초록

평가배경 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’은 암 환자에서 테모졸로마이드 등 알킬화 항암제의 반응성을 예측하기 위해 O-6-메틸구아닌-디옥시리보핵산메틸 전이효소(O6-methylguanine-DNA methyltransferase, MGMT) 유전자의 메틸화 여부를 평가하는 검사이다. 본 검사는 메틸화특이중합효소연쇄반응(Methylation-specific polymerase chain reaction, MSP)을 이용하여 MGMT 유전자의 촉진자(promoter) 부위 메틸화를 확인하는 방법이다. 본 검사는 신의료기술평가제도 도입 이전인 2006년 비급여 항목으로 처음 도입되었으며(보건복지부 고시 제2006-91호, 2006.11.22.), 이후 2016년 ‘사람유전자 검사 급여전환 및 비급여 목록 정비’ 시행에 따라 현재의 고시명으로 변경되어 현재까지 사용되고 있다(보건복지부 고시 제2016-229호, 2016.12.8.). 국내에서 본 검사에 대한 의료기술평가가 수행된 바는 없었다. ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’은 내부모니터링을 통해 의료기술재평가 안건으로 선정되었다. 동 검사의 안전성 및 효과성에 대한 근거를 확인하고자, 2024년 제6차 의료기술재평가위원회(2024.6.14.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 ‘뇌신경교종 최초 진단 환자’에서 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’는 MGMT 유전자 촉진자(promoter)의 메틸화를 확인하여 표준치료(방사선 치료와 테모졸로마이드 병용요법)의 예후를 예측하기 위한 목적으로 사용 시 안전하고 효과적인지에 대한 근거를 제공하고 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 뇌신경교종 최초 진단 환자 중 표준치료를 시행한 환자에서 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’의 안전성 및 효과성을 재평가하기 위하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자) 소위원회(이하 ‘소위원회’라 한다)’의 논의를 거쳐 확정하였다. 소위원회는 신경외과 2인, 병리과 1인, 진단검사의학과 1인, 혈액종양내과 1인, 방사선 종양학과 1인, 근거기반의학 1인의 전문가 7인으로 구성하였다. 본 평가의 대상자는 뇌신경교종 최초 진단 환자 중 표준치료(방사선 치료와 테모졸로마이드 병용요법)를 수행한 환자로 결정하였다. 비교검사는 현재 국민건강보험요양급여에 등재되어 있는 검사인 ‘메틸화 특이적 다중결찰의존프로브증폭(Methylation-specific Multiplex ligation-dependent probe amplification, MS-MLPA)’, ‘면역조직(세포)화학 검사[종목당](Immunohistochemistry, IHC)’로 결정하였다. 결과변수는 안전성과 효과성을 확인하였고, 안전성은 합병증 및 부작용 등으로 결정하였다. 효과성의 일차지표는 생존기간 및 생존율(전체 혹은 무진행), 사망위험으로 결정하였고, 이차지표는 예측정확도 등 예측관련 결과지표, 메틸화 검출률, 검사 간 일치율로 결정하였다. 소위원회에서는 다른 검사와의 비교를 통해 중재검사의 효과를 확인하는 것이 적절하다고 판단하여 연구유형을 비교연구만 포함하기로 결정하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2024. 7. 30.). 최종 선택된 연구의 비뚤림위험 평가는 연구설계, 분석방법에서 예후요인의 효과추정치에 대한 결함 정도를 평가하고자 Quality In Prognostic factor Studies (QUIPS)를 사용하여 두 명의 검토자가 독립적으로 시행하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 표로 제시하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과 최종 선택된 연구는 총 6편(대상자 수, 총 364명)이었고, 연도별로 2007년부터 2013년까지 출판되었다. 연구유형은 6편 모두 코호트 연구이었다. 대상질환은 6편 중 5편에서 교모세포종, 1편에서는 뇌신경교종이었고, 교모세포종을 대상으로 한 5편의 연구 중 1편에서는 소아를 대상으로 하였다. 비교검사별로는 MSP와 MS-MLPA를 비교한 연구 2편, MSP와 IHC를 비교한 연구 2편, MSP와 MS-MLPA, IHC 둘 다 비교한 연구가 2편이었다. 연구의 비뚤림위험 평가결과, 일부 연구에서 ‘다른 예후인자에 대한 보정’ 영역의 비뚤림위험이 높은 것으로 평가하였으나, 대부분의 영역에서 비뚤림위험이 낮음 혹은 중등도로 평가하였다. 안전성 MSP의 안전성은 합병증 또는 이상반응으로 평가하고자 하였으나, 이를 보고한 연구는 없었다. 효과성 뇌신경교종 최초 진단 환자에서 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’의 효과성은 MS-MLPA와 IHC 두 가지 검사를 비교하여 생존기간 및 생존율(전체(overall) 혹은 무진행(progression-free)), 사망위험, 예측관련 결과지표, 메틸화 검출률, 검사 간 일치율로 평가하였다. 생존기간 및 생존율 MSP와 MS-MLPA를 비교한 생존기간 및 생존율은 4편에서 보고되었다. 전체 생존기간을 평가한 3편 중 1편에서는 MSP군에서 메틸화 환자가 비메틸화 환자보다 유의하게 생존기간이 길었으나(p<0.04), MS-MLPA군에서는 모든 임계값에서 유의한 차이가 없었다(cut-off=3.6, 10.4, 12.8, p>0.05). 다른 1편에서는 MSP로 확인한 메틸화 환자의 전체 생존기간이 평균 28.29개월(95% Confidance Interval (CI) 21.84~34.73), 비메틸화 환자는 평균 16.31개월(95% CI 12.75~19.86)로 유의한 차이가 있었으나 MS-MLPA군에서 메틸화 환자는 평균 18.85개월(95% CI 15.75~21.94), 비메틸화 환자는 평균 16.77개월(95% CI 12.21~22.41)로 유의한 차이가 없었다. 3편에서 MSP군과 MS-MLPA군 간 전체 생존기간에 대한 차이는 유의하지 않거나 통계적 언급이 없었다. 소아 교모세포종을 대상으로 한 연구 1편에서는 MSP로 확인된 메틸화 환자가 없어 비교가 불가능했다. 무진행 생존기간은 2편에서 보고하였고, 1편에서는 MSP군에서 메틸화 환자가 비메틸화 환자보다 유의하게 생존기간이 길었다(p<0.001). MS-MLPA군에서는 임계값 3.6에서만 메틸화 환자가 비메틸화 환자보다 유의하게 연장되었지만(p=0.002), MSP군과 MS-MLPA군 간 차이는 언급이 없었다. 소아 교모세포종 연구에서도 MSP로 확인된 메틸화 환자가 없어 비교할 수 없었다. 생존율은 1편에서 60개월 시점에 MSP군과 MS-MLPA군 모두 메틸화 환자에서 비메틸화 환자보다 생존율이 높은 경향이 있었고, 두 군 간 생존율이 비슷하였다. MSP와 IHC와 비교한 생존기간 및 생존율은 2편에서 보고하였다. 전체 생존기간을 보고한 2편 중 1편에서 MSP군의 메틸화 환자는 중간값 21.5개월, 비메틸화 환자는 중간값 15.2개월이었고, IHC군의 메틸화 환자는 중간값 16개월, 비메틸화 환자는 중간값 12.8개월이었다. 전체 생존기간은 MSP군(p=0.003)과 IHC(p=0.040)군 모두 메틸화 환자가 비메틸화 환자보다 더 길었다. 다른 1편의 전체 생존기간에서 MSP군의 메틸화 환자는 중간값 22.4개월, 비메틸화 환자는 중간값 14.9개월, IHC군의 메틸화 환자는 중간값 22.4개월, 비메틸화 환자는 중간값 14.6개월로 나타나 MSP군(p=0.000081)과 IHC군(p=0.00051) 모두 메틸화 환자가 비메틸화 환자보다 더 길었다. 그러나 2편 모두 두 군 간 차이는 언급이 없었다. 무진행 생존기간은 1편에서 보고되었으며, MSP군에서 메틸화 환자가 12.2개월, 비메틸화 환자가 9.1개월(p=0.0024), IHC군에서 메틸화 환자가 12.2개월, 비메틸화 환자가 9개월(p=0.036)로, 두 검사 모두에서 메틸화 환자의 무진행 생존기간이 더 길었으나, 두 군 간 유의성에 대한 언급은 없었다. 생존율은 1편에서 24개월 시점 결과가 보고되었으며, MSP군에서 메틸화 환자가 41.5%, 비메틸화 환자가 7.8%로 메틸화 환자가 비메틸화 환자보다 유의하게 높았다(p<0.05). IHC군에서는 메틸화 환자가 29.9%, 비메틸화 환자가 17.2%로 메틸화 환자가 비메틸화 환자보다 더 높았으나, 두 군 간 유의한 차이는 언급이 없었다. 사망위험 사망위험은 MSP와 IHC를 비교한 1편에서 위험비(Hazard ratio, HR)로 보고하였다. MSP군의 메틸화 환자는 비메틸화 환자보다 사망위험이 58.7% 낮았다(HR 0.413, 95% CI 0.189~0.904, p=0.027). IHC군에서는 메틸화 환자에서 비메틸화 환자보다 사망위험이 5.1% 낮았지만 통계적으로 유의한 차이는 없었다(HR 0.949, 95% CI 0.433~2.080, p=0.896). 예측관련 결과지표 예측관련 결과지표는 MSP와 MS-MLPA를 비교한 연구는 1편이었다. 그 결과 18개월 시점에서 무진행 혹은 전체 생존기간에 대한 예측오차는 MSP가 MS-MLPA보다 낮은 경향이 있었다. MSP와 IHC를 비교한 예측정확도는 1편에서 최소 6개월 이상의 추적관찰 후 테모졸로마이드 치료 반응으로 보고 하였다. 테모졸로마이드 치료 반응은 최소 6개월 이상의 추적 관찰 후 촬영된 최신 Magnetic Resonance Imaging (MRI) 영상과 첫 수술 후 촬영된 MRI 영상을 비교하여 정의되었다. 그 결과 MSP는 민감도 85%, 특이도 17%, 양성예측도 77.3%, 음성예측도 79.9%이었고, IHC는 민감도 95.2%, 특이도 66.7%, 양성예측도 78.4%, 음성예측도 91.7%이었다. MSP는 IHC보다 예측정확도가 낮았다. 메틸화 검출률 메틸화 검출률을 보고한 연구는 총 7편이었다. MSP와 MS-MLPA를 비교한 결과는 총 4편이었고, MS-MLPA의 메틸화 임계값은 0.1~0.3 범위이었다. MSP의 메틸화 검출률은 0%~42%(14/33), MS-MLPA는 9%(3/34)~47.9%(23/48)로 다양하게 나타났다. MSP와 IHC를 비교한 연구 3편에서는 MSP의 메틸화 검출률이 31.6%(12/38)~63%(58/92), IHC는 59%(58/99)~76.3%(29/38)로 보고하였다. 메틸화 검사 간 일치율 MSP와 MS-MLPA와 비교하여 메틸화 검사 간 일치율은 2편에서 보고하였다. 1편에서는 80%의 일치율을, 다른 1편에서는 검사 간 일치율이 높았다고 하였다(Cohen's Kappa k=0.84). MSP와 IHC를 비교한 3편 중 1편에서는 83.7%, 다른 1편에서는 39.5%의 일치율을 보였다. 이외 다른 1편에서는 MSP와 IHC에서 메틸화 검사는 두 군 간 상관관계가 낮다고 보고 하였다. 결론 및 권고결정 의료기술재평가 소위원회에서는 뇌신경교종 최초 진단 환자에서 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’에 대해 현재 평가결과에 근거하여 다음과 같이 제언하였다. 체계적 문헌고찰 결과 동 검사의 안전성은 합병증 또는 이상반응으로 평가하고자 하였으나, 안전성 결과를 보고한 연구는 없었다. 그러나 동 검사는 체외에서 시행하는 검사이므로 직접적인 위해를 가하지 않아 안전하다는 의견이었다. 효과성의 경우 생존기간 및 생존율에서 MSP와 비교검사(MS-MLPA 혹은 IHC)의 MGMT 유전자 촉진자 메틸화 환자가 비메틸화 환자보다 유의하게 길거나 향상되었지만, 검사 간 비교 시 유의한 차이가 없거나 유의성을 보고하지 않았다. 사망위험은 MSP군에서 메틸화 환자가 비메틸화 환자보다 더 낮았고, IHC군에서는 차이가 없으며, 두 군 간 통계적 유의성에 대한 언급은 없었다. 예측관련 결과는 6개월 시점의 테모졸로마이드 치료반응을 확인하는 데 MSP가 IHC에 비해 예측정확도가 낮았으나, 생존기간 예측 오차에서는 MSP가 MS-MLPA보다 더 작은 경향이 있었다. 국내 임상 상황에서 MGMT 유전자 촉진자의 메틸화 여부를 확인하기 위해 MS-MLPA는 숙련된 전문가가 필요하고, IHC는 검사자의 판독에 따른 차이가 발생할 수 있는 단점이 있다. 이에 반해 MSP는 PCR방법으로 편의성이 있고, 검사시간이 짧아 많은 환자에게 정확한 정보를 제공할 수 있다는 의견을 제시하였다. 이에 소위원회에서는 뇌신경교종 최초 진단 환자를 대상으로 MGMT 유전자 촉진자(promoter)의 메틸화를 확인하여 표준치료(방사선 치료와 테모졸로마이드 병용요법)의 예후를 예측하기 위한 목적으로 사용 시 안전하고, 비교검사 대비 유사하거나 효과적인 검사로 평가하였다. 2025년 제2차 의료기술재평가위원회(2024.2.14.)는 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상 상황에서 뇌신경교종 최초 진단 환자를 대상으로 표준치료(방사선 치료와 테모졸로마이드 병용요법)의 예후를 예측하기 위해 MGMT 유전자 촉진자의 메틸화를 확인하는 목적으로 사용 시 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’ 검사 사용을 ‘권고함’으로 심의하였다. 또한 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응’은 검사법의 명칭이고 본 평가에서는 뇌신경교종 MGMT 유전자 촉진자에서 동 검사의 효과를 평가한 것이므로 본 평가의 안건명을 ‘사람유전자 분자병리 검사-비유전성 유전자 검사-메틸화특이중합효소연쇄반응(MGMT 유전자 촉진자)’으로 수정하기로 결정하였다.


Background The Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter) is a test to evaluate the methylation status of the O6-methylguanine-DNA methyltransferase (MGMT) gene; the results are used to predict responsiveness to alkylating agents such as temozolomide in patients with cancer. This test identifies methylation in the promoter region of the MGMT gene using a methylation-specific polymerase chain reaction (MSP). The test was first introduced as a non-reimbursed item in 2006 prior to the implementation of the New Health Technology Assessment system (Ministry of Health and Welfare Bulletin No. 2006-91, November 22, 2006); it was later reclassified under its current name following systematic revision of the reimbursement system for human gene tests (Ministry of Health and Welfare Bulletin No. 2016-229, December 8, 2016). A health technology assessment has not previously been conducted on this test in Korea. Internal monitoring selected this technology for reassessment to evaluate the evidence on its clinical safety and effectiveness. The reassessment plan and subcommittee composition were reviewed and approved at the 6th 2024 Health Technology Reassessment Committee meeting (June 14, 2024). Objective This reassessment was conducted to provide scientific evidence on the clinical safety and effectiveness of the Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter) in predicting the prognosis of standard treatment (concurrent radiotherapy and temozolomide) by identifying methylation of the MGMT gene promoter and to provide the strength of recommendation for the use of this technology. Methods A systematic literature review was conducted to assess the clinical safety and effectiveness of the Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter) in newly diagnosed patients with neuroglial tumors who underwent standard therapy. The Subcommittee on the Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter) (hereinafter referred to as “the Subcommittee”) held discussions to finalize all assessment methods. The Subcommittee comprised seven members: two neurosurgeons, one pathologist, one laboratory medicine specialist, one hemato-oncologist, one radiation oncologist, and one evidence-based medicine expert. The target population was defined as newly diagnosed patients with neuroglial tumors who received standard therapy (concurrent radiotherapy and temozolomide). Comparators were selected from currently reimbursable tests under the National Health Insurance System: methylation-specific multiplex ligation-dependent probe amplification (MS-MLPA) and immunohistochemistry (IHC). Outcome measures included safety and effectiveness. Safety was assessed based on complications and adverse events. For effectiveness, primary outcomes included overall survival (OS) or progression-free survival (PFS) and mortality risk; secondary outcomes included prognostic accuracy measures, methylation detection rates, and agreement between tests. The Subcommittee determined that including only comparative studies was appropriate for evaluating the effectiveness of the index test. The literature search was conducted using three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews - Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, KMbase, and RISS). The final search date was July 30, 2024. Risk of bias in the included studies was assessed using the Quality In Prognostic factor Studies (QUIPS) tool, which evaluates the degree of potential bias in prognostic factor effect estimates based on study design and analysis. Data extraction was performed using a pre-specified form, and results were tabulated. All processes were conducted independently by two reviewers, and discrepancies were resolved by reaching a consensus. The final recommendation was determined by the Health Technology Reassessment Committee based on the findings of the Subcommittee. Results The systematic literature search identified six studies (n = 364) published between 2007 and 2013; all six were cohort studies. Of these, five included patients with glioblastoma, and one included patients with glioma of the brain . One of the glioblastoma studies involved pediatric patients. Two studies compared MSP with MS-MLPA, two compared MSP with IHC, and two compared MSP with both MS-MLPA and IHC. In the risk of bias assessments, some studies showed a high risk of bias in the “adjustment for other prognostic factors” domain; however, most studies were rated as having low to moderate risk across all other domains. Safety No studies reported outcomes on complications or adverse events associated with MSP. Effectiveness The effectiveness of the Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter) was assessed by comparing it with MS-MLPA and IHC. Effectiveness was analyzed based on outcomes, including survival, mortality risk, predictive performance, methylation detection rate, and agreement between tests. Survival Four studies reported overall survival (OS) and survival rate comparing MSP and MS-MLPA. Among three studies reporting OS, one found significantly longer OS in the MSP-detected methylated group than in the unmethylated group (p < 0.04), whereas the MS-MLPA groups showed no significant differences across all cutoff values (cut-off = 3.6, 10.4, and 12.8; p > 0.05). One study reported that the mean OS was significantly longer in the MSP-detected methylated group (28.29 months, 95% confidence interval (CI): 21.84–34.73) than in the unmethylated group (16.31 months, 95% CI: 12.75–19.86). However in the MS-MLPA group, the mean OS was 18.85 months (95% CI: 15.75–21.94) in methylated patients and 16.77 months (95% CI: 12.21–22.41) in unmethylated patients, with no significant difference. In the third study, no statistically significant difference was observed between MSP and MS-MLPA in OS, or statistical results were not reported. One study on pediatric glioblastoma included no patients with MSP-detected methylated promoters, so a comparison could not be made. PFS was reported in two studies. In one, the MSP-detected methylated group had significantly longer survival than the unmethylated group (p < 0.001); in contrast, the MS-MLPA-detected methylated group showed significantly longer survival only at the 3.6 cutoff value (p = 0.002), although no direct comparisons between the MSP and MS-MLPA groups were reported. The pediatric glioblastoma study also did not include patients with MSP-detected methylated promoters, so a comparison could not be made. One study reported survival rates at 60 months, showing that patients with methylated promoters tended to have higher survival rates than those with unmethylated promoters in both the MSP and MS-MLPA groups, with similar rates in both groups. Two studies compared OS and survival rates between MSP and IHC. Among two studies that reported OS, one study reported a median OS of 21.5 months in the MSP-detected methylated group and 15.2 months in the unmethylated group, and 16 months vs. 12.8 months in the IHC-detected groups, respectively. Patients with methylated promoters had longer survival in both the MSP (p = 0.003) and IHC (p = 0.040) groups. In the other study, the median OS was 22.4 months in the MSP-detected methylated group and 14.9 months in the unmethylated group, and 22.4 months vs. 14.6 months in the IHC-detected groups, respectively. Survival was also significantly longer in the methylated groups for both MSP (p = 0.000081) and IHC (p = 0.00051) testing. However, neither study reported a direct comparison between the MSP and IHC groups. PFS was reported in one study. In the MSP group, the median PFS was 12.2 months in patients with methylated promoters and 9.1 months in those with unmethylated promoters (p = 0.0024); in the IHC group, the median PFS was 12.2 months and 9.0 months, respectively (p = 0.036). Methylation was associated with longer PFS in both test groups, but statistical significance was not reported for differences between the testing methods. One study reported 24-month survival rates. These rates were significantly higher in MSP-detected methylated group than in the unmethylated group (41.5% vs. 7.8%, p < 0.05). In the IHC group, the survival rate was 29.9% in patients with methylated promoters and 17.2% in those with unmethylated promoters, although statistical significance was not reported. Mortality risk One study comparing MSP and IHC reported hazard ratios (HR) for mortality. In the MSP group, the risk of mortality was 58.7% lower in patients with methylated promoters than in those with unmethylated promoters (HR = 0.413, 95% CI: 0.189–0.904, p = 0.027). In the IHC group, the risk of mortality was 5.1% lower in patients with methylated promoters, although the difference was not statistically significant (HR = 0.949, 95% CI: 0.433–2.080, p = 0.896). Predictive outcomes Predictive outcomes were reported in one study comparing MSP and MS-MLPA. At 18 months, MSP showed a tendency for lower prediction errors for PFS and OS compared with MS-MLPA. One study comparing MSP and IHC evaluated predictive accuracy based on response to temozolomide after a minimum of six months of follow-up. Treatment response was defined by comparing the most recent MRI obtained after at least six months of follow-up with the initial post-surgical MRI. MSP showed a sensitivity of 85%, specificity of 17%, positive predictive value (PPV) of 77.3%, and negative predictive value (NPV) of 79.9%; IHC demonstrated a sensitivity of 95.2%, specificity of 66.7%, PPV of 78.4%, and NPV of 91.7%. Overall, MSP had a lower predictive accuracy than IHC. Methylation detection rates Methylation detection rates were reported in seven studies. Four studies compared MSP and MS-MLPA, with MS-MLPA methylation thresholds ranging from 0.1 to 0.3. The methylation detection rate ranged from 0%–42% (14/33) for MSP, and from 9% (3/34) to 47.9% (23/48) for MS-MLPA. In three studies comparing MSP and IHC, the detection rate for MSP ranged from 31.6% (12/38) to 63% (58/92), whereas it ranged from 59% (58/99) to 76.3% (29/38) for IHC. Agreement between methylation tests The agreement between MSP and MS-MLPA was reported in two studies. One study reported an agreement rate of 80%, whereas the other reported a high level of agreement (Cohen’s kappa = 0.84). Among three studies comparing MSP and IHC, one reported an agreement rate of 83.7% and another reported 39.5%; the remaining study stated that the correlation between the two tests was low. Conclusions and Strength of Recommendation The Subcommittee reviewed the current evidence on “Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter)” in newly diagnosed patients with neuroglial tumors and provided the following assessment. The safety of the test was assessed based on complications or adverse events, but no studies reported safety-related data. However, as this test is performed in vitro, it was considered to be safe due to the absence of direct harm to patients. In terms of effectiveness, patients with MGMT promoter methylation had significantly longer OS and better survival rates than those without methylation, regardless of the test used to assess methylation (MSP, MS-MLPA, or IHC); no significant differences were reported between the test methods. The risk of mortality was lower in patients with methylated MGMT promoters than in those with unmethylated promoters when assessed by MSP, whereas no significant difference was observed when IHC was used for testing; no studies reported a statistical comparison of the two testing methods. For predictive outcomes, MSP showed lower predictive accuracy than IHC in identifying responses to temozolomide treatment after six months of follow-up. However, MSP tended to have lower prediction errors for survival outcomes compared with MS-MLPA. In Korean clinical settings, MS-MLPA requires trained specialists to assess MGMT promoter methylation, and IHC has the limitation of interpretative variability. In contrast, MSP is a PCR-based method that offers greater convenience and shorter turnaround times, which may facilitate timely and accurate decision-making in a larger number of patients. Based on these findings, the Subcommittee concluded that MSP is a safe, effective, and comparable test for evaluating MGMT promoter methylation in newly diagnosed patients with neuroglial tumors, with the objective of predicting prognosis following standard treatment (radiotherapy with concomitant temozolomide). At the 2nd 2025 Health Technology Reassessment Committee meeting (February 14, 2024), the Committee deliberated on the “Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter).” Based on the Subcommittee’s findings regarding the clinical safety, effectiveness, and other evaluation criteria, the Committee concluded that the use of the “Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter)” is “recommended” for predicting prognosis following standard treatment (radiotherapy with concomitant temozolomide) by assessing MGMT promoter methylation in newly diagnosed patients with neuroglial tumors. Additionally, as “Methylation-specific Polymerase Chain Reaction” refers to the test method itself and this reassessment specifically evaluated its use in detecting MGMT promoter methylation in neuroglial tumors, the official title of this technology was revised to “Human Gene Molecular Pathology Test–Non-hereditary Gene Test–Methylation-specific Polymerase Chain Reaction (MGMT Gene Promoter).”
