Noninvasive Cardiac Function Monitoring [Bioreactance]

초록

평가배경 혈역학적 감시(hemodynamic monitoring)란 혈역학적 불안정성을 보이는 중증 환자에서 진단과 치료를 목적으로 혈역학 지표들을 지속적으로 확인하는 것을 말한다. 혈압, 맥박수, 중심정맥압(central venous pressure, CVP), 말초산소포화도 등의 기본적인 방법부터 일회박출량변동(stroke volume variation, SVV), 맥박압변동(pulse pressure variation, PPV), 심초음파(echocardiography), 비침습적 생체임피던스(bioimpedence) 등 다양한 방법들이 혈역학적 감시를 목적으로 사용하고 있다. 비침습적 심기능 측정 [바이오리엑턴스법]은 지속적으로 심박출량 측정 및 감시가 필요한 환자를 대상으로 흉부에 4개의 스티커형 전극을 부착하고 본체에 연결하여 심박출량을 측정 및 감시하는 검사로 2011년 신의료기술평가를 거쳐 2016년 2월에 선별급여(80%)로 등재되어 임상에서 사용 중이다. 동 기술은 2025년 선별급여 적합성 평가시기를 고려하여 내부 모니터링을 통해 재평가 주제로 발굴하였고, 다양한 대체 의료기술이 존재하는 현 시점에서 비교기술 대비 동 기술의 안전성과 효과성에 대한 근거를 확인하고자 2024년 제8차 의료기술재평가위원회(2024.8.9.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 ‘비침습적 심기능 측정 [바이오리엑턴스법]’은 지속적으로 비침습적 심기능 측정이 필요한 환자에서 심박출량을 측정 및 감시하는데 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 ‘비침습적 심기능 측정 [바이오리엑턴스법]’에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “비침습적 심기능 측정 공동 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 순환기내과 3인, 호흡기내과(중환자의학) 2인, 흉부외과 2인, 마취통증의학과 2인, 근거기반의학 1인의 전문가 10인으로 구성하였다. 본 평가의 대상자는 지속적으로 심기능 측정이 필요한 환자이며, 참고표준검사는 스완-간즈 카테터법에 의한 심박출량 측정(열희석법을 이용한 폐동맥 카테터 심기능 측정)이며, 비교검사는 동맥압에 기초한 심기능 측정, 열희석법 보정 후 동맥압에 기초한 심기능 측정 등으로 정의하였다. 결과변수로는 안전성과 효과성을 확인하였으며, 안전성은 검사 관련 부작용 및 이상반응 지표로 설정하였고, 효과성은 측정 정확성(참고표준과의 상관성, 일치도, 추세능력, 의료결과에의 영향을 지표로 평가하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2024. 10. 11.). 최종 선택된 연구의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를 사용하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과 최종 선택된 연구는 총 11편이었다. 선택 연구 11편 중 연구대상자는 수술 중이거나 수술 직후의 환자가 7편으로 다수였으며, 만성 심부전 환자, 혈액학적 감시가 필요한 환자가 연구대상자이었다. 연구의 비뚤림위험 평가 결과, 진단적 코호트연구는 환자선택 영역에서 구체적인 선정 및 배제기준을 제시하지 않아 ‘불확실’이 21.4%, 중재검사 영역에서 각 검사의 측정 방법과 측정 시점이 명확하지 않아 ‘불확실’이 21.4%였다. 무작위배정 비교임상시험에서는 배정순서 은폐에 대한 언급이 없어 불확실이 100%이었다. 안전성 체계적 문헌고찰 결과, 11편의 선택문헌에서 비침습적 심기능 측정 [바이오리엑턴스법] 검사장비 및 검사수행으로 인한 검사 관련 합병증이나 심각한 이상반응을 보고한 문헌은 없었다. 효과성 비침습적 심기능 측정 [바이오리엑턴스법]은 심기능 측정이 지속적으로 필요한 환자에서 심박출량 측정 및 감시하는 검사로서 측정정확성으로 평가하였다. 또한, 측정정확성은 참고표준검사인 폐동맥 카테터를 이용한 열희석법(PAC-CO(TD), 이하 PCA)과의 상관성, 일치도, 추세능력으로 평가하였다. 다만, 전반적으로 동 검사와 참고표준검사와의 심박출량 및 심박출지수의 일치도는 선택연구의 대상자특성, 측정 방법, 측정 시점 등이 연구마다 각기 상이하여 일관된 연구결과를 확인하고 일반화하기는 제한점이 있어 정성적으로 평가하였다. 동 검사와 참고표준검사의 심박출량 일치도는 11편 중 7편에서 심박출량의 편향(bias)이 –0.71~-0.68L/min 이었고 백분율 오차(Percentage Error, PE)는 7편에서 16.9~66.3% 이었다. 수술 중 PE를 측정한 4편에서는 53.8~66.3%인 반면, 그 외 만성심부전 환자이거나 심장수술 이후 집중치료실 등에서의 PE를 측정한 3편은 16.9~47.0%으로 PE의 허용 가능한 범위인 30% 이내 기준(Critchley et al. 1999)에는 2편이 부합하였다. 참고표준검사의 상관성은 9편에서 상관계수(r)를 보고하였으며, 상관계수(r)의 범위는 0.28~0.88이었다. 이 중 응급상황인 Cardiogenic shock 환자인 1편에서 0.28이었으며, 그 외 8편은 0.51~0.88이었다. 참고표준검사와의 추세능력 분석은 2편에서 보고하였고 간 이식 환자의 수술 중 심박출량 추세능 일치율(Concordance Rate, CR)은 54.8%이었으며, 만성심부전 환자에서 휴식기와 스트레스(능동적 하지 들어올리기) 상황에서의 심박출량 추세능력 일치율(CR)은 93.0%이다. 의료결과에의 영향은 국내 의료환경에서 심장 판막수술을 받은 심방세동환자에서 동 검사 기반의 모니터링 효과를 확인한 무작위배정 임상연구에서 중재군(NICOM)이 참고표준검사(PCA)에 비해 평균 입원기간이 짧고 두 군 간 유의한 차이는 없어 동 검사의 활용가능성을 언급하였으나, 한 편의 연구결과로 일반화하기에는 제한이 있었다. 소위원회에서는 침습적 방법으로 심박출량 모니터링을 적용하기 어려운 환자에서 지속적인 심박출량의 추이를 확인하기 위한 목적으로 동 검사를 이용해 볼 수는 있으나, 본 평가에서는 침습적 참고표준검사와의 비교를 통한 측정정확도 등을 평가하여 동맥혈관을 확보하기 어려운 소아 혹은 성인에서의 문헌적 근거를 확인할 수는 없었다. 또한, 최신 연구에서 동 검사의 측정정확도(일치도 및 상관성)가 낮아 기 등재된 참조표준검사(PCA)를 대체하기 어렵다고 보고하였으나, 소위원회에서는 선택문헌의 대상자 특성, 측정 방법, 측정 시점 등이 연구마다 각기 상이하여 일관된 연구결과를 확인하기에는 제한점이 있다고 판단하였다. 소위원회에서는 2015년 이후, 최신연구 9편 중 6편이 수술 중이거나 수술 직후의 측정정확도를 제시한 결과로 수술 시에는 다양한 요소들(침습적 의료행위, 약제 사용 등)이 관여되는 점, 침습적 검사를 통한 모니터링이 가능한 점 등을 고려한다면, 수술 시에는 동 검사의 유용성이 다소 낮다는 의견을 제시하였다. 다만, 임상적으로 응급 중증쇼크환자를 제외한 8편에서 참고표준검사와의 상관계수(r)는 0.51~0.88 범위를 보고하고 있고, 대상자특성, 측정 방법 및 측정 시점 등의 이질성에도 불구하고 총 11편에서 심박출량(CO) 혹은 심박출지수(CI) 편향(bias)의 절대값이 1L/min 이내 범위로서 수술 이외 측정 시점 3편의 심박출량(CO) 편향(bias)은 –0.09~0.48로 절대값이 0.5L/min를 초과하지 않을 뿐 아니라, 만성심부전 환자(Pandhita, 2021)에서의 백분율오차(PE)는 28%로 낮고 추세능(TA) 일치율(CR)은 93%로 높아 허용가능한 수준이라고 판단하였다. 동 검사의 고시 세부인정기준을 고려한다면 적용가능한 환자 수가 적고 남용가능성도 낮아 실제 임상현장에서의 이용량은 감소 추세라는 의견을 제시하였다. 결론 및 권고결정 의료기술재평가 소위원회에서는 ‘비침습적 심기능 측정 [바이오리엑턴스법]’ 검사로 인한 합병증이나 심각한 이상반응을 보고한 문헌이 없고, 체외에서 이루어지는 비침습적 모니터링 검사로 환자에게 직접적인 위해를 가하지 않아 안전한 기술로 판단하였다. 체계적 문헌고찰 결과, 동 검사의 효과성은 소위원회에서 참고표준검사인 침습적 심박출량 검사법(PCA)과의 측정정확도(편향, 일치한계, 백분율 오차)는 대상자 특성, 측정 방법, 측정 시점 등의 이질성으로 일관된 결과를 보이지는 않았지만, 응급상황을 제외한 참고표준검사와의 상관성은 유지되었으며, 수술 상황에서의 측정 시점을 제외한 편향의 범위는 수용 가능한 수준이었고 임상적응증에 해당하는 만성심부전 환자에서의 백분율 오차와 추세능의 일치율은 허용가능한 수준이라고 판단하였다. 또한, 동 검사의 임상적 유용성은 높지 않으나, 침습적 심기능 측정법을 지속적으로 적용하기 어려운 의료환경 또는 환자의 경우, 비침습적 검사로서의 안전성, 편의성, 지속적 측정가능성 등을 고려할 때 임상적으로 수용가능하다고 제언하였다. 이에 소위원회에서는 지속적으로 심박출량 측정 및 감시를 위해 침습적인 기존 검사법을 적용하기 어려운 환자에서 심박출량의 추이를 지속적으로 감시하기 위한 목적으로 효과성은 임상적으로 수용 가능하다고 판단하였다. 2025년 제3차 의료기술재평가위원회(2025. 3. 14.)는 ‘비침습적 심기능 측정 [바이오리엑턴스법]’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 동 기술의 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 지속적으로 심박출량 측정 및 감시를 위해 침습적인 기존 검사법을 적용하기 어려운 환자에서 심박출량의 추이를 지속적으로 감시하기 위한 목적으로 비침습적 심기능 측정 [바이오리엑턴스법] 사용을 ‘약하게 권고함’으로 결정하였다.


Background Hemodynamic monitoring is the continuous assessment of cardiovascular parameters, such as blood pressure, heart rate, central venous pressure, and peripheral oxygen saturation, in critically ill patients for diagnosis and therapeutics. Monitoring methods include stroke volume variation, pulse pressure variation, echocardiography, and noninvasive bioimpedance-based techniques. “Noninvasive Cardiac Function Monitoring [Bioreactance]” is a technique in which four adhesive chest electrodes connected to a main unit are used to measure and monitor cardiac output in patients requiring continuous cardiac monitoring. This technique, assessed in the 2011 New Health Technology Assessment, has been listed as a selective benefit item (80% coinsurance) since February 2016. Consequent to the selective benefit review scheduled in 2025, internal monitoring deemed the technique eligible for reassessment to evaluate the current evidence on its clinical safety and effectiveness in comparison with those of alternative techniques. The reassessment was conducted after the eighth 2024 Health Technology Reassessment Committee (August 9, 2024) had reviewed and approved the reassessment plan and constitution of the Subcommittee. Objective To provide scientific evidence on the clinical safety and effectiveness of “Noninvasive Cardiac Function Monitoring [Bioreactance],” a technique for continuous noninvasive cardiac output assessment and monitoring in critically ill patients, and to determine the strength of recommendation for its continued use. Method Safety and effectiveness were assessed using a systematic literature review. All assessment methods were deliberated and finalized, accordant with the objective of the assessment, by the “Joint Subcommittee on Noninvasive Cardiac Output Monitoring” (hereinafter, the “Subcommittee”). The Subcommittee comprised nine members: two cardiologists, two thoracic surgeons, two critical care pulmonologists, two anesthesiologists, and one evidence-based medicine expert. The target population was patients requiring continuous cardiac function monitoring. The reference standard was cardiac output measurement using a pulmonary artery catheter with thermodilution (PAC-CO [TD]). The comparators included arterial pressure-based cardiac output monitoring (with or without thermodilution calibration). Outcome measures included safety (technique-related adverse events and complications) and effectiveness (measurement accuracy based on agreement and correlation with the reference standard, trending ability, and impact on clinical outcomes). Three international (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews – Cochrane Central Register of Controlled Trials) and three Korean (KoreaMed, KMbase, and RISS) databases were searched (Final search date: October 11, 2024). As all included studies were diagnostic cohort studies, risk of bias was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 tool. All processes were independently conducted by two reviewers, and any discrepancies were resolved consensually. The Health Technology Reassessment Committee proffered the final recommendation following a review based on the findings of the Subcommittee. Results Of the 11 selected studies, seven included patients undergoing surgery or in the immediate postoperative period and the remaining included patients with chronic heart failure or those requiring hemodynamic monitoring. Regarding risk of bias, 21.4% of the studies were rated “unclear” for the patient selection domain owing to unspecified inclusion and exclusion criteria, and 21.4% were rated as “unclear” for the index technique domain owing to insufficient information on measurement methods or timing. In the randomized controlled trials (RCT), all studies (100%) were rated “unclear” for allocation concealment. Safety No technique-related adverse events or complications were reported in any of the 11 selected studies. Effectiveness Regarding effectiveness, measurement accuracy was evaluated based on agreement and correlation with PAC-CO (TD) and trending ability. However, the agreement between the index technique and PAC-CO (TD) varied according to patient characteristics, measurement methods, and timing, limiting generalizability. Thus, a qualitative analysis was conducted. Agreement between the index technique and PAC-CO (TD) for cardiac output was reported in 7/11 studies, with bias ranging from –0.71 to –0.68 L/min and a percentage error (PE) 16.9–66.3%. In four studies conducted during surgery, the PE was 53.8–66.3%. In three studies conducted outside the surgical setting, the PE was 16.9–47.0%, and two of these studies met the commonly accepted threshold of 30% (Critchley et al., 1999). Correlation with PAC-CO (TD) was reported in nine studies, with correlation coefficients (r) ranging from 0.28 to 0.88. One study in patients with cardiogenic shock reported an r value of 0.28, and the remaining eight studies had r values of 0.51–0.88. Trending ability was evaluated in two studies. One intraoperative study involving patients undergoing liver transplantation reported a concordance rate (CR) of 54.8% based on cardiac output trends, and another study involving chronic heart failure patients reported a CR of 93.0% between resting and stress (passive leg raise) conditions. Impact on clinical outcomes was reported in a single RCT conducted in Korea, which found a shorter average length of stay in the intervention (noninvasive cardiac monitoring) compared with the reference standard (PAC); however, the difference was not statistically significant. Although the study reported the potential applicability of the index technique, the results could not be generalized since it was based on a single study. The Subcommittee noted that the index technique may be considered for monitoring cardiac output trends in patients for whom invasive monitoring is difficult. However, no studies were identified that supported its use in pediatric or adult patients with limited arterial access. A recent study reported that the measurement accuracy of the index technique was insufficient to replace the reference standard. Nevertheless, the Subcommittee deemed that the heterogeneous characteristics and methods across the selected studies precluded consistent conclusions. Among the nine studies published since 2015, six evaluated measurement accuracy during surgery or in the immediate postoperative period. Considering the influence of factors such as invasive procedures and medication use during surgery, and the feasibility of invasive monitoring in such settings, the Subcommittee deemed the intraoperative utility of the index technique as relatively low. Excluding a study involving patients in emergent critical cardiogenic shock, eight studies reported r values of 0.51–0.88. Across all the 11 studies, the absolute value of bias for cardiac output or cardiac index was within 1 L/min. Among three studies conducted outside the surgical setting, cardiac output bias ranged from –0.09 to 0.48 L/min, all within ±0.5 L/min. A study involving patients with chronic heart failure (Pandhita, 2021) reported a PE and CR of 28% and 93%, respectively, indicating an acceptable level of accuracy. Considering the narrow patient eligibility defined in the reimbursement criteria and the low risk of overuse, the Subcommittee noted that clinical use of the technique is declining. Conclusion and Strength of Recommendation The Subcommittee found no studies reporting technique-related adverse events or complications, and since the technique involved noninvasive external monitoring, deemed it clinically safe. For effectiveness, although agreement with PAC-CO (TD) (reference standard) varied owing to heterogeneity in patient characteristics, measurement methods, and timing, correlation was maintained in most studies (excluding emergencies), and the bias values (excluding intraoperative measurements) were within acceptable ranges. In patients with chronic heart failure, the PE and trending ability CR were within acceptable limits. The overall clinical utility of the technique was considered limited; nevertheless, the Subcommittee deemed the clinical effectiveness of the technique acceptable because of its safety, convenience, and ability to provide continuous noninvasive monitoring in cases where invasive cardiac function monitoring is not feasible because of patient or healthcare system constraints. The third 2025 Health Technology Reassessment Committee (March 14, 2025) reviewed the technique, based on the clinical safety and effectiveness and other relevant factors, and concluded the following: In the current Korean clinical setting, “Noninvasive Cardiac Function Monitoring [Bioreactance]” is “weakly recommended” for continuous cardiac output monitoring in patients for whom invasive cardiac output measurement is difficult.
