Pronunciation and Phonation Test – Articulation Disorder

초록

평가배경 ‘발음 및 발성검사’는 성대결절, 폴립, 성대마비, 후두악성종양 등 음성장애를 유발하는 모든 질환자를 대상으로 음성장애의 유무 및 정도를 평가하고, 수술 후 추적관찰 및 수술·음성치료 효과 분석을 목적으로 비급여로 등재된 기술이다. 그러나 동 검사는 실제 임상 현장에서 음성장애뿐 아니라 조음·발음장애 환자의 말 산출 및 음향학적 특성 평가에 활용되고 있어, 발음장애 환자에서의 임상적 활용과 유용성에 대한 근거 검토가 필요하다고 보았다. 이에 동 기술은 유관기관 수요조사를 통해 발굴되어 2025년 제4차 의료기술재평가위원회(2025.4.11.)에서 평가계획서 및 소위원회 구성안을 심의받은 후 재평가가 수행되었다. 평가목적 본 평가의 목적은 발음장애 환자에서 발음 및 발성검사의 임상적 유용성에 대한 근거를 검토하여 관련 정보를 제공하기 위함이다. 평가방법 본 평가는 발음 및 발성검사의 임상적 효과성을 확인하기 위해 “발음 및 발성검사 공동소위원회(이하 ‘소위원회’)”의 논의를 거쳐 평가방법을 확정하였다. 소위원회는 의료기술재평가위원회에서 심의한 평가계획에 따라 이비인후과 2인, 재활의학과 2인, 근거기반의학 1인을 포함한 총 5인으로 구성하였다. 발음 및 발성검사는 소리를 분석하는 기능 검사로 해부학적 병변 및 움직임 등을 확인하는 영상 검사를 비교검사로 설정하기는 어렵다고 판단하였다. 이에 본 평가에서는 체계적 문헌고찰을 통해 비교검사와의 효과성을 비교·분석하기보다는 의학교과서 및 국내외 임상진료지침 검토를 통해 임상적 유용성을 확인하고자 하였다. 또한, 검사항목이 다섯 가지(공기역학검사, 음성음향검사, 청각심리검사, 전기성문파검사, 비음측정검사)로 구성된 점을 고려하여 검사 사용 현황을 파악하기 위해 한 개의 데이터베이스를 토대로 간략 문헌검색을 수행하였다. 본 평가에서는 구음장애, 조음장애, 말소리장애 또는 발음장애가 혼용되어 사용되고 있으나, 검사명을 고려하여 ‘발음장애’로 제시하였다. 평가 핵심질문은 ‘발음 및 발성검사는 발음장애 환자를 진단 및 평가하기 위해 임상적으로 효과적인가?’로 설정하였다. 교과서는 이비인후과, 재활의학 분야를 검토하였고, 임상진료지침은 국내외 데이터베이스 검색 후 소위원회 논의를 통해 포함 여부를 확정하였다. 문헌고찰은 핵심질문을 토대로 Medline 데이터베이스를 이용하여 수행하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종 심의하였다. 평가결과 발음장애를 일으키는 모든 질환에서 발음 및 발성검사의 유용성을 확인하기 위하여 교과서, 임상진료지침 및 관련 일차 연구를 검토하였다. 교과서 검토 국내 재활의학 및 소아재활의학 교과서를 검토한 결과, 발음장애 평가를 위해 그림자음검사, 한국어발음검사, 최대발성시간 등이 주로 활용하고 있으며, 발음 및 발성검사에 대한 구체적인 기술은 확인하기 어려웠다. 다만 재활의학 및 소아재활의학 교과서에서 음성음향검사의 활용 가능성과 음성검사에 대한 이해의 필요성이 언급되어 있었다. 임상진료지침 검토 총 8편의 임상진료지침을 검토한 결과, 대부분 전반적인 언어 평가의 필요성을 제시하였고, 일부 지침에서 청지각적 평가 및 객관적인 음향학적 검사의 필요성을 제시하였다. 질환별로 살펴보면, 뇌졸중 환자를 대상으로 한 임상진료지침 4편 중 3편에서 말실행증과 조음장애 등의 말운동장애에 대한 전반적인 언어 평가의 필요성을 제시하였고, 나머지 1편은 구음장애가 있는 뇌졸중 환자의 개인 맞춤형 재활 중재에 대한 권고만 확인되었다. 두경부암 환자 관련 1편의 지침은 암 생존자의 전반적인 언어 평가의 필요성을 언급하였다. 외상성 뇌손상 지침은 성인 환자 대상인 경우, 구음장애 치료에 대한 언급만 확인되었고, 소아 환자 대상 지침에서는 구음장애 및 음성장애 평가를 위한 청지각적 평가 및 Visi-Pitch 등과 같은 객관적인 음향학적 평가가 필요하다는 전문가 합의문이 확인되었다. 뇌성마비 소아·청소년 환자를 대상으로 한 지침은 언어장애 향상을 위한 효과적인 중재가 필요하다고 제시하였으나, 치료 후 청지각 및 음성음향검사 등에 대한 개별 연구 사례만 제시하였고, 검사에 대한 별도의 권고사항은 없었다. 일차연구 검토 발음장애 대상으로 발음 및 발성검사의 사용현황을 파악하고자 문헌검색을 통해 총 20편의 연구를 선택하였다. 연구대상자의 분포는 구순구개열이 있는 아동 대상 연구가 6편, 말소리장애 아동 2편, 뇌성마비로 인한 조음장애 아동 2편으로 소아청소년을 대상으로 한 연구는 10편이었고, 성인에서는 파킨슨병(3편), 근위축성 측삭경화증(2편), 다발성경화증(2편), 중증근무력증(2편), 프리드라이히 운동실조증(1편)으로 인한 구음장애 환자를 대상으로 한 연구가 10편이었다. 발음 및 발성검사의 종류로는 음성음향검사가 18편(90.0%)으로 대부분을 차지하였다. 또한, 구순구개열 아동을 대상으로 비음측정검사를 활용한 연구가 3편이었다. 음성음향검사의 주요 장치는 음성분석 프로그램인 Praat을 이용한 연구가 11편으로 가장 많았으며, 음성음향검사와 함께 청지각적 조음 평가, 환자 자가보고 도구 등을 함께 사용하고 있었다. 발음 및 발성검사의 사용 목적은 조음 및 구음장애와의 연관성을 보고한 연구가 15편(75.0%), 치료 전·후 질병 경과를 평가하거나 효과를 비교하기 위한 경우는 5편(25.0%)으로 질병과의 연관성을 관찰한 연구가 더 많았다. 주요 검사 지표는 음성음향검사의 경우 기본 주파수(fundamental frequency, F0), 지터, 쉬머, 켑스트럼 최고치 돌출값(cepstral peak prominence, CPP) 등이 활용되었고, 비음측정검사에서는 비강음 점수 등의 객관적인 지표를 사용하였다. 결론 및 제언 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같이 결과 및 의견을 제시하였다. 소위원회는 교과서에서 발음장애 평가 시 조음검사와 함께 음성음향검사의 활용이 간략하게 언급되고 있음을 확인하였다. 총 8편의 임상진료지침 중 뇌졸중, 두경부암 환자를 대상 4편의 지침에서 전반적인 언어 평가의 필요성을 제시하고 있었으나, 구음장애에 대한 발음 및 발성검사의 권고는 확인되지 않았다. 소아 외상성 뇌손상 지침은 청지각적 평가도구와 함께 Visi-Pitch 등과 같은 음성음향검사로 발음장애를 평가하여 적절한 시기에 치료 개입이 이루어지도록 해야 한다는 전문가 합의문을 확인하였다. 또한, 관련 임상 연구를 검토한 결과, 동 검사는 파킨슨병으로 인한 구음장애, 구순구개열 아동, 뇌성마비 아동에서 치료 효과 및 질병 상태를 확인하기 위해 활용되고 있었다. 이에 소위원회는 발음 및 발성검사가 구개열 또는 구개순으로 인한 발음장애, 뇌졸중·뇌성마비·파킨슨병 등 뇌병변으로 인한 발음(구음)장애와 실어증, 소아 발달성 조음장애의 질병 상태를 평가하고 치료 경과를 관찰하는 데 유용하며, 대부분의 검사가 환자에게 위험성이 없는 비침습적 검사라는 의견이었다. 다만, 국내 임상 상황에서 식약처 허가 사항 및 건강보험심사평가원 고시 등을 종합적으로 고려할 때, 발음 및 발성검사가 여러 개의 검사로 이루어져 행위 정의가 모호한 측면이 있으므로 이를 명확히 정의하는 것이 필요하며, 의료기관마다 장비와 소프트웨어가 다양하여 검사 결과의 참고치와 해석 기준에 대한 표준화가 필요하다는 의견을 제시하였다. 2025년 제3차 재평가전문위원회*(2025.11.14.)는 소위원회 검토 결과를 바탕으로 ‘발음 및 발성검사-발음장애’에 대해 심의하였으며, 소위원회의 검토 의견을 원안대로 의결하였다.


Background The Pronunciation and Phonation Test includes auditory perceptual testing, voice-acoustic testing, aerodynamic testing, electroglottography, and nasal resonance measurement, targeting all patients with conditions causing voice disorders, such as vocal nodules, polyps, vocal paralysis, and laryngeal malignancies. This test is a non-reimbursed health technology for evaluating voice disorders, postoperative follow-up, and the effectiveness of surgical treatment and voice therapy. However, since this test is used to evaluate speech production and acoustic characteristics in patients with articulation and speech sound disorders, it was considered necessary to review the evidence regarding patients with articulation disorders. This health technology was identified as a reassessment item in a demand survey conducted by the Health Technology Reassessment (HTR) Program. This health technology was selected for reassessment at the 4th Health Technology Reassessment Committee (April 11, 2025), and the reassessment was completed accordingly. Objective The objective of this assessment is to evaluate the evidence regarding the clinical utility of the Pronunciation and Phonation Test in patients with articulation disorders. Methods All assessment methods were finalized by the "Subcommittee on the Pronunciation and Phonation Test" ("Subcommittee") after deliberation and aligned with the assessment objectives. The Subcommittee comprised five experts: two otolaryngologists, two rehabilitation medicine specialists, and one evidence-based medicine specialist. In this evaluation, clinical utility was verified through a review of medical textbooks and clinical practice guidelines. Additionally, a brief literature search was conducted using a single database to identify current usage status. Medical textbooks in otolaryngology and rehabilitation medicine were reviewed. Clinical practice guidelines were identified through domestic and international database searches, after which their eligibility was determined through discussion within the Subcommittee. The literature review was performed using the Ovid-MEDLINE database. Based on the Subcommittee's review, the Health Technology Reassessment Committee reviewed and approved. Results Textbooks on rehabilitation medicine and pediatric rehabilitation medicine state that articulation assessment is primarily performed using the Picture Consonant Articulation Test (PCAT), the Korean Standard Picture of Articulation and Phonological Test (KS-PAPT), the Urimal Test of Articulation and Phonology (U-TAP), and maximum phonation time (MPT). The potential usefulness of acoustic voice analysis and the need to understand voice assessment were mentioned, but there were no detailed descriptions of the Pronunciation and Phonation Test. A total of eight clinical practice guidelines were reviewed. Of the four guidelines for stroke, three recommended comprehensive speech and language evaluation for patients with motor speech disorders, including apraxia of speech and dysarthria. One practice guideline for head and neck cancer emphasized the importance of comprehensive speech and language assessment in cancer survivors. Clinical practice guidelines for pediatric patients with traumatic brain injury (TBI) suggested an expert consensus recommending auditory-perceptual evaluation and acoustic assessment using instruments such as Visi-Pitch for the evaluation of dysarthria and voice disorders. One study for pediatric and adolescent patients with cerebral palsy only presented research cases involving voice evaluation after treatment, and there were no specific recommendations for testing. A literature search identified 20 studies evaluating the clinical use of the Pronunciation and Phonation Test in patients with articulation disorders. The study subjects were children with cleft lip and palate (6 cases), children with speech disorders (2 cases), children with articulation disorders due to cerebral palsy (2 cases), Parkinson's disease (3 cases), amyotrophic lateral sclerosis (2 cases), multiple sclerosis (2 cases), myasthenia gravis (2 cases), and Friedreich's ataxia (1 case). Among the assessment modalities, acoustic voice analysis was the most frequently used, appearing in 18 studies (90.0%). In addition, three studies involving children with cleft lip and palate used nasometry to evaluate resonance. The main measurement tools used the voice analysis program Praat in 11 studies (55.5%), and most studies performed perceptual articulation assessment and patient-reported outcome measures. The main measurement indicators included fundamental frequency (F0), Jitter, Shimmer, Cepstral Peak Prominence (CPP), and nasalance score. The purpose of the Pronunciation and Phonation Test included the association of articulation or dysarthria (15 studies) and evaluation and comparison of treatment effects (5 studies). Conclusion and Recommendations Based on a literature review of textbooks, clinical practice guidelines, and clinical studies, the Pronunciation and Phonation Test is used to evaluate articulation disorders. Still, there is a lack of evidence in textbooks or guidelines. The Subcommittee was of the opinion that the Pronunciation and Phonation Test is clinically useful for assessing disease severity and monitoring treatment outcomes for articulation disorders caused by cleft palate and lip, dysarthria resulting from stroke, cerebral palsy, and Parkinson's disease, as well as developmental articulation disorders in children. Furthermore, since these tests are non-invasive procedures, they pose minimal risk to patients. However, the Subcommittee identified several issues requiring further consideration in the Korean healthcare setting. Because the Pronunciation and Phonation Test consists of multiple individual assessment components, the definition of the procedure remains ambiguous and should be more clearly specified. In addition, considerable variation exists among medical institutions with respect to testing equipment and software platforms. Therefore, establishing standardized reference values, measurement protocols, and interpretation criteria is necessary to improve the consistency and comparability of test results. The 3rd Health Technology Reassessment Expert Committee* (November 14, 2025) reviewed the Subcommittee's assessment findings. The Expert Committee endorsed the Subcommittee's conclusions without modification. *With the amendment to the "Rules on the Evaluation of New Health Technology" (Ministry of Health and Welfare Decree No. 1098, effective September 7, 2025), a new Health Technology Reassessment Expert Committee has been formed and has been operating since September 2025.
