초록

평가배경 비디오 전기안진검사(Video-Nystagmography, VNG)는 어지럼 환자에서 머리 운동에 따른 안구의 움직임을 적외선 비디오를 이용하여 측정 및 분석함으로써 전정기능을 정량화하는 검사이다. 동 기술은 신의료기술평가제도 도입 이전인 2001년에 비급여로 등재된 항목으로, 유관기관의 수요조사를 통해 재평가 주제로 선정되었다. 이에 2025년 제4차 의료기술재평가위원회(2025.4.11)에서 재평가 수행 및 소위원회 구성안에 대한 심의를 거쳐 재평가를 수행하였다. 평가목적 본 평가의 목적은 비디오 전기안진검사가 어지럼을 호소하는 환자에서 안구운동을 평가하여 전정기능장애 감별에 보조적으로 도움을 주는 검사로서 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고, 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하는 데 있다. 평가방법 본 평가는 평가목적에 따라 “어지럼검사 공동 소위원회(이하 ‘소위원회’)”의 논의를 거쳐 확정하였으며, 소위원회는 의료기술재평가위원회에서 심의한 평가계획에 따라 이비인후과 2인, 신경과 1인, 근거기반의학 1인, 총 4인으로 구성하였다. 소위원회는 비디오 전기안진검사가 다양한 어지럼검사 중 하나로서 특정 비교검사를 설정하여 상대적 효과를 평가하기 어려운 특성이 있고, 임상에서 단독으로 사용되기보다는 여러 전정기능검사와 함께 상호보완적으로 활용된다는 점을 고려할 때, 체계적 문헌고찰 방식보다는 문헌검토 방식이 적절하다고 보았다. 또한 어지럼 관련 검사는 특정 질환에 한정되어 사용되기보다 임상적 관찰 및 병태생리 평가과정에서 활용되므로 적응증 관련 평가범위 설정에 어려움이 있다는 점도 함께 고려되었다. 이에 대상 적응증 확인 및 평가기준 설정을 위해 국내 3개 관련 학회(대한평형의학회·대한이비인후과학회·대한신경과학회) 의견과 국내 의학 교과서 5편, 국제평형의학회(Bárány Society)의 전문가 합의문 18편을 검토 자료로 선정하여 동 검사의 임상적 안전성 및 효과성을 확인하기로 하였다. 특히 대한평형의학회 최신 교과서 『어지럼과 현훈』 제3판(2025)은 국제평형의학회의 최신 진단기준을 반영하고 있고. 어지럼 관련 질환을 총 23개로 체계적으로 정리하고 있어 이를 주된 기준(reference source)으로 삼아, 해당 23개 질환을 어지럼 증상 특성에 따라 말초성 질환(11개), 중추성 질환(6개), 비전정성 질환(3개) 및 기타 질환(3개)으로 구분하여 정리하였다. 평가는 안전성과 효과성으로 구분하여 수행하였으며, 안전성은 검사 시행과 관련된 이상반응 및 사용 시 주의사항을 중심으로, 효과성은 23개 질환별 임상적 특성과 진단기준을 정리한 후 동 검사의 임상적 활용 가능성을 중심으로 검토하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 비디오 전기안진검사의 안전성은 검사 시행과 관련된 이상반응 및 주의사항을 중심으로, 효과성은 어지럼 관련 23개 질환을 말초성 질환, 중추성 질환, 비전정성 질환 및 기타 질환으로 구분하여 각 질환별 임상적 특성과 진단기준을 정리한 후, 동 검사의 임상적 활용 가능성을 중심으로 검토하였다. 안전성 비디오 전기안진검사는 검사 과정에서 어지럼, 구역감, 피로감 등이 나타날 수 있으나 대부분 일과성이거나 경미한 수준으로 보고되었으며, 이 외 중대한 이상반응이나 합병증에 대한 보고는 확인할 수 없었다. 동 검사는 전극과 같은 소모품이 별도로 필요하지 않아 검사 부담이 비교적 적고, 환자와 검사자 모두에게 편의성이 높은 검사로 확인되였다. 다만 벤조디아제핀·디아제팜 등의 진정제나 기타 신경안정제 및 전정억제제 복용 시 검사 결과의 신뢰도가 저하될 수 있어, 검사 전 약 24~48시간 동안 약물 복용 여부를 확인하는 등 주의가 필요하다. 효과성 비디오 전기안진검사는 적외선 카메라를 이용해 안구 움직임을 기록하고, 안진의 방향, 형태, 속도 및 지속시간을 정량적으로 분석함으로써 전정기능 이상을 평가하는 검사로 다양한 어지럼 질환에서 전정기능 평가에 활용되고 있다. 검토 결과, 전정질환 평가는 단일 검사로 이루어지기보다 안진, 전정안반사, 자세 조절 등 서로 다른 기능 영역을 평가하는 검사들이 상호보완적으로 활용되는 특성이 있으며, 비디오 전기안진검사는 이와 같은 평가 과정에서 안진을 기반으로 전정기능 이상을 확인하는 기본적인 검사로 활용된다. 일부 질환에서는 검사 결과가 정상 또는 비특이적으로 나타나더라도 감별진단 과정에서 임상적으로 의미있는 정보를 제공할 수 있다. 말초성 질환에서 양성 돌발성 체위성 현훈의 경우 비디오 전기안진검사를 통해 두위변환 시 유발되는 체위성 안진을 관찰함으로써 진단에 직접 활용되며, 안진의 방향을 통해 병변 반고리관을 추정하는 데 이용된다. 급성 일측 전정병증에서는 비디오 전기안진검사를 이용하여 자발안진 및 온도안진반응을 평가함으로써 병변측 전정기능 저하를 판단하는 데 유용하며, 메니에르병에서는 발작기와 비발작기 사이의 전정기능 변동을 평가하는 데 참고된다. 또한 상반고리관피열증후군에서는 비디오 전기안진검사로 소리 또는 압력 자극에 의해 유발되는 안진을 관찰하여 진단에 직접 활용되며, 외상성 어지럼이나 외림프누공 등에서는 자발안진 및 체위안진을 확인하여 다른 전정질환과의 감별 과정에 활용되고, 양측 전정병증의 경우에는 다른 전정기능검사와 함께 전정기능 저하를 평가하는 과정에서 활용된다. 중추성 질환에서는 비디오 전기안진검사를 통해 다양한 안진 양상을 분석하여 말초성 질환과의 감별에 유용하게 활용된다. 전정편두통, 뇌혈관성 어지럼, 소뇌 및 뇌간 병변 등에서는 주시유발안진, 방향변환안진, 혼합안진 등 중추성 안진 소견을 확인할 수 있어 병소의 위치나 중추성 병변의 가능성을 추정하는 데 참고되고, 중추체위현훈의 경우에는 체위 변화에 따른 비전형적인 안진 양상을 분석하여 말초성 체위성 현훈과 구분하는 데 활용된다. 비전정성 및 기타 질환에서는 비디오 전기안진검사가 주로 감별진단 과정에서 활용된다. 지속체위지각어지럼과 같은 기능성 어지럼에서는 일반적으로 특이적인 안진 소견이 관찰되지 않으며 다른 전정질환을 배제하는 데 참고되고, 기립어지럼이나 내과적 질환과 관련된 어지럼에서도 전정질환과의 구분을 위해 다른 검사와 함께 종합적으로 해석된다. 소아 어지럼에서는 전정기능검사의 일환으로 비디오 전기안진검사를 통해 자발안진 및 유발안진을 기록하여 전정기능 평가에 활용되며, 노인 어지럼에서는 비디오 전기안진검사가 다양한 전정기능검사와 함께 전정질환 및 중추성 원인 감별을 위한 평가 과정에서 활용된다. 결론 및 권고결정 의료기술재평가 소위원회에서는 현재 확인된 평가결과에 근거하여 다음과 같은 의견을 제시하였다. 안전성의 경우, 비디오 전기안진검사는 주요 이상반응으로 어지럼, 구역감, 피로감 등 경미하고 일과성인 증상이 주로 언급되고 있으며, 중대한 이상반응 및 합병증에 대한 언급은 확인할 수 없었다. 이에 의료기술재평가 소위원회에서는 동 검사가 비침습적이고 방사선 노출이 없는 검사로 반복 시행이 가능하며, 표준화된 검사 절차를 준수하고 검사 과정에서의 주의사항을 고려하여 숙련된 검사자에 의해 시행될 경우 임상적 안전성은 충분히 확보될 수 있는 검사라는 의견을 제시하였다. 효과성의 경우, 비디오 전기안진검사는 안진의 방향, 형태 및 지속시간을 정량적으로 분석하여 말초성 및 중추성 어지럼을 감별하는 진단 보조검사로서, 말초 전정 질환에서는 병변 반고리관 또는 병변 측 전정 기능 이상을 추정하는 데 유용하고, 중추성 또는 비전정성 원인이 의심되는 경우에는 중추성 특징을 구분하는 데 활용 가능한 검사로 판단하였다. 또한 비디오 전기안진검사는 진단 및 추적 관찰 과정에서 다른 전정기능검사와 상호보완적으로 사용될 수 있다는 의견을 제시하였다. 이와 같은 임상적 활용 특성을 종합할 때, 소위원회는 비디오 전기안진검사의 적응증을 특정 질환으로 한정하기보다는, 어지럼·현훈 환자에서 임상적 관찰 및 병태생리 평가를 보조하는 진단 보조검사로서 임상적 가치가 있는 기술로 평가하였다. 아울러 일부 질환에서 비디오 전기안진검사의 검사 결과가 정상 또는 특징적 소견이 없더라도, 배제 진단 및 치료 경과 모니터링 측면에서 임상적으로 의미 있는 정보로 해석될 수 있다고 판단하였다. 다만, 본 판단은 2025년 기준 국제평형의학회 및 대한평형의학회 교과서를 포함한 국내 교과서 5편에 제시된 최신 근거를 바탕으로 도출된 결과이므로, 향후 임상진료지침 개정 및 진단기술 발전 등에 따라 비디오 전기안진검사의 검사 적용 질환, 진단 및 판정 기준은 변동될 수 있어 지속적인 확인이 요구된다는 의견을 추가 제시하였다. 또한 해외에서는 어지럼 검사를 전정기능 배터리검사(Vestibular Function Test Battery)로 통합하여 포괄적으로 평가하되, 기본적으로 비디오 전기안진검사를 중심으로 하고 필요 시 회전검사, 동적체평형검사, 비디오두부충동검사 등을 필요에 따라 추가 수행(add-on)하는 체계로 운영되고 있다. 반면 국내에서는 검사항목이 급여·비급여·선별급여로 분리되어 급여항목과 유사한 비디오 기반의 검사들이 비급여나 선별급여 형태로 청구되는 운영 실태가 관찰되고 있다. 이에, 관련 학회들을 중심으로 환자 중심의 진료 경로(clinical pathway)를 반영한 표준화된 검사 프로토콜을 마련할 필요가 있다는 의견을 제시하였다. 또한, 향후 표준화된 진료지침을 활용하여 급여 및 비급여 구조의 개선 가능성 등을 포함한 정책적 검토의 필요성도 함께 제시되었다. 2026년 제1차 재평가전문위원회(2026.1.9.)*는 임상적 안전성과 효과성의 근거 등을 종합적으로 고려하였을 때, 국내 임상 상황에서 어지럼을 호소하는 환자를 대상으로 안구운동을 평가하여 전정기능장애 감별에 보조적으로 도움을 주는 검사로서 ‘비디오 전기안진검사’의 사용을 ‘약하게 권고함’으로 심의하였다. 다만, 본 권고결정은 교과서 등의 문헌 근거를 바탕으로 이루어진 것으로, 향후 국내 환자 중심의 진료 경로를 반영한 표준 검사 프로토콜 확립이 필요하며, 이에 따른 추가적 재평가가 필요하다는 의견이었다.


Background Video-Nystagmography (VNG) is a test that quantifies vestibular function in patients with dizziness by using infrared video to measure and analyze eye movements induced by head motion. This technology was listed as a non-reimbursed item in 2001, before the introduction of the New Health Technology Assessment system. It was selected for reassessment through a needs survey of relevant organizations. Accordingly, the reassessment was conducted after the reassessment plan and subcommittee composition were reviewed at the 4th 2025 Health Technology Reassessment Committee meeting (April 11, 2025). Objective The objective of this reassessment was to provide evidence on whether Video-Nystagmography is clinically safe and effective as a test for evaluating eye movements to aid in differentiating vestibular dysfunction in patients presenting with dizziness, and to determine the appropriate strength of recommendation for the use of this technology. Methods In accordance with the objective of the assessment, this reassessment was finalized through discussions with the “Joint Subcommittee on Dizziness Tests” (hereinafter, “the Subcommittee”), which was composed of four experts—two otolaryngologists, one neurologist, and one evidence-based medicine expert—in accordance with the assessment plan reviewed by the Health Technology Reassessment Committee. The Subcommittee determined that, because Video-Nystagmography is one of several tests for dizziness for which it is difficult to designate a specific comparator to assess relative effectiveness, and because it is generally used in a complementary manner alongside other vestibular function tests rather than as a stand-alone test in clinical practice, a literature review approach was more appropriate than a systematic literature review. It was also considered that dizziness-related tests are used in clinical observation and pathophysiological assessment rather than being confined to specific diseases, making it difficult to define the scope of assessment in relation to indications. Accordingly, to confirm the target indications and establish assessment criteria, the opinions of three relevant domestic societies (the Korean Balance Society, the Korean Society of Otorhinolaryngology-Head and Neck Surgery, and the Korean Neurological Association), five domestic medical textbooks, and 18 expert consensus statements from the Bárány Society were selected as review materials to confirm the clinical safety and effectiveness of this test. In particular, the latest textbook of the Korean Balance Society, Dizziness and Vertigo, 3rd edition (2025), reflects the most recent diagnostic criteria of the Bárány Society and systematically organizes a total of 23 dizziness-related diseases; this textbook was therefore adopted as the primary reference source, and the 23 diseases were classified according to the characteristics of dizziness symptoms into peripheral diseases (11), central diseases (6), non-vestibular diseases (3), and other diseases (3). The assessment was conducted separately for safety and effectiveness: safety was assessed by focusing on adverse events associated with the test and precautions for its use, and effectiveness was assessed by organizing the clinical characteristics and diagnostic criteria for each of the 23 diseases, then examining the test’s clinical applicability. The Health Technology Reassessment Expert Committee determined the final level of recommendation based on the Subcommittee’s review. Results The safety of Video-Nystagmography was assessed by focusing on adverse events and precautions associated with the test, and effectiveness was assessed by classifying the 23 dizziness-related diseases into peripheral, central, non-vestibular, and other diseases, organizing the clinical characteristics and diagnostic criteria for each disease, and then examining the clinical applicability of the test. Safety During Video-Nystagmography, symptoms such as dizziness, nausea, and fatigue may occur. Still, these were mostly transient or mild, and no serious adverse events or complications were reported. This test does not require separate consumables such as electrodes, resulting in a relatively low testing burden, and it has been shown to offer high convenience for both patients and examiners. However, because the reliability of test results may be reduced by the use of sedatives such as benzodiazepines or diazepam, or other tranquilizers and vestibular suppressants, caution is required, including checking medication use for approximately 24–48 hours before the test. Effectiveness Video-Nystagmography records eye movements using an infrared camera and quantitatively analyzes the direction, pattern, velocity, and duration of nystagmus to assess vestibular dysfunction; it is used to evaluate vestibular function in a variety of dizziness-related diseases. The review found that the assessment of vestibular disorders is generally not based on a single test but rather relies on the complementary use of tests that evaluate different functional domains, such as nystagmus, the vestibulo-ocular reflex, and postural control; within this process, Video-Nystagmography is used as a basic test to confirm vestibular dysfunction based on nystagmus. Even when test results are normal or non-specific in some diseases, they can still provide clinically meaningful information during differential diagnosis. Among peripheral diseases, in benign paroxysmal positional vertigo, Video-Nystagmography is used directly for diagnosis by observing positional nystagmus induced by positional changes of the head, and the direction of the nystagmus is used to estimate the affected semicircular canal. In acute unilateral vestibulopathy, it is useful for determining the extent of lesion-side vestibular function decline by assessing spontaneous nystagmus and caloric responses. In Ménière’s disease, it serves as a reference for assessing fluctuations in vestibular function between attacks and inter-attack periods. In superior semicircular canal dehiscence syndrome, Video-Nystagmography is used directly in diagnosis by observing nystagmus induced by sound or pressure stimuli, and in traumatic dizziness or perilymphatic fistula, it is used to identify spontaneous and positional nystagmus to aid differentiation from other vestibular disorders; in bilateral vestibulopathy, it is used together with other vestibular function tests in the process of assessing vestibular function decline. In central diseases, Video-Nystagmography is useful for differentiating central from peripheral diseases by analyzing various nystagmus patterns. In vestibular migraine, cerebrovascular dizziness, and cerebellar or brainstem lesions, central nystagmus findings such as gaze-evoked nystagmus, direction-changing nystagmus, and mixed nystagmus can be identified, providing a reference for estimating the location of the lesion or the possibility of a central lesion; in central positional vertigo, atypical nystagmus patterns associated with positional changes are analyzed to differentiate it from peripheral positional vertigo. For non-vestibular and other diseases, Video-Nystagmography is mainly used in the process of differential diagnosis. In functional dizziness such as persistent postural-perceptual dizziness (PPPD), specific nystagmus findings are generally not observed, and the test is used as a reference to exclude other vestibular disorders; in dizziness related to orthostatic conditions or medical illness, it is also interpreted comprehensively together with other tests to differentiate it from vestibular disease. In pediatric dizziness, Video-Nystagmography is used as part of vestibular function testing to record spontaneous and evoked nystagmus and assess vestibular function. In dizziness in the elderly, it is used alongside other vestibular function tests to differentiate vestibular disease from central causes. Conclusions and Recommendations Based on the currently available assessment results, the Health Technology Reassessment Subcommittee presented the following opinions. In terms of safety, the main adverse events reported for Video-Nystagmography were mild, transient symptoms such as dizziness, nausea, and fatigue; no serious adverse events or complications were reported. Accordingly, the Subcommittee determined that, as this test is non-invasive, involves no radiation exposure, and can be repeated, its clinical safety can be sufficiently ensured provided that standardized test procedures are followed, relevant precautions are observed, and the test is performed by a skilled examiner. In terms of effectiveness, the Subcommittee determined that Video-Nystagmography, as a diagnostic aid that quantitatively analyzes the direction, pattern, and duration of nystagmus to differentiate peripheral from central dizziness, is useful for estimating the affected semicircular canal or lesion-side vestibular dysfunction in peripheral vestibular disorders, and can be used to identify central features when a central or non-vestibular cause is suspected. The Subcommittee further determined that Video-Nystagmography can be used in a complementary manner alongside other vestibular function tests during diagnosis and follow-up. Taken together, these clinical usage characteristics led the Subcommittee to assess Video-Nystagmography as a technology of clinical value—not as a diagnostic test limited to specific diseases, but as an auxiliary diagnostic test that assists clinical observation and pathophysiological assessment in patients with dizziness/vertigo. The Subcommittee further determined that, even when the results of Video-Nystagmography are normal or show no characteristic findings in some diseases, they can still be interpreted as clinically meaningful information for exclusionary diagnosis and monitoring of treatment course. However, because this assessment was based on the most recent evidence presented in five domestic textbooks, including the 2025 textbooks of the Bárány Society and the Korean Balance Society, the Subcommittee additionally noted that the diseases to which Video-Nystagmography applies, as well as the diagnostic and determination criteria, may change in the future as clinical practice guidelines are revised and diagnostic technology advances, and that continued monitoring is therefore required. The Subcommittee also noted that, internationally, dizziness tests are comprehensively assessed as part of an integrated Vestibular Function Test Battery, which is generally centered on Video-Nystagmography, with the rotary chair test, dynamic posturography, and the Video Head Impulse Test added as needed. In Korea, by contrast, test items are separated into reimbursed, non-reimbursed, and selective benefit categories, and it has been observed that video-based tests similar to reimbursed items are being billed as non-reimbursed or selective benefit items. The Subcommittee therefore expressed the opinion that relevant societies should take the lead in developing a standardized test protocol that reflects a patient-centered clinical pathway. Further, it noted the need for policy review, including the potential to improve the structure of reimbursed and non-reimbursed items, using standardized clinical practice guidelines in the future. At the 1st 2026 Health Technology Reassessment Expert Committee meeting (January 9, 2026),* after comprehensively considering the evidence on the clinical safety and effectiveness of the technology, the Committee determined that the use of Video-Nystagmography—as a test that evaluates eye movements to aid in the differentiation of vestibular dysfunction in patients presenting with dizziness in the Korean clinical setting—should be classified as “Weak Recommendation.” However, the Committee noted that this recommendation was based on evidence from the literature, such as textbooks, and that establishing a standardized test protocol that reflects a patient-centered clinical pathway in Korea, along with a subsequent reassessment based on that protocol, will be necessary in the future.
