초록

평가배경 비스쿰 알붐(Viscum album)’, ‘이뮤노시아닌(Immunocyanin)’, ‘싸이모신 알파 1(Thymosin α1)’은 암 환자 및 면역기능 저하자를 대상으로 종양 치료, 항암 보조요법, 암 재발 예방, 면역기능 보조요법 등의 목적으로 사용되고 있는 비급여 주사제이다. ‘비급여 상세내역조사’ 자료에 따르면, 해당 약제들은 종양용약 중에서도 진료비 규모가 큰 항목에 속한다. 이들 기술은 ‘건강보험 비급여 관리강화 종합대책’ 수립(’20.12.31.)에 따라, 적정한 의료서비스 제공및 합리적 의료이용을 지원하기 위해 유관기관으로부터 의료기술재평가를 요청받았다. 2024년 제4차 의료기술재평가위원회(2024.4.12.)에서 의료기술재평가 계획서 및 소위원회 구성안을 심의하고, 임상적 안전성과 유효성에 대한 전문적이고 심층적인 검토를 통해 재평가를 수행하였다. 본 평가의 목적은 비스쿰 알붐(Viscum album), 이뮤노시아닌(Immunocyanin) 및 싸이모신 알파 1(Thymosin α1)의 임상적 안전성과 유효성에 대한 객관적인 근거를 도출하고, 해당 기술에 대한 의료기술재평가 권고등급을 제시하는 데 있다. 본 보고서에는 그 중 비스쿰 알붐의 안전성 및 유효성 평가 내용이 포함되어 있다. 평가방법 비스쿰 알붐의 안전성 및 유효성을 평가하기 위해 체계적 문헌고찰(Systematic Review)을 수행하였다. 세부 평가 방법 및 결과는 「비스쿰 알붐ㆍ이뮤노시아닌ㆍ싸이모신 알파 1 약물의 안전성 및 유효성 평가 통합 소위원회」(이하 ‘소위원회’)의 검토와 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 심의한 평가계획에 따라 혈액종양내과 2인, 가정의학과 2인, 근거기반의학 전문가 2인으로 구성되어 총 6인이 참여하였다. 본 평가의 대상자는 암 환자로 설정하였다. 안전성 평가는 심각한 이상반응 및 부작용, 일반적인 이상반응 및 부작용 등을 주요 지표로 설정하였다. 유효성은 생존율, 재발 및 질병 진행, 항암반응률, 삶의 질을 결과지표로 설정하였다. 해당 약물이 주로 암 환자에게 사용된다는 점을 고려하여 본 평가는 선택 비뚤림 및 교란 요인 등을 최소화할 수 있는 연구설계인 무작위배정 비교임상시험(Randomized Controlled Trials, RCTs) 연구로 한정하여 문헌을 선정하였다. 문헌 검색은 핵심질문을 기반으로 국외 3개 데이터베이스(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials)와 국내 3개 데이터베이스(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS))에서 수행되었다(최종검색일 2024. 7. 25.). 최종 선정된 연구에 대해서는 ‘비뚤림위험(Risk of Bias, RoB)’ 평가 도구를 활용하여 평가를 수행하였고, 자료추출은 사전에 정의한 양식에 따라 시행하였다. 양적 분석이 가능한 경우에는 메타분석을 실시하였다. 모든 과정은 2인의 평가자가 독립적으로 수행하였으며, 의견 불일치 시 합의를 통해 최종 결과를 도출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거 수준을 평가하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회의 심의를 거쳐 의료기술재평가 권고등급이 결정되었다. 평가결과 체계적 문헌고찰 결과, 최종 선택연구는 18개의 무작위배정 비교임상시험 연구(문헌 28편)로 임상시험 단위로 평가하였다. 출판연도는 2010년 이전 연구가 12개, 2010년 이후 6개이었고, 출판국가는 독일이 가장 많았다. 연구대상자는 폐암, 유방암, 췌장암, 방광암, 신장암 등 다양하였고, 악성흉막삼출액 환자도 포함하였다. 중재치료는 수술 후 또는 항암치료에 추가적인 치료로 이루어졌고, 비교치료는 수술 후 무치료, 수술 후 표준치료, 항암치료 등이었다. 비뚤림위험 평가는 각 항목별 기준에 따라 수행하였다. 먼저, 무작위배정 순서 생성과 순서 은폐 방법에 대한 기술이 없는 경우에는 관련 정보의 부족으로 인해 비뚤림위험을 ‘불확실’로 평가하였다. 눈가림 항목의 경우, 생존율, 무질병 생존율 등 객관적 지표를 결과로 제시한 연구는 눈가림의 영향을 받지 않을 것으로 판단하여 비뚤림위험을 ‘낮음’으로 평가하였다. 반면, 삶의 질과 같은 주관적 지표를 활용한 연구는 비뚤림위험 ‘불확실’로 평가하였다. 불충분한 결과 자료 항목은 추적관찰 손실이 20% 이상 발생한 경우 ‘불확실’로 평가하였다. 선택적 보고에 대한 항목은 안전성 또는 유효성 지표 중 하나만 보고한 경우 ‘불확실’로 평가하였다. 기타 비뚤림위험 항목은 연구 수행에 있어 약물을 제조사로부터 제공받았거나 연구비를 지원받은 경우는 비뚤림위험 ‘높음’으로 평가하였다. 안전성 심각한 이상반응은 총 3편의 연구에서 보고되었으며, 이 중 2편의 연구는 군별 부작용 건수를 보고하였고 나머지 1편의 연구인 Woode 등(2024)은 비스쿰 알붐군에서 20.7%, 비교군에서 19.6% 발생하였다. 약물 및 연구 중단을 보고한 연구는 5편이었다. 이 중 Kleeberg 등(2004)은 비스쿨 알붐 치료군에서 WHO grade 3-4에 해당하는 독성으로 인해 연구 중단 사례가 4.9% 발생하였다. Semiglasov 등(2004)은 고용량 비스쿨 알붐 치료군에서 한 명의 환자가 홍반 발생으로 인해 약물을 중단한 후 회복되었으나 며칠 후 사망한 사례가 보고되었고, 연구자와 약리위원회는 비스쿰 알붐 치료와는 관련이 없다고 판단하였다. 나머지 3편에서 약물 중단을 보고하였으며 비스쿰 알붐군에서 5.4~15.4%, 비교군에서 0.0~5.4% 사이로 발생하였다. 일반적인 이상반응 및 부작용은 약물관련 이상반응, 항암치료로 인한 이상반응을 포함하여 연구마다 다양하였다. 전체 이상반응 및 부작용을 보고한 5편 중 3편의 연구는 항암치료 관련 부작용을 포함하여 비스쿰 알붐군에서 10.0~46.6%, 비교군에서 30.9~45.5% 사이로 보고하였다. Semiglasov 등(2004)은 약물관련 부작용으로 오한, 근육통, 알레르기성 피부반응 등이 비스쿰 알붐 치료군에서 1.6~4.5%로 발생하였다. 부작용 등으로 인한 입원은 4편의 연구에서 보고하였고, 비스쿰 알붐군에서 0.0~24.2%, 비교군에서 0.9~53.8% 사이로 나타났다. 항암치료 관련 부작용은 백혈구감소증, 혈역학적 독성, 위장관계 질환 등을 보고하였으며, 7편 중 6편의 연구를 메타분석한 결과, 비스쿰 알붐 치료군과 비교군 사이 유의한 차이는 없었다(Risk ratio (RR)=0.85, 95% confidence interval (CI) 0.63~1.13, I2=0%). 메타분석에 포함되지 않은 Piao 등(2004) 연구는 비스쿰 알붐군에서 부작용 건수가 좀 더 낮게 나타났다. 또한, 백혈구감소증 및 호중구감소증을 보고한 4편 중 3편의 연구를 메타분석한 결과, 군 간 유의한 차이는 없었다(RR=0.83, 95% CI 0.47~1.47, I2=0%). Longhi 등(2014) 연구는 비스쿰 알붐군에서 호중구감소증이 발생하지 않았고, 비교군에서 12건을 보고하였다. 비스쿰 알붐 주사제 사용으로 인한 부작용 중 국소 피부반응을 비롯하여 가려움, 발진, 부분 통증, 두드러기 등을 보고한 연구는 9편이었다. 비스쿰 알붐은 피하주사 후 0.0~66.4%의 분포로 부작용을 보고하였으며, 대부분의 증상은 시간이 지나면서 사라지는 경향이 있었다. 유효성 5년 생존율, 생존위험비, 사망자 수, 중앙생존기간은 12편의 연구에서 보고되었다. 5년 생존율은 2편의 연구에서 비스쿰 알붐군 62.0~66.7%, 비교군 40.0~60.0%의 분포를 보였고, 비스쿰 알붐군과 비교군 간 위험비를 보고한 4편 중 3편은 군 간 유의한 차이가 없었다. 사망자 수를 보고한 3편 중 2편의 연구는 비스쿰 알붐군에서 사망 발생이 다소 낮은 경향이 있었고, 1편의 연구(Kleeberg 등, 2004)는 비스쿰 알붐군에서 암 특이적 사망이 높게 나타난 경향이 있었다. 중앙 생존기간은 9편 중 5편은 유의한 차이가 없었고(Lenartz 등, 2000; Brinkmann 등, 2004; Longhi 등, 2020; Bar-Sela 등, 2013; Wode 등, 2024), 2편은 비스쿰 알붐군에서 중앙 생존기간이 유의하게 길게 나타나는 경향이 있었으며(Cazacu 등, 2003; Troger 등, 2013), 나머지 2편은 통계적 유의성을 확인할 수 없어 군 간 차이를 확인할 수 없었다(Salzer 등, 1991; Heiny 등 1997). 재발 및 질병진행 등은 9편의 연구에서 보고되었다. 무질병, 무재발 생존율을 보고한 연구는 2편으로 Steuer-Vogt 등(2001)은 군 간 유의한 차이가 없었고, Longhi 등(2020)은 비스쿰 알붐군에서 길게 나타났다. 무질병, 무재발 생존 위험비를 보고한 3편 중 2편의 연구는 군 간 유의한 차이가 없었다. 무질병, 무재발에 대해 각각 메타분석한 결과, 두 결과지표 모두 군 간 유의한 차이가 없었다(무질병에 대한 RR=1.00, 95% CI 0.79~1.27, I2=0%; 무재발에 대한 RR=1.00, 95% CI 0.82~1.21, I2 =22%). 무질병, 무진행, 무재발 기간을 보고한 6편 중 2편의 연구는 군 간 유의한 차이가 없었고(Longhi 등, 2020; Bar-Sela 등, 2013), 나머지 4편의 연구는 표준편차 및 범위 등을 제시하지 않아 군 간 차이를 확인할 수 없었다(Salzer 등, 1991; Lenartz 등, 2000; Heiny 등, 1997; Goebell 등, 2002). 항암반응률과 관련해서 관해율은 2편에서 평가되었다. 완전 및 부분 관해율은 비스쿰 알붐군 0.0~21.4%, 비교군 22.6~29.7%으로 비스쿰 알붐군에서 낮게 나타나는 경향이 있었다. 악성흉막삼출액 환자의 임상반응률은 2편의 연구에서 보고하였고, 성공 또는 부분 성공 비율이 비스쿰 알붐군 61.5~76.7%, 비교군 30.0~80.0%로 비스쿰 알붐군에서 높게 나타나는 경향이 있었다. 삶의 질은 11편의 연구에서 보고하였다. 이 중 시점별 측정값을 보고한 연구 2편 중 Lenartz 등(1996) 연구는 비스쿰 알붐 치료군에서 수술 24주 후 삶의 질이 상당히 개선되었다고 보고하였고, Heiny 등(1997)은 비스쿰 알붐군에서 12주 이후 삶의 질이 비교군에 비해 유의하게 개선되었다고 보고하였다. 군별 변화량을 보고한 6편 중 4편의 연구는 표준화 평균 차이에 대한 메타분석을 수행한 결과, 군 간 유의한 차이는 없었고 이질성이 높아 결과 해석에 주의가 필요하였다(Standardized mean difference (SMD)=0.40, 95% CI –0.25~1.05, I2=83%). 2편의 연구는 표준편차 등의 정보가 없어 메타분석을 할 수 없었고, 각각의 연구에서 삶의 질이 군 간 유의한 차이가 없다고 보고하였다(Pelzer 등, 2018; Woode 등, 2024). 군 간 평균 차이를 보고한 3편 중 Steuer-Vogt 등(2006)의 연구는 군 간 유의한 차이가 없었고, 나머지 2편의 연구는 비스쿰 알붐군에서 삶의 질이 유의하게 개선되었다(Kim 등, 2012; Troger 등, 2014). 결론 및 의료기술재평가 권고결정 의료기술재평가 소위원회는 국내에서 비스쿰 알붐이 종양치료 및 종양수술 후 재발 예방 등으로 허가되었으며 항악성종양제로 분류되어 있는 점을 고려하여 본 평가는 비스쿰 알붐의 암치료 및 항암 보조요법으로서의 안전성과 유효성을 중심으로 수행하였다. 체계적 문헌고찰 결과, 비스쿰 알붐은 기존 치료군과 비교하여 심각한 이상반응 및 부작용, 전체 이상반응 및 부작용, 항암치료 관련 부작용 등이 유사한 수준으로 발생하였고, 비스쿰 알붐군에서 주사 부위에 경미하고 일시적인 피부반응이 있으나 위해가 크지 않아 안전하다고 평가하였다. 유효성은 비스쿰 알붐이 보조적인 암 치료로 다양한 암(유방암, 폐암, 췌장암 등) 환자를 대상으로 연구되었지만, 생존율, 재발 및 질병 진행, 삶의 질 등을 개선하는데 추가적인 이득이 있다고 판단하기 어려웠다. 본 평가는 무작위배정 비교임상시험 연구를 중심으로 수행하였으나, 개별 연구의 표본 크기가 작은 점, 암 유형과 병기 및 진행 단계가 일관되지 않은 점 등으로 인해 진행성 또는 전이성 암 환자에서 임상적 이점을 입증하기 어렵다고 보았다. 또한, 상당수 연구들이 2010년 이전에 진행되어 현행 암 치료 실태를 반영하지 못한다는 점에서 최신 근거가 부족하다는 의견이었다. 이외에도 비스쿰 알붐 치료 관련 연구 상당수가 한 국가(독일)에서 수행된 점, 무작위배정 방법, 순서은폐 방법 등을 명확히 보고하지 않는 등 비뚤림위험이 존재하였다. 이에 따라 전체적으로 근거수준은 ‘낮음’ 또는 ‘매우 낮음’으로 평가되었다. 2024년 제12차 의료기술재평가위원회(2024.12.13.)에서는 의료기술재평가사업 관리지침 제4조제10항에 근거하여 소위원회 검토 결과를 바탕으로 비스쿰 알붐에 대한 최종 심의를 진행하였다. 위원회는 비스쿰 알붐이 암 환자에게 통상적인 항암치료에 추가로 사용될 경우, 안전성은 기존 치료와 유사한 수준으로 판단하였다. 그러나 암종별로 환자 특성이 이질하고 임상 결과가 일관되지 않은 점, 연구의 비뚤림위험에 대한 우려, 최신 연구 부족 등으로 인해 유효성에 대한 문헌 근거수준은 낮다고 평가하였다. 이에 따라, 의료기술재평가위원회는 임상적 안전성과 유효성 등에 대한 근거를 종합적으로 고려하였을 때, 국내 임상상황에서 암 환자에게 종양 치료 및 재발 예방 목적으로 기존 통상적인 암 치료에 비스쿰 알붐을 추가 투여하는 것을 ‘권고하지 않음’으로 결정하였다.


Background Viscum album, Immunocyanin, and Thymosin α1 are non-reimbursable injectable drugs used in patients with cancer or impaired immune function for purposes such as tumor treatment, adjuvant cancer therapy, prevention of cancer recurrence, and immune support therapy. According to data from the “Detailed Survey of Non-reimbursable Items,” these drugs rank among the highest in medical expenditure in the category of antineoplastic agents. In accordance with the “Comprehensive Plan to Strengthen the Management of Non-Reimbursable Services in the National Health Insurance” (December 31, 2020), these drug-based techniques were referred for health technology reassessment by relevant institutions to promote appropriate healthcare delivery and rational healthcare utilization. The reassessment protocol and subcommittee composition were reviewed at the 4th 2024 Health Technology Reassessment Committee meeting (April 12, 2024), and the reassessment was conducted through a professional and in-depth review of clinical safety and effectiveness. The objective of this reassessment was to provide objective evidence on the clinical safety and effectiveness of Viscum album, Immunocyanin, and Thymosin α1 and to determine the strength of recommendation for each technology. This report presents the assessment results for Viscum album. Method The safety and effectiveness of Viscum album were assessed using a systematic literature review. The detailed methodology and results were finalized through review and discussion by the “Joint Subcommittee on the Safety and Effectiveness Assessment of Viscum album, Immunocyanin, and Thymosin α1” (hereafter referred to as the “Subcommittee”). The Subcommittee comprised six members: two hematology-oncology specialists, two family medicine specialists, and two evidence-based medicine experts, in accordance with the reassessment plan approved by the Health Technology Reassessment Committee. The target population was patients with cancer. Safety was assessed based on severe adverse events (AEs) and complications, as well as general AEs and complications. Effectiveness outcomes included survival rate, recurrence and disease progression, antitumor response rate, and quality of life. Considering that the agent is primarily used in patients with cancer, the literature review was limited to randomized controlled trials (RCTs) to minimize selection bias and confounding. The literature search was performed using key questions across three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews – Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, KMbase, and RISS), with the final search conducted on July 25, 2024. Risk of bias was assessed for each included study using the Risk of Bias (RoB) tool, and data were extracted using a predefined form. A meta-analysis was performed when quantitative synthesis was feasible. All processes were conducted independently by two reviewers, with any discrepancies resolved through consensus. The level of evidence was assessed using the GRADE (Grading of Recommendations Assessment, Development and Evaluation) approach. The final strength of recommendation was determined through review by the Health Technology Reassessment Committee based on the Subcommittee’s findings. Results A total of 18 RCTs (reported in 28 publications) were identified, and the data were analyzed by trial. Twelve studies were published before 2010 and six thereafter; most studies were conducted in Germany. Study populations included patients with lung, breast, pancreatic, bladder, and kidney cancers, as well as those with malignant pleural effusion. The intervention was Viscum album administered postoperatively or in combination with chemotherapy, while comparators included no postoperative treatment, postoperative standard therapy, or chemotherapy alone. Risk of bias was assessed by domain. Studies that did not describe random sequence generation or allocation concealment were rated “unclear” for these domains due to insufficient information. For blinding, studies reporting objective outcomes (e.g., survival and disease-free survival) were considered unaffected by lack of blinding and rated “low” risk, whereas those reporting subjective outcomes (e.g., quality of life) were rated “unclear.” For incomplete outcome data, studies with >20% loss to follow-up were rated “unclear.” For selective reporting, studies reporting only safety or only effectiveness outcomes were rated “unclear.” For other sources of bias, studies that received study drugs or funding from the manufacturer were rated “high.” Safety Severe AEs were reported in three studies. Two studies reported the number of events by group, and one study (Woode et al., 2024) reported rates of 20.7% in the Viscum album group and 19.6% in the control group. Five studies reported treatment discontinuation due to AEs. In the study by Kleeberg et al. (2004), 4.9% of patients in the Viscum album group discontinued treatment due to WHO grade 3–4 toxicity. In the study by Semiglasov et al. (2004), one patient in the high-dose Viscum album group discontinued treatment due to erythema and subsequently recovered but died several days later; both the investigator and pharmacology committee deemed the event unrelated to Viscum album. The remaining three studies reported treatment discontinuation rates of 5.4%–15.4% in the Viscum album group and 0.0%–5.4% in the control group. General AEs and complications varied by study and included both drug-related and chemotherapy-related events. Among five studies reporting overall AEs (including chemotherapy-related events), three reported rates of 10.0%–46.6% in the Viscum album group and 30.9%–45.5% in the control group. In the study by Semiglasov et al. (2004), drug-related AEs included chills, myalgia, and allergic skin reactions, occurring in 1.6%–4.5% of patients receiving Viscum album. Hospitalization due to AEs was reported in four studies, with rates of 0.0%–24.2% in the Viscum album group and 0.9%–53.8% in the control group. Chemotherapy-related AEs included leukopenia, hematologic toxicity, and gastrointestinal disorders. A meta-analysis of six of seven studies reporting chemotherapy-related AEs found no significant difference between groups (RR = 0.85, 95% CI 0.63–1.13, I² = 0%). In the study by Piao et al. (2004), which was not included in the meta-analysis, the number of AEs was lower in the Viscum album group. A meta-analysis of three of four studies reporting leukopenia or neutropenia also found no significant difference (RR = 0.83, 95% CI 0.47–1.47, I² = 0%). In the study by Longhi et al. (2014), no cases of neutropenia were reported in the Viscum album group, compared with 12 cases in the control group. Nine studies reported local skin reactions and other injection-site AEs, such as pruritus, rash, localized pain, and urticaria. The reported rate of local reactions following subcutaneous injection was 0.0%–66.4%, with most resolving over time. Effectiveness Twelve studies reported 5-year survival rates, hazard ratios for survival, number of deaths, or median survival. Two studies reported 5-year survival rates of 62.0%–66.7% in the Viscum album group and 40.0%–60.0% in the control group. Of four studies reporting hazard ratios, three found no significant difference between groups. Of three studies reporting mortality, two reported slightly lower mortality in the Viscum album group, while one (Kleeberg et al., 2004) reported higher cancer-specific mortality in the Viscum album group. Median survival was reported in nine studies. In five studies, no significant difference was observed between the Viscum album and control groups (Lenartz et al., 2000; Brinkmann et al., 2004; Longhi et al., 2020; Bar-Sela et al., 2013; Wode et al., 2024). Two studies (Cazacu et al., 2003; Troger et al., 2013) showed a trend toward significantly longer median survival in the Viscum album group. In the remaining two studies (Salzer et al., 1991; Heiny et al., 1997), statistical significance could not be determined. Recurrence or disease progression was reported in nine studies. Two studies reported disease-free survival or recurrence-free survival: one study (Steuer-Vogt et al., 2001) found no significant difference between the Viscum album and control groups, while the other (Longhi et al., 2020) reported longer recurrence-free survival in the Viscum album group. Of three studies reporting hazard ratios for disease-free survival or recurrence-free survival, two found no significant difference between groups. A meta-analysis found no significant difference for either disease-free survival (RR = 1.00, 95% CI 0.79–1.27, I² = 0%) or recurrence-free survival (RR = 1.00, 95% CI 0.82–1.21, I² = 22%). Among six studies reporting disease-free survival, progression-free survival, or recurrence-free survival, two found no significant difference between groups (Longhi et al., 2020; Bar-Sela et al., 2013), while the remaining four did not provide SD or range data, precluding assessment of group differences (Salzer et al., 1991; Lenartz et al., 2000; Heiny et al., 1997; Goebell et al., 2002). Regarding antitumor response rate, remission rate was evaluated in two studies. Complete or partial remission rates were 0.0%–21.4% in the Viscum album group and 22.6%–29.7% in the control group, indicating a lower trend in the Viscum album group. Clinical response rate in patients with malignant pleural effusion was reported in two studies, with success rates or partial success rates of 61.5%–76.7% in the Viscum album group and 30.0%–80.0% in the control group, indicating a higher trend in the Viscum album group. Quality of life was reported in 11 studies. Of two studies reporting serial measurements, one (Lenartz et al., 1996) found substantial improvement at 24 weeks postoperatively in the Viscum album group, while the other (Heiny et al., 1997) reported significant improvement from week 12 onward in the Viscum album group compared with that in the control group. Among six studies reporting changes from baseline, four were included in a meta-analysis, which found no significant difference between groups (SMD = 0.40, 95% CI –0.25–1.05, I² = 83%); however, high heterogeneity warranted caution in interpretation. Two studies did not provide SD data, and each reported no significant difference between groups (Pelzer et al., 2018; Woode et al., 2024). Of three studies reporting mean differences between groups, one (Steuer-Vogt et al., 2006) found no significant difference, while two (Kim et al., 2012; Troger et al., 2014) reported significant improvement in the Viscum album group. Conclusion and Strength of Recommendation The Subcommittee noted that Viscum album is approved in Korea for treatment of tumors and prevention of cancer recurrence after tumor surgery and is classified as an antineoplastic agent. This assessment therefore focused on its safety and effectiveness as a cancer treatment or adjuvant therapy. The systematic review found that rates of severe AEs, overall AEs, and chemotherapy-related AEs were similar between groups. Injection-site reactions in the Viscum album group were mild, transient, and not clinically significant, and therefore, the treatment was considered safe. Regarding effectiveness, although Viscum album has been studied as an adjunctive cancer therapy in various malignancies (e.g., breast, lung, and pancreatic cancers), there was insufficient evidence to conclude that it provides additional benefit in terms of survival, recurrence, disease progression, or quality of life. This reassessment was based on RCTs; however, the small sample sizes of individual studies, along with inconsistencies in cancer type, stage, and disease status, limited the ability to demonstrate clinical benefit in patients with advanced or metastatic cancer. Furthermore, many studies were conducted before 2010, limiting their applicability to current cancer treatment practices, and there was consensus that more recent evidence is lacking. Additional concerns included that a substantial proportion of studies on Viscum album were conducted in a single country (Germany) and that randomization methods and allocation concealment were often not clearly reported, both of which increased the risk of bias. Overall, the level of evidence was rated as “low” to “very low.” At the 12th 2024 Health Technology Reassessment Committee meeting (December 13, 2024), the Committee reviewed the Subcommittee’s findings in accordance with Article 4(10) of the Health Technology Reassessment Project Management Guidelines. The Committee concluded that, while the safety of adding Viscum album to standard cancer treatment is comparable to that of the control group, the level of evidence for effectiveness is low due to heterogeneity in patient characteristics by cancer type, inconsistency in clinical outcomes, concerns regarding risk of bias, and lack of recent studies. Accordingly, the Health Technology Reassessment Committee determined that the addition of Viscum album to standard cancer treatment for treatment of tumors treatment or prevention of cancer recurrence is “not recommended” in the current Korean clinical setting.
