상부소화관 공초점 내시경검사

초록
평가배경 상부소화관 공초점 내시경검사는 식도, 위의 이형성이나 암성 병변이 의심되는 환자에서 암성 병변 감별 및 생검위치 확인을 위해 시행하는 기술이다. 본 기술은 2017년 신의료기술로 평가되었으며, 이후 보건복지부 고시 제2020-4호(2020. 1. 7.)에 따라 2020년부터 선별급여 90% 항목으로 건강보험에 등재되어 임상에서 제한적 급여 적용이 가능하게 되었다. 본 기술은 내부 모니터링 과정을 통해 의료기술재평가 대상 안건으로 발굴되었으며, 이에 따라 ‘상부소화관 공초점 내시경검사’에 대한 임상적 안전성 및 유효성에 대한 과학적 근거를 종합적으로 검토하고, 의료기술의 적정 사용 여부를 판단하기 위한 정책적 의사결정을 지원하고자 재평가가 진행되었다. 2024년 제8차 의료기술재평가위원회(2024. 8. 9.)에서는 해당 기술에 대한 재평가 계획서 및 소위원회 구성안이 심의·의결되었으며, 이를 토대로 본 재평가가 수행되었다. 평가목적 본 평가의 목적은 식도 또는 위의 이형성이나 암성 병변이 의심되는 환자에서 상부소화관 공초점 내시경검사를 암성 병변 감별 및 생검 위치 확인 목적으로 사용 시 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 식도와 위의 이형성이나 암성 병변이 의심되는 환자에서 상부소화관 공초점 내시경검사의 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “상부소화관 공초점 내시경검사 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회의 결정에 따라 소화기내과 2인, 병리과 2인, 근거기반의학 1인, 총 5인으로 구성하였다. 본 평가의 대상자는 식도 또는 위의 이형성이나 암성 병변 의심환자, 중재검사는 프로브 기반의 상부소화관 공초점 내시경검사(probe-based confocal laser endomicroscopy, pCLE)로 설정하였고 참고표준검사는 조직병리학적 생검을 기준으로 하였다. 비교검사는 비교검사가 없는 경우, 기본 내시경검사를 단독 시행한 경우, 기본 내시경검사에 영상증강기법을 병행한 경우, 그리고 최종 진단 전 조직검사로 구분하였다. 결과지표로는 안전성과 효과성으로 구분하여 평가하였다. 안전성은 검사 관련 합병증이나 부작용을 결과지표로 하였고 효과성은 진단정확성과 더불어 병변 또는 환자 단위에서의 추가 발견율, 그리고 조직검사 성공률 향상 또는 불필요한 생검감소 등 의료결과에 미치는 영향을 중심으로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 데이터베이스 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials)와 국내 데이터베이스 3개(KoreaMed, 의학논문데이터베이스, 한국교육학술정보원)를 활용하여 연구를 검색하였다(최종검색일 2024. 10. 16.). 최종 선택된 연구 중 진단법 평가연구의 비뚤림위험 평가는 QUADAS-2를 사용하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 진단정확도를 표로 제시하고, 양적 분석이 가능한 경우 메타분석을 하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과 총 33편의 연구가 평가에 최종 선택되었다. 33편 중 30편은 진단법 평가연구, 3편은 진단정확성을 보고하지 않은 무작위배정 임상시험연구(randomized controlled trial, RCT)였다. 식도의 이형성이나 암성 병변 의심환자를 대상으로 하는 연구가 15편, 위의 이형성이나 암성 병변 의심환자를 대상으로 하는 연구가 16편이었으며, 나머지 2편은 식도와 위를 모두 포함한 상부소화관 병변 의심환자를 대상으로 하였다. 중재검사는 대부분 백색광 내시경(white-light endoscopy, WLE)에 공초점 내시경 프로브를 결합하여 시행한 형태였다. 비교검사는 두 가지 유형으로 구분되었다. 첫째, 기존 내시경검사 또는 최종진단 전 조직검사와 pCLE 단독을 비교한 경우, 둘째, 동일한 비교검사에 pCLE를 추가로 병행한 경우와 비교검사만 단독으로 시행한 경우를 비교한 연구가 이에 해당한다. 연구의 비뚤림위험 평가결과 대체로 평가영역별 비뚤림위험이 ‘낮음’으로 평가되었다. 안전성 안전성은 검사 관련 합병증이나 부작용으로 평가하였다. 33편 중 11편에서는 검사 관련 합병증이나 부작용이 발생하지 않았다고 보고하였으며, 6편에서는 경미한 출혈이나 조영제 투여에 따른 알레르기 반응 등이 보고되었다. 이러한 이상반응은 모두 즉각적인 처치 후 회복되었고, 추가적인 의료적 조치가 필요하지 않았으며, 중대한 합병증은 보고되지 않았다. 효과성 효과성은 진단정확성과 병변/환자 추가 발견율, 의료결과에 미치는 영향으로 평가하였다. 식도의 이형성이나 암성 병변 의심환자를 대상으로 한 연구 중 2X2표 추출이 가능한 연구 16편을 대상으로 메타분석한 결과, pCLE의 진단정확성은 통합 민감도 0.89 (95% 신뢰구간(confidence interval, CI) 0.79-0.95), 통합 특이도 0.89 (95% CI 0.80-0.94), 통합 곡선하면적(area under the curve, AUC) 0.95 (95% CI 0.93-0.97)였고, 바렛식도 대상자로 하위군 분석 결과(10편) 통합 민감도 0.87 (95% CI 0.66-0.96), 통합 특이도 0.87 (95% CI 0.73-0.95), 통합 AUC 0.93 (95% CI 0.91-0.95)이었다. 위의 이형성이나 암성 병변 의심환자 대상 연구 13편을 메타분석한 결과 pCLE의 진단정확성은 통합 민감도 0.90 (95% CI 0.85-0.94), 통합 특이도 0.95 (95% CI 0.93-0.97), 통합 AUC 0.98 (95% CI 0.96-0.99)이었다. 비교검사의 진단정확성은 비교검사의 종류와 pCLE의 단독 시행 또는 병행 여부에 따라 비교 조건이 상이하여, 메타분석은 수행하지 않고 각 연구에서 보고한 결과를 중심으로 질적으로 정리하였다. 식도에서 이형성 또는 암성 병변이 의심되는 환자를 대상으로 진단정확성을 보고한 연구는 총 8편이 확인되었으며, 이 중 5편은 기존 내시경검사와, 3편은 최종진단 전 시행된 조직검사와 비교한 것이었다. 이들 중 1편을 제외하고는 모두 pCLE의 진단정확성이 비교검사보다 우수한 것으로 보고하였다. 위에서 이형성 또는 암성 병변이 의심되는 환자를 대상으로 진단정확성을 보고한 연구는 총 7편이었으며, 이 중 4편은 기존 내시경검사와, 3편은 조직검사와 비교하였다. 마찬가지로 1편을 제외한 전 연구에서 pCLE의 진단정확성이 비교검사보다 높은 것으로 나타났다. 바렛식도 및 식도의 암성 병변이 의심되는 환자를 대상으로 한 7편의 연구에서는 pCLE를 시행함으로써 기존 비교검사에서는 관찰되지 않았던 병변 또는 환자가 추가로 확인된 것으로 보고하였다. 위암 환자를 대상으로 한 1편의 연구에서는 pCLE로 유도한 타겟 생검이 백색광 내시경(white light endoscopy, WLE)으로 유도한 생검에 비해 유의하게 많은 미분화형 위암을 검출하였다. 종양의 병리학적 검출률 및 검출 수를 보고한 2편의 연구에서는 pCLE 기반 생검이 기존 내시경검사에 비해 더 많은 암 조직을 포함한 것으로 나타났으며, 진단율(diagnostic yield)을 보고한 2편에서도 pCLE의 진단율이 비교검사보다 유의하게 높은 것으로 확인되었다. 치료계획의 변화를 보고한 1편에서는 pCLE 결과에 따라 치료 방향이 긍정적으로 변경되었고, 생검감소 효과를 분석한 3편의 연구에서는 기존 검사 또는 비교검사에 pCLE를 병행하였을 때 생검 횟수가 유의하게 감소한 것으로 보고하였다. 결론 및 권고결정 의료기술재평가 소위원회에서는 현재 평가결과에 근거하여 다음과 같은 의견을 제시하였다. 체계적 문헌고찰 결과 동 검사로 인한 합병증이나 부작용을 보고한 연구가 적고(33편 중 6편), 대부분 경미하고 일시적인 증상으로 중대한 부작용은 없었다. 이에 소위원회에서는 상부소화관 공초점 내시경검사의 안전성은 일반 내시경검사와 유사하므로 수용가능한 수준이라는 의견이었다. 효과성 측면에서 식도의 이형성이나 암성 병변 의심환자에서 참고표준인 조직병리학적 검사결과를 기준으로 하였을 때 중재검사의 진단정확성은 민감도와 특이도 모두 0.89로 높은 수준을 보였다. 중재검사를 단독으로 시행하거나 기존 검사(내시경검사 또는 최종진단 전 조직검사)에 추가로 시행한 경우 진단정확성은 전반적으로 비교검사와 유사한 수준이었으나, 바렛식도 환자에서는 기존 비교검사로는 관찰되지 않았던 병변 또는 환자가 pCLE로 추가 확인되었으며, 불필요한 생검 횟후의 감소 또는 치료계획의 긍정적인 변경이 보고되었다. 위의 이형성이나 암성 병변 의심환자에서 참고표준인 조직병리학적 검사를 기준으로 평가한 결과, 중재검사의 진단정확성은 민감도와 특이도 모두 0.90 이상으로 높은 수준을 보였다. 중재검사를 단독으로 시행하거나 기존 검사(내시경검사 또는 최종진단 전 조직검사)에 추가로 시행한 경우, 진단정확성은 전반적으로 비교검사와 유사하였으나, 일부 연구에서는 pCLE를 활용해 타겟 생검한 검체에서 비교검사에 비해 유의하게 많은 암 조직이 포함되었다고 보고하였다. 이에 소위원회에서는 본 검사가 식도 및 위에서 각각 이형성 또는 암성 병변의 추적관찰 중 병변의 악성 여부를 평가하고, 이형성 및 암에 대한 진단정확도를 높이며, 조직검사 위치 결정에 도움이 될 수 있는 검사라는 의견이었다. 다만, 국내에서 바렛식도의 유병률이 낮은 점, 비교검사와의 진단정확성 비교가 다양한 조건에서 이루어져 메타분석 수행이 불가능하였던 점, 치료계획의 변경 등 실제 임상에서 환자에게 미치는 이익에 대한 근거가 아직 충분하지 않다는 점 등을 고려할 때, 임상 적용 시 신중한 판단이 필요하다는 의견도 함께 제시되었다. 그럼에도 불구하고, 본 검사는 검사 소요시간이 길어 남용 가능성은 낮으며, 병변을 고해상도로 정밀하게 관찰할 수 있어 조기 진단율을 높이는 데 유용할 수 있다는 의견이 있었다. 2025년 제4차 의료기술재평가위원회(2025. 4. 11.)는 ‘상부소화관 공초점 내시경검사’에 대해 다음과 같이 심의하였다. 식도의 이형성이나 암성 병변 의심 환자를 대상으로 동 기술을 사용 시 병변의 조기발견이 가능하여 안전성 및 효과성에 대한 근거는 확인되었으나, 장비가 고가이고 높은 숙련도가 요구된다는 점, 비교기술 대비 동 기술의 추가적인 효과에 대한 근거가 충분하지 않다는 점, 주 적응증인 바렛식도의 국내 유병률이 낮다는 점에서 동 기술의 사용이 필요한 경우가 제한적이라고 판단하였다. 이에 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 식도의 이형성이나 암성 병변 의심 환자를 대상으로 상부소화관 공초점 내시경검사의 사용을 ‘약하게 권고함’으로 결정하였다. 위의 이형성이나 암성 병변 의심 환자를 대상으로 동 기술을 사용 시 병변의 조기발견이 가능하여 안전성 및 효과성에 대한 근거는 확인되었으나, 장비가 고가이고 높은 숙련도가 요구된다는 점, 비교기술 대비 동 기술의 추가적인 효과에 대한 근거가 충분하지 않다는 점에서 동 기술의 사용이 필요한 경우가 제한적이라고 판단하였다. 이에 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 위의 이형성이나 암성 병변 의심 환자를 대상으로 상부소화관 공초점 내시경검사의 사용을 ‘약하게 권고함’으로 결정하였다.
Background Confocal Laser Endomicroscopy (CLE) of the upper gastrointestinal tract (GIT) is a diagnostic technology used to differentiate malignant lesions and identify optimal biopsy targets in patients with suspected dysplastic or malignant lesions of the esophagus or stomach. This technology was first evaluated in 2017 through the New Health Technology Assessment and was subsequently included in the National Health Insurance program in 2020 for selective reimbursement (90% copayment), as announced in the Ministry of Health and Welfare Bulletin No. 2020-4 (January 7, 2020). CLE was selected for reassessment through the internal monitoring process to comprehensively review updated scientific evidence regarding its clinical safety and effectiveness and to inform policy decisions on its appropriate use. At the 8th Health Technology Reassessment Committee meeting (August 9, 2024), the reassessment plan and subcommittee composition were reviewed and approved, and the reassessment was conducted accordingly. Objective The objective of this reassessment was to evaluate the clinical safety and effectiveness of CLE of the upper GIT in differentiating malignant lesions and identifying appropriate biopsy sites in patients with suspected dysplastic or malignant lesions of the esophagus or stomach, as well as to determine the appropriate strength of recommendation for its clinical use. Method A systematic literature review was conducted to evaluate the safety and effectiveness of CLE of the upper GIT in patients with suspected dysplastic or malignant lesions of the esophagus or stomach. All assessment methodologies were finalized through discussions within the Subcommittee on CLE (hereinafter referred to as “the Subcommittee”). The Subcommittee comprised five experts, including two gastroenterologists, two pathologists, and one specialist in evidence-based medicine. The target population included patients with suspected dysplastic or malignant lesions of the esophagus or stomach. The intervention was probe-based CLE (pCLE), and the reference standard was histopathologic biopsy. Comparator tests included conventional endoscopy alone, endoscopy combined with image-enhanced modalities, or pre-diagnostic biopsy, where applicable. Study outcomes were categorized into safety and effectiveness. Safety outcomes included procedure-related complications and adverse events, while effectiveness outcomes encompassed diagnostic accuracy, detection rates of additional lesions or patients, biopsy success rates or avoidance of unnecessary biopsies, and the impact on clinical decision-making. Systematic searches were conducted across three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews–Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, Korean Medical Database, and the Research Information Sharing Service). The final search date was October 16, 2024. The risk of bias in diagnostic evaluation studies was assessed using the QUADAS-2 tool. Data were extracted using a pre-specified form, and diagnostic performance measures were summarized in tabular format. Where appropriate, a meta-analysis was conducted to provide a quantitative synthesis. All review processes were performed independently by two reviewers, with discrepancies resolved through consensus. The findings of the Subcommittee were reviewed and endorsed by the Health Technology Reassessment Committee, which subsequently determined the final grade of recommendation for this technology. Results A total of 33 studies were included in this reassessment. Of these, 30 were diagnostic evaluation studies, and three were randomized controlled trials that did not report diagnostic accuracy outcomes. Notably, 15 studies focused on patients with suspected dysplastic or malignant esophageal lesions, 16 on those with suspected gastric lesions, and two included both esophageal and gastric lesions. In most studies, the intervention involved combining a confocal endomicroscopy probe with conventional white-light endoscopy (WLE). Comparator tests were categorized into two groups: (i) pCLE versus conventional endoscopy or pre-diagnostic biopsy alone, and (ii) pCLE in addition to conventional endoscopy or pre-diagnostic biopsy versus the comparator alone. The overall risk of bias across the included studies was generally assessed as low. Safety Safety was evaluated based on test-related complications or adverse events. Among the 33 included studies, 11 reported no test-related complications or adverse events. Six studies reported mild hemorrhage or allergic reactions to the contrast agent. All reported adverse events resolved following immediate management and did not require further medical intervention. No serious adverse events were reported. Effectiveness Effectiveness was assessed based on diagnostic accuracy, additional lesion or patient detection rate, and impact on medical outcomes. A meta-analysis of 16 studies providing two-by-two diagnostic data for patients with suspected dysplastic or malignant esophageal lesions demonstrated a pooled sensitivity of 0.89 (95% confidence interval [CI]: 0.79–0.95), pooled specificity of 0.89 (95% CI: 0.80–0.94), and an area under the curve (AUC) of 0.95 (95% CI: 0.93–0.97). In a subgroup analysis of Barrett’s esophagus (10 studies), the pooled sensitivity and specificity were both 0.87, with an AUC of 0.93 (95% CI: 0.91–0.95). A meta-analysis of 13 studies involving patients with suspected gastric dysplasia or malignancy yielded a pooled sensitivity of 0.90 (95% CI: 0.85–0.94), pooled specificity of 0.95 (95% CI: 0.93–0.97), and an AUC of 0.98 (95% CI: 0.96–0.99). Due to heterogeneity in comparator types and study designs, a meta-analysis of comparator tests was not performed, and findings were qualitatively synthesized. Among eight studies evaluating diagnostic accuracy for esophageal lesions, five compared pCLE with conventional endoscopy and three with pre-diagnostic biopsy; all but one reported higher diagnostic accuracy for pCLE. Among seven studies on gastric lesions, four compared pCLE with conventional endoscopy and three with biopsy, with all but one reporting superior accuracy for pCLE. Seven studies involving patients with Barrett’s esophagus or suspected esophageal carcinoma reported that pCLE identified additional lesions or patients not detected by the comparator tests. In one study of patients with gastric cancer, pCLE-guided targeted biopsy detected a significantly higher number of poorly differentiated gastric carcinomas than WLE-guided biopsy. Additionally, two studies assessing histopathologic tumor detection rates found that pCLE-guided biopsy specimens contained a higher proportion of malignant tissue, while two additional studies demonstrated significantly higher diagnostic yield with pCLE. In one study that evaluated changes in treatment planning, pCLE findings led to favorable modifications in clinical management, and in three studies, the addition of pCLE significantly reduced the number of biopsies required. Conclusion and Grade of Recommendation The Health Technology Reassessment Subcommittee presented the following conclusions based on the current evidence. Among the 33 studies included in the systematic review, only six reported adverse events or complications, all of which were mild and transient. No serious adverse events were reported. Accordingly, the Subcommittee determined that the safety profile of CLE for the upper GIT is comparable to that of conventional endoscopy and is considered clinically acceptable. Regarding effectiveness, the diagnostic accuracy of pCLE for differentiating malignant lesions of the esophagus was high when compared with histopathologic findings as the reference standard, with pooled sensitivity and specificity values of approximately 0.89. Although overall diagnostic performance was similar to that of comparator tests—whether performed alone or in combination with conventional modalities such as endoscopy or pre-diagnostic biopsy—pCLE demonstrated added value in specific clinical contexts. In patients with Barrett’s esophagus, pCLE identified additional lesions or patients not detected by conventional testing, reduced unnecessary biopsies, and contributed to favorable changes in treatment planning. For patients with suspected dysplastic or malignant lesions of the stomach, diagnostic accuracy against histopathologic findings as the reference standard demonstrated both sensitivity and specificity exceeding 0.90. While diagnostic accuracy was generally comparable to that of comparator tests, several studies reported that pCLE-guided targeted biopsies contained a significantly higher proportion of malignant tissue compared to specimens obtained using conventional methods. Based on these findings, the Subcommittee concluded that CLE may be beneficial for evaluating malignancy during follow-up of dysplastic or malignant lesions in the esophagus and stomach, improving diagnostic accuracy, and supporting biopsy site selection. However, given the low prevalence of Barrett’s esophagus in Korea, the heterogeneity of comparator settings that precluded meta-analysis, and the limited evidence regarding patient-level outcomes—such as treatment modification or long-term clinical benefit—the Subcommittee recommended cautious clinical application. Nonetheless, recognizing that the procedure is time-intensive and thus unlikely to be overused, the Subcommittee considered CLE potentially valuable for improving early detection through high-resolution, real-time visualization. At the 4th Health Technology Reassessment Committee meeting (April 11, 2025), the Committee reviewed these findings and reached the following decisions. For patients with suspected dysplastic or malignant lesions of the esophagus, evidence supporting the safety and effectiveness of CLE was identified. However, due to the high cost of the device, technical complexity requiring advanced operator expertise, limited incremental benefit over existing modalities, and low prevalence of Barrett’s esophagus in Korea, the Committee concluded that the clinical need for this technology is limited. Based on a comprehensive evaluation of clinical safety, effectiveness, and other evaluation criteria, the use of CLE for suspected dysplastic or malignant esophageal lesions in the Korean clinical setting is “weakly recommended.” For patients with suspected dysplastic or malignant lesions of the stomach, evidence supporting safety and effectiveness was also identified. However, considering the high equipment cost, need for specialized training, and limited evidence of additional clinical benefit over existing technologies, the Committee determined that the clinical indications for its use are limited. Accordingly, based on a comprehensive assessment of clinical safety, effectiveness, and other evaluation criteria, the use of CLE for suspected dysplastic or malignant gastric lesions in the Korean clinical setting is “weakly recommended.”
