, quality of life, and patient satisfaction. Only comparative studies were included in the assessment. Based on the key question, a literature search was conducted in three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews-Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, Korean Medical Database [KMbase], and Research Information Sharing Service [RISS]). The final search date was August 13, 2024. Risk of bias was assessed using the Cochrane Risk of Bias (RoB) tool for randomized controlled trials (RCTs) and the Risk of Bias for Nonrandomized Studies (RoBANS) version 2.0 for non-randomized studies (NRSs). The level of evidence of the systematic review results was evaluated using the GRADE (Grading of Recommendations Assessment, Development, and Evaluation) method. Data were extracted using predefined extraction forms, and meta-analyses were performed when quantitative analysis was feasible. All processes were independently performed by two reviewers, and disagreements were resolved through consensus. The final recommendation level was determined by the Health Technology Reassessment Committee based on the findings of the Subcommittee. Results A total of 23 studies (n = 1,442) met the inclusion criteria: one RCT and 22 NRSs. Overall, 9 studies included patients with benign musculoskeletal tumors and 14 included patients with malignant musculoskeletal tumors. For benign tumors, eight studies compared RFA versus surgery, and one study compared surgery plus RFA versus surgery alone. For malignant tumors, seven studies compared surgery plus RFA versus surgery alone, five compared RFA versus cryoablation, and two compared RFA versus radiotherapy. The overall risk of bias in both RCTs and NRSs was assessed as moderate. Safety The safety of RFA was assessed based on adverse events and complications. In eight studies comparing RFA versus surgery in patients with benign tumors, no significant differences were observed in safety indicators, including pain, and both groups showed a similar incidence of transient or mild adverse events. One study comparing surgery plus RFA versus surgery alone reported no radiation-related or nerve-related complications in either group. Among the five studies comparing RFA or cryoablation alone or RFA plus surgery and cryoablation plus surgery in patients with malignant tumors, one reported a significantly higher rate of mild complications in the RFA group. The remaining studies reported similar incidence of transient or mild adverse events in both groups. In one study comparing surgery plus RFA versus surgery plus radiotherapy, no significant differences were reported in all outcome measures between the two groups. In five studies comparing surgery alone versus surgery plus RFA, no significant differences were found, and both groups reported similar incidence of transient or mild adverse events. Effectiveness The effectiveness of RFA was evaluated based on pain indicators, treatment success rate, oncological outcomes, functional outcomes, operation time, length of hospital stay, quality of life, and patient satisfaction. Among studies comparing RFA versus surgery in patients with benign tumors, one of three studies reported significantly greater pain reduction in the RFA group, whereas the remaining two studies showed favorable trends for the intervention RFA group without reporting statistical significance. For treatment success rate, three of four studies found no significant difference between groups, and the remaining study reported comparable outcomes but did not report statistical significance. A meta-analysis of six studies on recurrence rate showed significantly higher recurrence in the RFA group, and one study reported significantly lower disease-free survival in the RFA group. For functional outcomes, one study reported no significant difference between the groups. Among three studies assessing length of hospital stay, one reported significantly shorter length of hospital stay in the RFA group, whereas two showed a tendency toward shorter length of hospital stay without reporting statistical significance. For operation time, one of two studies reported significantly shorter operation time in the RFA group. Regarding patient satisfaction, one study reported a higher rate of “very satisfied” responses in the RFA group, though without reporting statistical significance. One study comparing surgery plus RFA versus surgery alone in patients with benign tumors reported significantly better pain and functional outcomes in the RFA group; other outcomes were not reported. In studies comparing RFA alone and cryoablation alone or surgery plus RFA or surgery plus cryoablation for malignant tumors, two of four studies reported significantly better pain outcomes in the comparator group, whereas the remaining two studies favored the comparator without reporting statistical significance. Treatment success rate, oncological outcomes, and functional outcomes were not reported. Regarding operation time, one study found no significant difference between the groups. For length of hospital stay, one study reported significantly longer length of hospital stay in the RFA group. Regarding quality of life, one study found better quality of life in the comparator group, but statistical significance was not reported. Among studies comparing surgery plus RFA versus radiotherapy plus surgery for malignant tumors, one study reported better pain outcomes in the RFA group, whereas the other favored the comparator. One study reported significantly better functional outcomes in the RFA group, and other outcomes were not reported. A meta-analysis of four studies comparing surgery plus RFA versus surgery alone in patients with malignant tumors showed that the RFA group had significantly better pain outcomes at both one and six months. For treatment success rate, two studies reported a 100% treatment success rate in both groups. Regarding recurrence rate, a meta-analysis of three studies showed significantly lower recurrence rates in the RFA group. Among six studies assessing functional outcomes, three reported significantly better outcomes in the RFA group, whereas three found no significant difference. Regarding operation time, a meta-analysis of three studies found no significant difference between groups. For quality of life, one study reported no significant difference between the groups. Conclusions and Strength of Recommendation Based on the currently available evidence, the Subcommittee presented the following conclusions regarding the safety and effectiveness of RFA for musculoskeletal tumors: In patients with benign tumors, some studies reported that RFA is a minimally invasive procedure with transient adverse events such as post-procedural pain and nerve injury. Therefore, the Subcommittee determined that RFA is a safe technology for benign tumors. Although recurrence rates after RFA were higher than those after surgery, some studies showed favorable outcomes for RFA in terms of pain reduction, shorter operation time, and reduced length of hospital stay. Furthermore, combining RFA with surgery resulted in significantly improved pain and functional outcomes compared with surgery alone. Thus, the Subcommittee determined that RFA is an effective treatment option for patients with benign musculoskeletal tumors. In patients with malignant tumors, some studies reported that the minimally invasive RFA also resulted in only transient adverse events, including pain and nerve injury. Therefore, the Subcommittee considered RFA as a safe technology for malignant tumors. Although RFA showed inferior outcomes compared to cryoablation in terms of some pain-related indicators, it showed comparable effectiveness to radiotherapy and greater improvement in pain, recurrence rate, and functional outcomes when combined with surgery. Therefore, the Subcommittee determined that RFA is an effective treatment option for patients with malignant musculoskeletal tumors. In addition, the Subcommittee noted that RFA is preferred over cryoablation in the Korean clinical setting due to greater equipment accessibility and procedural convenience. Furthermore, RFA tends to be used as first-line treatment for osteoid osteoma. At the 2nd 2025 Health Technology Reassessment Committee meeting (February 14, 2025), the following decisions were made regarding “Radiofrequency Ablation for Musculoskeletal Tumors”: Considering the clinical evidence on safety and effectiveness and other assessment factors, the Committee determined that the use of RFA for tumor removal and symptom relief is “weakly recommended” in patients with benign musculoskeletal tumors in the Korean clinical setting. Considering the clinical evidence on safety and effectiveness and other assessment factors, the Committee determined that the use of RFA for tumor removal and symptom relief is “weakly recommended” in patients with malignant musculoskeletal tumors in the Korean clinical setting. ※ This procedure is recommended for limited use under the judgment of a specialist in cases where surgical treatment is difficult, or the patient does not wish to undergo surgery
