초록
평가배경 ‘증상 및 행동 평가척도(우울척도)-신경증우울평가’는 우울(의심)장애 환자의 우울 증상을 평가하기 위한 검사로, 특정한 평가척도가 명확히 정해져 있지 않은 특징이 있다. 「국민건강보험 비급여정보포털」의 행위설명에 따르면, 본 검사는 환자의 우울감을 확인하기 위한 자기보고형 또는 검사자 평가형 검사로, 우울의 양상 및 정도를 측정하여 진단, 감별진단 및 치료 경과 파악 등에 활용되는 것으로 정의되어 있다. 해당 기술은 의료기술 재평가 대상 선정을 위한 수요조사 과정에서 유관기관으로부터 제안되었으며, 재평가 대상 선정 절차를 거쳐 재평가 대상기술로 선정되었다. 평가목적 본 평가의 목적은 ‘증상 및 행동 평가척도(우울척도)-신경증우울평가’의 현황과 사용 가능한 도구를 파악하여 임상 현장의 활용에 도움이 될 수 있는 관련 정보를 제공하기 위함이다. 평가방법 ‘증상 및 행동 평가척도(우울척도)-신경증우울평가’의 임상 현황과 사용 가능한 도구를 파악하기 위해 문헌검토를 수행하였다. 이를 위해 ‘증상 및 행동 평가척도(기타)-한국판성격평가척도(KPAI), 이화방어기제검사’ 평가와 함께 공동 소위원회를 구성하였으며, 정신건강의학과 3인(의원급 포함), 임상심리전문가 2인, 근거기반의학 1인의 전문가 6인으로 구성하였다. 평가방법은 평가목적을 고려하여 “증상 및 행동 평가척도(기타)-한국판성격평가척도(KPAI), 이화방어기제검사, 증상 및 행동 평가척도(우울척도)-신경증우울평가’ 공동 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. ‘증상 및 행동 평가척도(우울척도)-신경증우울평가’의 임상에서의 사용 현황을 파악하기 위해 관련 학회(대한신경정신의학회, 대한정신건강의학과의사회, 한국임상심리학회)에 해당 기술의 사용 여부 및 현황, 대체기술, 임상적 필요성의 의견을 수렴하였다. 또한 평가도구별 세부정보 확인을 위해 Ovid MEDLINE, APA PsycInfo, PubMed, RISS 등 전자데이터베이스와 구글, 네이버 등 검색엔진을 활용하여 관련 문헌을 검색하였으며, ‘depression’, ‘depression inventory’, ‘신경증우울평가’, ‘FY713’ 등의 주요어를 사용하였다. 수집된 문헌은 질적 검토(Qualitative review)를 통해 정리하였다. 평가결과 ‘증상 및 행동 평가척도(우울척도)-신경증우울평가(FY713)’의 임상적 사용 현황과 활용 가능한 평가도구를 파악하기 위하여 관련 학회 의견수렴과 문헌검토를 수행하였다. 학회 의견수렴을 통해 해당 기술의 임상적 활용 현황과 적용 범위를 확인하였고, 문헌검토를 통해 임상 상황에서 활용 가능한 평가도구의 예시와 각 도구의 주요 특성을 정리하였다. 관련 학회 의견을 종합한 결과, ‘신경증우울평가(FY713)’는 현행 급여로 고시된 우울척도만으로 평가가 어려운 우울장애 의심 환자를 대상으로 보다 심층적인 증상 평가를 위해 보완적으로 활용되는 것으로 파악되었다. 또한 해당 행위에 사용하는 평가척도는 특정화되어 있지 않으며, 급여항목에 포함되지 않은 다양한 우울 평가척도가 임상적 필요에 따라 선택적으로 사용되고 있는 것으로 확인되었다. 본 평가는 해당 기술에 적용되는 평가척도를 특정화하기 위한 것이 아니라, 임상 상황에 따라 활용 가능한 평가척도의 예시와 기본 정보를 제시하는 데 목적이 있다. 이에 따라 활용 가능한 평가도구의 예시로 HDSQ, MADRS, RDS, ATQ-N/P, MHS:D, NDS를 확인하여 관련 정보를 정리하였다. 총 6개의 평가도구가 확인되었으며, 이 중 4개는 해외에서, 2개는 국내에서 개발된 도구였다. 해외에서 개발된 도구는 모두 국내에서 신뢰도와 타당도가 검증된 것으로 확인되었고, 전반적으로 국내 타당화 연구 결과는 각 도구의 신뢰도와 타당도를 지지하였다. 국내에서 개발된 도구 또한 신뢰도와 타당도가 전반적으로 높은 수준으로 보고되었다. 결론 및 권고결정 의료기술재평가 소위원회에서는 ‘증상 및 행동 평가척도(우울척도)-신경증우울평가’에 대해 현재 평가결과에 근거하여 다음과 같이 제언하였다. ‘증상 및 행동 평가척도(우울척도)–신경증우울평가’는 현행 급여로 고시된 우울척도만으로는 평가가 어려운 우울(의심)장애 환자를 대상으로, 보다 심층적인 증상 평가를 위해 보완적으로 활용되는 도구로 판단하였다. 다만, 임상적 활용 시에는 신뢰도와 타당도가 검증되었거나 학술적 근거가 확보된 평가도구에 한정하여 사용하는 것이 바람직하다고 보았다. 활용 가능한 평가도구를 특정하여 제한하기는 어려우나, 소위원회 논의를 통해 HDSQ, MADRS, BSS, ATQ-N/P, MHS:D, NDS를 예시로 제시하였으며, 실제 임상에서는 각 평가척도의 특성과 환자 상태를 종합적으로 고려하여 임상전문가의 판단과 경험에 기반한 적절한 도구 선택이 필요하다는 의견이었다. 한편, 현재 확인된 타당화 연구는 표본의 대표성 및 문화적 차이 측면에서 일부 한계가 있어, 한국인을 대상으로 한 평가도구의 개발과 기존 한국어판 도구의 표준화 및 임상적 유용성 검증을 위한 추가 연구가 필요하다고 판단하였다. 아울러, 행위명 및 정의의 개편 필요성도 제기되었다. 행위명에 포함된 ‘신경증우울(Neurotic Depression)’은 현행 진단체계에서 사용되지 않는 용어로, 실제 임상 적용 범위를 반영하여 용어의 수정·개편이 필요하다고 보았다. 또한, 고시 항목에서 정의 및 적응증이 ‘우울척도(FY711) 행위설명 참고’로만 제시되어 있어 적용 기준이 불명확하므로, ‘신경증우울평가(FY713)’의 정의, 적용 대상 및 범위를 명확히 기술하여 임상적 활용과 행정적 해석의 일관성을 확보할 필요가 있다고 제언하였다. 2025년 제4차 재평가전문위원회*(2025.12.8.)는 ‘증상 및 행동 평가척도(우울척도)-신경증우울평가’에 대해 다음과 같이 심의하였다. 재평가전문위원회는 소위원회의 검토 결과에 동의하였으며, 추가로 해당 검사들이 제도적 관리 범위 밖에서 임의로 활용되는 일이 없도록 향후 제도적 보완이 필요하다는 의견을 제시하였다.
Background The "Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment" is a test used to evaluate depressive symptoms in patients with (suspected) depressive disorder, and is characterized by the absence of a specifically designated rating scale. According to the procedure description in the National Health Insurance Non-Covered Services Information Portal, this test is defined as a self-report or examiner-administered assessment designed to identify depressive symptoms in patients, measuring the pattern and severity of depression for use in diagnosis, differential diagnosis, and monitoring of treatment progress. This technology was proposed by a relevant organization during the demand survey process for the selection of health technology reassessment subjects, and was subsequently selected as a reassessment subject through the established selection procedures. Objectives The aims of this assessment were to examine the current status of the "Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment" and identify available assessment tools, with the purpose of providing relevant information that can support its use in clinical practice. Method A literature review was conducted to examine the current clinical status of the "Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment" and to identify available assessment tools. For this purpose, a joint subcommittee was formed together with the assessment of the "Symptom and Behavioral Rating Scale (Other) — Korean Personality Assessment Inventory (KPAI) and Ewha Defense Mechanism Test." The subcommittee consisted of six experts: three specialists in psychiatry (including a clinic-level practitioner), two licensed clinical psychologists, and one specialist in evidence-based medicine. All evaluation methods were discussed and finalized by the "Joint Subcommittee for Assessment of Symptom and Behavioral Rating Scale (Other) — KPAI, Ewha Defense Mechanism Test, and Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment" (hereinafter referred to as the "subcommittee"), in consideration of the purpose of the assessment. To examine the current clinical use of the "Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment," opinions were solicited from relevant academic societies (the Korean Neuropsychiatric Association, the Korean Academy of Anxiety and Mood Disorders, and the Korean Clinical Psychology Association) regarding the status and extent of use of this technology, available alternative tools, and clinical necessity. In addition, electronic databases including Ovid MEDLINE, APA PsycInfo, PubMed, and RISS, as well as search engines such as Google and Naver, were used to search for relevant literature to identify detailed information on each assessment tool. Key search terms included 'depression,' 'depression inventory,' '신경증우울평가,' and 'FY713.' The collected literature was organized through a qualitative review. Results A survey of relevant academic societies and a literature review were conducted to examine the current clinical use of the "Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment (FY713)" and to identify available assessment tools. The survey of academic societies confirmed the current clinical utilization and scope of application of this technology, and the literature review identified examples of assessment tools available for use in clinical settings along with their key characteristics. Based on the consolidated opinions from relevant academic societies, it was found that the "Neurotic Depression Assessment (FY713)" is used as a supplementary tool to enable more in-depth symptom evaluation in patients with (suspected) depressive disorder who cannot be adequately assessed using only the depression scales currently listed under insurance coverage. It was also confirmed that no specific rating scale is designated for use with this procedure, and that various depression rating scales not included in the covered items are selectively used according to clinical need. The purpose of this assessment was not to designate or restrict the rating scales applicable to this technology, but rather to present examples of rating scales available for use in various clinical situations along with their basic information. Accordingly, HDSQ, MADRS, RDS, ATQ-N/P, MHS:D, and NDS were identified as examples of available assessment tools, and relevant information was compiled. A total of six assessment tools were identified: four developed internationally and two developed domestically. All internationally developed tools were confirmed to have established reliability and validity in Korean populations, and the Korean validation studies broadly supported the reliability and validity of each tool. The domestically developed tools were also reported to demonstrate generally high levels of reliability and validity. Conclusions and Recommendations Based on the current assessment results, the Health Technology Reassessment Subcommittee provided the following recommendations regarding the "Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment." The "Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment" was determined to be a supplementary tool intended to enable more in-depth symptom evaluation in patients with (suspected) depressive disorder who cannot be adequately assessed using only the depression scales currently listed under insurance coverage. However, it was considered desirable that clinical use of this technology be limited to assessment tools for which reliability and validity have been established or for which adequate academic evidence exists. While it was not feasible to designate or restrict the available assessment tools to specific instruments, the subcommittee identified HDSQ, MADRS, BSS, ATQ-N/P, MHS:D, and NDS as examples through its deliberations. It was noted that in actual clinical practice, appropriate tool selection should be based on the clinical professional's judgment and experience, with comprehensive consideration of the characteristics of each rating scale and the patient's condition. Furthermore, the currently identified validation studies have certain limitations in terms of sample representativeness and cultural differences, and it was therefore determined that additional research is needed for the development of assessment tools specifically for the Korean population, as well as for the standardization of existing Korean-language tools and verification of their clinical utility. It was also noted that revisions to the procedure name and definition are necessary. The term "Neurotic Depression," included in the current procedure name, is no longer used in contemporary diagnostic classification systems, and it was recommended that the terminology be revised to reflect the actual scope of clinical application. Additionally, since the definition and indications listed in the official notice refer only to "see procedure description for Depression Scale (FY711)," leaving the application criteria unclear, it was recommended that the definition, target population, and scope of application of the "Neurotic Depression Assessment (FY713)" be explicitly described to ensure consistency in clinical use and administrative interpretation. The 4th Health Technology Reassessment Committee of 2025* (December 8, 2025) deliberated on the "Symptom and Behavioral Rating Scale (Depression Scale) — Neurotic Depression Assessment" and reached the following conclusion: The committee agreed with the findings of the subcommittee and additionally noted that institutional measures should be taken in the future to prevent these assessments from being used arbitrarily outside the scope of regulatory oversight. *Following the amendment to the "Rules on New Health Technology Assessment" (Ministry of Health and Welfare Ordinance No. 1098, partially amended, effective September 7, 2025), the Health Technology Reassessment Committee was newly organized and has been in operation since September 2025.
