Background The quantitative sensory test (QST) is used to diagnose abnormalities in sensory function, monitor treatment response, and follow up patients by quantitatively measuring the stimulus intensity at which sensation can be perceived in response to stimuli such as vibration, temperature, and electrical currents of specific frequencies in the distal parts of the upper and lower extremities. The test is used to assess the presence and severity of neurosensory impairment caused by peripheral neuropathy. Among QST modalities, the current perception threshold (electrical pain threshold) test measures the threshold at which a patient perceives pain using electrical currents of specific frequencies, namely 5, 250, and 2,000 Hz. This technology was listed in 2002, before the introduction of the New Health Technology Assessment system, as an item fully paid out-of-pocket by patients, and was subsequently converted to a non-covered item in 2006, pursuant to Ministry of Health and Welfare Notification No. 2005-89, dated December 22, 2005. The need to provide public information on this technology as an item under the non-covered services reporting system was identified through a demand survey conducted among relevant organizations. Following the relevant selection process, the technology was finally selected for reassessment. Accordingly, to provide information on the clinical effectiveness of this technology, a reassessment was conducted after deliberation on the assessment protocol and the proposed composition of the subcommittee at the 8th Health Technology Reassessment Committee meeting in 2025, held on August 8, 2025. Objective The objective of this assessment was to review the evidence on the target population and assessment tools for the “quantitative sensory test – current perception threshold (electrical pain threshold)” and to provide information supporting its appropriate use in clinical practice. Methods Considering that the quantitative sensory test is used for heterogeneous diseases with various etiologies and that index and comparator tests are used in a stepwise and complementary manner, this assessment reviewed textbooks and clinical practice guidelines to reflect the clinical context, instead of conducting a systematic review focused on comparative effectiveness. All assessment methods were finalized through discussion by the Joint Subcommittee for the Quantitative Sensory Test, hereafter referred to as the “Subcommittee,” taking into account the objective of the assessment. The Subcommittee consisted of five experts: two neurologists, two endocrinologists, and one rehabilitation medicine specialist. The key question of this assessment was: “Is the quantitative sensory test – current perception threshold (electrical pain threshold) clinically effective for evaluating sensory nerve function in peripheral neuropathy?” Regarding the target population, the current Ministry of Health and Welfare notification specifies “peripheral neuritis.” However, because peripheral neuritis and peripheral neuropathy are used interchangeably in current clinical practice, and most published literature describes the condition as peripheral neuropathy, the Subcommittee discussed and defined the target population of this assessment as patients with “peripheral neuropathy.” Regarding comparator tests, the diagnosis of diabetic peripheral neuropathy is currently based on initial screening using tests not listed in the National Health Insurance reimbursement schedule, such as the Michigan Neuropathy Screening Instrument (MNSI) self-administered questionnaire and the monofilament test, followed by nerve conduction studies, which are reimbursed and used as a standard test when abnormal findings are identified. However, because nerve conduction studies mainly evaluate Aβ nerve fibers and therefore have limitations in independently identifying all types of nerve injury, the Subcommittee considered that the index and comparator tests are used complementarily in clinical practice. Accordingly, no comparator test was specified. The medical outcome assessed was clinical utility. Textbooks in the fields of neurology, neuromuscular diseases, diabetology, and rehabilitation medicine were reviewed. Clinical practice guidelines were searched in the Korean Medical Guideline Information Center, Guideline International Network, and Trip Medical Database using combinations of key terms such as “neuropathy,” “quantitative sensory,” “QST,” “vibration,” “thermal,” “current/current perception,” “electrical,” “pain,” and “threshold.” In addition, using the retrieved clinical practice guidelines, manual searches were conducted for clinical practice guidelines published by domestic and international health technology assessment agencies and professional societies. The search date was September 10, 2025. The appropriateness of the textbooks and clinical practice guidelines was subsequently confirmed by the Subcommittee. The final deliberation of this assessment was conducted by the Reassessment Expert Committee based on the Subcommittee’s review findings. Results Textbook Review A review of four textbooks found that the quantitative sensory test was presented as a test that objectively quantifies the function of small nerve fibers, including thinly myelinated or unmyelinated nerve fibers (Aδ and C fibers), which are difficult to evaluate using nerve conduction studies. The textbooks also stated that QST is useful for the early detection of nerve injury at the asymptomatic stage in conditions such as diabetic neuropathy and small fiber neuropathy, and that it is used as an adjunctive test to complement the limitations of nerve conduction studies. The test components include assessment of the vibration perception threshold, which evaluates large nerve fibers; the thermal threshold, which evaluates small nerve fibers; and the current perception threshold, which separately evaluates unmyelinated, small, and large nerve fibers. These components allow quantitative assessment of the extent and severity of nerve injury as well as functional changes. However, no specific description of the current perception threshold (electrical pain threshold) was identified in the reviewed textbooks. Clinical Practice Guideline Review A review of 14 domestic and international clinical practice guidelines found that the quantitative sensory test was presented as an adjunctive diagnostic test for evaluating sensory function and the degree of nerve injury in patients with peripheral neuropathy, including diabetic peripheral neuropathy and small fiber neuropathy. QST was described as a test that can objectively assess not only negative sensory abnormalities, such as sensory loss or reduced pain perception, but also positive sensory abnormalities, such as allodynia and hyperalgesia, by quantifying sensory thresholds for stimuli including vibration, temperature, current perception, and pain perception. However, most clinical practice guidelines did not clearly present specific classification criteria for the types of nerve fibers, such as Aβ, Aδ, and C fibers, associated with vibration, temperature, current perception, and pain perception stimuli. Many clinical practice guidelines noted that QST results may be influenced by the examiner’s skill level and the subjective responses of the examinee, and therefore have limitations as a stand-alone diagnostic test. Accordingly, they proposed that QST should be used as an adjunctive test interpreted in conjunction with clinical findings and other tests, such as nerve conduction studies, skin biopsy, and autonomic function tests. Some clinical practice guidelines also stated that QST can be used to quantify sensory thresholds and document a sensory profile that systematically records the overall pattern and characteristics of a patient’s sensory function, and that this information can be used to assess treatment response and prognosis. However, no specific description of the current perception threshold (electrical pain threshold) was identified in the reviewed clinical practice guidelines. Conclusions and Recommendations Based on the current assessment results, the Health Technology Reassessment Subcommittee presented the following conclusions and opinions. The quantitative sensory test is presented in textbooks and clinical practice guidelines as a test for identifying abnormalities in sensory function among patients with peripheral neuropathy. In particular, it has complementary clinical value because pain perception varies among individuals and because the test can measure small myelinated and unmyelinated nerve fibers that are difficult to assess using nerve conduction studies. However, despite the advantage of presenting patients’ subjective symptoms as objective measurements, the Subcommittee considered that the test has several limitations, including interindividual differences in pain thresholds, inconsistency in measured values across devices, low sensitivity and specificity of individual tests, and variability in results depending on examiner proficiency. Accordingly, the Subcommittee considered that the quantitative sensory test can be used as a complementary test for identifying sensory nerve abnormalities in patients with peripheral neuropathy, rather than as a stand-alone test. The Subcommittee also recommended further research to establish population-specific reference values by age, sex, race, and other factors, and to evaluate the reliability and validity of the test by disease category in order to standardize the testing method. Among quantitative sensory tests, the current perception threshold (electrical pain threshold) is a test that measures the pain threshold through current perception threshold testing. Although it can present patients’ subjective symptoms as objective measurements, the Subcommittee considered that it has limitations as a stand-alone test and that its future use is expected to decrease because the relevant medical device is no longer being manufactured. The 1st Reassessment Expert Committee meeting in 2026, held on January 9, 2026, deliberated on the “quantitative sensory test – current perception threshold (electrical pain threshold)” based on the Subcommittee’s review findings and approved the Subcommittee’s conclusion as originally proposed.
