초록

평가배경 초음파를 활용한 뇌혈류 기능검사는 두개경유도플러(transcranial doppler, TCD)를 이용한 특수검사로 미세기포를 이용한 좌우단락검사, 경두개 혈관운동반응성검사, 미세색전신호(microembolic signals, MES) 모니터링, 기립경사도 경두개 초음파가 포함되며, 뇌졸중, 일과성 뇌허혈증 등 환자에게 시행되는 기술이다. 신의료기술평가 없이 비급여로 등재되어 사용되고 있다. 동 기술은 국민건강보험공단의 비급여 보고제도 대상 항목이며 수요조사(유관기관 평가 요청)를 통해 재평가 대상으로 발굴하였고, 2025년 제5차 의료기술재평가위원회(2025.5.16.)에서 재평가 대상으로 심의를 받고 재평가를 수행하였다. 평가목적 본 평가는 뇌혈류 및 뇌혈관 기능 상태 평가가 필요한 환자를 대상으로 "초음파를 활용한 뇌혈류 기능검사”의 사용대상과 기능검사에 대한 근거를 검토하여, 임상현장에서의 적절한 적용 및 활용을 지원하기 위한 정보를 제공하는 것을 목적으로 한다. 평가방법 초음파를 활용한 뇌혈류 기능검사의 평가방법은 평가목적을 고려하여 “초음파를 활용한 뇌혈류 기능검사 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 신경과 3인, 신경외과 1인, 총 4인으로 구성하였다. 본 검토에서는 중재기술에 포함하고 있는 세부 검사를 4개(미세기포를 이용한 좌우단락검사, 혈관운동반응성검사, MES 모니터링, 기립경사도 경두개 초음파) 기술로 정의하고 각 세부 검사별로 임상적 적용 대상 및 활용을 확인하기 위해, 교과서 및 임상진료지침을 종합적으로 검토하였다. 평가결과 본 평가에서는 『신경초음파(2021)』, 『신경학』 및 『뇌졸중』 최신판(2024) 교과서와, 국내외 DB 간략검색을 통해 확인된 총 10편의 관련 임상진료지침 및 전문가 합의문을 검토하였으며, 각 세부 검사별 검토된 내용은 다음과 같다. 미세기포를 이용한 좌우단락검사 『신경초음파(2021)』, 『신경학(2024)』, 『뇌졸중(2024)』 교과서에서는 열린타원구멍(patent foramen ovale, PFO) 등 우좌션트 진단 시 TCD의 유용성을 언급하고 있었다. 미세기포를 이용한 좌우단락검사는 식도경유심초음파검사(transesophageal echocardiography, TEE)에 비해 비침습적이며 시행이 용이한 검사로, TEE 검사가 어려운 삼킴곤란 환자에게도 적용할 수 있다고 기술하고 있다. 임상진료지침에서도 미세기포를 이용한 좌우단락검사가 TEE, 흉벽경유심초음파검사(transthoracic echocardiography, TTE)와 유사한 민감도·특이도(약 92-97%)를 갖는 비침습적 선별검사로 권장한다고 제시하였다. 다만, 해부학적 정보 제공의 한계로 심장초음파를 대체할 수는 없으며 상호보완적인 검사로 활용하는 것이 적절하다고 교과서 및 지침 모두에서 일관되게 제시하고 있다. 경두개 혈관운동반응성 검사 『신경초음파(2021)』, 『뇌졸중(2024)』 교과서에서 경두개 혈관운동반응성 검사는 이산화탄소 반응, 아세타졸아미드 투약 등 다양한 자극을 통해 뇌혈관 예비능(reserve capacity)을 평가하는 도구로, 뇌혈류 자동조절 능력 평가 및 혈류역학적 협착의 진단에 활용된다고 제시되어 있다. 임상진료지침에서도 혈관운동반응성 검사는 경동맥 협착 환자의 허혈성 뇌졸중 발생 및 재발위험도 예측, 뇌혈관 협착/폐색 환자에서 혈역학적 평가, 치료결정(경동맥내막절제술, 스텐트 시술 등)에 참고할 수 있는 유용한 평가지표로 권장하고 있다. 미세색전신호(MES) 모니터링 『신경초음파(2021)』,『뇌졸중(2024)』 교과서에서는 MES가 색전 감시, 뇌졸중 위험도 평가, 치료효과 판정의 지표로 활용될 수 있는 검사이며, 특히 유증상 경동맥 협착에서 MES 검사의 임상적 유용성이 있다고 제시하고 있다. 다만, 지속적 관찰이 필요하고 검사 실행에 시간이 소요되는 한계가 존재한다고 제시되었다. 임상진료지침에서는 TCD를 통해 순환 중 발생하는 색전의 존재와 빈도를 탐지하여 뇌졸중 위험도와 치료효과를 평가하는 검사로, 경동맥 협착/폐색 환자에서 뇌졸중 위험도 평가, 항혈전제 치료반응 평가, 경동맥내막절제술/스텐트 삽입술, 심장수술 등 시술 전후 색전 위험 감시에 활용 가능한 검사로 권장되고 있다. 기립경사도 경두개 초음파 『신경초음파(2021)』, 『신경학(2024)』 교과서에서 실신 환자의 병태생리 평가를 위해 기립경사검사 시 TCD 활용이 제시되었고, 기립 시 혈압·맥박·중대뇌동맥 혈류 속도의 변화를 관찰하여 신경매개실신, 기립 저혈압, 체위기립빈맥증후군(postural orthostatic tachycardia synd rome, POTS), 대뇌실신을 감별하는 데 도움을 준다고 제시되었다. 관련 가이드라인 및 국제 합의문에서는 TCD에 대해 기립성 조절장애 및 일과성 의식소실 등 실신 평가 시 기립경사검사와 함께 측정하는 보조적 평가도구 중 하나로 권고하고 있으며 특히, 심인성 가성실신(pseudopsychogenic syncope) 감별진단하는 데 유용하다고 제시하였다. 결론 초음파를 활용한 뇌혈류 기능검사 소위원회는 교과서 및 임상진료지침을 종합적으로 검토한 결과 다음과 같은 의견을 제시하였다. 초음파를 활용한 뇌혈류 기능검사는 TCD 기술을 기반으로 하는 기능검사로, 미세기포를 이용한 좌우단락검사, 경두개 혈관운동반응성검사, 미세색전신호(MES) 모니터링, 기립경사도 경두개 초음파의 세부 검사 방법을 포함하고 있다. 각 세부 검사에 대한 검토 결과는 다음과 같다. 첫째, 미세기포를 이용한 좌우단락검사는 열린타원구멍(PFO) 등 우좌션트를 확인하기 위한 비침습적이고 시행이 용이한 선별검사로서, 식도경유심초음파검사와 유사한 민감도와 특이도를 보이는 것으로 확인되었으며 심장초음파 검사의 한계를 보완하는 상호보완적인 검사로 활용 가치가 있다. 또한, 국내외 교과서 및 진료지침에서 ‘Right-to-Left shunt’ 용어가 표준적으로 사용됨에 따라, 국내 비급여 행위설명에 제시된 국문 용어를 ‘우좌션트검사’로 변경할 필요가 있다고 제언하였다. 둘째, 경두개 혈관운동반응성 검사는 경동맥 협착 및 뇌혈관 폐색 환자에서 혈관 예비능을 평가하여 뇌졸중 발생 및 재발 위험도를 예측하고, 경동맥내막절제술이나 스텐트 삽입술 등 치료방침 결정을 보조하는 지표로 유용하다. 셋째, 미세색전신호(MES) 모니터링은 순환 중 발생하는 색전의 존재와 빈도를 실시간으로 확인할 수 있는 검사로, 경동맥 협착/폐색 환자, 심장성 색전증 위험 환자, 뇌졸중, 수술 전후 환자에서 색전 위험도 평가와 치료효과 감시에 유용한 보조적 검사로 확인되었다. 넷째, 기립경사도 경두개 초음파는 기립 시 혈압, 맥박, 중대뇌동맥 혈류 속도의 변화를 관찰하여 기립경사 검사와 병행하여 평가하는 보조검사로 실신 원인을 파악하는 데 유용하며, 모든 실신 환자에게 필요한 일반적 검사는 아니나 일부 환자군(신경매개실신, 대뇌실신 등)에서 감별진단에 도움을 줄 수 있는 것으로 확인되었다. 또한, TCD 기반의 평가는 검사자 숙련도, 검사 수행 방법, 자극 기법, 해석 기준 등에 따라 결과에 변동성이 발생할 수 있어 검사 수행 및 판독 기준의 프로토콜을 잘 준수하고, 검사의 질 관리가 병행되는 것이 바람직하다고 제언하였다. 더불어, 본 평가는 특정 교과서와 임상진료지침·전문가 합의문을 중심으로 한 근거 검토에 기반한 것으로, 각 세부기술에 대한 심층 문헌고찰을 수행한 평가는 아니라는 점에서 실제 임상에서 적용 가능한 모든 대상질환과 활용범위를 완전히 포괄하지 못한 한계가 있다. 2026년 제1차 재평가전문위원회*(2026. 1. 9.)는 소위원회 검토 결과를 바탕으로 ‘초음파를 활용한 뇌혈류 기능검사’에 대해 논의하였으며, 소위원회 결론을 원안대로 심의·의결하였다.


Background Cerebral blood flow function tests using ultrasound are specialized examinations based on transcranial Doppler (TCD) that include the right-to-left shunt test using microbubbles, the transcranial vasomotor reactivity test, microembolic signal (MES) monitoring, and head-up tilt transcranial Doppler; these tests are performed in patients with stroke, transient ischemic attack, and other related conditions. This technology has been listed and used as a non-reimbursed item without having undergone a New Health Technology Assessment. As an item subject to the National Health Insurance Service’s non-reimbursed service reporting system, it was identified as a candidate for reassessment through a needs survey (an assessment request from relevant organizations). The reassessment was conducted after being reviewed as a reassessment subject at the 5th 2025 Health Technology Reassessment Committee meeting (May 16, 2025). Objective The objective of this assessment was to review the evidence on the target population and the clinical utility of “cerebral blood flow function tests using ultrasound” in patients requiring evaluation of cerebral blood flow and cerebrovascular function, and to provide information to support their appropriate application in clinical practice. Methods The assessment methods for cerebral blood flow function tests using ultrasound were finalized, in consideration of the assessment objective, through discussions with the “Subcommittee on Cerebral Blood Flow Function Tests Using Ultrasound” (hereinafter, “the Subcommittee”). The Subcommittee, as determined by the Health Technology Reassessment Committee, comprised four experts: three neurologists and one neurosurgeon. For this review, the index technology was defined as comprising four specific tests—the right-to-left shunt test using microbubbles, the vasomotor reactivity test, MES monitoring, and head-up tilt transcranial Doppler—and textbooks and clinical practice guidelines were comprehensively reviewed to confirm the target population and clinical utility of each test. Results This assessment reviewed the textbooks Neurosonology (2021), Neurology, and the most recent edition of Stroke (2024), along with 10 related clinical practice guidelines and expert consensus statements identified through a brief search of domestic and international databases. The findings for each specific test are presented below. Right-to-Left Shunt Test Using Microbubbles The textbooks Neurosonology (2021), Neurology (2024), and Stroke (2024) all note the usefulness of TCD in diagnosing right-to-left shunts, such as patent foramen ovale (PFO). The right-to-left shunt test using microbubbles is described as a non-invasive test that is easier to perform than transesophageal echocardiography (TEE) and can be performed even in patients with dysphagia, in whom TEE is difficult. Clinical practice guidelines similarly recommend the right-to-left shunt test using microbubbles as a non-invasive screening test with sensitivity and specificity (approximately 92–97%) comparable to those of TEE and transthoracic echocardiography (TTE). However, both the textbooks and the guidelines consistently note that, because of its limitations in providing anatomical information, this test cannot replace echocardiography and is more appropriately used as a complement. Transcranial Vasomotor Reactivity Test The textbooks Neurosonology (2021) and Stroke (2024) describe the transcranial vasomotor reactivity test as a tool for evaluating cerebrovascular reserve capacity using various stimuli, such as carbon dioxide reactivity and acetazolamide administration, and note that it is used to assess cerebral autoregulation and diagnose hemodynamically significant stenosis. Clinical practice guidelines also recommend the vasomotor reactivity test as a useful assessment index to predict the occurrence and recurrence risk of ischemic stroke in patients with carotid stenosis, to perform hemodynamic assessment in patients with cerebrovascular stenosis or occlusion, and to inform treatment decisions such as carotid endarterectomy or stenting. Microembolic Signal (MES) Monitoring The textbooks Neurosonology (2021) and Stroke (2024) describe MES as an index that can be used for embolus monitoring, stroke risk assessment, and evaluation of treatment effect, and note that MES monitoring has particular clinical utility in symptomatic carotid stenosis. However, it has the limitation of requiring continuous observation and being time-consuming to perform. Clinical practice guidelines recommend TCD-based detection of the presence and frequency of emboli occurring during circulation as a means of assessing stroke risk and treatment effect, and note that it can be used to assess stroke risk and antithrombotic treatment response in patients with carotid stenosis or occlusion, and to monitor embolic risk before and after procedures such as carotid endarterectomy, stenting, and cardiac surgery. Head-Up Tilt Transcranial Doppler The textbooks Neurosonology (2021) and Neurology (2024) describe the use of TCD during head-up tilt testing to assess the pathophysiology of syncope, noting that observing changes in blood pressure, pulse, and middle cerebral artery blood flow velocity during standing can help differentiate neurally mediated syncope, orthostatic hypotension, postural orthostatic tachycardia syndrome (POTS), and cerebral syncope. Relevant guidelines and international consensus statements recommend TCD as a supplementary assessment tool to be measured alongside head-up tilt testing when evaluating syncope, including orthostatic dysregulation and transient loss of consciousness, and note that it is particularly useful for differentiating psychogenic pseudosyncope. Conclusion Based on a comprehensive review of textbooks and clinical practice guidelines, the Subcommittee on Cerebral Blood Flow Function Tests Using Ultrasound presented the following opinions. Cerebral blood flow function tests using ultrasound are TCD-based functional tests that include the right-to-left shunt test with microbubbles, the transcranial vasomotor reactivity test, MES monitoring, and head-up tilt transcranial Doppler. The findings for each specific test are summarized below. First, the right-to-left shunt test using microbubbles is a non-invasive, easy-to-perform screening test for confirming right-to-left shunts, such as PFO; it has been shown to have sensitivity and specificity comparable to those of transesophageal echocardiography and serves as a complementary test that addresses the limitations of echocardiography. In addition, given that the term “right-to-left shunt” is used as the standard term in domestic and international textbooks and clinical practice guidelines, the Subcommittee recommended that the Korean term used in the domestic description of this non-reimbursed procedure be changed to “right-to-left shunt test.” Second, the transcranial vasomotor reactivity test is useful as an index for predicting the risk of stroke occurrence and recurrence by assessing vascular reserve capacity in patients with carotid stenosis or cerebrovascular occlusion, and for assisting treatment decisions such as carotid endarterectomy or stent placement. Third, MES monitoring is a test that can identify the presence and frequency of emboli occurring during circulation in real time, and it was confirmed to be a useful adjunctive test for assessing embolic risk and monitoring treatment effect in patients with carotid stenosis or occlusion, patients at risk of cardiogenic embolism, stroke patients, and perioperative patients. Fourth, head-up tilt transcranial Doppler is a supplementary test, performed alongside head-up tilt testing, that observes changes in blood pressure, pulse, and middle cerebral artery blood flow velocity during standing to help identify the cause of syncope. However, it is not a routine test for all patients with syncope; it is helpful for differential diagnosis in certain patient groups, such as those with neurally mediated or cerebral syncope. The Subcommittee further recommended that, because TCD-based assessment results may vary depending on examiner skill, test performance methods, stimulation techniques, and interpretation criteria, the protocols for test performance and interpretation should be strictly followed, along with parallel quality control of the test. In addition, because this assessment was based on a review of evidence centered on specific textbooks, clinical practice guidelines, and expert consensus statements rather than an in-depth systematic literature review of each specific technique, it has the limitation of not fully covering all target diseases and the scope of clinical application that may be relevant in actual clinical practice. At the 1st 2026 Health Technology Reassessment Expert Committee meeting* (January 9, 2026), the Committee discussed “cerebral blood flow function tests using ultrasound” based on the Subcommittee’s review and resolved to adopt the Subcommittee’s conclusions as originally presented.
