주의력검사
초록
평가배경 주의력검사의 한 종류인 연속수행검사(Continuous Performance Test, CPT)는 컴퓨터 프로그램을 이용하여 특정 기호나 숫자 등 주어지는 자극에 대해 대상자의 반응 또는 억제를 평가함으로써 주의력결핍 과잉행동장애(Attention-Deficit/Hyperactivity Disorder, ADHD) 환자의 신경심리학적 이상소견을 측정하는 검사이다. 동 기술은 신의료기술평가 제도 도입 이전 2005년 비급여로 등재되었다. 동 기술은 유관기관의 수요조사를 통해 의료기술재평가 주제로 제안되어, 관련 검토 절차를 통해 재평가 필요성이 인정되어 재평가 대상 항목으로 선정되었다. 다양한 대체 의료기술이 존재하는 현 시점에서 비교기술 대비 동 기술의 안정성 및 효과성에 대한 근거를 확인하고자, 2025년 제5차 의료기술재평가위원회(2025.5.16.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 평가의 목적은 ADHD (의심)환자의 ADHD 진단과 약물효과 평가에서 주의력검사가 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고 등급을 결정하기 위함이다. 평가방법 주의력검사에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “주의력검사 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 정신건강의학과 3인, 소아청소년과 2인, 근거기반의학 1인의 전문가 6인으로 구성하였다. 본 평가의 대상자는 주의력결핍 과잉운동장애 의심 환자이며,중재 검사는 주의력검사 중 CPT, 참고표준검사는 종합적 임상진단, 비교검사는 설문지 검사(증상 및 행동 평가 척도 등)로 정의하였다. 결과변수로는 안전성과 효과성을 확인하였으며, 안전성은 검사 관련 부작용으로 정하였고 효과성은 진단정확도와 치료 효과 모니터링을 지표로 평가하였다. 또한 체계적 문헌고찰에서 충분하게 검토되지 못한 국내에서 주로 사용하는 주의력검사에 대하여 추가적인 검색을 진행하였다. 국내에서 많이 사용하는 종합주의력검사(Comprehensive Attention Test, CAT)와 정밀주의력검사(Advanced Test of Attention, ATA)의 신뢰성 및 타당성을 중심으로 검토하였다. 연구문헌은 핵심질문을 토대로 국외 4개(Ovid MEDLINE, Embase(via Ovid), EBM Reviews- Cochrane Central Register of Controlled Trials, APA PsycInfo(via Ovid)), 국내 2개(KoreaMed, 한국교육학술정보원(RISS)) 데이터베이스에서 검색하였다(최종검색일 2025.6.19.). 최종 선택된 연구의 비뚤림위험 평가는 Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2)를 사용하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 진단정확도를 민감도, 특이도, 곡선하면적(Area Under the Curve, AUC)으로 제시하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고 등급을 결정하였다. 평가결과 문헌 24편 중 소아·청소년 대상 연구는 18편(총 1,921명, ADHD 825명, 비교군 1,096명), 성인 대상은 6편(총 3814명, ADHD 1,338명, 비교군 2,476명)이었다. 한국과 이스라엘 연구는 주로 소아 ADHD를, 유럽 연구는 성인 ADHD를, 미국·캐나다는 다양한 CPT 진단정확도 비교에 중점을 두었다. 모든 연구는 정신질환의 진단 및 통계 편람(Diagnostic and Statistical Manual of Mental Disorders, DSM) 진단을 기준으로 집단 간 진단정확도를 평가하였다. 안전성 선정 문헌에서 주의력검사 관려 부작용을 보고한 연구는 없었다. 다만 전문가의 자문을 통하여 CPT 검사는 비침습적이며 컴퓨터 기반의 단순 반응 과제로 구성되어 있어 임상적 안전성이 높고 부작용이 거의 없는 검사로 평가된다. 부작용 보고는 거의 모두 약물 병용 연구에서의 자극제 관련 반응으로 제한되었으며, CPT 자체는 아동·청소년 및 성인 대상 모두에서 안전한 도구로 확인되었다. 효과성 주의력검사의 진단정확도를 보고한 연구는 15편(소아·청소년 9편, 성인 6편)이었다. 소아·청소년 ADHD 진단에서 임상적 평가 기준과 유의한 연관을 보였다. 전산화 신경인지검사(Computerized Neurocognitive Test, CNT)(1편)는 민감도 83%, 특이도 79%, AUC 0.86이었고, Conners CPT(2편)는 민감도 66.7%를 보였다. MOXO-CPT(3편)는 연령별 민감도 81~91%, 주의력 변인 검사(Test of Variables of Attention, TOVA)(1편) 기반 모델은 AUC 0.867이었다. 다중지표 CPT(4편)(의 AUC는 0.76~0.88로 일치율 80% 이상이었다. 성인 ADHD 진단은 감사에 따라 연구 간 진단정확도의 변이가 컸다. CPT 단독 검사의 민감도 33~76%, 특이도 43~91%였다. 치료 효과 평가 관련 연구는 3편으로 약물치료 후 TOVA 지표의 비교검사와 유의한 상관관계를 나타내었으며, 정량화 행동검사(Quantified Behavior Test, QbTest)(4편)의 종합 점수의 상관계수는 0.28 ~ 0.38 사이였다. 국내 CPT 검사에서 종합주의력검사(Comprehensive Attention Test, CAT)(6편)는 검사-재검사의 상관계수는 0.6 ~ 0.85, 내적 일치도 ≥0.75, 기존 인지검사와 상관계수 0.50 ~ 0.70으로 신뢰도와 수렴타당도가 양호했고, ADHD-정상군 간 모든 지표에서 유의한 차이를 보였다. 한국형 전산화 주의력검사(Advanced Test of Attention,ATA)(3편)의 내적 일치도는 시각에서 0.82, 청각에서 0.86, 검사-재검사 상관계수 0.73 ~ 0.88, 기존 검사와의 상관계수 0.65 ~ .80으로 변별 타당도가 확인되었다. 결론 및 제언 의료기술재평가 소위원회에서는 현재 확인된 평가결과에 근거하여 다음과 같은 의견을 제시하였다. 체계적 문헌 검토 및 국내 문헌 검토 결과를 종합하면 CPT 검사는 ADHD 환자의 주의력, 충동성 및 반응억제 기능을 객관적으로 평가할 수 있는 유용한 도구로 확인되었다. 소아·청소년 대상 연구에서는 민감도 66.7~91%, 특이도 79% 이상, AUC 0.76~0.88 수준의 진단 성능을 보여 ADHD와 정상군을 효과적으로 구분하였으며, CAT과 ATA 또한 높은 신뢰도와 타당도를 나타냈다. 또한 CPT는 약물치료 후 누락오류, 반응속도 및 반응속도 변동성의 개선을 민감하게 반영하여 치료반응 평가에도 활용 가능하였다. CPT는 ADHD를 단독으로 진단하는 검사라기보다는 임상적 평가를 보완하고 치료효과를 객관적으로 추적하는 보조적 평가도구로서 임상적 유용성이 있는 것으로 판단된다. 주의력검사(CPT)는 통제된 조건 하에서 주의 유지, 반응 억제, 정보 처리 속도 등 기저 인지 과정을 직접 측정함으로써, 평정 척도 간 결과 불일치 사례나 외현적 증상이 뚜렷하지 않은 부주의 우세형 주의력결핍·과잉 행동장애(ADHD)의 인지적 특성을 확인하는 데 활용될 수 있고, 약물 치료 전후의 주의력 및 충동 억제 변화를 정량적으로 제시하여, 적정 용량 설정 및 치료 반응 모니터링을 위한 임상적 근거를 제공한다. 다만, 주의력검사는 피검자의 각성 수준, 주변 소음, 기기 설정 등 검사 환경과 개인의 상태 요인에 따라 결괏값이 변동될 수 있는 특성을 지닌다. 피검자의 수면부족, 피로도, 지시사항 전달의 일관성 등이 누락 및 오경보 오류 수치에 직접적인 영향을 미칠 수 있으므로, 결과 해석 시에는 임상적 관찰 기록과의 병행 검토가 필수적이라고 제언하였다. 따라서 CPT는 단독 진단 도구로 활용되기보다, 면담 및 행동 평정 척도 결과와 보조적으로 통합되어 진단의 정확도를 높이는 객관적인 임상 보조도구로 유용한 것으로 검토하였다. 2026년 제2차 재평가전문위원회*(2026. 2. 6.)는 ‘주의력검사’에 대하여 다음과 같이 심의하였다. 재평가전문위원회는 CPT에 대하여 임상적 안전성과 효과성의 근거 및 그 외 평가항목을 종합적으로 고려하였을 때, 국내 임상 상황에서 주의력결핍 과잉운동장애 (의심)환자를 대상으로 주의력결핍 과잉운동장애의 진단 및 약물효과 평가를 목적으로 사용할 경우, 단독 진단 도구로의 활용에는 한계가 있으나 면담 및 행동 평정 척도와 병행하는 임상적 보조 도구로 사용하는 것에 한하여 약하게 권고함으로 심의하였다.
Background The Continuous Performance Test (CPT) is a computerized neuropsychological test used to assess attention and response inhibition in patients with Attention-Deficit/Hyperactivity Disorder (ADHD). It evaluates sustained attention and inhibitory control by measuring a subject's responses to target stimuli and the ability to withhold responses to non-target stimuli, such as specific symbols or numbers. This technology was listed as a non-reimbursed service in 2005, prior to the implementation of the New Health Technology Assessment system. The technology was proposed as a candidate for health technology reassessment through a survey of relevant stakeholders. After review, the need for reassessment was confirmed, and the technology was selected for reassessment. To evaluate the safety and effectiveness of this technology compared with currently available alternative technologies, the 5th Health Technology Reassessment Committee in 2025 (May 16, 2025) reviewed and approved the reassessment plan and the proposed composition of the subcommittee, and subsequently initiated the reassessment. Objective The objective of this assessment was to evaluate the safety and effectiveness of the Continuous Performance Test (CPT) for the diagnosis of Attention-Deficit/Hyperactivity Disorder (ADHD) and the assessment of medication effects in patients with suspected or confirmed ADHD, and to establish the recommendation grade for its clinical use through the Health Technology Reassessment Program. Methods A systematic literature review was conducted to evaluate the safety and effectiveness of the Continuous Performance Test (CPT). The assessment methods were developed and finalized through discussions by the CPT Subcommittee (hereinafter referred to as "the Subcommittee"). The Subcommittee consisted of six experts, including three psychiatrists, two child and adolescent psychiatrists, and one specialist in evidence-based medicine. The target population comprised patients with suspected attention-deficit/hyperactivity disorder (ADHD). The index test was defined as the Continuous Performance Test (CPT), the reference standard was comprehensive clinical diagnosis, and the comparator consisted of questionnaire-based assessments, including symptom and behavioral rating scales. The assessment outcomes included safety and effectiveness. Safety was evaluated based on test-related adverse events, while effectiveness was assessed in terms of diagnostic accuracy and the ability to monitor treatment response. In addition, supplementary literature searches were conducted for attention tests that are widely used in Korea but were not adequately represented in the systematic review. This supplementary review focused on the reliability and validity of the Comprehensive Attention Test (CAT) and the Advanced Test of Attention (ATA), both of which are commonly used in clinical practice in Korea. Based on the predefined key questions, literature searches were conducted in four international databases (Ovid MEDLINE, Embase via Ovid, EBM Reviews–Cochrane Central Register of Controlled Trials, and APA PsycInfo via Ovid) and two domestic databases (KoreaMed and the Research Information Sharing Service [RISS]). The final search was conducted on June 19, 2025. The risk of bias of the included studies was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool. Data were extracted using a standardized data extraction form, and diagnostic accuracy was summarized using sensitivity, specificity, and the area under the receiver operating characteristic curve (AUC). Study selection, risk-of-bias assessment, and data extraction were performed independently by two reviewers. Any disagreements were resolved through discussion until consensus was reached. Based on the findings of the Subcommittee, the final recommendation grade was determined by the Health Technology Reassessment Committee after its final deliberation. Results Of the 24 included studies, 18 involved children and adolescents (1,921 participants: 825 with ADHD and 1,096 controls), and 6 involved adults (3,814 participants: 1,338 with ADHD and 2,476 controls). Studies conducted in South Korea and Israel predominantly evaluated pediatric ADHD populations, whereas those conducted in Europe primarily investigated adult ADHD. Studies from the United States and Canada mainly compared the diagnostic performance of different Continuous Performance Tests (CPTs). In all studies, diagnostic accuracy was evaluated against clinical diagnosis based on the Diagnostic and Statistical Manual of Mental Disorders (DSM) criteria, which served as the reference standard. Safety: None of the selected studies reported any adverse events related to the attention test. Furthermore, expert consultation indicated that the CPT is highly safe in clinical settings with almost no side effects, as it is non-invasive and consists of simple computer-based response tasks. Reported side effects were almost exclusively limited to stimulant-related reactions in combined medication studies, confirming that the CPT itself is a safe tool for both children/adolescents and adults. Effectiveness: Fifteen studies reported the diagnostic accuracy of the attention test (9 in children/adolescents, 6 in adults). In diagnosing pediatric and adolescent ADHD, it showed a significant association with clinical assessment criteria. The Computerized Neurocognitive Test (CNT) (1 study) demonstrated a sensitivity of 83%, a specificity of 79%, and an AUC of 0.86, while the Conners CPT (2 studies) showed a sensitivity of 66.7%. The MOXO-CPT (3 studies) showed an age-specific sensitivity of 81-91%, and a model based on the Test of Variables of Attention (TOVA) (1 study) yielded an AUC of 0.867. Multi-indicator CPTs (4 studies) showed an AUC ranging from 0.76 to 0.88, with a concordance rate of over 80%. In diagnosing adult ADHD, substantial variations in diagnostic accuracy were observed across studies depending on the specific test. The CPT alone demonstrated a sensitivity of 33-76% and a specificity of 43-91%. Three studies evaluated treatment effects, showing a significant correlation between post-medication TOVA indicators and comparative assessments, while the correlation coefficient for the composite score of the Quantified Behavior Test (QbTest) (4 studies) ranged between 0.28 and 0.38. Regarding domestic CPT variants, the Comprehensive Attention Test (CAT) (6 studies) demonstrated good reliability and convergent validity, with a test-retest correlation coefficient of 0.6-0.85, an internal consistency of ≥0.75, and a correlation coefficient of 0.50-0.70 with existing cognitive tests. It also showed significant differences across all indicators between the ADHD and normal control groups. The Korean Advanced Test of Attention (ATA) (3 studies) confirmed its discriminant validity, showing an internal consistency of 0.82 for visual and 0.86 for auditory modalities, a test-retest correlation coefficient of 0.73-0.88, and a correlation coefficient of 0.65-0.80 with existing tests. Conclusion and Recommendation Based on the available evidence, the Health Technology Reassessment Subcommittee reached the following conclusions. The findings of the systematic literature review and supplementary review of domestic studies indicate that the Continuous Performance Test (CPT) is a useful tool for objectively assessing attention, impulsivity, and response inhibition in patients with attention-deficit/hyperactivity disorder (ADHD). In studies involving children and adolescents, the CPT demonstrated diagnostic performance with sensitivities ranging from 66.7% to 91%, specificities of at least 79%, and area under the receiver operating characteristic curve (AUC) values ranging from 0.76 to 0.88, enabling discrimination between patients with ADHD and healthy controls. The Comprehensive Attention Test (CAT) and the Advanced Test of Attention (ATA) also demonstrated satisfactory reliability and validity. Furthermore, the CPT was sensitive to changes in omission errors, response time, and response time variability following pharmacological treatment, supporting its usefulness for monitoring treatment response. The CPT objectively measures cognitive functions, including sustained attention, response inhibition, and information-processing speed, under standardized testing conditions. It may therefore provide additional information when clinical findings and behavioral rating scales are inconsistent or when inattentive symptoms are difficult to identify. In addition, quantitative changes in CPT performance before and after medication treatment may assist clinicians in evaluating treatment response and optimizing therapeutic management. However, CPT results may be influenced by testing conditions and individual factors, including arousal level, sleep deprivation, fatigue, environmental distractions, device settings, and the consistency of test administration. Accordingly, CPT findings should be interpreted in conjunction with clinical interviews, behavioral rating scales, and other clinical information, rather than being used in isolation. Based on these findings, the Subcommittee concluded that the CPT should not be used as a standalone diagnostic tool for ADHD. Instead, it has clinical value as an objective adjunctive assessment that complements comprehensive clinical evaluation and supports the assessment of treatment response. At its second meeting in 2026 (February 6, 2026), the Health Technology Reassessment Committee deliberated on the evidence and concluded that, although the CPT has demonstrated clinical safety and effectiveness, the available evidence does not support its use as a standalone diagnostic tool for diagnosing ADHD or evaluating medication effects in patients with suspected or confirmed ADHD. Therefore, the Committee issued “a weak recommendation” for the use of the CPT only as an adjunctive clinical assessment, in combination with clinical interviews and behavioral rating scales. *The Health Technology Reassessment Expert Committee was newly formed and has been in operation since September 2025 due to the amendment of the 「Rules on the New Healthcare Technologies Assessment」(Ministry of Health and Welfare Ordinance No. 1098, partially amended, effective September 7, 2025).
