초록

평가배경 정량적 감각기능 검사(quantitative sensory test)는 말초신경염에 의한 감각신경의 손상 유무와 심한 정도를 상⸱하지 말단 부위에서 진동감각, 온도감각, 특정 주파수 전류 등 자극에 대해 감각을 인지할 수 있는 자극 강도를 정량적으로 측정하여 감각기능의 이상을 진단, 치료에 대한 반응을 관찰, 추적하는 목적으로 사용되는 검사이다. 이 중 온도역치 검사는 가열수조 등의 온도조절 장치를 이용하여 단계별로 온도감각을 특정 피부에 가하여 온도의 변화를 감지할 수 있는 임계 수치를 측정하는 검사로, 신의료기술평가제도 도입 이전인 2002년 100분의 100 본인부담 항목으로 등재된 후, 2006년 비급여로 전환되었다(보건복지부 고시 제2005-89호, 2005.12.22.). 동 기술은 비급여 보고제도 대상 항목으로, 대국민 정보제공을 위한 유관기관의 수요조사를 통해 발굴되었다. 2025년 제8차 의료기술재평가위원회(2025.8.8.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 ‘정량적 감각기능 검사-온도역치’에 대한 사용대상 및 평가도구에 대한 근거를 검토하여 임상 현장에서의 적절한 사용을 지원하기 위한 정보를 제공하기 위함이다. 평가방법 정량적 감각기능 검사는 다양한 병인을 가진 이질적인 질환에서 사용되고, 중재 및 비교검사가 단계적, 상호보완적으로 활용되는 특성을 고려하여, 본 평가에서는 효과성 비교 중심의 체계적 문헌고찰 대신 임상적 맥락을 반영하기 위해 교과서 및 임상진료지침을 검토하였다. 모든 평가방법은 평가목적을 고려하여 “정량적 감각기능 검사 공동 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 신경과 2인, 내분비내과 2인, 재활의학과 1인, 총 5인의 전문가로 구성하였다. 본 평가의 핵심질문은 “정량적 감각기능 검사-온도역치는 말초신경병증에서 감각신경 기능을 평가하는 데 임상적으로 효과적인가?”로 설정하였다. 대상 환자와 관련하여 현행 복지부 고시에서 ‘말초신경염’으로 규정되어 있으나 현재 임상에서 말초신경염과 말초신경병증을 혼용해 사용하고, 출판된 문헌에서도 대부분 말초신경병증으로 기술하고 있어 소위원회 논의를 거쳐 본 평가의 대상 환자는 ‘말초신경병증’으로 설정하였다. 비교검사와 관련하여 당뇨병 말초신경병증의 진단은 현재 건강보험 요양급여목록에 등재되지 않은 당뇨병 신경병증 선별검사(Michigan neuropathy screening instrument) 자가 설문과 모노필라멘트 검사로 1차 선별을 시행한 후, 이상 소견 시 등재 급여인 신경전도검사(nerve conduction study)를 표준검사로 활용하고 있다. 다만 신경전도검사는 주로 Aβ 신경섬유를 평가하여 모든 신경 손상을 단독으로 확인하기 어려우므로 중재 및 비교검사는 상호보완적으로 임상에서 활용되고 있다는 소위원회 의견에 따라 비교검사는 설정하지 않았다. 의료결과는 임상적 유용성을 확인하였다. 교과서는 신경학, 신경근육질환, 당뇨병학, 재활의학 분야를 검토하였다. 임상진료지침은 국내 임상진료지침 정보센터(Korean Medical Guideline Information Center), 국제 진료지침 네트워크(Guideline International Network), Trip Medical Database 등에서 ‘neuropathy’, ‘quantitative sensory’, ‘QST’, ‘vibration’, ‘thermal’, ‘current/current perception’, ‘electrical’, ‘pain’, ‘threshold’ 등 주요어를 조합하여 검색하였다(2025.9.10). 이후 소위원회에서 교과서 및 임상진료지침의 적절성을 확인하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종 심의하였다. 평가결과 교과서 검토 결과 교과서 4편을 검토한 결과, 정량적 감각기능 검사는 신경전도검사로 평가하기 어려운 얇은 유수초 또는 무수초 신경섬유(Aδ, C)를 포함한 소신경섬유의 기능을 정량적으로 평가하는 검사로 제시하였다. 또한 당뇨병 신경병증이나 소섬유신경병증(small fiber neuropathy, SFN) 등에서 무증상 단계의 신경 손상을 조기에 발견하는 데 유용하며, 신경전도검사의 한계를 보완하는 보조적 검사로 활용될 수 있다고 보고하였다. 검사 항목에는 진동감각역치(대신경섬유 평가), 온도역치(소신경섬유 평가), 전류감각역치(무수·소·대신경섬유 구분 평가) 등을 포함하며, 이를 통해 신경 손상의 범위와 정도, 기능 변화를 정량적으로 평가할 수 있다. 이 중 온도역치 검사는 소신경섬유의 기능적 상태를 반영하는 지표로, 온감 및 냉감을 통해 측정할 수 있으며, 일반적으로 온감역치가 냉감역치보다 더 민감한 것으로 제시하였다. 또한 당뇨병 환자에서 온도감각 저하로 인한 화상 위험 증가와 관련이 있어 임상적으로 중요한 지표로 활용될 수 있다. 아울러 SFN의 확정진단은 신경전도검사가 정상인 경우, 발 부위의 온도감각역치 이상과 표피내 신경섬유 밀도(intraepidermal nerve fiber density, IENFD) 감소를 함께 확인하도록 제시하고 있어 온도역치 검사는 소신경섬유의 기능 이상을 평가하는 중요한 진단적 근거로 활용될 수 있다. 임상진료지침 검토 결과 국내외 임상진료지침 14편을 검토한 결과, 정량적 감각기능 검사는 당뇨병 (말초)신경병증과 SFN을 포함한 말초신경병증 환자의 감각기능과 신경 손상 정도를 평가하기 위한 보조적 진단검사로 제시하였다. 구체적으로는 진동, 온도, 전류인지, 통증인지 자극에 대한 감각역치를 정량화하여 감각 저하 및 통각 감소와 같은 음성 감각이상뿐 아니라 이질통 및 과민통 등의 양성 감각이상을 정량적으로 평가할 수 있는 검사로 보고하였다. 다만 자극 유형과 관련된 신경섬유 유형(Aβ, Aδ, C 등)의 구체적 구분 기준은 대부분의 지침에서 명확히 제시하지 않았다. 또한 다수의 지침에서 정량적 감각기능 검사는 검사자의 숙련도나 피검자의 주관적 반응에 따라 결과가 영향을 받을 수 있어 단독 진단검사로 활용하기에는 한계가 있는 것으로 언급하였다. 이에 임상 소견과 함께 신경전도검사, 피부 생검, 자율신경기능 검사 등과 병행하여 해석하는 보조적 검사로 활용할 것을 제안하였다. 일부 지침에서는 정량적 감각기능 검사를 통해 감각역치를 정량화하여 환자의 감각기능 전반의 양상과 패턴을 체계적으로 기록한 감각 프로파일을 문서화할 수 있으며, 이를 기반으로 치료 반응 및 예후 평가에 활용할 수 있다고 제시하였다. 이 중 온도역치 검사는 Aδ 및 C 신경섬유의 기능을 반영하는 핵심 평가항목으로서 온감, 냉감 및 열통각역치를 측정하여 소섬유신경 기능 변화를 민감하게 평가할 수 있는 것으로 공통적으로 언급하였다. 신경병증성 통증에서는 열역치가 IENFD와 유의한 상관성을 보여 보조적 진단검사로 제시하였으며, 소아‧청소년에서 SFN 또는 자가면역신경병증이 의심되는 경우에도 동 검사의 활용 가능성을 제시하였다. 또한 당뇨병 신경병증에서는 열지각역치가 감각 변화의 기록 및 종단적 평가에 유용할 가능성이 있는 것으로 보고하였다. 결론적으로 대부분의 임상진료지침에서는 온도역치를 포함한 정량적 감각기능 검사 결과는 임상 소견, 피부 생검 등 다른 임상 검사와 함께 종합적으로 해석할 것을 제안하였다. 결론 의료기술재평가 소위원회에서는 현재의 평가 결과에 근거하여 다음과 같이 제언하였다. 정량적 감각기능 검사는 교과서 및 임상진료지침에서 말초신경병증 환자의 감각기능 이상을 확인하는 검사로 활용되고 있으며, 사람마다 통증을 느끼는 정도가 다르고 신경전도검사로 확인하기 어려운 작은 유수초 및 무수초 신경섬유를 측정할 수 있다는 점에서 진단 보조적 가치가 있다고 판단하였다. 다만 환자의 주관적 증상을 정량화할 수 있는 이점에도 불구하고, 개인별 통증역치의 차이, 장비 간 측정값의 비일관성, 개별 검사들의 낮은 민감도 및 특이도, 검사자 숙련도에 따른 결과 변동 등 여러 한계를 고려할 필요가 있다고 보았다. 정량적 감각기능 검사 중 온도역치 검사는 온도 자극에 대한 감각역치를 측정하여 주관적 증상을 정량적으로 제시할 수 있으나, 단독 검사로 활용하기에는 한계가 있어 진동역치 등과 함께 감각기능 이상을 평가하는 보조적 검사로 활용될 수 있다는 의견이었다. 종합하면, 정량적 감각기능 검사는 소섬유신경병증 등 말초신경병증 환자에서 감각신경 이상을 확인하는 데 있어 단독 진단검사로 사용하기보다는 다른 임상 검사와 병행하는 상호보완적 검사로 활용할 수 있다. 또한, 검사방법의 표준화를 위해 연령, 성별, 인종 등 인구집단 특이적 참고치 마련과 함께, 질환별 검사의 신뢰도 및 타당도에 대한 추가 연구가 필요하다고 제언하였다. 2026년 제1차 재평가전문위원회*(2026.1.9.)는 소위원회 검토 결과를 바탕으로 ‘정량적 감각기능 검사-온도역치’에 대해 심의하였으며, 소위원회의 결론을 원안대로 의결하였다.


Background Quantitative sensory testing (QST) quantifies the intensity of stimuli—such as vibration, temperature, and specific-frequency electrical current—required for a patient to perceive sensation at the distal upper and lower extremities, and is used to diagnose sensory nerve damage caused by peripheral neuropathy, and to monitor and track treatment response. Among QST modalities, thermal threshold testing measures the threshold at which a change in temperature can be detected, using a thermode connected to a temperature-controlled device (e.g., a heating water bath) applied stepwise to a specific area of skin. This test was listed in 2002, prior to the introduction of the new health technology assessment system, as a 100% patient-copayment item, and was converted to non-reimbursed status in 2006 (Ministry of Health and Welfare Notification No. 2005-89, December 22, 2005). This technology is subject to the non-reimbursement reporting system and was identified through a demand survey conducted by relevant institutions for the purpose of providing information to the public. At the 8th Health Technology Reassessment Committee meeting of 2025 (August 8, 2025), the reassessment plan and subcommittee composition were deliberated, and the reassessment was subsequently conducted. Purpose The purpose of this assessment is to review the evidence on the target population and assessment tools for "quantitative sensory testing—thermal threshold," in order to provide information supporting its appropriate use in clinical practice. Methods Considering that QST is used across a heterogeneous range of diseases with diverse etiologies, and that the intervention and comparator tests are applied in a stepwise, complementary manner, this assessment reviewed textbooks and clinical practice guidelines to reflect clinical context, rather than conducting an effectiveness-focused systematic literature review. All assessment methods were finalized through discussion by the "Quantitative Sensory Testing Joint Subcommittee (hereinafter the 'Subcommittee')," which was composed of five experts (2 neurologists, 2 endocrinologists, and 1 rehabilitation medicine specialist). The key question was set as: "Is quantitative sensory testing—thermal threshold clinically effective for evaluating sensory nerve function in peripheral neuropathy?" Although the current Ministry of Health and Welfare notification specifies "peripheral neuritis," the Subcommittee set the target population as "peripheral neuropathy," reflecting current clinical usage and the published literature. No comparator test was designated, based on the Subcommittee's opinion that the intervention and existing tests (e.g., screening questionnaires, monofilament testing, and nerve conduction studies) are used in a complementary rather than substitutive manner in diagnosing diabetic peripheral neuropathy. The health outcome was clinical utility. Textbooks in neurology, neuromuscular disease, diabetology, and rehabilitation medicine were reviewed, and clinical practice guidelines were searched in the Korean Medical Guideline Information Center, the Guidelines International Network, and the Trip Medical Database using keywords including "neuropathy," "quantitative sensory," "QST," "vibration," "thermal," "current/current perception," "electrical," "pain," and "threshold" (search date: September 10, 2025). Based on the Subcommittee's review, the Reassessment Expert Committee conducted the final deliberation. Results A review of 4 textbooks indicated that QST quantitatively evaluates the function of small nerve fibers (Aδ, C), including thinly myelinated and unmyelinated fibers that are difficult to assess with nerve conduction studies, and is useful for the early detection of subclinical nerve damage in conditions such as diabetic neuropathy and small fiber neuropathy (SFN), serving as an adjunctive test that complements the limitations of nerve conduction studies. Among the QST modalities, thermal threshold testing—assessed via warm and cold sensation—reflects the functional status of small nerve fibers, with warm-sensation thresholds generally reported as more sensitive than cold-sensation thresholds. This test is also clinically relevant given its association with an increased risk of burns in diabetic patients due to impaired thermal sensation, and is used alongside reduced intraepidermal nerve fiber density to support the diagnosis of SFN when nerve conduction studies are normal. A review of 14 domestic and international clinical practice guidelines indicated that QST is presented as an adjunctive diagnostic test for evaluating sensory function and the degree of nerve damage in patients with peripheral neuropathy, including diabetic peripheral neuropathy and SFN, by quantifying sensory thresholds for vibration, thermal, current-perception, and pain stimuli—capturing both negative sensory phenomena (e.g., hypoesthesia, hypoalgesia) and positive phenomena (e.g., allodynia, hyperalgesia). Most guidelines noted that QST results can be influenced by examiner skill and subjective patient response, limiting its use as a standalone diagnostic test, and recommended its use alongside nerve conduction studies, skin biopsy, and autonomic function testing. Some guidelines also noted that QST can be used to document a systematic sensory profile to support the evaluation of treatment response and prognosis. Thermal threshold testing was commonly identified as a key modality reflecting Aδ and C fiber function, with warm, cold, and heat-pain thresholds sensitively capturing changes in small-fiber function; heat threshold showed a significant correlation with intraepidermal nerve fiber density in neuropathic pain, and potential utility was also noted in pediatric and adolescent patients with suspected SFN or autoimmune neuropathy, as well as for longitudinal monitoring of sensory change in diabetic neuropathy. Most guidelines recommended that thermal threshold results be interpreted comprehensively together with clinical findings and other tests such as skin biopsy. Conclusion Based on the assessment results, the Health Technology Reassessment Subcommittee concluded that QST has adjunctive diagnostic value, as it is used in textbooks and clinical practice guidelines to identify sensory function abnormalities in patients with peripheral neuropathy and can measure small myelinated and unmyelinated nerve fibers that are difficult to assess with nerve conduction studies, given individual variability in pain perception. However, several limitations were noted, including inter-individual variation in pain threshold, inconsistency of measurements across devices, low sensitivity and specificity of individual tests, and result variability depending on examiner skill. Thermal threshold testing can quantitatively represent subjective sensory symptoms by measuring the sensory threshold to thermal stimuli, but has limitations as a standalone test and is more appropriately used as an adjunctive test alongside vibration threshold and other modalities to evaluate sensory dysfunction. Overall, QST is more appropriately used as a complementary test in conjunction with other clinical tests, rather than as a standalone diagnostic test, for identifying sensory nerve abnormalities in patients with peripheral neuropathy, including SFN. The Subcommittee further recommended establishing population-specific reference values (by age, sex, ethnicity, etc.) to standardize testing methods, along with additional research on the reliability and validity of the test across different disease conditions. At the 1st Reassessment Expert Committee meeting of 2026* (January 9, 2026), the Committee deliberated on "quantitative sensory testing—thermal threshold" based on the Subcommittee's review, and resolved to adopt the Subcommittee's conclusion as originally proposed.
