이뮤노시아닌
초록
평가배경 ‘비스쿰 알붐(Viscum album)’, ‘이뮤노시아닌(Immunocyanin)’, ‘싸이모신 알파 1(Thymosin α1)’은 암 환자 및 면역기능 저하자를 대상으로 종양 치료, 항암 보조요법, 암 재발 예방, 면역기능 보조요법 등의 목적으로 사용되고 있는 비급여 주사제이다. ‘비급여 상세내역조사’ 자료에 따르면, 해당 약제들은 종양용약 중에서도 진료비 규모가 큰 항목에 속한다. 이들 기술은 ‘건강보험 비급여 관리강화 종합대책’ 수립(’20.12.31.)에 따라, 적정한 의료서비스 제공및 합리적 의료이용을 지원하기 위해 유관기관으로부터 의료기술재평가를 요청받았다. 2024년 제4차 의료기술재평가위원회(2024.4.12.)에서 의료기술재평가 계획서 및 소위원회 구성안을 심의하고, 임상적 안전성과 유효성에 대한 전문적이고 심층적인 검토를 통해 재평가를 수행하였다. 본 평가의 목적은 비스쿰 알붐(Viscum album), 이뮤노시아닌(Immunocyanin) 및 싸이모신 알파 1(Thymosin α1)의 임상적 안전성과 유효성에 대한 객관적인 근거를 도출하고, 해당 기술에 대한 의료기술재평가 권고등급을 제시하는 데 있다. 본 보고서에는 그 중 이뮤노시아닌의 안전성 및 유효성평가 내용이 포함되어 있다. 평가방법 이뮤노시아닌의 안전성 및 유효성을 평가하기 위해 체계적 문헌고찰(Systematic Review)을 수행하였다. 세부 평가 방법 및 결과는 「비스쿰 알붐ㆍ이뮤노시아닌ㆍ싸이모신 알파 1 약물의 안전성 및 유효성 평가 통합 소위원회」(이하 ‘소위원회’)의 검토와 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 심의한 평가계획에 따라 혈액종양내과 2인, 가정의학과 2인, 근거기반의학 전문가 2인으로 구성되어 총 6인이 참여하였다. 본 평가의 대상자는 암 환자로 설정하였다. 안전성 평가는 심각한 이상반응 및 부작용, 일반적인 이상반응 및 부작용 등을 주요 지표로 설정하였다. 유효성은 생존율, 재발 및 질병진행, 항암반응률, 삶의 질을 결과지표로 설정하였다. 해당 약물이 주로 암 환자에게 사용된다는 점을 고려하여 본 평가는 선택 비뚤림 및 교란 요인 등을 최소화할 수 있는 연구설계인 무작위배정 비교임상시험(Randomized Controlled Trials, RCTs) 연구로 한정하여 문헌을 선정하였다. 문헌 검색은 핵심질문을 기반으로 국외 3개 데이터베이스(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials)와 국내 3개 데이터베이스(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS))에서 수행되었다(최종검색일 2024. 7. 25.). 최종 선정된 연구에 대해서는 ‘비뚤림위험(Risk of Bias, RoB)’ 평가 도구를 활용하여 평가를 수행하였고, 자료추출은 사전에 정의한 양식에 따라 시행하였다. 양적 분석이 가능한 경우에는 메타분석을 실시하였다. 모든 과정은 2인의 평가자가 독립적으로 수행하였으며, 의견 불일치 시 합의를 통해 최종 결과를 도출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거 수준을 평가하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회의 심의를 거쳐 의료기술재평가 권고등급이 결정되었다. 평가결과 체계적 문헌고찰 결과, 최종 선택연구는 무작위배정 비교임상시험 연구 7편이었다. 출판연도는 2000년 이전 연구가 6편, 2012년 1편이었고, 출판국가는 독일이 가장 많았다. 연구대상자는 표재성 방광암으로 경요도 절제술을 받은 환자를 대상으로 한 연구가 6편, 신장암으로 신절제술을 받은 환자 대상 연구가 1편이었다. 중재치료는 수술 후 이뮤노시아닌 단독 치료군이었고, 비교치료는 신장암 환자에서 수술 후 무치료, 나머지 방광암 환자에서는 미토마이신C, 에토글루시드, BCG 치료와 비교하였다. 비뚤림위험 평가는 각 항목별 기준에 따라 수행하였다. 먼저, 무작위배정 순서 생성과 순서 은폐 방법에 대한 기술이 없는 경우에는 관련 정보의 부족으로 인해 비뚤림위험을 ‘불확실’로 평가하였다. 눈가림 항목의 경우, 생존율, 무질병 생존율 등 객관적 지표를 결과로 제시한 연구는 눈가림의 영향을 받지 않을 것으로 판단하여 비뚤림위험을 ‘낮음’으로 평가하였다. 반면, 삶의 질과 같은 주관적 지표를 활용한 연구는 비뚤림위험 ‘불확실’로 평가하였다. 불충분한 결과 자료 항목은 추적관찰 손실이 20% 이상 발생한 경우 ‘불확실’로 평가하였다. 선택적 보고에 대한 항목은 안전성 또는 유효성 지표 중 하나만 보고한 경우 ‘불확실’로 평가하였다. 기타 비뚤림위험 항목은 연구 수행에 있어 약물을 제조사로부터 제공받았거나 연구비를 지원받은 경우는 비뚤림위험 ‘높음’으로 평가하였다. 안전성 심각한 이상반응 및 부작용을 보고한 연구는 5편으로 모두 방광암 환자를 대상으로 수행되었다. 이 중 4편의 연구에서 심각한 부작용은 없었다고 보고하였고, Kalble 등(1991)에서는 심한 방광염 및 연구 중단 보고가 이뮤노시아닌군은 발생하지 않았고(19명 중 0명), 비교군에서 12%(25명 중 3명) 발생하였다. 일반적인 합병증 및 부작용을 보고한 연구는 6편으로 모두 방광암 환자를 대상으로 수행되었다. Jurincic 등(1988)은 전신 독성 및 알레르기 반응이 없었다고 보고하였고, Lammers 등(2012)은 발열, 감기 증상, 피로 같은 일반적인 부작용은 이뮤노시아닌군에서 좀 더 많이 발생하였고, 알레르기 및 피부 반응은 이뮤노시아닌군에서 좀 더 적게 발생하였다. 경증 방광염을 보고한 4편의 연구를 메타분석한 결과, 이뮤노시아닌군에서 유의하게 낮게 발생하였다(RR=0.09, 95% CI 0.04~0.20, I2=0%). 발열을 보고한 5편의 연구를 메타분석한 결과, 군 간 유의한 차이는 없었다(RR=3.66, 95% CI 0.42~32.14, I2=48%). 유효성 5년 생존율, 중앙 생존기간, 사망자 수를 보고한 연구는 1편으로 신장암 환자에서 5년 생존율은 이뮤노시아닌군이 60.0%, 비교군 56.5%로 보고하였으며, 중앙 생존기간은 두 군 사이 유의한 차이가 없었다(Jurincic- Winkler 등, 1994). 재발 및 질병진행 등은 6편의 연구에서 보고되었다. 모두 방광암 환자를 대상으로 무질병, 무진행, 무재발 생존율을 보고한 연구 3편 모두 군 간 유의한 차이가 없었고, 무질병, 무진행 생존위험비를 보고한 Lammers 등(2012) 연구는 이뮤노시아닌군에서 무재발 생존위험비가 유의하게 높았고, 무진행 위험비는 군 간 유의한 차이가 없었다. 재발 및 질병 진행에 대해 메타분석한 결과, 두 결과지표 모두 군 간 유의한 차이가 없었다(재발에 대한 RR=1.21, 95% CI 0.77~1.88, I2=76%; 질병 진행에 대한 RR=0.65, 95% CI 0.32~1.33, I2=7%). 무질병, 무진행, 무재발 기간을 보고한 연구 5편 중 2편의 연구는 이뮤노시아닌군에서 무질병, 무재발 기간이 비교군에 비해 유의하게 짧았고(Flamm 등, 1990; Lammers 등, 2012), 나머지 3편은 군 간 유의한 차이가 없었다(Flamm 등, 1990; Jurincic- Winkler 등(1994); Flamm 등, 1994). 항암반응률 및 삶의 질을 보고한 연구는 없었다. 결론 및 의료기술재평가 권고결정 의료기술재평가 소위원회는 국내에서 이뮤노시아닌이 경요도 절제 후 및 기존 약물치료 실패 후 방광암 재발 방지에 사용되는 항암제로 허가되었으며 기타의 종양치료제 분류되어 있는 점을 고려하여 본 평가는 이뮤노시아닌의 항암 보조요법으로서의 안전성과 유효성을 중심으로 수행하였다. 체계적 문헌고찰 결과, 이뮤노시아닌은 기존 치료와 비교하여 심각한 이상반응 및 부작용이 보고되지 않았고, 일반적인 부작용인 발열만 관찰되어 안전하다고 평가하였다. 유효성은 신장암 환자의 생존율을 비교한 1편의 연구만 확인되었으며, 표본 크기가 작아 평가하기에 근거가 부족하였다. 표재성 방광암 환자에서 경요도 절제술 후 이뮤노시아닌은 표준치료(미토마이신C, BCG, 에토글루시드)와 비교 시, 재발이나 질병 진행에 차이가 없었다. 이는 표재성 방광암에서 이뮤노시아닌 치료는 추가적인 이점을 제공하지 않으며 국내에서는 현재 이뮤노시아닌을 표재성 방광암의 치료로 사용하지 않는다는 의견이 있었다. 본 평가는 무작위배정 비교임상시험 연구를 중심으로 수행하였으나, 개별 연구의 표본 크기가 작은 점, 무작위 배정방법 및 순서 은폐방법 등에 대한 보고가 부족한 점 등으로 인해 비뚤림에 대한 우려가 있어 전반적인 근거수준이 ‘낮음’으로 평가되었다. 또한, 대부분의 연구가(7편 중 6편) 2000년 이전에 출판되어 현재의 암 치료 실태를 반영하지 못한다는 점에서 최신 근거가 부족하다는 의견이었다. 국내에서는 이뮤노시아닌이 고형 종양 환자에서 추가 치료로 사용되는 경우가 있으나, 고형 종양에서 치료에 대한 유효성의 근거는 부족하다고 평가하였다. 2024년 제12차 의료기술재평가위원회(2024.12.13.)에서는 의료기술재평가사업 관리지침 제4조제10항에 근거하여 소위원회 검토 결과를 바탕으로 이뮤노시아닌에 대한 최종 심의를 진행하였다. 위원회는 이뮤노시아닌이 암 환자에게 통상적인 항암치료에 추가로 사용될 경우, 안전성은 기존 치료와 유사한 수준으로 판단하였다. 그러나 방광암 외 고형 종양에 대한 근거가 부족한 점, 연구의 비뚤림위험에 대한 우려, 최신 연구 부족 등으로 인해 유효성에 대한 문헌 근거수준은 낮다고 평가하였다. 이에 따라, 의료기술재평가위원회는 임상적 안전성과 유효성 등에 대한 근거를 종합적으로 고려하였을 때, 국내 임상 상황에서 암 환자에게 종양 치료 및 재발 예방 목적으로 기존 통상적인 암 치료에 이뮤노시아닌을 추가 투여하는 것을 ‘권고하지 않음’으로 결정하였다.
Background Viscum album, Immunocyanin, and Thymosin α1 are non-reimbursable injectable drugs used in patients with cancer or impaired immune function for purposes such as tumor treatment, adjuvant cancer therapy, prevention of cancer recurrence, and immune support therapy. According to data from the “Detailed Survey of Non-reimbursable Items,” these drugs rank among the highest in medical expenditure in the category of antineoplastic agents. In accordance with the “Comprehensive Plan to Strengthen the Management of Non-Reimbursable Services in the National Health Insurance” (December 31, 2020), these drug-based techniques were referred for health technology reassessment by relevant institutions to promote appropriate healthcare delivery and rational healthcare utilization. The reassessment protocol and subcommittee composition were reviewed at the 4th 2024 Health Technology Reassessment Committee meeting (April 12, 2024), and the reassessment was conducted through a professional and in-depth review of clinical safety and effectiveness. The objective of this reassessment was to provide objective evidence on the clinical safety and effectiveness of Viscum album, Immunocyanin, and Thymosin α1 and to determine the strength of recommendation for each technology. This report presents the assessment results for Immunocyanin. Method A systematic literature review was conducted to evaluate the safety and effectiveness of Immunocyanin. The detailed methodology and results were finalized through review and discussion by the “Joint Subcommittee on the Safety and Effectiveness Assessment of Viscum album, Immunocyanin, and Thymosin α1” (hereafter referred to as the “Subcommittee”). The Subcommittee comprised six members: two hematology-oncology specialists, two family medicine specialists, and two evidence-based medicine experts, in accordance with the reassessment plan approved by the Health Technology Reassessment Committee. The target population was patients with cancer. Safety was assessed in terms of severe adverse events (AEs) and complications, as well as general AEs and complications. Effectiveness outcomes included survival rate, recurrence and disease progression, antitumor response rate, and quality of life. As the agent is primarily used in patients with cancer, the literature review was limited to randomized controlled trials (RCTs) to minimize selection bias and confounding. The literature search was guided by predefined key questions and performed in three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews – Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, KMbase, and RISS), with the final search conducted on July 25, 2024. Risk of bias was assessed for each included study using the Risk of Bias (RoB) tool, and data were extracted with a predefined form. Meta-analysis was conducted when quantitative synthesis was feasible. All processes were performed independently by two reviewers, with discrepancies resolved by consensus. The level of evidence was evaluated using the GRADE (Grading of Recommendations Assessment, Development, and Evaluation) approach. The final strength of recommendation was determined through review by the Health Technology Reassessment Committee based on the Subcommittee’s findings. Results The systematic literature review identified a total of seven RCTs. Six studies were published before 2000 and one in 2012, with Germany being the most common country of publication. Six studies enrolled patients with superficial bladder cancer who had undergone transurethral resection, and one study enrolled patients with renal cancer who had undergone nephrectomy. The intervention was postoperative Immunocyanin monotherapy. In studies on patients with renal cancer, the control group received no postoperative treatment, whereas in the studies on patients with bladder cancer, the control group received mitomycin C, ethoglucid, or BCG therapy. Risk of bias was assessed by domain. Studies lacking descriptions of random sequence generation or allocation concealment were rated “unclear” for these domains due to insufficient information. For blinding, studies reporting objective outcomes such as survival rate and disease-free survival were judged unlikely to be affected by lack of blinding and were rated “low” risk, whereas those reporting subjective outcomes such as quality of life were rated “unclear.” For incomplete outcome data, studies with loss to follow-up exceeding 20% were rated “unclear.” For selective reporting, studies that reported only safety or only effectiveness outcomes were rated “unclear.” For other sources of bias, studies that received the study drug or funding from the manufacturer were rated “high.” Safety Five studies, all in patients with bladder cancer, reported severe AEs or complications. Four studies reported no severe complications. In Kalble et al. (1991), severe cystitis and treatment discontinuation occurred in 0% (0/19) of patients in the Immunocyanin group and in 12% (3/25) of patients in the control group. Six studies, all in patients with bladder cancer, reported general complications or AEs. Jurincic et al. (1988) reported no systemic toxicity or allergic reactions. Lammers et al. (2012) found that general AEs such as fever, cold symptoms, and fatigue occurred more frequently in the Immunocyanin group, while allergic and skin reactions were less frequent than that in the control group. A meta-analysis of four studies reporting mild cystitis showed a significantly lower incidence in the Immunocyanin group (RR = 0.09, 95% CI 0.04–0.20, I² = 0%). A meta-analysis of five studies reporting fever found no significant difference between groups (RR = 3.66, 95% CI 0.42–32.14, I² = 48%). Effectiveness One study reported 5-year survival rate, median survival, and number of deaths. In patients with renal cancer, the 5-year survival rate was 60.0% in the Immunocyanin group and 56.5% in the control group, with no significant difference in median survival between groups (Jurincic-Winkler et al., 1994). Recurrence or disease progression was reported in six studies, all conducted in patients with bladder cancer. Three studies reported disease-free, progression-free, or recurrence-free survival rates, with no significant difference between groups. In Lammers et al. (2012), the hazard ratio for recurrence-free survival was significantly higher in the Immunocyanin group, whereas the hazard ratio for progression-free survival showed no significant difference between groups. Meta-analysis found no significant difference between groups for recurrence (RR = 1.21, 95% CI 0.77–1.88, I² = 76%) or disease progression (RR = 0.65, 95% CI 0.32–1.33, I² = 7%). Among five studies reporting disease-free, progression-free, or recurrence-free survival duration, two found significantly shorter disease-free or recurrence-free duration in the Immunocyanin group compared with the control group (Flamm et al., 1990; Lammers et al., 2012). The remaining three studies found no significant difference between groups (Flamm et al., 1990; Jurincic-Winkler et al., 1994; Flamm et al., 1994). No studies reported antitumor response rate or quality of life outcomes. Conclusion and Strength of Recommendation The Subcommittee noted that Immunocyanin is approved in Korea as an antineoplastic agent for prevention of bladder cancer recurrence following transurethral resection (TUR) or after failure of prior drug therapy and is classified as an “other antineoplastic agent.” Accordingly, this assessment focused on the safety and effectiveness of Immunocyanin as adjuvant therapy for cancer. The systematic literature review found no reports of severe AEs or complications associated with Immunocyanin compared with existing therapies; only fever, a general AE, was observed. Immunocyanin was therefore considered safe. Regarding effectiveness, only one study in patients with renal cancer reported survival outcomes, and the small sample size limited the strength of the evidence. In patients with superficial bladder cancer following TUR, Immunocyanin did not differ from standard therapies (mitomycin C, BCG, or ethoglucid) in recurrence or disease progression. The Subcommittee noted that Immunocyanin treatment for superficial bladder cancer provides no additional benefit and is not currently used in Korea for this treatment indication. This reassessment was based on RCTs; however, concerns regarding risk of bias were noted due to the small sample sizes of individual studies and insufficient reporting of randomization and allocation concealment methods. Overall, the level of evidence was rated as “low.” Furthermore, most studies (six of seven) were published before 2000, limiting their applicability to current cancer treatment practices. Although Immunocyanin is sometimes used as an adjunct therapy in patients with solid tumors in Korea, the Subcommittee concluded that there is insufficient evidence to support its effectiveness in solid tumors. At the 12th 2024 Health Technology Reassessment Committee meeting (December 13, 2024), the Committee reviewed the Subcommittee’s findings in accordance with Article 4(10) of the Health Technology Reassessment Project Management Guidelines. The Committee concluded that Immunocyanin demonstrates a comparable level of safety to conventional treatments when used as an adjuvant to standard cancer treatment. However, the level of evidence for effectiveness was rated as low due to the lack of evidence on solid tumors other than bladder cancer, concerns regarding risk of bias, and the lack of recent studies. Accordingly, the Health Technology Reassessment Committee determined that, in the current Korean clinical setting, the use of Immunocyanin in addition to standard cancer treatment for treatment of tumors or prevention of cancer recurrence is “not recommended.”
