초록

평가배경 호기산화질소 측정(Exhaled Nitric Oxide Measurement)은 천식 환자에서 비침습인 방법을 이용하여 호산구성 기도 염증의 정도를 파악하기 위해 사용하는 기술이다. 천식은 가장 흔한 만성 기도질환 중 하나로, 호흡곤란, 천명, 가슴답답함, 기침, 호기 기류제한과 같은 특징을 보인다. 호기산화질소 측정은 2013년 신의료기술평가를 거쳐 비급여 등재된 기술이다(보건복지부 고시 제2015-59호, 2015.4.15.). 동 기술은 내부 모니터링을 통해 재평가 안건으로 발굴되었고, 다양한 대체 의료기술이 존재하는 현 시점에서 비교기술 대비 동 기술의 효과성에 대한 근거를 확인하고자, 2024년 제4차 의료기술재평가위원회(2024.4.12.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 호기산화질소 측정검사가 천식 환자에서 호산구성 기도 염증의 정도를 파악하는데 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 호기산화질소 측정에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “호기산화질소 측정 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 호흡기내과 2인, 소아청소년과 2인, 근거기반의학 1인의 전문가 5인으로 구성하였다. 소위원회 논의를 통해 진단정확도가 확립되지 않은 소아천식에서의 호기산화질소 측정의 진단정확도를 확인하는 것과 천식 환자에서의 치료반응 모니터링 및 약제 반응성을 확인하는 것에 대한 안전성과 효과성을 확인하고자 하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2024.7.1.). 최종 선택된 연구의 비뚤림위험 평가는 평가 가능한 연구를 대상으로 수행하였다. 소아천식에서의 진단정확도를 보고한 13편의 진단법 평가연구는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를, 천식에서의 치료반응 모니터링 결과를 보고한 14편의 무작위배정 임상시험 연구는 Cochrane의 Risk of Bias (RoB)를 이용하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 진단정확도를 2 x 2표로 제시하고, 양적 분석이 불가능하다고 판단하여 메타분석은 수행하지 않고 결과값의 범위와 산포도로 제시하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과, 최종 선택된 연구는 총 48편이었고 연구대상자는 천식(의심) 진단을 받은 소아 또는 성인이었다. 비뚤림위험 평가결과, 소아천식의 진단정확도 보고 연구에서는 대체로 비뚤림위험이 ‘낮음’이긴 하나 환자선택, 중재검사 영역에서 ‘불확실’이 각 21.4%, 28.6%이고 참고표준검사 영역에서 임상기준에 따른 진단 또는 언급이 명확하지 않아 ‘불확실’이 50.0%, 연구진행과 시점 영역에서 비뚤림위험을 높일 가능성이 있는 경우에 ‘높음’으로 평가하였다(14.3%). 천식에서의 치료반응 모니터링 및 약제 반응 연구에서는 환자선택 영역에서는 ‘불확실’이 50%, 연구참여자, 연구자 및 결과평가에 대한 눈가림 언급이 불확실하거나 눈가림되지 않아 ‘불확실’과 ‘높음’(각 50%, 14%)으로, 민간 연구비 지원을 받은 경우 비뚤림위험 ‘높음’으로 평가하였다(42.9%). 안전성 호기산화질소 측정의 안전성은 검사 관련 부작용 및 이상반응으로 평가하고자 하였으나, 해당 지표를 보고한 연구는 없었다. 효과성 호기산화질소 측정의 효과성은 소아천식 환자에서의 진단정확도, 천식(소아, 성인) 환자에서의 치료반응 모니터링과 약제(생물학적 제제) 반응성을 지표로 평가하였다. 소아천식 환자에서 천식 진단 보조 목적으로 호기산화질소 측정 검사가 효과적인지 확인하기 위해 일차 지표를 진단정확도, 이차 지표를 비교검사와의 상관성으로 설정하였고, 이를 보고한 연구 총 32편(진단정확도 14편, 비교검사와의 상관성 19; 1편 중복)을 확인하였다. 진단정확도를 보고한 연구 14편이었다. 임계치 25ppb(7편)를 기준으로 했을 때의 진단정확도는 민감도 0.27~0.85, 특이도 0.88~0.97, AUC 0.63~0.93이었으며, 임계치 35ppb(4편)를 기준으로 했을 때의 진단정확도는 민감도 0.23~0.40, 특이도 0.94~0.99, ACU 0.54~0.69이었다. 비교검사가 메타콜린 기관지유발검사인 경우(2편)의 진단정확도는 민감도 0.83~0.88, 특이도 0.69~0.83, AUC 0.72~0.90이었으며, 혈액 호산구수와 비교한 경우(2편)의 진단정확도는 민감도 0.76~0.85, 특이도 0.72~0.89, AUC 0.78~0.92, 객담 호산구수(1편)의 민감도 0.99, 특이도 0.917, AUC 0.96이었다. 비교검사와의 상관성을 보고한 연구 19편 중 유의한 상관성을 보고한 경우의 결과값만 포함하여 범위를 확인한 결과, 메타콜린 기관지유발검사와의 상관성(7편)은 -0.411~0.338이었고, 혈액 호산구수와의 상관성(7편)은 0.112~0.522, 객담 내 호산구수와의 상관성(3편)은 0.297~0.438이었다. 천식 환자에서 호기산화질소 측정을 활용한 치료반응 모니터링 관련 연구는 총 14편(소아 8편, 성인 6편)이 확인되었다. 호기산화질소 측정을 기반으로 한 조절법과 기존의 증상 또는 임상진료지침에 기반한 조절법을 비교한 결과, 소아와 성인 모두에서 증상 악화 관련 지표(증상 악화 발생 횟수, 무증상 기간)에 대해 중재검사가 기존 조절법보다 효과적이거나 유사한 수준이었다. 삶의 질에서는 두 군 간 유의한 차이가 없었으며, 의료 이용 관련 지표에서도 대부분의 연구에서 중재검사가 효과적이거나 두 군 간 차이가 없었다. 다만, 일부 연구(2편)에서는 호기산화질소 측정을 단독으로 적용한 경우, 약물 용량 모니터링 측면에서 기존 조절법이 더 효과적이었다. 천식 환자에서 호기산화질소 측정을 이용한 약제(생물학적 제제) 반응성 확인 관련 연구 3편을 확인하였으나, 비교검사 대비 동 검사의 효과성의 확인하기에는 문헌이 충분하지 않았다. 결론 및 권고결정 의료기술재평가 소위원회에서는 호기산화질소 측정의 안전성과 효과성 결과를 현재 확인된 근거를 바탕으로 다음과 같이 제시하였다. 체계적 문헌고찰 결과, 천식 환자에서 진단보조 목적으로 사용하는 호기산화질소 측정검사의 관련 부작용 및 이상반응 결과를 보고한 연구는 없었으나, 동 검사는 비침습적인 방법으로 시행되는 검사이므로 안전하다고 평가하였다. 동검사의 효과성은 소아천식 환자와 천식 환자로 나누어 확인하였다. 소아천식 환자에서 호기산화질소 측정은 천식 진단을 보조하는 검사로서 비교검사인 기관지유발검사(메타콜린), 혈액 호산구수 측정검사와 유사한 수준의 효과성이 확인되고, 특히 검사에 대한 협조가 용이하다는 장점이 있어 진단적 가치가 있는 것으로 판단하였다. 천식에서의 약제 및 치료반응 모니터링 목적으로 사용하는 호기산화질소 측정검사의 관련 부작용 및 이상반응 결과를 보고한 연구는 없었으며 천식 환자에서 동 검사가 비침습적인 방법으로 시행되므로 안전하다고 평가하였다. 효과성의 경우 천식 환자에서 치료반응을 모니터링하기 위한 목적으로 호기산화질소 측정과 증상 또는 가이드라인 기반으로 한 기존 조절법을 비교 시, 증상악화 관련 결과에서는 소아ㆍ성인 모두 중재검사가 비교검사 대비 효과가 있거나 효과성에 차이가 없었고, 삶의 질에서는 두 군 간 유의한 차이가 없었다. 의료이용 관련 결과에서는 대부분의 연구에서 중재검사에서 효과가 있거나 두 군 간 차이가 없었으나, 일부 연구(2편)에서 호기산화질소 측정을 단독으로 검사 시 약물이용량을 모니터링 하는데 비교검사가 더 효과적인 것을 확인하였다. 천식에서 호기산화질소 측정을 이용한 약제(생물학적 제제) 반응성 예측 관련 연구 총 3편을 확인하였으나, 비교검사 대비 동 검사의 효과성을 확인하기에는 문헌이 충분하지 않았다. 다만, 호기산화질소 측정은 혈액 호산구 수보다 결과를 즉시 확인할 수 있으며, 특히 혈중 호산구 수가 높지 않은 제2형 염증 환자에서도 생물학적 제제에 대한 반응이 양호한 것으로 보고되고 있어, 이러한 환자군을 선별하는 데 있어 호기산화질소가 혈중 호산구 수를 보완하는 지표로 활용될 수 있다는 의견이 제시되었다. 이에 따라 호기산화질소 측정은 생물학적 제제 사용 결정의 근거로 활용 가능성이 있으나, 이를 입증하기 위한 추가 연구가 필요하다고 제언하였다. 이에 소위원회에서는 동 검사가 소아천식 환자에서 천식 진단을 보조하는데 임상적으로 안전하고 효과적인 기술로 판단하였다. 또한 천식 환자에서 치료반응을 모니터링하고 생물학적 제제 사용 결정에 도움을 줄 수 있는 안전하고 효과적인 기술로서 소아와 성인 모두에서 검사 대안 중 하나가 될 수 있는 기술이라고 판단하였다. 실제 임상에서 동 검사는 비침습적이고 검사시간이 짧아 소아천식 환자의 천식 진단을 목적으로 사용 시 더 유용할 뿐 아니라 중증천식 환자의 생물학적 제제 사용이 점차 증가하는 현시점에서 동 검사가 치료방향을 결정하는데 도움이 될 수 있어 임상적으로 중요성이 점차 증가하고 있다는 의견이었다. 2025년 제1차 의료기술재평가위원회(2025.1.10.)는 ‘호기산화질소 측정’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 등을 종합적으로 고려하였을 때, 국내 임상상황에서 소아천식 환자에서 호기산화질소 측정 검사를 진단보조 목적으로 사용하는 것을 ‘권고함’으로 결정하였다(단, 검사의 협조가 어려운 5세 미만의 소아에서는 검사 결과의 판단에 유의가 필요함). 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 등을 종합적으로 고려하였을 때, 국내 임상상황에서 천식 환자에서 약제 및 치료반응 모니터링 목적으로 사용하는 호기산화질소 측정 검사를 ‘권고함’으로 결정하였다.


Background Exhaled nitric oxide measurement is a non-invasive technique used to assess eosinophilic airway inflammation in patients with asthma. Asthma, characterized by symptoms such as shortness of breath, wheezing, chest tightness, cough, and expiratory airflow limitation, is one of the most common chronic airway diseases. This technology was evaluated through the New Health Technology Assessment in 2013 and listed as a non-reimbursable item (MOHW Bulletin No. 2015-59, April 15, 2015). The technology was identified as a candidate for reassessment through internal monitoring. Given the availability of various alternative medical technologies, the 4th 2024 New Health Technology Reassessment Committee (held on April 12, 2024) reviewed and approved the reassessment plan and subcommittee composition to evaluate the effectiveness of this technology compared to alternative modalities. Objectives The objectives of this reassessment were to determine whether exhaled nitric oxide measurement is clinically safe and effective for assessing eosinophilic airway inflammation in patients with asthma and determine the appropriate strength of recommendation for its continued use. Method The safety and effectiveness of exhaled nitric oxide measurement were assessed through a systematic literature review. All assessment methods were finalized after discussions within the “Subcommittee for Exhaled Nitric Oxide Measurement” (hereinafter the “Subcommittee”) based on the reassessment objective. The Subcommittee comprised five experts: two pulmonologists, two pediatricians, and one evidence-based medicine expert. The Subcommittee aimed to assess the diagnostic accuracy of exhaled nitric oxide measurement in pediatric asthma, where diagnostic criteria are not well established, as well as its safety and effectiveness for treatment monitoring and drug responsiveness in patients with asthma. Literature searches were conducted in three international databases (Ovid MEDLINE, Ovid EMBASE, and EBM Reviews - Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, KMbase, and RISS) (Final search date July 1, 2024). The risk of bias was assessed for eligible studies. The Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool was used for 13 diagnostic evaluation studies in pediatric asthma, and the Cochrane Risk of Bias (RoB) tool was used for 14 randomized controlled trials (RCTs) evaluating treatment monitoring in asthma. Data were extracted using a predefined format, and diagnostic accuracy was presented using 2 × 2 tables. Meta-analysis was not conducted due to heterogeneity; instead, diagnostic accuracy ranges and dispersion plots were presented. All steps were conducted independently by two reviewers, and discrepancies were resolved by consensus. The final strength of recommendation was determined based on Subcommittee findings and reviewed at the nHTA meeting. Results A total of 48 studies were included in the final systematic review. Study participants were pediatric or adult patients with a diagnosis of or suspected asthma. Most diagnostic evaluation studies in pediatric asthma were rated as having a “low” overall risk of bias; however, the domains for patient selection and index test were rated as having an “unclear” risk of bias in 21.4% and 28.6% of studies, respectively. For the reference standard domain, a bias risk of 50.0% was rated as having an “unclear” risk of bias due to a lack of clarity regarding the clinical diagnostic criteria. The flow and timing domain was rated as having a “high” risk of bias in 14.3% of studies due to factors that may have introduced bias. For studies assessing treatment monitoring and drug responsiveness in asthma, 50% had an “unclear” risk of bias in patient selection, and the blinding of participants, investigators, and outcome assessment was either unclear or not performed, resulting in “unclear” and “high” risk of bias ratings in 50% and 14% of studies, respectively. Studies funded by private industry were rated as having a “high” risk of bias in 42.9% of cases. Safety We aimed to assess the safety of exhaled nitric oxide measurement based on procedure-related adverse events and complications; however, no studies reported outcomes for these indicators. Effectiveness The effectiveness of exhaled nitric oxide measurement was assessed based on diagnostic accuracy in pediatric asthma and treatment monitoring and drug (biologic) responsiveness in pediatric and adult patients with asthma. Diagnostic accuracy was set as the primary outcome, and correlation with comparator tests as the secondary outcome to evaluate whether exhaled nitric oxide measurement is effective as a diagnostic adjunct in pediatric asthma. A total of 32 studies were identified (14 reporting diagnostic accuracy, 19 reporting correlations, and 1 including both). Fourteen studies reported data on diagnostic accuracy. At the 25 ppb threshold (seven studies), sensitivity ranged from 0.27 to 0.85, specificity from 0.88 to 0.97, and area under the curve (AUC) from 0.63 to 0.93. At the 35 ppb threshold (four studies), sensitivity ranged from 0.23 to 0.40, specificity from 0.94 to 0.99, and AUC from 0.54 to 0.69. In studies using methacholine bronchial provocation tests as comparators (two studies), sensitivity ranged from 0.83 to 0.88, specificity from 0.69 to 0.83, and AUC from 0.72 to 0.90. In studies comparing with blood eosinophil counts (two studies), sensitivity ranged from 0.76 to 0.85, specificity from 0.72 to 0.89, and AUC from 0.78 to 0.92. One study comparing it with sputum eosinophil count reported a sensitivity of 0.99, specificity of 0.917, and AUC of 0.96. Among the 19 studies reporting correlations with comparator tests, the range of statistically significant correlation coefficients was as follows: methacholine challenge test (seven studies), −0.411 to 0.338; blood eosinophil count (seven studies), 0.112–0.522; and sputum eosinophil count (three studies), 0.297–0.438. A total of 14 studies (8 pediatric and 6 adult) evaluated the use of exhaled nitric oxide measurement for treatment monitoring in asthma. When comparing adjustment strategies based on exhaled nitric oxide (fractional exhaled nitric oxide, FeNO) levels versus symptom-based or guideline-based management, the intervention demonstrated comparable or greater effectiveness in terms of outcomes related to symptom exacerbation (e.g., number of exacerbations and duration of symptom-free periods) in both children and adults. There were no significant differences between groups regarding quality of life. Most studies reported that the intervention was either more effective or similar in terms of healthcare utilization outcomes. However, two studies reported that the comparator approach was more effective in monitoring medication dosage when exhaled nitric oxide measurement alone was used to guide treatment. Three studies evaluated exhaled nitric oxide measurement as a tool for assessing drug (biologic) responsiveness in asthma. However, the evidence was insufficient to determine its effectiveness compared to comparator tests. Conclusion and Strength of Recommendation Based on the current evidence, the Subcommittee presented the following findings regarding its safety and effectiveness. No studies reported adverse events or complications related to exhaled nitric oxide measurement when used as a diagnostic adjunct in patients with asthma. However, the procedure is non-invasive and was therefore considered safe. Effectiveness was assessed separately for pediatric and adult asthma populations. Exhaled nitric oxide measurement was effective as a diagnostic adjunct in pediatric patients, as evidence suggested diagnostic accuracy comparable to that of methacholine challenge testing and blood eosinophil count. The test was considered particularly valuable in pediatric populations due to its ease of cooperation. Given its non-invasive nature and the lack of studies reporting adverse events or complications associated with the procedure, it was considered safe regarding treatment monitoring and drug responsiveness in patients with asthma. In terms of effectiveness, exhaled nitric oxide-based management showed either superior or comparable outcomes in reducing symptom exacerbations compared to conventional strategies based on symptoms or clinical guidelines in both pediatric and adult patients. No significant differences in quality of life were found between the two groups. For healthcare utilization outcomes, most studies reported that the intervention was either more effective or showed no significant difference compared to the comparator. However, two studies found that the comparator was more effective for monitoring medication dosage compared to using exhaled nitric oxide measurement alone. Three studies investigated the use of exhaled nitric oxide measurement to predict drug (biologic) responsiveness in asthma; however, the evidence was insufficient to determine its comparative effectiveness. Furthermore, the Subcommittee noted that exhaled nitric oxide levels are available immediately, unlike blood eosinophil count, and may be a useful supplementary indicator to blood eosinophil count in identifying patients with Type 2 inflammation who exhibit a good response to biological therapy despite having low eosinophil levels. Thus, while the test shows potential for informing decisions regarding the use of biologic therapies, further studies are needed to confirm its utility in this context. Accordingly, the Subcommittee determined that exhaled nitric oxide measurement is a clinically safe and effective diagnostic adjunct for pediatric asthma. It was also considered a safe and effective option for monitoring treatment response and informing decisions on biological therapy in patients with asthma, and may be used as one of several alternatives in both pediatric and adult populations. In clinical practice, this test is particularly advantageous for diagnosing asthma in pediatric patients owing to its non-invasiveness and short test time. Additionally, the test is gaining clinical significance for its value in informing treatment decisions with the increased use of biological therapies in severe asthma. At the 1st 2025 Health Technology Reassessment Committee meeting (January 10, 2025), the committee reviewed exhaled nitric oxide measurement as follows. Considering the clinical safety and effectiveness, the Health Technology Reassessment Committee determined that exhaled nitric oxide measurement is “recommended” for use in pediatric asthma as a diagnostic adjunct in the Korean clinical setting. (Note: Careful interpretation is advised when applying this test in children younger than 5 due to challenges in test cooperation.) Considering the clinical safety and effectiveness, the committee also determined that exhaled nitric oxide measurement is “recommended” for use in treatment monitoring and drug responsiveness assessment in patients with asthma in the Korean clinical setting.
