Enhanced External Counterpulsation (EECP)

초록

평가배경 증진된 외부 역박동술(Enhanced External Counterpulsation, EECP)은 기존 약물치료나 중재적 시술로 시행할 수 없는 불인성(intractable) 만성 안정형 협심증 환자의 혈류를 개선하고 심근 부담을 감소시켜 심장질환 증상을 호전시키는 목적으로 시행하는 기술로, 환자의 종아리·대퇴부·둔부 등에 외부압박용 커프를 착용한 뒤 심전도로 심장의 박동파형을 파악한 후 압박커프에 규칙적으로 수축과 팽창을 반복한다. 2012년 신의료기술평가를 거쳐 2014년 비급여로 등재된 후(보건복지부 고시 제2014 -44호, 2014. 3. 25.), 2020년에 건강보험 보장성 강화 대책에 따라 선별급여(본인부담률 50%)로 등재(보건복지부 고시 제2020-96호, 2020. 5. 21.)되었다. 해당 기술은 선별급여 적합성 평가주기가 도래하여 내부모니터링을 통해 재평가 주제로 선정되었고, 2024년 제7차 의료기술재평가위원회(2024.7.12.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다. 평가목적 본 평가의 목적은 불인성 만성 안정형 협심증 환자에서 혈류개선 및 심근 부담 감소를 통한 심장질환의 호전을 위해 증진된 외부 역박동술을 사용하는 것이 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 불인성 만성 안정형 협심증 환자에서 증진된 외부 역박동술(EECP)에 대한 안전성 및 효과성을 평가하고자 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “증진된 외부 역박동술 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 순환기내과 2인, 심장혈관흉부외과 1인, 영상의학과 1인, 근거기반의학 1인, 총 5인으로 구성하였다. 본 평가의 대상자는 불인성 만성 안정형 협심증 환자였고, 비교시술은 거짓치료, 심장재활, Control(약물+EECP vs 약물의 경우, 약물군)로 정의하였다. 안전성은 시술 관련 부작용 및 합병증을 결과지표로 보았고, 효과성은 일차지표로 협심증 중증도 변화, 이차지표로 약물사용량 감소, 운동기능평가 향상, 삶의 질, 재원기간, 심근관류지표, 병원이용률 및 입원일수를 검토하였다. 연구유형은 비교연구 이상의 연구만 평가에 포함하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS)) 데이터베이스에서 검색하였다(최종검색일 2024. 9. 12.). 최종 선택연구의 비뚤림위험 평가는 무작위배정비교임상연구(Randomized Controlled Trial, RCT)인 경우 Cochrane의 Risk of Bias (RoB)를, 비무작위 연구(Non-randomized study, NRS)는 Risk of Bias Assessment for Non- randomized Studies (RoBANS ver 2.0)를 사용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하고, 평가결과를 토대로 권고등급을 결정하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 체계적 문헌고찰 결과 최종 선택된 연구는 총 9편(문헌 12편, 대상자 수 456명)으로 RCT 5편, NRS 4편이었다. 연구대상자는 최대 약물치료와 경피적 관상동맥 중재술 및 관상동맥 우회로술과 같은 중재적 시술을 시행할 수 없는 불인성 만성 안정형 협심증 환자였다. 비뚤림위험 평가결과, RCT의 경우 배정순서 은폐, 연구참여자 및 연구자에 대한 눈가림 영역에서 대부분 ‘불확실’로 평가하였고, NRS의 경우 교란변수, 노출측정에서 대부분 ‘불확실’로 평가하였으며 그 외 영역에서는 대부분 ‘낮음’으로 평가하였다. 안전성 안전성은 총 6편(RCT 4편, NRS 2편)의 연구를 토대로 거짓치료 및 약물 단독치료와 비교하여 시술 관련 부작용 및 합병증을 검토하였다. 약물 단독치료와 비교한 연구 2편 및 거짓치료와 비교한 연구 3편에서 부작용과 합병증이 발생하지 않았다고 보고하였다. 그러나 RCT 1편에서는 거짓치료와 비교 시 부작용이 EECP군에서 유의하게 많았다. 특히 장비관련 부작용으로 다리통증, 피부찰과상, 부종, 감각이상 등을 보고하였고, 비장비관련 부작용으로 어지럼증, 불안함, 이명 등을 보고하였다. 효과성 효과성은 총 12편(RCT 8편, NRS 4편)의 연구를 토대로 일차지표인 협심증 중증도 변화, 이차지표인 약물사용량, 운동기능평가, 삶의 질, 재원기간, 심근관류지표, 병원이용률 및 입원일수를 검토하였다. 협심증 중증도 변화는 Canadian Cardiovascular Society (CCS) 등급과 협심증 에피소드로 각각 확인하였다. CCS 등급은 낮을수록 협심증 증상이 개선된 것으로 해석하며, CCS 등급을 보고한 연구는 총 7편(RCT 4편(문헌 6편), NRS 3편)이었다. 거짓치료군과 비교 시 대부분의 연구에서 EECP군의 CCS 등급이 개선되었으나 정량적 합성이 가능한 RCT 연구 3편을 메타분석한 결과 군 간 유의한 차이는 없었고 이질성이 매우 높아 해석에 주의가 필요하였다(Mean Difference (MD) -1.21 (95% Confidence Interval (CI) -4.17, 1.74, I2=99%)). 심장재활군과 비교한 1편에서는 CCS 등급별 협심증 개선 비율을 확인하였고, CCS Ⅱ로 개선된 비율은 EECP군이 심장재활군보다 유의하게 높았으나 CCSⅠ으로 개선된 비율은 EECP군이 심장재활군보다 유의하게 낮았다. NRS 3편에서는 단독 약물치료군 대비 약물치료 및 EECP 병행군에서 CCS 등급이 유의하게 낮아 협심증 증상의 개선을 확인하였다. 협심증 에피소드를 본 연구는 모두 거짓치료군과 비교한 RCT 연구 3편으로, 대체로 EECP군에서 협심증 에피소드가 줄어든 경향을 보였으나, 메타분석한 결과 군 간 유의한 차이는 없었고 이질성이 매우 높아 해석에 주의가 필요하였다(MD –0.78 (95% CI –2.26, 0.71, I2=98%)). 이차지표 중 약물사용량을 보고한 연구는 총 4편(RCT 3편(문헌 5편), NRS 1편)이었다. 거짓치료군과 비교 시 EECP군에서 약물사용량이 감소한 경향을 보였으나, 메타분석 결과 군 간 유의한 차이는 없었고 이질성이 매우 높아 해석에 주의가 필요하였다(MD –0.68 (95% CI –1.49, 0.12, I2=91%)). 단독 약물치료군과 비교한 NRS 연구 1편에서는 시술 후 모든 시점에서 EECP군의 약물사용량이 유의하게 감소하였다. 운동기능평가를 보고한 연구는 모두 거짓치료군과 비교한 RCT 3편(문헌 4편)이었다. 운동 지속 시간, 최대 협심증 등급, 최대 산소흡수 등 다양한 지표를 검토한 결과, 시술 후 최대 심박수를 제외한 모든 지표에서 EECP군의 유의한 개선을 확인하였다. 삶의 질을 보고한 연구는 RCT 3편이었다. 거짓치료군과 비교한 RCT 연구 2편 중 1편에서는 36-Item Short Form Survey (SF-36) 중 일부 영역(사회적 기능, 감정적 역할 제한, 활력, 통증)에서만 EECP군이 유의하게 좋았으나, 다른 1편에서는 군 간 유의한 차이가 없었다. 심장재활군과 비교한 1편에서도 군 간 유의한 차이는 없었다. 또 다른 지표인 심근관류지표와 병원이용률 및 입원일수를 보고한 연구는 확인할 수 없었다. 결론 및 권고결정 의료기술재평가 소위원회에서는 불인성 만성 안정형 협심증 환자에서 증진된 외부 역박동술에 대해 현재 확인된 근거를 바탕으로 다음과 같이 제언하였다. 안전성의 경우, 대부분의 연구에서 증진된 외부 역박동술 시술 관련 부작용 및 합병증이 없는 것으로 보고하였다. 다만 RCT 1편에서 거짓치료와 비교 시 높은 부작용을 보이는 것으로 보고하였으나 피부찰과상, 멍, 수포 등의 증상으로 심각한 수준이 아니므로 비교적 안전한 시술이라고 평가하였다. 효과성의 경우, EECP는 거짓치료와 비교 시 대부분의 지표(협심증 중증도 변화, 약물 사용량 감소, 운동기능평가 향상, 삶의 질)에서 일부 유의한 효과가 있거나 유의한 차이가 없는 등 결과가 혼재하였다. 심장재활과 비교한 RCT 1편에서는 시술 후 협심증 개선 정도가 유의하게 낮았고 삶의 질에 대한 유의한 차이는 없었다. 1편의 NRS 연구에서는 EECP가 약물치료와 병행 시 약물치료만 한 경우에 비해 약물 사용량이 감소하는 효과가 있었으나 관련 근거가 부족하다고 평가하였다. 경제성의 경우, 비용 절감, 비용효과성을 보고한 연구는 없어 국내 임상상황에서 경제성을 평가할 수 없었다. 이에 소위원회에서는 현재 연구들의 근거를 토대로 보았을 때, 증진된 외부 역박동술의 효과성을 입증할 타 시술과의 최근 비교연구가 부족하고, 문헌 내 대상자 수가 적어 결과를 일반화하기 어려운 점, GRADE 근거수준이 ‘낮음’ 또는 ‘매우 낮음’인 점을 고려하여 동 시술의 효과성을 확인하기 위한 잘 설계된 추가연구가 필요하다고 제언하였다. 또한 증진된 외부 역박동술은 최대 약물치료나 관상동맥 중재시술 및 관상동맥 우회로술 등 중재적 시술을 시행할 수 없는 불인성 만성 안정형 협심증 환자를 대상으로 임상현장에서 적절한 사용이 필요하며, 오남용 하지 않도록 주의해야 한다고 제언하였다. 2025년 제2차 의료기술재평가위원회(2025. 2. 14.)에서는 소위원회 검토 결과에 근거하여 의료기술 재평가사업 관리지침 제4조 10항에 의거 ‘증진된 외부 역박동술’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 등을 종합적으로 고려하였을 때, 국내 임상상황에서 불인성 만성 안정형 협심증 환자의 혈류개선 및 심근 부담 감소를 통한 심장질환 호전을 목적으로 증진된 외부 역박동술의 사용을 ‘권고보류’로 결정하였다.


Background Enhanced external counterpulsation (EECP) is a technique used to improve blood flow and reduce myocardial workload in patients with refractory angina pectoris who are not candidates for conventional medical therapy or interventional procedures. The procedure involves applying external pressure cuffs to the patient’s calves, thighs, and buttocks; these cuffs are rhythmically inflated and deflated in synchronization with the cardiac cycle, as monitored by electrocardiography. EECP was initially evaluated in 2012 as part of the New Health Technology Assessment and was classified as a non-reimbursable benefit in 2014 (Ministry of Health and Welfare Bulletin No. 2014-44, March 25, 2014). However, in 2020, it was listed as a selective benefit with a 50% copayment under the health insurance coverage expansion initiative (Bulletin No. 2020-96, May 21, 2020). During the selective benefit eligibility review cycle, EECP was selected for reassessment through internal monitoring. The reassessment was conducted after the reassessment plan was approved, and a subcommittee was appointed at the 7th 2024 Health Technology Reassessment Committee meeting (July 12, 2024). Objective The objective of this reassessment was to determine the clinical safety and effectiveness of EECP in improving cardiac symptoms by enhancing blood flow and reducing myocardial workload in patients with refractory angina pectoris and to determine the appropriate strength of recommendation for this technology. Methods A systematic literature review was conducted to assess the clinical safety and effectiveness of EECP in patients with chronic refractory angina. The “Subcommittee on Enhanced External Counterpulsation” (hereinafter referred to as “the Subcommittee”) held discussions to finalize all assessment methods. The Subcommittee comprised two cardiologists, one cardiothoracic surgeon, one radiologist, and one evidence-based medicine expert. The target population was patients with chronic refractory angina. The comparator was defined as sham treatment, cardiac rehabilitation, or control (i.e., the drug therapy group in studies comparing drug therapy plus EECP against drug therapy alone). Safety was assessed based on procedure-related adverse events and complications. The primary outcome measure for evaluating effectiveness was changes in angina severity; secondary outcomes included reduction in medication use, improvement in exercise capacity, quality of life, length of hospital stay, myocardial perfusion indices, healthcare utilization, and number of days in hospital. Only comparative studies were included in the assessment. The literature search was conducted using three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews – Cochrane Central Register of Controlled Trials) and three Korean databases (KoreaMed, KMbase, and RISS). The final search date was September 12, 2024. The risk of bias in the included studies was assessed using the Cochrane Risk of Bias (RoB) tool for randomized controlled trials (RCTs) and the Risk of Bias Assessment for Non-randomized Studies (RoBANS ver. 2.0) for non-randomized studies (NRS). The overall quality of evidence was evaluated using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) method, and the strength of recommendation was determined accordingly. All processes were independently performed by two reviewers, and any discrepancies were resolved by reaching a consensus. The final level of recommendation was determined by the Health Technology Reassessment Committee, based on the findings of the Subcommittee. Results The systematic literature review identified nine studies (12 publications, with a total sample size of 456 patients), comprising five RCTs and four NRSs. The study population included patients with chronic refractory angina who were not candidates for invasive interventions, such as maximal medical therapy, percutaneous coronary intervention (PCI), or coronary artery bypass grafting (CABG). In the risk-of-bias assessment, most RCTs were rated as “unclear” in domains related to allocation concealment and blinding of participants and personnel. For the NRSs, the domains of confounding and exposure measurement were mostly rated as “unclear,” whereas the remaining domains were generally rated as “low” risk. Safety Safety was assessed based on procedure-related adverse events and complications reported in six studies (four RCTs and two NRSs) that compared EECP with sham treatment or drug therapy alone. Two studies comparing EECP with drug therapy alone and three studies comparing EECP with sham treatment reported no adverse events or complications. However, one RCT reported a significantly higher rate of adverse events in the EECP group than in the sham group; device-related adverse events included leg pain, skin abrasions, edema, and sensory disturbances, whereas non-device-related adverse events included dizziness, anxiety, and tinnitus. Effectiveness Effectiveness was assessed based on the primary and secondary outcomes reported in 12 studies (eight RCTs and four NRSs). The Canadian Cardiovascular Society (CCS) classification and the number of angina episodes were used to assess angina severity. A lower CCS class indicates symptom improvement. Seven studies (four RCTs [six publications] and three NRSs) reported CCS class changes. Most studies found that the CCS class improved in the EECP group compared with sham treatment. However, meta-analysis of the three RCTs with available quantitative data showed high heterogeneity, without significant between-group differences, warranting cautious interpretation (mean difference [MD], –1.21; 95% confidence interval [CI], –4.17 to 1.74; I2 = 99%). One study comparing EECP with cardiac rehabilitation reported a significantly higher proportion of patients improving to CCS class II in the EECP group but a significantly lower proportion improving to CCS class I. In the three NRSs, the CCS class was significantly lower in the EECP plus drug therapy group than in the drug therapy alone group, suggesting symptom improvement. The number of angina episodes was assessed in three RCTs which compared EECP with sham treatment. Although the EECP group tended to have fewer angina episodes compared with the sham treatment group, the meta-analysis showed no significant between-group differences and high heterogeneity (MD, –0.78; 95% CI, –2.26 to 0.71; I2 = 98%). Four studies (three RCTs [totaling five publications] and one NRS) assessed medication use. Although a decreasing trend in medication use was observed in the EECP group compared with sham treatment, meta-analysis showed no significant between-group differences and high heterogeneity (MD, –0.68; 95% CI, –1.49 to 0.12; I2 = 91%). The one NRS comparing EECP plus drug therapy to drug therapy alone reported a significant reduction in medication use in the EECP group at all time points. Exercise capacity was assessed in three RCTs (totaling four publications), which compared EECP with sham treatment; measures included exercise duration, peak angina grade, and peak oxygen uptake. Significant improvements were observed in all measures except for peak heart rate. Three RCTs reported quality of life outcomes. Of the two studies comparing EECP with sham treatment, one study reported significant improvements in select domains of the 36-Item Short Form Survey (SF-36) (i.e., social functioning, emotional role limitations, vitality, and pain), whereas the other demonstrated no significant differences. One study comparing EECP with cardiac rehabilitation found no significant differences in quality-of-life outcomes between the groups. No studies reported myocardial perfusion indices, healthcare utilization, or length of hospital stay. Conclusions and Strength of Recommendation Based on current evidence, the Subcommittee made the following recommendations regarding EECP for patients with chronic refractory angina. In terms of safety, most studies have reported no adverse events or complications related to the procedure. Although one RCT reported a higher incidence of adverse events in the EECP group compared with the sham treatment group, these were generally minor (e.g., skin abrasions, bruising, or blisters); thus, the procedure was considered relatively safe. In terms of effectiveness, EECP showed mixed results compared with sham treatment, with some indicators (angina severity, medication use, exercise capacity, and quality of life) showing significant improvements and others showing no significant differences. One RCT comparing EECP with cardiac rehabilitation found a lower degree of improvement in angina severity in the EECP group and no significant differences in quality of life. One NRS reported that EECP combined with drug therapy reduced medication use compared with drug therapy alone; however, the overall level of evidence was insufficient. No studies assessed economic outcomes such as cost savings or cost-effectiveness in the Korean clinical setting. Given the lack of recent comparative studies, the small sample sizes in existing studies, and the overall low or very low GRADE quality of evidence, the Subcommittee concluded that additional well-designed studies are required to establish the effectiveness of EECP. EECP should be used appropriately in clinical practice for patients with chronic refractory angina who are not candidates for maximal medical therapy or revascularization procedures such as PCI or CABG, and caution should be taken to avoid misuse or overuse. At the 2nd 2025 Health Technology Reassessment Committee meeting (February 14, 2025), the Committee drew the following conclusion regarding EECP based on the Subcommittee’s findings and in accordance with Article 4–10 of the Guidelines on the Management of the Health Technology Reassessment Program: Considering the clinical safety and effectiveness of the technology, the Committee determined that the use of EECP for improving blood flow and reducing myocardial burden in patients with chronic refractory angina in the Korean clinical setting should be classified as “Withhold Recommendation.”
