초록

평가배경 비타민[정밀면역검사-간이검사]25-OH-Vitamin D, total (Vitamin D Quantitative Point of Care Immmunoassay) (이하 비타민[정밀면역검사-간이검사])는 혈액(혈청/혈장) 검체를 간이검사 장비를 통해 분석하여 비타민 D 결핍 또는 과잉 상태를 선별·추적하는 검사법이다. 본 기술은 신의료기술평가를 거치지 않고 2021년 4월 선별급여(본인부담 90%)로 등재되었으며, 유관기관 수요조사와 관련 검토 절차를 거쳐 재평가 대상 기술로 선정되었다. 이후 2025년 제3차 의료기술재평가위원회(2025.3.14.)에서 재평가 계획서 및 소위원회 구성안이 심의되어 본 재평가를 수행하였다. 평가목적 평가의 목적은 비타민 D 결핍 및 과잉 환자에서 ‘비타민[정밀면역검사-간이검사]’의 임상적 효과성에 대한 근거를 제공하고, 동 기술 사용에 대항 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 비타민[정밀면역검사-간이검사]에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “비타민[정밀면역검사-간이검사] 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 내분비내과 2인, 진단검사의학과 2인, 근거기반의학 1인, 가정의학과 1인의 전문가 6인으로 구성하였다. 본 평가의 대상자는 비타민 D 결핍 및 과잉 (의심)환자이며, 참고표준검사는 비타민[정밀분과-질량분석]검사이고, 비교검사는 비타민[정밀면역검사]로 정의하였다. 결과변수로는 효과성을 확인하였으며, 효과성은 진단정확도 및 참고표준검사와의 일치도, 상관성 지표로 평가하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2024. 2. 20.). 최종 선택된 연구의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를 사용하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 진단정확도를 2 x 2표로 제시하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다. 평가결과 최종 선택연구는 단면관찰연구 2편(대상자 수 352명)으로, 모두 참고표준검사인 액체크로마토그래피-질량분석법(Liquid Chromatography–Tandem Mass Spectrometry, LC-MS/MS)과의 상관성을 보고하였다. 그러나 진단정확도, 검사 신속성, 편의성에 따른 이득을 평가한 연구는 확인되지 않았다. 비뚤림위험 평가 결과, 표본 규모가 작고 일부 연구에서 맹검(blinding) 절차가 불분명하여 임상적 적용성에는 제한이 있는 것으로 판단하였다. 비타민 D 정밀면역 간이검사(Point-of-Care test, POCT)의 효과성은 LC–MS/MS 참고표준검사와의 상관성 및 일치도로 평가하였다. Albrecht 등(2021)은 상관계수 r=0.89, Lin의 일치도계수(Lin’s conordance correlation coefficient) rc=0.88로 신선·냉동 검체 모두 안정적이었으며, 변동계수(coefficient of variation, CV는 9~15%로 정밀도가 양호하였다. Palermiti 등(2024)은 AFIAS-1® r=0.932, AFIAS-10® r=0.910으로 매우 높은 상관성을 보였고, 고농도 구간에서 약간의 과대평가를 제외하면 전반적으로 LC–MS/MS와 양호한 일치도를 보였다. 결론 및 권고결정 의료기술재평가 소위원회는 비타민[정밀면역검사-간이검사]25-OH-Vitamin D, total는 안전하고 표준검사와 높은 상관성과 일치도를 보이기는 하나 근거문헌의 수가 적고 연구에 사용된 표본의 수가 적은 근거의 수준에 한계가 있어 정밀면역검사 또는 정밀분광-질량분석을 대체할 수준은 아닌 것으로 평가하였다. 따라서 본 검사는 검사실이 갖추어지지 않은 환경에서 비타민 D 농도를 확인하기 위하여 보조적으로 사용하는데 유용한 기술로 제안하였다. 2025년 제2차 재평가전문위원회*(2025. 10. 17.)는 “비타민[정밀면역검사-간이검사]”에 대해 다음과 같이 심의하였다. 재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 비타민D 결핍 및 과잉이 의심되는 환자의 비타민D 농도를 측정하는 목적으로 비타민[정밀면역검사-간이검사]의 사용을 ‘약하게 권고함’으로 결정하였다.


Background The Vitamin D [High Quality Immunoassay -Handy test] 25-OH Vitamin D, Total (hereafter referred to as the "Vitamin D Handy test") is a diagnostic test that analyzes blood samples (serum/plasma) using a point-of-care testing device to screen for and monitor vitamin D deficiency or excess. This technology was listed as a selective benefit (90% patient co-payment) in April 2021 without undergoing the new health technology assessment process. It was later selected as a target for re-evaluation following demand surveys and related review procedures by relevant organizations. Subsequently, the re-evaluation plan and the composition of the subcommittee were reviewed and approved at the third Health Technology Reassessment Committee (March 14, 2025), and this re-evaluation was conducted accordingly. Objective The objective of this evaluation is to provide evidence on the clinical effectiveness of the Vitamin D Handy test in patients with vitamin D deficiency or excess and to determine the recommendation grade for this technology through the health technology re-assessment process. Methods A systematic review was conducted to assess the safety and effectiveness of the Vitamin D Handy test. All evaluation methods were finalized through discussions by the Subcommittee, taking into account the evaluation objectives. The Subcommittee consisted of six experts: two in endocrinology, two in laboratory medicine, one in evidence-based medicine, and one in family medicine. The target population included patients with suspected vitamin D deficiency or excess. The reference standard test was defined as the Vitamin D Liquid Chromatography-Tandem Mass Spectrometry( LC-MS/MS), and the comparator test was defined as the laboratory-based immunoassay. Effectiveness outcomes included diagnostic accuracy, agreement with the reference standard, and correlation measures. Results The final included studies comprised two cross-sectional observational studies (total sample size: 352 participants), both of which reported correlations with the reference standard, LC-MS/MS. However, no studies were identified that evaluated benefits in terms of diagnostic accuracy, test turnaround time, or usability. The risk of bias assessment indicated limitations in clinical applicability due to small sample sizes and unclear blinding procedures in the studies. The effectiveness of the Vitamin D Handy test was evaluated based on its correlation and agreement with the LC-MS/MS reference standard. In Albrecht et al. (2021), the correlation coefficient was r = 0.89 and Lin's concordance correlation coefficient (rc) was 0.88, demonstrating stable performance in both fresh and frozen samples. The coefficient of variation (CV) ranged from 9% to 15%, indicating acceptable precision. In Palermiti et al. (2024), very high correlations were observed (AFIAS-1Ⓡ: r = 0.932; AFIAS-10Ⓡ: r = 0.910). Overall agreement with LC-MS/MS was good, with only slight overestimation observed in the high-concentration range. Conclusion and Recommendation The Subcommittee evaluated that although Vitamin [High Quality Immunoassay -Handy test] 25-OH-Vitamin D, total is safe and shows high correlation and consistency with the standard test, it is not at a level to replace precision immunoassay or LS-MS/MS due to the small number of evidence documents and small sample sizes used in the studies, which limit the level of evidence. Therefore, the subcommittee proposed this test as a useful technology for auxiliary use to check vitamin D concentrations in environments where a laboratory is not equipped. The second Health Technology Reassessment Expert Committee* (October 17, 2025) deliberated on the Vitamin D [High Quality Immunoassay -Handy test] as follows : When comprehensively considering the evidence of clinical safety and effectiveness and other assessment items, the Health Technology Reassessment Expert Committee decided to 'weakly recommend' the use of Vitamin D [High Quality Immunoassay -Handy test] for the purpose of measuring vitamin D concentration in patients suspected of vitamin D deficiency and excess.
