초록
평가배경 정신분석적 정신치료(Psychoanalytic Psychotherapy)는 정상-신경증 그룹을 위한 것으로 장애까지는 아니지만 일차적으로 성격적 어려움을 가진 사람들이 자신의 삶의 목표를 달성하기 위한 성격변화, 잠재력을 극대화하는 기능개선, 그리고 기본적으로 만족스러운 삶을 사는 데 방해받는 만성적인 문제들을 극복하여 삶의 질을 향상시키기 위한 기술이다. 동 기술은 2018년도 정신치료 건강보험 수가 개편을 통해 비급여로 등재된 기술이다. 해당 기술은 의료기술 재평가 대상 선정을 위한 수요조사 과정에서 유관기관으로부터 제안되었으며, 재평가 대상 선정 절차를 거쳐 재평가 대상기술로 선정되었다. 2025년 제5차 의료기술재평가위원회(2025.5.16.)에서 신속검토 대상으로 선정되었으며, 소위원회 논의를 거쳐 관련 문헌을 검토하고 임상 활용에 필요한 정보를 제공하고자 재평가를 수행하였다. 평가목적 본 평가는 정신분석적 정신치료와 관련된 문헌을 검토하고, 그 결과를 바탕으로 전문가 논의를 거쳐 임상 현장에서의 활용에 참고가 될 수 있는 정보를 제공하고자 수행하였다. 평가방법 본 평가는 정신분석적 정신치료에 대하여 신속검토 방법으로 수행하였다. 이를 위하여 국내외 교과서, 임상진료지침, 선행 의료기술평가, 이차연구 및 관련 종설 등을 검색·검토하였으며, 검토 결과는 교과서, 가이드라인, 선행의료기술평가, 연구 현황 및 연구의 제한점을 중심으로 정리하였다. 또한 문헌 검토 결과에 대해서는 소위원회 전문가의 검토와 논의를 거쳐 임상적 활용에 참고할 수 있는 정보로 제시하였다. 평가결과 교과서 검토 결과, 최신정신의학 제7판(민성길, 김찬형, 2023)과 신경정신의학 제3판(대한신경정신의학회, 2019)에서 정신분석적 정신치료는 정신사회적 치료 또는 정신역동적 정신치료의 한 유형으로 제시되고 있음을 확인하였다. 가이드라인(American Psychiatric Association, European Psychiatric Association, Royal Australian and New Zealand College of Psychiatrists) 및 선행 의료기술평가(NICE) 검토 결과, 정신분석적 또는 정신역동적 정신치료는 우울장애, 공황장애, 사회불안장애, 범불안장애, 양극성장애, 강박장애 등을 포함한 다양한 정신질환에서 환자의 특성, 기존 치료의 적합성 및 반응 여부를 고려하여 선택적으로 활용할 수 있는 치료 옵션으로 제시되고 있었다. 이차연구 검토 결과, 연구대상은 양극성장애, 일반 정신질환, 우울장애, 복합정신장애, 자살·자해, 성적 학대 경험군 등으로 다양하였으며, 중재의 정의와 범위 또한 연구마다 다양하였다. 전반적으로는 정신분석적 또는 정신역동적 치료 전반을 포괄하고 있었고, 일부 연구에서는 치료 빈도와 치료 세팅(couch 사용 여부, 의자에 마주 앉는 방식 등)을 기준으로 정신분석과 정신분석적 정신치료를 구분하여 제시하였다. 관련 종설에서는 정신분석적 정신치료 연구의 제한점도 함께 제시하고 있었다. 장기 치료의 특성상 무작위배정비교연구 설계에 현실적인 제약이 크고, 치료방법의 표준화가 어려우며, 결과 중심의 연구만으로는 자기이해, 성숙, 관계 변화 등 치료의 내면적·과정적 변화를 충분히 반영하거나 정밀하게 측정하기 어렵다는 점이 지적되었다. 또한 일부에서는 측정 과정 자체가 치료적 과정을 방해할 가능성도 제시하였다. 본 평가의 최종 검토 문헌은 총 31편(교과서 2편, 가이드라인 11편, 선행 의료기술평가 5편, 이차 연구 10편, 기타 문헌 3편)이었다. 결론 의료기술재평가 소위원회에서는 현재 평가결과에 근거하여 다음과 같은 의견을 제시하였다. 정신분석적 정신치료는 백 년 이상의 역사를 가진 정신의학의 근간이 되는 전통적인 치료법으로, 수년간 전문 훈련을 받은 전문의에 의해 시행되는 기술이다. 동 기술은 성격적 어려움이 있는 환자(ICD-11 personality difficulty)가 적용 대상이며, 다양한 정신질환에서 선택적 치료법으로 권고되고 있다. 치료방법과 기간은 환자의 특성과 치료자의 접근에 따라 수개월에서 수년까지 다양하다. 동 기술은 정량적인 비교를 통한 평가보다는 치료의 고유한 과정적 특성을 고려하여 신중히 평가할 필요가 있다는 의견이었다. 2026년 제1차 재평가전문위원회*(2026.1.9.)는 소위원회 검토 결과에 근거하여 다음과 같이 심의하였다. 정신분석적 정신치료는 백 년 이상의 역사를 가진 정신의학의 근간이 되는 전통적인 치료법으로, 전문 훈련을 받은 전문의에 의해 시행되는 기술이다. 동 기술은 성격적 어려움이 있는 환자(ICD-11 personality difficulty)가 적용 대상이며, 다양한 정신질환에서 선택적 치료법으로 권고되고 있다. 환자의 특성과 치료자의 접근에 따라 치료 방법과 기간은 수개월에서 수년까지 다양하다. 동 기술은 정량적 비교 중심의 접근보다는 치료가 진행되는 고유한 과정적 특성과 임상적 맥락을 충분히 고려하여 신중히 검토할 필요가 있다.
Background Psychoanalytic psychotherapy is intended for individuals within the normal-to-neurotic spectrum who do not necessarily meet the diagnostic criteria for a mental disorder but primarily experience personality-related difficulties. It aims to facilitate personality change that enables individuals to achieve their life goals, maximize their potential and improve functioning, overcome chronic problems that interfere with leading a satisfying life, and ultimately improve their quality of life. This intervention was listed as a non-reimbursed service following the 2018 revision of the National Health Insurance reimbursement system for psychotherapy in Korea. Psychoanalytic psychotherapy was proposed by a relevant professional organization during the demand survey conducted to identify candidate technologies for Health Technology Reassessment. Following the established selection process, it was selected for reassessment. At the 5th Health Technology Reassessment Committee meeting held on May 16, 2025, the intervention was designated for a Rapid Review. Following deliberation by the Health Technology Reassessment Subcommittee, the relevant literature was reviewed to provide information that may support its use in clinical practice. Objective This assessment aimed to review the available literature on psychoanalytic psychotherapy and, based on the findings and expert deliberation, provide information that may serve as a reference for its use in clinical practice. Methods This assessment was conducted using the Rapid Review methodology. Domestic and international textbooks, clinical practice guidelines, previous health technology assessments, secondary research, and relevant review articles were searched and reviewed. The findings were summarized with a focus on textbooks, clinical practice guidelines, previous health technology assessments, the current state of the evidence, and the limitations of the available research. The results of the literature review were subsequently reviewed and discussed by the Health Technology Reassessment Subcommittee and are presented as information to support clinical practice. Results The review of textbooks found that Latest Psychiatry, 7th edition (Min Seong-Gil & Kim Chan-Hyung, 2023), and Textbook of Neuropsychiatry, 3rd edition (Korean Neuropsychiatric Association, 2019), describe psychoanalytic psychotherapy as a form of psychosocial therapy or psychodynamic psychotherapy. The review of clinical practice guidelines published by the American Psychiatric Association, the European Psychiatric Association, and the Royal Australian and New Zealand College of Psychiatrists, as well as previous health technology assessments conducted by NICE, indicated that psychoanalytic or psychodynamic psychotherapy may be selectively considered as a treatment option for a variety of mental disorders, including depressive disorders, panic disorder, social anxiety disorder, generalized anxiety disorder, bipolar disorder, and obsessive-compulsive disorder, depending on patient characteristics, the appropriateness of existing treatments, and treatment response. The review of secondary research demonstrated considerable heterogeneity in both study populations and intervention definitions. Study populations included patients with bipolar disorder, general psychiatric disorders, depressive disorders, complex mental disorders, suicidal or self-harming behaviors, and individuals with a history of sexual abuse. Overall, the studies broadly encompassed psychoanalytic or psychodynamic therapies. Some studies distinguished psychoanalysis from psychoanalytic psychotherapy according to treatment frequency and therapeutic setting, such as the use of a couch or face-to-face sessions conducted while seated in chairs. Relevant review articles also identified several limitations in the evidence on psychoanalytic psychotherapy. Because treatment is typically long-term, randomized controlled trials are subject to considerable practical constraints, and standardization of the intervention is difficult. Furthermore, outcome-focused research alone may not adequately capture or precisely measure internal and process-related therapeutic changes, such as increased self-understanding, psychological maturation, and changes in interpersonal relationships. Some authors have also suggested that the assessment process itself may interfere with the therapeutic process. A total of 31 publications were included in the final review, comprising 2 textbooks, 11 clinical practice guidelines, 5 previous health technology assessments, 10 secondary research publications, and 3 additional references. Conclusion Based on the assessment findings, the Health Technology Reassessment Subcommittee provided the following opinion. Psychoanalytic psychotherapy is a traditional treatment modality with a history of more than 100 years and has served as one of the foundations of psychiatry. It is delivered by psychiatrists who have completed several years of specialized training. The intervention is intended for patients with ICD-11 personality difficulty and is recommended as a selective treatment option for a variety of mental disorders. Treatment methods and duration vary from several months to several years depending on patient characteristics and the therapist's therapeutic approach. The Subcommittee considered that psychoanalytic psychotherapy should be assessed with careful consideration of its distinctive therapeutic process, rather than primarily through quantitative comparative evaluation. Based on the findings of the Health Technology Reassessment Subcommittee, the 1st Health Technology Reassessment Expert Committee (January 9, 2026) concluded as follows. Psychoanalytic psychotherapy is a traditional treatment modality with a history of more than 100 years and has served as one of the foundations of psychiatry. It is delivered by psychiatrists who have received specialized training. The intervention is intended for patients with ICD-11 personality difficulty and is recommended as a selective treatment option for a variety of mental disorders. Treatment methods and duration vary from several months to several years depending on patient characteristics and the therapist's therapeutic approach. Therefore, psychoanalytic psychotherapy should be reviewed with careful consideration of its distinctive therapeutic process and clinical context, rather than through an approach focused primarily on quantitative comparis
