초록
평가 배경 안드로스테네디온(androstenedione)은 혈중의 안드로스테네디온 농도를 측정하여 부신피질과 관련된 질환을 감별·진단하는 검사이다. 안드로스테네디온은 부신피질자극호르몬에 반응하여 부신피질에서 생성 및 분비되는 성 호르몬으로서, 생체 내에서 테스토스테론과 에스트론의 전구물질 역할을 한다. 안드로스테네디온은 과잉 또는 결핍 시 쿠싱증후군, 선천성 부신과형성증, 부신기능부전 등 다양한 부신피질 관련 질환의 발생에 관여한다. 안드로스테네디온 검사는 신의료기술평가제도 시행 이전에 비급여로 등재되어 현재까지 사용하고 있다. 동 기술은 내부 모니터링을 통해 재평가 안건으로 발굴되었다. 다양한 대체 의료기술이 존재하는 현 시점에서 비교기술 대비 동 기술의 안전성과 효과성에 대한 근거를 확인하고자 2024년 제6차 의료기술재평가위원회(2024.6.14.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 선천성 부신과형성증, 다낭성 난소증후군 (의심) 환자에서 안드로스테네디온 검사가 질환의 진단 보조 및 모니터링 목적으로 사용 시 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 안드로스테네디온에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “안드로스테네디온 및 DHEA 통합 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 내분비대사내과 2인, 산부인과(생식내분비학) 2인, 진단검사의학과 2인, 근거기반의학과 1인, 총 7인으로 구성하였다. 본 평가의 대상 환자는 안드로스테네디온 검사가 임상에서 주로 사용되는 주요 질환인 선천성 부신과형성증(Congenital Adrenal Hyperplasia, CAH) 및 다낭성 난소증후군(Polycystic Ovary Syndrome, PCOS) (의심) 환자로 하였으며, 참고표준검사는 최종임상진단, 비교검사는 대상질환에 따라 CAH는 17-하이드록시프로게스테론(17 hydroxyprogesterone, 17-OHP), 코티솔, 부신피질자극호르몬(adrenocorticotropic hormone, ACTH) 검사 등으로 설정하였고, PCOS는 생화학적 호르몬 안드로겐 검사(총 테스토스테론 등)로 정의하였다. 결과변수로는 안전성과 효과성을 확인하였으며, 안전성은 검사와 관련된 이상반응 및 부작용을 지표로, 효과성은 진단정확성, 비교검사와의 상관성, 질환(임상)과의 관련성, 의료결과에의 영향(치료모니터링) 지표로 평가하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews-Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2024. 7. 25.). 최종 선택된 연구의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를 사용하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여, 진단정확도를 2 x 2표로 제시하고, 양적 분석이 가능한 경우 메타분석을 수행하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과 최종 선택된 연구는 총 19편으로 연구유형은 진단법 평가연구 18편, 코호트 연구 1편이었다. 연구대상자에 따라 선천성 부신과형성증 관련 연구 2편(대상자 수 131명, 중재군 70명, 대조군 61명), 다낭성 난소증후군 관련 연구 17편(대상자 수 3,683명, 중재군 2,536명, 대조군 1,147명)이었다. 진단법 평가연구에 대한 비뚤림위험 평가결과, 모든 연구가 환자-대조군 설계의 진단법 평가연구로 환자선택 영역에서 비뚤림위험 ‘높음’이 100%였고, 중재검사 영역에서는 ‘높음’이 36.9%, ‘불확실’이 52.6%이었다. 안전성 안전성은 검사와 관련된 이상반응 및 부작용을 통해 평가하고자 하였으나, 선택된 연구에서는 관련 결과를 보고한 연구는 확인되지 않았다. 해당 검사는 체외에서 수행되는 검사로, 검사 수행으로 인한 안전성에는 문제가 없을 것으로 판단하였다. 또한, 검사 결과의 위음성 또는 위양성으로 인한 위험성에 대해서는 해당 검사가 다른 검사와 상호보완적으로 사용된다는 점을 고려할 때 검사 결과에 따른 위해는 크지 않을 것으로 보았다. 효과성 효과성은 연구대상자(선천성 부신과형성증, 다낭성 난소증후군) 별로 구분하여 제시하였다. 선천성 부신과형성증(CAH) 관련 연구는 2편에서 보고하였다. 진단정확성은 1편에서 제시하였고, 염분소실형 CAH와 건강대조군 감별에 대한 진단정확성 결과는 안드로스테네디온은 민감도 100%, 특이도 97%, 비교검사인 17-OHP는 민감도 100%, 특이도 98%로 보고되었다. 단순 남성화형 및 비고전형 CAH와 건강대조군 감별에 대한 진단정확성 결과는 안드로스테네디온은 민감도 72%, 특이도 97%, 비교검사인 17-OHP는 민감도 12%, 특이도 98%로, 안드로스테네디온이 17-OHP보다 민감도가 높았다. 치료반응 모니터링 관련 결과는 1편에서 보고하였다. 21-수산화효소결핍증(21-hydroxylase deficiency, 21-OHD) 성인 환자에서 경구 글루코코르티코이드(oral glucocorticoid, OG)와 지속적 피하 하이드로코티손 주입(continuous subcutaneous hydrocortisone infusion) 치료 시, 안드로스테네디온은 다른 검사와 유사하게 전반적으로 OG 치료 동안에 농도가 더 높았으며, 부신피질자극호르몬(ACTH)과의 상관성은 0.68로 보고하였다. 다낭성 난소증후군(PCOS) 연구는 17편에서 보고하였으며, 진단정확성은 단독 검사와 병용 검사로 나누어 검토하였다. 단독 검사에서 생화학적 고안드로겐혈증 관련 PCOS 환자를 건강대조군과 감별 진단하는데 진단정확성은 15편의 연구에서 보고하였고, 안드로스테네디온의 진단정확성은 민감도 39.8-91%, 특이도 44.6.-90.7%, 곡선하면적(Area under the curve, AUC) 값 0.63-0.957으로 보고하였다. 다른 생화학적 안드로겐 비교검사의 진단정확성은 총 테스토스테론(Total Testosterone, TT)은 민감도 70-92.9%, 특이도 59-82.9%, AUC값 0.684-0.939, 유리 테스토스테론은 민감도 62-91%, 특이도 74.1-89%, AUC값 0.837-0.954, DHEA-S (Dehydroepiandrosterone-sulfate)는 민감도 50-91%, 특이도 22.2-79%, AUC값 0.502-0.775, 유리 안드로겐 지표(Free Androgen Index, FAI)는 민감도 70-90%, 특이도 70.5-95.7%, AUC값 0.79-0.951, 황체형성호르몬(Luteinizing Hormone, LH)은 민감도 35%, 77.3%, 특이도 61.1%, AUC값 0.62-0.897, 난포자극호르몬(Follicle Stimulating Hormone, FSH)은 민감도 65.9%, 특이도 66.3%, AUC값 0.699, LH/FSH는 민감도 41-67.4%, 특이도 67.4, 68.5%, AUC값 0.638-0.980, 성호르몬결합글로불린(Sex Hormone Binding Globulin, SHBG)은 민감도 27.3-67.7%, 특이도 61-80%, AUC값 0.29-0.765로 보고하였다. 병용검사로서 진단정확성은 6편에서 보고하였다. 1편을 제외한 대부분의 연구에서 PCOS를 진단하는데 있어 각 지표를 개별적으로 사용하는 것보다 안드로스테네디온을 포함하여 병용검사 시에 진단정확도를 향상시키는 것으로 보고하였다. 질병과의 연관성은 1편에서 보고하였으며, 연령, 체질량지수(Body mass index, BMI)를 보정하였을 때 안드로스테네디온 수치가 높은 경우에 다낭성 난소증후군 질환 발생과의 연관성이 있었다(Odds ratio (OR)=5.75, 95% 신뢰구간(Confidence interval, CI) 3.74-8.84, p<0.05). 안드로스테네디온과 호르몬 비교검사와의 상관성은 3편에서 보고하였다. 2편은 TT, FAI, DHEA-S, LH와 유의한 상관성을 보고였으며, 그 외 1편에서는 안드로스테네디온 수치는 임상적 또는 대사적 매개변수와 상관관계를 보이지 않는다고 보고하였다. 결론 및 제언 의료기술재평가 소위원회에서는 선천성 부신과형성증 및 다낭성 난소증후군 (의심)환자에서 안드로스테네디온 검사의 안전성과 효과성 결과를 현재 확인된 근거를 바탕으로, 적응질환별로 구분하여 다음과 같이 제시하였다. 선천성 부신과형성증 질환에 대한 체계적 문헌고찰 결과, 소위원회에서는 평가에 포함된 문헌적 근거가 제한적이지만, 현재 선천성 부신과형성증을 진단하는 데 있어 17-OHP, 코티솔, ACTH 등의 기존 검사를 대체하는 검사가 아니라 상호보완적으로 활용되며, 질환의 세부 유형(subtype)을 감별하는데 유용하다는 의견이었다. 또한, 임상에서 17-OHP 검사 결과 양성으로 21-수산화효소 결핍성 선천성 부신과형성이 의심되는 신생아의 진단에 스테로이드 화합물 검사(정밀분광-질량분석)가 사용되고 있으며, 이 검사에는 안드로스테네디온이 포함된 점을 고려할 때, 안드로스테네디온 검사는 선천성 부신과형성증 진단을 보조하는데 임상적으로 의미가 있다고 평가하였다. 다낭성 난소증후군 질환에 대한 체계적 문헌고찰 결과, 소위원회에서는 현재 다낭성 난소증후군의 표준 진단 기준에 생화학적 고안드로겐혈증(hyperandrogenism) 확인이 포함되어 있으며, 안드로스테네디온이 생화학적 고안드로겐혈증을 확인하는 지표 중 하나로서 진단 정확성은 신뢰할 수 있는 수준이므로 다낭성 난소증후군의 고안드로겐혈증 진단을 보조하는데 효과적인 검사로 평가하였다. 다만, 최근 국제 임상진료지침에서는 다낭성 난소증후군 진단 시 생화학적 고안드로겐혈증 평가를 위해 총 테스토스테론과 유리 테스토스테론(유리 안드로겐 지수 포함)을 1차적으로 사용하는 것을 권고하며, 이들 수치가 상승하지 않을 경우 안드로스테네디온 검사를 고려하도록 하고 있고, 검사 정확도를 높이기 위해 질량분석법을 사용하는 것을 제안하고 있는 점을 반영할 필요가 있다고 제언하였다. 2025년 제2차 의료기술재평가위원회(2025. 2. 14.)는 ‘안드로스테네디온 검사’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 검토한 문헌들의 근거 수준이 높지 않은 점과 해당검사가 기존검사의 대체검사가 아니라는 점을 고려하였다. 이에, 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 선천성 부신과형성증, 다낭성 난소증후군 (의심)환자의 진단 보조 및 모니터링 목적으로 안드로스테네디온 검사의 사용을 ‘약하게 권고함’으로 심의하였다.
Background Androstenedione testing is a diagnostic test used to measure serum androstenedione levels to aid in the differential diagnosis of adrenal cortex-related disorders. Androstenedione is a sex hormone produced and secreted by the adrenal cortex in response to adrenocorticotropic hormone (ACTH), and it serves as a precursor of testosterone and estrone. Androstenedione excess or deficiency is associated with various adrenal disorders, including Cushing syndrome, congenital adrenal hyperplasia (CAH), and adrenal insufficiency. This test has been used as a non-reimbursable procedure since before the implementation of the New Health Technology Assessment system. Internal monitoring identified this technology for reassessment, which was conducted to evaluate the evidence on the clinical safety and effectiveness of androstenedione testing. Reassessment was conducted after the reassessment plan was reviewed and approved and the subcommittee was appointed at the 6th 2024 Health Technology Reassessment Committee meeting (June 14, 2024). Objective The objective of this reassessment was to provide scientific evidence on the clinical safety and effectiveness of androstenedione testing when used as an adjunctive diagnostic test and for monitoring patients with (suspected) CAH or polycystic ovary syndrome (PCOS), with the aim of determining the strength of recommendation for continued use. Methods A systematic literature review was conducted to assess the safety and effectiveness of androstenedione testing. The “Androstenedione and DHEA Joint Subcommittee” (hereinafter referred to as the “Subcommittee”) held discussions to finalize all assessment methods based on the objectives of the reassessment. The Subcommittee was appointed by the Health Technology Reassessment Committee and comprised seven members, including two endocrinologists, two obstetricians and gynecologists (reproductive endocrinology), two laboratory medicine specialists, and one evidence-based medicine expert. The target population included patients in whom androstenedione testing was primarily used in clinical practice, specifically those with (suspected) CAH or PCOS. The reference standard was the final clinical diagnosis. For CAH, the comparators included 17-hydroxyprogesterone (17-OHP), cortisol, and ACTH testing. For PCOS, the comparators were biochemical androgen tests (e.g., total testosterone [TT]). Outcome measures included safety and effectiveness. Safety was assessed based on adverse events and complications associated with testing. Effectiveness was assessed based on diagnostic accuracy, correlation with comparator tests, association with disease (clinical), and impact on medical outcomes (treatment monitoring). The literature search was conducted using three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews - Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, KMbase, and RISS). The final search date was July 25, 2024. The risk of bias in selected studies was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool. Data were extracted using a predefined form, and 2×2 tables were constructed to assess diagnostic accuracy. Meta-analysis was conducted when quantitative synthesis was feasible. All processes were independently conducted by two reviewers, and discrepancies were resolved by reaching a consensus. Final recommendation was made by the Health Technology Reassessment Committee based on the findings of the Subcommittee. Results The systematic review identified 19 studies, including 18 diagnostic accuracy studies and one cohort study. Two studies included patients with CAH (131 patients: 70 in the intervention group and 61 in the control group), and 17 included those with PCOS (3,683 patients: 2,536 in the intervention group and 1,147 in the control group). The risk of bias was high in all diagnostic evaluation studies due to their case-control design. Specifically, 100% of studies had a high risk of bias in the patient selection domain, whereas 36.9% and 52.6% had a high or unclear risk in the index test domain, respectively. Safety Safety was assessed based on test-related adverse events and complications. However, none of the included studies reported relevant data. As this is an in vitro diagnostic test, procedural safety concerns are minimal. Additionally, given that the test is typically used in conjunction with other diagnostic tests, the potential clinical risk of false positives or negatives was considered low. Effectiveness Effectiveness was analyzed separately for CAH and PCOS. Two studies reported data on CAH. One study reported diagnostic accuracy in differentiating patients with salt-wasting CAH from healthy controls. Androstenedione testing had a sensitivity of 100% and specificity of 97%, whereas 17-OHP had a sensitivity of 100% and specificity of 98%. When distinguishing patients with simple virilizing or non-classic CAH from healthy controls, androstenedione testing had a higher sensitivity (72%) than 17-OHP (12%), with comparable specificities (97% vs. 98%). One study reported treatment monitoring outcomes. In adults with 21-hydroxylase deficiency treated with either oral glucocorticoids (OG) or continuous subcutaneous hydrocortisone infusion, androstenedione levels were generally higher during OG treatment, which was similar to other hormones; the correlation with ACTH testing was 0.68. For PCOS, 17 studies reported diagnostic accuracy, which was examined based on standalone or combination tests. For standalone tests, 15 studies reported the diagnostic accuracy of differentiating individuals with PCOS with biochemical hyperandrogenism from healthy controls. Androstenedione testing had a sensitivity of 39.8%–91%, specificity of 44.6%–90.7%, and area under the curve (AUC) of 0.63–0.957. For other biochemical androgen markers, TT demonstrated a sensitivity of 70–92.9%, specificity of 59–82.9%, and AUC of 0.684–0.939; free testosterone showed a sensitivity of 62–91%, specificity of 74.1–89%, and AUC of 0.837–0.954; dehydroepiandrosterone sulfate (DHEA-S) had a sensitivity of 50–91%, specificity of 22.2–79%, and AUC of 0.502–0.775; free androgen index (FAI) showed a sensitivity of 70–90%, specificity of 70.5–95.7%, and AUC of 0.79–0.951; luteinizing hormone (LH) demonstrated a sensitivity of 35% and 77.3%, specificity of 61.1%, and AUC of 0.62–0.897; follicle-stimulating hormone (FSH) had a sensitivity of 65.9%, specificity of 66.3%, and AUC of 0.699; LH/FSH ratio showed a sensitivity of 41–67.4%, specificity of 67.4% and 68.5%, and AUC of 0.638–0.980; and sex hormone-binding globulin (SHBG) had a sensitivity of 27.3–67.7%, specificity of 61–80%, and AUC of 0.29–0.765. The diagnostic accuracy of androstenedione testing as an add-on test was reported in six studies. In all but one of these studies, adding androstenedione to other markers improved diagnostic accuracy for PCOS compared with using each marker individually. The association between androstenedione and disease occurrence was reported in one study, which found that higher androstenedione levels were associated with the onset of PCOS after adjusting for age and body mass index (odds ratio = 5.75, 95% confidence interval: 3.74–8.84, p < 0.05). Correlations between androstenedione and other hormonal markers were reported in three studies. Two studies showed significant correlations with TT, FAI, DHEA-S, and LH, whereas the other reported no significant correlation between androstenedione and clinical or metabolic parameters. Conclusions and Recommendations Based on the currently available evidence, the Subcommittee reviewed the clinical safety and effectiveness of androstenedione testing in patients with (suspected) CAH and PCOS and provided the following assessment based on the target condition. For CAH, androstenedione testing was not used as a replacement for existing tests such as 17-OHP, cortisol, or ACTH, but rather served as an adjunctive test used to differentiate between subtypes of the disease; however, the Subcommittee noted that the available evidence was limited. In current clinical practice, steroid compound testing (tandem mass spectrometry), which includes androstenedione, is used to diagnose suspected 21-hydroxylase deficiency CAH in newborns who test positive on the 17-OHP test. Based on this evidence, the Subcommittee concluded that androstenedione is clinically meaningful as an adjunct in the diagnosis of CAH. For PCOS, the Subcommittee noted that current diagnostic criteria include biochemical hyperandrogenism and that androstenedione is one of the markers used to confirm hyperandrogenism. The Subcommittee determined that the diagnostic accuracy of androstenedione testing is sufficiently reliable and that the test is effective as an adjunct in diagnosing hyperandrogenism in patients with PCOS. Furthermore, the Subcommittee noted that current international clinical practice guidelines recommend TT and free testosterone (including FAI) as first-line markers for evaluating biochemical hyperandrogenism, with androstenedione as a second-line marker when these values are not elevated. The guidelines also recommend the use of mass spectrometry to improve analytical accuracy; therefore, the Subcommittee advised taking these recommendations into consideration. The Subcommittee’s findings were reviewed at the 2nd 2025 Health Technology Reassessment Committee meeting (February 14, 2025), and the Committee drew the following conclusions on androstenedione testing. The Committee noted that the quality of evidence in the included studies was not high and that this test was not used as a replacement for existing diagnostic tests. Considering the clinical safety, effectiveness, and other relevant assessment factors, the Health Technology Reassessment Committee determined that the use of the androstenedione test for diagnostic support and monitoring in patients with (suspected) CAH and PCOS is “weakly recommended” in the current Korean clinical setting.
