증상 및 행동 평가 척도(기타)-한국판성격평가척도(KPAI)
초록
평가배경 한국판성격평가척도(Korean Personality Assessment Inventory, KPAI)는 Morey(1991)가 개발한 성격평가 질문지(PAI)를 우리나라 표집을 대상으로 표준화한 성인용 자가보고형 질문지로서, 환자 집단의 성격 및 정신병리적 특징 평가에 이용하는 검사이다. 동 검사는 신의료기술평가 제도 도입 이전인 2002년에 100분의 100 본인부담으로 등재되었고, 2005년 비급여로 전환되었다. 해당 기술은 2025년 의료기술재평가 수요조사에서 유관기관으로부터 제안된 이후 관련 대상 선정 절차를 거쳐, 2025년 제3차 의료기술재평가위원회(2025.3.14.)에서 재평가 대상으로 선정되었다. 이에 따라 현시점에서 동 기술의 효과성을 확인하기 위하여, 2025년 제5차 의료기술재평가위원회(2025.5.16.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 거쳐 재평가를 수행하였다. 평가목적 본 평가의 목적은 한국판성격평가척도(KPAI)의 임상적 효과성에 대한 정보를 제공하기 위함이다. 평가방법 본 평가에서는 ‘정신의학적 장애(의심) 환자 대상에서 한국판성격평가척도(KPAI)는 정신병리 또는 성격장애 진단 보조, 치료계획 수립 및 모니터링에 임상적으로 효과적인가?’를 핵심질문으로 설정하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “이화방어기제검사 재평가 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였으며, 소위원회는 정신건강의학과 3인, 임상심리전문가 2인, 근거기반의학 1인, 총 6인으로 구성하였다. 본 평가는 정신의학적 장애(의심) 환자를 대상으로 수행하였으며, 비교검사는 건강보험요양급여비용 목록에 등재된 자가보고식 검사, 참고표준검사는 정신질환 진단 및 통계편람 제4판 및 제5판에 근거한 종합적 임상판단으로 설정하였다. 결과변수는 효과성에 대해 진단분류 예측률, 비교검사와의 상관성, 질환과의 관련성 및 의료결과에 대한 영향을 중심으로 평가하였다. 한편, 한국판성격평가척도(KPAI)는 환자가 자가보고식 검사지를 이용하여 응답하는 검사로서 환자에게 직접적인 위해를 가하지 않는다고 판단하여, 중재검사 수행에 따른 안전성은 별도로 평가하지 않았다. 연구문헌은 핵심질문을 토대로 국외 4개(Ovid MEDLINE, Embase, Cochrane Central Register of Controlled Trials, APA PsycInfo), 국내 2개(KoreaMed, 한국교육학술정보원(RISS)) 데이터베이스에서 검색하였다(최종검색일 2025.6.27.). 연구선정 과정은 2명의 평가자가 독립적으로 수행하였고, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 동 검사의 특성 및 평가에 포함된 연구설계에 적합한 비뚤림위험 평가 도구가 확인되지 않아 비뚤림위험 평가는 수행하지 않았으며, 사전에 정한 자료추출 서식을 활용해 자료를 추출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종 심의하였다. 평가결과 체계적 문헌고찰 결과, 최종 선택된 연구는 총 14편으로, 출판연도별로는 2020년대 4편, 2010년대 6편, 2000년대 4편이었다. 연구환경은 학교, 의료기관, 일반인 대상, 교정시설 등으로 다양하였고, 연구대상자는 대학생, 의과전문대학원생, 주요 우울장애·양극성장애 환자, 경도 외상성 뇌손상 환자, 정신분열병 환자, 입원치료 환자 등으로 확인되었다. 중재검사로는 PAI-BOR가 주로 사용되었고 일부 연구에서는 PAI가 활용되었다. 한국판성격평가척도(KPAI)의 효과성은 진단분류 예측률, 비교검사와의 상관성, 질환과의 관련성 및 의료결과에 대한 영향을 중심으로 평가하였다. 다만, 의료결과에 대한 영향을 보고한 연구는 확인되지 않았다. 진단분류 예측률은 총 3편에서 보고되었다. 1편에서는 PAI의 우울 및 정신분열병 척도를 예측변수로 활용하였을 때 우울증군과 정신분열병군을 구분하는 예측정확도가 77.1%로 보고되었다. 다른 1편에서는 PAI의 망상 및 정신분열병 척도를 예측변수로 활용하였을 때 정상성인군과 정신분열병군의 예측정확도가 각각 86.9%, 72.7%였다. 나머지 1편에서는 약물사용 척도를 제외한 10개 PAI 임상척도를 예측변수로 활용하였을 때 전체 예측정확도는 82.2%였으며, 집단별 예측정확도는 정상군 93.0%, 우울장애군 63.0%, 양극성장애군 57.6%, 정신분열병군 68.2%, 알코올 의존군 96.9%로 보고되었다. 비교검사와의 상관성은 총 9편에서 보고되었다. 2편에서는 PAI의 경계선적 특징(PAI-Borderline Features, PAI-BOR)이 영 심리도식 질문지(Young Schema Questionnaire-Short Form, YSQ-SF)와 유의한 정적상관을 보여, 경계선적 성격특성과 초기 부적응 도식 간 관련성이 확인되었다. 1편에서는 PAI-BOR이 미네소타 다면적 인성검사-II 인격 정신병리학 5요인 척도(Minnesota Multiphasic Personality Inventory-II, MMPI-II PSY-5)의 모든 하위척도와 유의한 상관관계를 보였고, 다른 1편에서는 간이정신진단검사(The revised version of Symptom Check List, SCL-47)의 우울, 불안, 대인관계민감, 편집 척도와 유의한 정적상관을 나타냈다. 1편에서는 의과대학 1학년 시점의 PAI 우울 점수가 낮을수록 3학년과 1학년 간 벡 우울 평가(Beck Depression Inventory, BDI) 점수 차이가 증가하였고, 1학년 시점의 PAI 불안관련장애 점수가 높을수록 3학년 시점의 BDI 점수가, 1학년 시점의 신체적 호소 점수가 높을수록 3학년 시점의 벡 불안 척도(Beck Anxiety Inventory, BAI) 점수가 증가하는 경향을 보였다. 다만 3학년과 1학년 간 BAI 점수 변화와 유의한 관련성을 보이는 1학년 시점의 PAI 임상척도는 확인되지 않았다. 또한 1편에서는 PAI-BOR이 알코올 사용 장애 선별 검사(Alcohol Use Disorders Identification Test, AUDIT)와 유의한 정적상관을 보였으며, 다른 1편에서는 PAI-BOR이 경조증 증상 척도(Hypomania Symptom Checklist-32, HCL-32), 지역사회 역학조사용 우울척도(Center for Epidemiological Studies-Depression Scale, CES-D), AUDIT과 모두 유의한 정적상관을 보였다. 1편에서는 PAI-BOR과 간이 우울 증상 평가척도(Quick Inventory of Depressive Symptomatology, QIDS) 간 유의한 관련성이 확인되지 않았으며, 다른 1편에서는 PAI의 신체적 호소, 불안, 우울 척도가 모두 Rey-Kim 기억검사와 유의한 부적상관을 보여, 해당 임상척도 점수가 높을수록 기억기능은 낮은 경향을 나타냈다. 질환과의 관련성은 총 2편에서 보고되었다. 1편에서는 PAI-BOR 총점이 양극성장애 단계의 이환과 유의한 관련성을 보이지 않았으며, 다른 1편에서는 PAI-BOR로 측정한 경계성 성격장애 성향이 주요 우울장애 환자에서 경계성 성격장애 동반 여부와 유의한 관련성을 보였다. 결론 및 제언 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같이 결과 및 의견을 제시하였다. 체계적 문헌고찰 결과, 한국판성격평가척도(KPAI)의 전체 척도가 아닌 일부 하위척도만을 사용한 연구가 대부분이었으나, 진단분류 예측률은 수용 가능한 수준을 보였고, 비교검사로 포함된 여러 도구와 유의한 상관성을 나타내었다. 다만 질환과의 관련성에 대한 연구결과는 이질적이었으며, 의료결과에 대한 영향을 보고한 연구가 없어 문헌적 근거는 다소 제한적이었으나, 다양한 인구집단과 적용 환경에서 신뢰도와 타당도가 입증된 도구로서 정신병리 또는 성격장애 진단 보조, 치료계획 수립 및 모니터링에 있어 동 검사의 임상적 효과성이 있을 것으로 보았다. 소위원회는 국내 임상 현장에서 건강보험 급여 항목인 미네소타 다면적 인성검사(Minnesota Multiphasic Personality Inventory, MMPI)가 가장 널리 활용되고 있으며, 한국판성격평가척도(KPAI)를 병행할 경우 환자 증상에 대한 추가적인 정보를 제공할 수 있다는 의견을 제시하였다. 한편, 본 평가는 현행 건강보험 급여 및 비급여에 해당하는 검사만을 비교검사로 포함하였으나, 환자의 심리적 원인에 의한 증상 및 행동을 객관적으로 평가하는 도구 중 건강보험 체계 내에 포함된 검사가 제한적이고, 이들 역시 수십 년 전 기준에 따라 포함된 검사로서 변화한 심리 이론과 평가 방법을 충분히 반영하지 못하였다는 점에서 평가의 제한이 있다고 보았다. 아울러 국내의 다양한 환자 집단을 대상으로 새롭게 개발되는 도구의 표준화 및 임상적 효과성 검증을 위한 지속적인 연구가 필요하다고 제언하였다. 2025년 제4차 재평가전문위원회*(2025.12.8.)는 ‘증상 및 행동 평가척도(기타) - 한국판성격평가척도(KPAI)’에 대해 다음과 같이 심의하였다. 재평가전문위원회는 소위원회의 검토 결과에 동의하였으며, 추가로 해당 검사가 제도적 관리 범위 밖에서 임의로 활용되는 일이 없도록 향후 제도적 보완이 필요하다는 의견을 제시하였다.
Background The Korean Personality Assessment Inventory (KPAI) is an adult self-report questionnaire standardized using a Korean sample based on the Personality Assessment Inventory (PAI) developed by Morey (1991). It is used to assess personality and psychopathological characteristics in patient populations. This test was listed in 2002, before the introduction of the New Health Technology Assessment system, as an item fully paid out-of-pocket by patients, and was subsequently converted to a non-covered item in 2005. This technology was proposed by a relevant organization during the 2025 demand survey for health technology reassessment. Following the relevant target-selection process, it was selected as a reassessment topic at the 3rd Health Technology Reassessment Committee meeting in 2025, held on March 14, 2025. Accordingly, to examine the effectiveness of this technology at the present time, a reassessment was conducted after deliberation on the reassessment protocol and the proposed composition of the subcommittee at the 5th Health Technology Reassessment Committee meeting in 2025, held on May 16, 2025. Objective The objective of this assessment was to provide information on the clinical effectiveness of the Korean Personality Assessment Inventory (KPAI). Methods This assessment conducted a systematic review based on the key question: “Is the Korean Personality Assessment Inventory (KPAI) clinically effective for assisting in the diagnosis of psychopathology or personality disorders, establishing treatment plans, and monitoring patients with suspected or confirmed psychiatric disorders?” All assessment methods were finalized through discussion by the Reassessment Subcommittee for the Korean Personality Assessment Inventory (KPAI) (hereafter referred to as the “Subcommittee”), taking into account the objective of the assessment. The Subcommittee consisted of six members: three psychiatrists, two clinical psychology specialists, and one evidence-based medicine expert. The target population of this assessment was patients with suspected or confirmed psychiatric disorders. The comparator tests were self-report tests listed in the National Health Insurance reimbursement schedule, and the reference standard was comprehensive clinical judgment based on the Diagnostic and Statistical Manual of Mental Disorders, Fourth and Fifth Editions. Effectiveness outcomes were evaluated with a focus on diagnostic classification prediction accuracy, correlation with comparator tests, association with disease, and impact on health outcomes. Meanwhile, because the KPAI is a self-report questionnaire completed by patients and was considered not to pose direct harm to patients, the safety of performing the index test was not separately assessed. Literature searches were conducted based on the key question in four international databases—Ovid MEDLINE, Embase, the Cochrane Central Register of Controlled Trials, and APA PsycInfo—and two domestic databases—KoreaMed and the Research Information Sharing Service (RISS). The final search date was June 27, 2025. Study selection was independently performed by two reviewers. In cases of disagreement, a consensus result was reached through discussion between the reviewers. Because no risk-of-bias assessment tool appropriate for the characteristics of this test and the study designs included in the assessment was identified, risk of bias was not assessed. Data were extracted using a predefined data extraction form. The final deliberation of this assessment was conducted by the Reassessment Expert Committee based on the Subcommittee’s review findings. Results A total of 14 studies were finally selected through the systematic review. By publication year, four studies were published in the 2020s, six in the 2010s, and four in the 2000s. The study settings varied and included schools, medical institutions, general-population settings, and correctional facilities. Study participants included university students, medical graduate students, patients with major depressive disorder or bipolar disorder, patients with mild traumatic brain injury, patients with schizophrenia, and inpatients. The PAI-BOR was mainly used as the index test, while some studies used the PAI. The effectiveness of the KPAI was evaluated with a focus on diagnostic classification prediction accuracy, correlation with comparator tests, association with disease, and impact on health outcomes. However, no studies reporting the impact on health outcomes were identified. Diagnostic classification prediction accuracy was reported in three studies. In one study, when the depression and schizophrenia scales of the PAI were used as predictor variables, the predictive accuracy for distinguishing the depression group from the schizophrenia group was 77.1%. In another study, when the paranoia and schizophrenia scales of the PAI were used as predictor variables, the predictive accuracies for the normal adult group and the schizophrenia group were 86.9% and 72.7%, respectively. In the remaining study, when 10 PAI clinical scales excluding the drug use scale were used as predictor variables, the overall predictive accuracy was 82.2%. Predictive accuracies by group were 93.0% for the normal group, 63.0% for the depressive disorder group, 57.6% for the bipolar disorder group, 68.2% for the schizophrenia group, and 96.9% for the alcohol dependence group. Correlation with comparator tests was reported in nine studies. In two studies, the PAI Borderline Features scale (PAI-BOR) showed a significant positive correlation with the Young Schema Questionnaire-Short Form (YSQ-SF), confirming an association between borderline personality traits and early maladaptive schemas. In one study, the PAI-BOR showed significant correlations with all subscales of the Minnesota Multiphasic Personality Inventory-II Personality Psychopathology Five scales (MMPI-II PSY-5). In another study, it showed significant positive correlations with the depression, anxiety, interpersonal sensitivity, and paranoid ideation scales of the revised version of the Symptom Checklist (SCL-47). In one study, lower PAI depression scores in the first year of medical school were associated with a greater increase in Beck Depression Inventory (BDI) scores between the first and third years. Higher PAI anxiety-related disorder scores in the first year tended to be associated with higher BDI scores in the third year, and higher somatic complaints scores in the first year tended to be associated with higher Beck Anxiety Inventory (BAI) scores in the third year. However, no PAI clinical scale measured in the first year showed a significant association with changes in BAI scores between the third and first years. In addition, one study reported that the PAI-BOR showed a significant positive correlation with the Alcohol Use Disorders Identification Test (AUDIT), while another study found that the PAI-BOR showed significant positive correlations with the Hypomania Symptom Checklist-32 (HCL-32), the Center for Epidemiological Studies-Depression Scale (CES-D), and AUDIT. One study found no significant association between the PAI-BOR and the Quick Inventory of Depressive Symptomatology (QIDS). Another study reported that the somatic complaints, anxiety, and depression scales of the PAI all showed significant negative correlations with the Rey-Kim Memory Test, indicating that higher scores on these clinical scales tended to be associated with lower memory function. Association with disease was reported in two studies. In one study, the total PAI-BOR score was not significantly associated with morbidity by stage of bipolar disorder. In another study, borderline personality disorder traits measured using the PAI-BOR showed a significant association with comorbid borderline personality disorder in patients with major depressive disorder. Conclusions and Recommendations Based on the current assessment results, the Health Technology Reassessment Subcommittee presented the following conclusions and opinions. The systematic review found that most studies used only selected subscales rather than the full KPAI. Nevertheless, diagnostic classification prediction accuracy was at an acceptable level, and significant correlations were observed with several instruments included as comparator tests. However, findings on association with disease were heterogeneous, and no studies reporting the impact on health outcomes were identified. Therefore, although the body of evidence from the literature was somewhat limited, the Subcommittee considered that the test is likely to have clinical effectiveness in assisting the diagnosis of psychopathology or personality disorders, establishing treatment plans, and monitoring patients, as it is an instrument whose reliability and validity have been demonstrated across diverse populations and settings. The Subcommittee noted that the Minnesota Multiphasic Personality Inventory (MMPI), which is reimbursed under the National Health Insurance system, is the most widely used instrument in domestic clinical practice. It also expressed the opinion that using the KPAI in parallel may provide additional information on patients’ symptoms. Meanwhile, although this assessment included only tests currently covered or non-covered under the National Health Insurance system as comparator tests, the Subcommittee recognized a limitation of the assessment in that few instruments objectively evaluating symptoms and behaviors arising from patients’ psychological causes are included within the National Health Insurance system. Furthermore, those instruments were incorporated based on criteria established several decades ago and may not sufficiently reflect changes in psychological theory and assessment methods. The Subcommittee also recommended continued research to standardize newly developed instruments for diverse patient populations in Korea and to verify their clinical effectiveness. The 4th Reassessment Expert Committee meeting in 2025, held on December 8, 2025, deliberated on “Symptomatic and Behavioral Evaluation Scale (Others): Korean Personality Assessment Inventory (KPAI)” as follows. The Reassessment Expert Committee agreed with the Subcommittee’s review findings and additionally stated that future institutional improvements are needed to prevent the test from being used arbitrarily outside the scope of institutional management.
