초록

평가배경 이화방어기제검사(Ewha Defense Mechanism Test, EDMT)는 정신역동이론에 기반하여 자아방어기제를 객관적으로 측정하는 자가보고식 지필검사로, 개인의 주요 방어기제와 적응행동 특성을 평가하기 위해 사용된다. 동 검사는 신의료기술평가 제도 도입 이전인 2002년에 100분의 100 본인부담으로 등재되었고, 2005년 비급여로 전환되었다. 해당 기술은 2025년 의료기술재평가 수요조사에서 유관기관으로부터 제안된 이후 관련 대상 선정 절차를 거쳐, 2025년 제3차 의료기술재평가위원회(2025.3.14.)에서 재평가 대상으로 선정되었다. 이에 따라 현시점에서 동 기술의 효과성을 확인하기 위하여, 2025년 제5차 의료기술재평가위원회(2025.5.16.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 거쳐 재평가를 수행하였다. 평가목적 본 평가의 목적은 이화방어기제검사의 임상적 효과성에 대한 정보를 제공하기 위함이다. 평가방법 본 평가에서는 ‘정신의학적 장애(의심) 환자 대상에서 이화방어기제검사는 정신병리 또는 성격장애 진단 보조, 치료계획 수립 및 모니터링에 임상적으로 효과적인가?’를 핵심질문으로 설정하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “이화방어기제검사 재평가 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였으며, 소위원회는 정신건강의학과 3인, 임상심리전문가 2인, 근거기반의학 1인, 총 6인으로 구성하였다. 본 평가는 정신의학적 장애(의심) 환자를 대상으로 수행하였으며, 비교검사는 건강보험요양급여비용 목록에 등재된 자가보고식 검사, 참고표준검사는 정신질환 진단 및 통계편람 제4판 및 제5판에 근거한 종합적 임상판단으로 설정하였다. 결과변수는 효과성에 대해 진단분류 예측률, 비교검사와의 상관성, 질환과의 관련성 및 의료결과에 대한 영향을 중심으로 평가하였다. 한편, 이화방어기제검사는 환자가 자가보고식 검사지를 이용하여 응답하는 검사로서 환자에게 직접적인 위해를 가하지 않는다고 판단하여, 중재검사 수행에 따른 안전성은 별도로 평가하지 않았다. 연구문헌은 핵심질문을 토대로 국외 4개(Ovid MEDLINE, Embase, Cochrane Central Register of Controlled Trials, APA PsycInfo), 국내 2개(KoreaMed, 한국교육학술정보원(RISS)) 데이터베이스에서 검색하였다(최종검색일 2025.6.27.). 연구선정 과정은 2명의 평가자가 독립적으로 수행하였고, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 동 검사의 특성 및 평가에 포함된 연구설계에 적합한 비뚤림위험 평가 도구가 확인되지 않아 비뚤림위험 평가는 수행하지 않았으며, 사전에 정한 자료추출 서식을 활용해 자료를 추출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종 심의하였다. 평가결과 체계적 문헌고찰 결과, 최종 선택연구는 총 6편이었다. 출판연도별로는 2010년대와 1990년대 연구가 각각 3편씩 포함되었다. 연구대상자는 일반 집단과 임상 집단을 모두 포함하고 있었으며, 대학생, 탈북 여성, 공황장애 환자, 학교폭력 피해자, 정신분열증 환자 등으로 다양하였다. 연구별 대상자 수는 22명에서 578명까지 분포하였고, 연구환경은 학교, 의료기관, 학교·의료기관, 북한이탈주민 정착지원시설 등으로 다양하였다. 선택연구에서는 주로 이화방어기제검사와 기존 심리검사 간 상관성을 평가하였다. 이화방어기제검사의 효과성은 진단분류 예측률, 비교검사와의 상관성, 질환과의 관련성 및 의료결과에 대한 영향을 중심으로 평가하였다. 다만, 진단분류 예측률, 질환과의 관련성 및 의료결과에 대한 영향을 보고한 연구는 확인되지 않았다. 비교검사와의 상관성은 총 6편에서 보고되었다. 1편에서는 이화방어기제검사의 성숙단계에 따른 척도와 지역사회 역학조사용 우울 척도 간 유의한 상관관계가 확인되지 않았다. 2편에서는 이화방어기제검사와 미네소타 다면적인성검사-II의 성격병리 5요인 척도 또는 간이정신진단검사의 정신증상 척도 간 전반적으로 유의한 상관관계가 보고되었으며, 특히 미성숙 단계 방어기제에서 관련성이 더 뚜렷하였다. 반면, 성숙 단계 방어기제와의 관련성은 제한적이었다. 다른 2편에서는 이화방어기제검사의 대부분 방어기제가 각각 벡 우울 척도 또는 상태·특성 분노 척도와 유의한 상관을 보이지 않았다. 나머지 1편에서는 이화방어기제검사의 모든 방어기제가 간편 정신상태 평정 척도와 유의한 관련성을 보였다. 결론 및 제언 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같이 결과 및 의견을 제시하였다. 체계적 문헌고찰 결과, 이화방어기제검사는 비교기술로서 포함된 검사 중 일부 검사와만 유의한 상관관계를 보였으며, 진단분류 예측률, 질환과의 관련성, 의료결과에 대한 영향을 보고한 연구는 확인되지 않았다. 이에 정신병리 또는 성격장애 진단 보조, 치료계획 수립 및 모니터링에 있어 동 검사의 임상적 효과성 여부를 판단하기에는 현재 문헌적 근거가 제한적이라는 의견이었다. 소위원회는 국내 임상 상황에서 환자의 방어기제를 측정할 수 있는 표준화된 검사가 이화방어기제검사 외에는 부재하다는 의견을 제시하였다. 한편, 본 평가는 현행 건강보험 급여 및 비급여에 해당하는 검사만을 비교검사로 포함하였으나, 환자의 심리적 원인에 의한 증상 및 행동을 객관적으로 평가하는 도구 중 건강보험 체계 내에 포함된 검사가 제한적이고, 이들 역시 수십 년 전 기준에 따라 포함된 검사로서 변화한 심리 이론과 평가 방법을 충분히 반영하지 못하였다는 점에서 평가의 제한이 있다고 보았다. 아울러 국내의 다양한 환자 집단을 대상으로 새롭게 개발되는 도구의 표준화 및 임상적 효과성 검증을 위한 지속적인 연구가 필요하다고 제언하였다. 2025년 제4차 재평가전문위원회*(2025.12.8.)는 ‘증상 및 행동 평가척도(기타) - 이화방어기제검사’에 대해 다음과 같이 심의하였다. 재평가전문위원회는 소위원회의 검토 결과에 동의하였으며, 추가로 해당 검사가 제도적 관리 범위 밖에서 임의로 활용되는 일이 없도록 향후 제도적 보완이 필요하다는 의견을 제시하였다.


Background The Ewha Defense Mechanism Test (EDMT) is a self-report, paper-and-pencil test based on psychodynamic theory that objectively measures ego defense mechanisms. It is used to assess an individual’s primary defense mechanisms and adaptive behavioral characteristics. This test was listed in 2002, before the introduction of the New Health Technology Assessment system, as an item fully paid out-of-pocket by patients, and was subsequently converted to a non-covered item in 2005. This technology was proposed by a relevant organization during the 2025 demand survey for health technology reassessment. Following the relevant target-selection process, it was selected as a reassessment topic at the 3rd Health Technology Reassessment Committee meeting in 2025, held on March 14, 2025. Accordingly, to examine the effectiveness of this technology at the present time, a reassessment was conducted after deliberation on the reassessment protocol and the proposed composition of the subcommittee at the 5th Health Technology Reassessment Committee meeting in 2025, held on May 16, 2025. Objective The objective of this assessment was to provide information on the clinical effectiveness of the Ewha Defense Mechanism Test. Methods This assessment conducted a systematic review based on the key question: “Is the Ewha Defense Mechanism Test clinically effective for assisting in the diagnosis of psychopathology or personality disorders, establishing treatment plans, and monitoring patients with suspected or confirmed psychiatric disorders?” All assessment methods were finalized through discussion by the Reassessment Subcommittee for the Ewha Defense Mechanism Test, hereafter referred to as the “Subcommittee,” taking into account the objective of the assessment. The Subcommittee consisted of six members: three psychiatrists, two clinical psychology specialists, and one evidence-based medicine expert. The target population of this assessment was patients with suspected or confirmed psychiatric disorders. The comparator tests were self-report tests listed in the National Health Insurance reimbursement schedule, and the reference standard was comprehensive clinical judgment based on the Diagnostic and Statistical Manual of Mental Disorders, Fourth and Fifth Editions. Effectiveness outcomes were evaluated with a focus on diagnostic classification prediction accuracy, correlation with comparator tests, association with disease, and impact on health outcomes. Meanwhile, because the Ewha Defense Mechanism Test is a self-report questionnaire completed by patients and was considered not to pose direct harm to patients, the safety of performing the index test was not separately assessed. Literature searches were conducted based on the key question in four international databases—Ovid MEDLINE, Embase, the Cochrane Central Register of Controlled Trials, and APA PsycInfo—and two domestic databases—KoreaMed and the Research Information Sharing Service (RISS). The final search date was June 27, 2025. Study selection was independently performed by two reviewers. In cases of disagreement, a consensus result was reached through discussion between the reviewers. Because no risk-of-bias assessment tool appropriate for the characteristics of this test and the study designs included in the assessment was identified, risk of bias was not assessed. Data were extracted using a predefined data extraction form. The final deliberation of this assessment was conducted by the Reassessment Expert Committee based on the Subcommittee’s review findings. Results As a result of the systematic review, a total of six studies were finally selected. By publication year, three studies were published in the 2010s and three in the 1990s. The study populations included both general and clinical populations and varied across university students, North Korean defector women, patients with panic disorder, victims of school violence, and patients with schizophrenia. The number of participants in each study ranged from 22 to 578. Study settings also varied and included schools, medical institutions, combined school and medical institution settings, and settlement support facilities for North Korean defectors. The selected studies mainly evaluated correlations between the Ewha Defense Mechanism Test and existing psychological tests. The effectiveness of the Ewha Defense Mechanism Test was evaluated with a focus on diagnostic classification prediction accuracy, correlation with comparator tests, association with disease, and impact on health outcomes. However, no studies reporting diagnostic classification prediction accuracy, association with disease, or impact on health outcomes were identified. Correlation with comparator tests was reported in six studies. In one study, no significant correlation was found between the scales according to maturity levels of defense mechanisms in the Ewha Defense Mechanism Test and the Center for Epidemiological Studies-Depression Scale (CES-D). In two studies, generally significant correlations were reported between the Ewha Defense Mechanism Test and either the Personality Psychopathology Five scales of the Minnesota Multiphasic Personality Inventory-II (MMPI-II PSY-5) or the psychiatric symptom scales of the Symptom Checklist (SCL-90R). In particular, the associations were more pronounced for immature-stage defense mechanisms. By contrast, associations with mature-stage defense mechanisms were limited. In two other studies, most defense mechanisms measured by the Ewha Defense Mechanism Test did not show significant correlations with either the Beck Depression Inventory (BDI) or the State-Trait Anger Scale (STAS). In the remaining study, all defense mechanisms measured by the Ewha Defense Mechanism Test showed significant associations with the Brief Psychiatric Rating Scale (BPRS). Conclusions and Recommendations Based on the current assessment results, the Health Technology Reassessment Subcommittee presented the following conclusions and opinions. The systematic review found that the Ewha Defense Mechanism Test showed significant correlations with only some of the tests included as comparator technologies. No studies reporting diagnostic classification prediction accuracy, association with disease, or impact on health outcomes were identified. Accordingly, the Subcommittee considered that the current body of evidence from the literature is limited for determining whether this test is clinically effective in assisting the diagnosis of psychopathology or personality disorders, establishing treatment plans, and monitoring patients. The Subcommittee noted that, in the domestic clinical context, there is no standardized test other than the Ewha Defense Mechanism Test that can measure patients’ defense mechanisms. Meanwhile, although this assessment included only tests currently covered or non-covered under the National Health Insurance system as comparator tests, the Subcommittee recognized a limitation of the assessment in that few instruments objectively evaluating symptoms and behaviors arising from patients’ psychological causes are included within the National Health Insurance system. Furthermore, those instruments were incorporated based on criteria established several decades ago and may not sufficiently reflect changes in psychological theory and assessment methods. The Subcommittee also recommended continued research to standardize newly developed instruments for diverse patient populations in Korea and to verify their clinical effectiveness. The 4th Reassessment Expert Committee meeting in 2025, held on December 8, 2025, deliberated on “Symptomatic and Behavioral Evaluation Scale (Others): Ewha Defense Mechanism Test” as follows. The Reassessment Expert Committee agreed with the Subcommittee’s review findings and additionally stated that future institutional improvements are needed to prevent the test from being used arbitrarily outside the scope of institutional management.
