자가치아 이식술
초록
평가배경 ‘자가치아 이식술(Tooth Autotransplantation)’은 치아의 결손이 있는 경우 인접된 치아를 주변 구조물의 손상 없이 발치한 후 결손 부위에 재이식하는 방법이다. 동 기술은 신의료기술평가 제도 도입 이전에 비급여로 등재된 기술(보건복지부 고시 제2002 – 49호, 2002. 7. 18.)이다. 본 기술은 수요조사를 통해 재평가 필요성이 확인되고 우선순위 심의를 거쳐 재평가 대상으로 선정되었으며, 2025년 5월 16일 개최된 제5차 의료기술재평가위원회에서 평가계획서 및 소위원회 구성이 심의·확정되었다. 평가목적 본 평가의 목적은 치아 결손이 있거나 예상되는 환자 중 결손 부위에 이식할 수 있는 건강한 다른 치아가 있는 경우, 결손 부위 회복 목적으로 시행한 ‘자가치아 이식술’이 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가 방법 본 평가에서는 ‘자가치아 이식술’의 안전성 및 효과성을 재평가하기 위하여 체계적 문헌고찰을 수행하였다. 평가방법은 평가목적에 따라 “자가치아 이식술 소위원회(이사 ’소위원회‘라 한다)”의 논의를 거쳐 확정하였으며, 소위원회는 치과보존과 2인, 구강악안면외과 2인, 근거기반전문가 1인의 전문가 5인으로 구성하였다. 평가의 핵심질문은 ‘치아 결손이 있거나 예상되는 환자 중 이식할 수 있는 건강한 다른 치아가 있는 경우, 결손 부위 회복 목적으로 시행한 ‘자가치아 이식술’은 임상적으로 안전하고 효과적인가?’이다. 비교기술은 임플란트, 고정성 국소의치(브릿지), 치아 재식술 등으로 설정하였고, 의료결과는 안전성의 경우 시술 관련 합병증 또는 이상반응, 효과성은 일차지표로 이식 치아의 생존율 및 성공률, 이차지표로 삶의 질, 환자 만족도, 심미적 결과 등으로 설정하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) 및 국내 데이터베이스 2개(KoreaMed, 한국교육학술정보원)에서 검색하였으며(최종검색일 2025.6.13.), 연구 선정기준 및 배제기준 적용을 통한 연구선택과 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치 시 제3자 합의를 통해 조정하였다. 최종 선택된 연구의 비뚤림위험 평가는 Risk of Bias Assessment for Nonrandomized Studies (RoBANS) ver. 2.0를 사용하였고, 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 질적 검토(qualitative review)를 수행하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 권고등급을 결정하였다. 평가결과 본 평가에 선택된 체계적 문헌고찰은 총 7편(연구대상자 총 169명)으로 연구유형별로는 전향적 코호트 연구와 전향적 스플릿 마우스(split-mouth) 관찰연구가 각각 1편, 후향적 스플릿 마우스 관찰연구가 5편이었다. 비교기술별로는 임플란트와 비교한 연구 1편, 반대측 자연치아와 비교한 연구 6편이었고, 고정성 국소의치(브릿지) 혹은 치아 재식술과 비교한 연구는 없었다. 추적관찰 기간은 13.4개월부터 74.4개월(6.2년)까지 다양하였다. 비뚤림위험 평가 결과, 교란변수에 대한 고려가 미흡한 문헌이 다수 확인되었다. 이에 따라 전체적인 근거의 신뢰도는 ‘낮음’으로 평가하였다. 안전성 자가치아 이식술의 안전성은 치아 결손이 있거나 결손이 예상되는 환자 중, 이식 가능한 건강한 자가치아가 있는 환자를 대상으로 시술 관련 합병증 및 이상반응을 중심으로 평가하였다. 임플란트와 비교한 전향적 코호트 연구 1편에 따르면, 자가치아 이식술군에서는 초기 골유착 25% (4/16명), 협측 구강점막 손상 6.3% (1/16명), 조기 부목 탈락 12.5% (2/16명), 전정부의 골괴사 조직 분리가 6.3% (1/16명)로 보고되었다. 한편, 임플란트군에서는 누공 형성 등으로 인해 임시지대주를 삭제(reduction)하거나 제거(removal)한 사례가 20% (3/15명) 발생하였다. 자가치아 이식술과 반대측 자연치아를 비교한 연구 3편에서는 시술 특성상 자가치아 이식술군에서만 시술 관련 합병증 및 이상반응이 보고되었다. 한 편에서는 잇몸 염증이 88.2% (15/17명)로 가장 많이 발생하였고, 골유착이 17.6% (3/17명) 발생하였다. 염증성 치근흡수는 두 편의 연구에서 각각 11.8% (2/17명)와 2.7% (1/37명)로 보고되었다. 이 중 한 편에서 치수괴사가 2.7% (1/37명) 발생하여 근관치료가 시행되었다. 수술 후 1주 시점에서 일시적인 촉진 시 통증이 보고되었으나, 이후 시점에서는 소실되었다. 이 외에도 치근단 방사선 투과성 병소, 병적 치주낭, 강직 소견, 뿌리 흡수 등 방사선학적 이상 소견이 일부 연구에서 보고되었다. 효과성 자가치아 이식술의 효과성은 이식 치아의 생존율 및 성공률, 환자의 주관적 결과, 골 재형성, 치주조직 치유, 저작기능으로 평가하였다. 평가 항목 중 삶의 질을 보고한 연구는 없었다. 이식 치아의 생존율 및 성공률은 1편에서 보고하였고, 평균 28개월의 추적관찰에서 생존율 100% (17/17명)를 보였으며, 수술 후 12개월 시점의 심미적·방사선학적 평가 결과를 기준으로 한 성공률은 76.5% (13/17명)이었다. 환자의 주관적 결과는 임플란트와 비교한 연구와 반대측 자연치아와 비교한 연구에서 보고하였으며, 주관적 통증, 주관적 염증, 시술에 대한 만족도, 전체 치료 만족도, 심미적 평가 점수로 평가하였다. 이 중 주관적 염증 점수에서 자가치아 이식술군이 더 높게 나타났으나(p=0.015), 이외 다른 결과에서는 유의한 차이가 없었다. 반대측 자연치아와 비교한 다른 1편에서는 자가치아 이식술군의 심미적 평가 점수(maxillary canine aesthetic index, MCAI)가 평균 6점(범위 2~12점)으로, ‘우수(excellent)’한 수준에 해당하였다. 저작기능은 반대측 자연치아와 비교한 연구 1편 보고되었으며 선호 저작측, 촉각 역치 및 교합력 관련 지표를 평가하였다. 실제 저작 시 더 자주 사용하는 측을 의미하는 선호 저작측의 비율은 반대측 치아군(60%, 12/20명)보다 자가치아 이식술군(40%, 8/20명)에서 더 높았다. 촉각 역치는 자극을 인지하는 데 필요한 최소 수준으로, 자가치아 이식술군에서 촉각 역치가 유의하게 높았다(p<0.05). 교합력은 씹을 수 있는 힘으로, 개별 교합력(Individual occlusal force), 교합력 비율(occlusal force ratio)은 자가치아 이식술군에서 반대측 자연치아군보다 유의하게 낮았다(p<0.05). 골 재형성(Bone remodeling)은 3편의 연구에서 보고되었으며, 모두 반대측 자연치아와 비교한 결과였다. 콘빔 전산화단층촬영 또는 디지털 캘리퍼로 측정한 결과, 대부분의 지표에서 두 군 간 유의한 차이가 없었다. 변연골(marginal bone)과 백악법랑경계(cementoenamel junction) 간 거리에서는 자가치아 이식술군이 반대측 자연치아보다 약간 짧았으나(p<0.05), 두 군 모두 정상 범위 내로 평가되었다. 치주조직 치유(periodontal healing)는 임플란트와 반대측 자연치아를 각각 비교하였다. 치은연에서 하방 3mm까지의 치은부피 감소량이 자가치아 이식술군에서 임플란트군보다 더 적었다(p<0.05). 반대측 자연치아와 비교한 연구 4편에서는 동요도, 플라그지수, 출혈지수, 치주낭 깊이 등 대부분의 임상 지표에서 두 군 간 차이가 없었다. 다른 한 편에서 치근 성장은 소구치 이식군에서 반대측 자연치아와 동일한 수준이었으며, 대구치 이식군에서는 반대측 자연치아와 비교하여 정상 82.5% (14/17명), 비정상 17.6% (3/17명)로 보고되었다. 안전성 및 효과성의 GRADE 근거수준은 ‘낮음(Low)’으로 평가하였다. 결론 및 권고결정 의료기술재평가 소위원회에서는 치아 결손이 있거나 예상되는 환자 중 이식할 수 있는 건강한 다른 치아가 있는 경우, 결손 부위 회복 목적으로 시행한 ‘자가치아 이식술’에 대해 현재 평가결과에 근거하여 다음과 같이 제시하였다. 소위원회는 치아 결손이 있거나 결손이 예상되는 환자에서 이식 가능한 건강한 치아가 있는 경우 결손 부위 회복 목적으로 시행한 자가치아 이식술은 전반적으로 안전한 기술로 평가하였다. 효과성 측면에서는 임플란트와 비교한 근거의 양이 적어 종합적인 판단은 어려웠으나, 반대측 자연치아와 비교 시 유사한 수준으로 회복된 점을 고려할 때 환자의 상태에 따라 선택적으로 사용할 수 있는 기술로 평가하였다. 또한 소위원회에서는 추가 고려사항 및 제언으로 자가치아 이식술의 성공률이 증례 선택(case selection)과 기술 민감도(technique sensitivity)에 영향을 받는 치료임을 고려하여, 적절한 환자를 선정하고 숙련된 전문가가 시술할 경우 임플란트와 유사한 성과를 기대할 수 있다는 의견이었다. 또한 치근 표면의 줄기세포가 우수한 골재생 능력을 보이는 등의 조직학적 특성을 감안할 때, 임플란트 시술이 어려운 환자에서 임상적으로 유효한 대안이 될 수 있으며, 향후 기술 발전과 응용 가능성을 고려하면 치료 선택지로서의 잠재력이 높다는 의견을 제시하였다. 2025년 제4차 재평가전문위원회*(2025.12.8.)는 임상적 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 치아 결손이 있거나 결손이 예상되는 환자 중 이식 가능한 건강한 치아가 있는 환자를 대상으로 결손 부위 회복을 목적으로 사용 시 ‘자가치아 이식술’ 사용을 ‘약하게 권고함’으로 심의하였다. 다만, 현재까지 축적된 근거가 충분하지 않아 향후 임플란트와 장기 효과를 비교한 연구가 필요하다고 제언하였다.
Background of Assessment "Tooth Autotransplantation" is a procedure in which, when a tooth is missing, an adjacent tooth is extracted without damaging the surrounding structures and re-implanted into the site of the missing tooth. This technology was listed as a non-reimbursed technology (Ministry of Health and Welfare Notification No. 2002-49, July 18, 2002) prior to the introduction of the New Health Technology Assessment system. Following a demand survey that confirmed the need for reassessment and a priority review, this technology was selected as a subject for reassessment. At the 5th Health Technology Reassessment Committee meeting held on May 16, 2025, the assessment plan and the composition of the subcommittee were reviewed and finalized. Purpose of Assessment The purpose of this assessment is to provide evidence on the safety and effectiveness of "Tooth Autotransplantation," performed to restore a missing tooth site in patients who have or are expected to have a missing tooth and who have another healthy tooth available for transplantation into the missing site, and to determine a health technology reassessment recommendation grade for the use of this technology. Assessment Methods A systematic literature review was conducted to reassess the safety and effectiveness of "Tooth Autotransplantation." The assessment methodology was finalized through discussions of the "Tooth Autotransplantation Subcommittee" (hereinafter the "Subcommittee"), which was composed of five experts: two in conservative dentistry, two in oral and maxillofacial surgery, and one evidence-based medicine expert. The key question of the assessment was: "In patients who have or are expected to have a missing tooth and who have another healthy tooth available for transplantation, is 'Tooth Autotransplantation' performed to restore the missing site clinically safe and effective?" The comparator technologies were set as implants, fixed partial dentures (bridges), and tooth reimplantation. Health outcomes were defined as procedure-related complications or adverse events for safety, and for effectiveness, the survival rate and success rate of the transplanted tooth as primary indicators, with quality of life, patient satisfaction, and aesthetic outcomes as secondary indicators. Based on the key question, the systematic literature review searched three international databases (Ovid MEDLINE, Ovid EMBASE, and the Cochrane Central Register of Controlled Trials) and two domestic databases (KoreaMed and the Korea Education and Research Information Service), with the final search conducted on June 13, 2025. Study selection, using the inclusion and exclusion criteria, as well as risk-of-bias assessment, was independently performed by two reviewers, with disagreements resolved through discussion with a third reviewer. The risk of bias of the finally selected studies was assessed using the Risk of Bias Assessment for Nonrandomized Studies (RoBANS) ver. 2.0, and data were extracted using a predetermined data extraction form for qualitative review. Based on the Subcommittee's review results, the final recommendation grade was determined following deliberation by the Reassessment Expert Committee. Assessment Results A total of 7 systematic literature reviews (169 study subjects in total) were selected for this assessment. By study design, there was 1 prospective cohort study, 1 prospective split-mouth observational study, and 5 retrospective split-mouth observational studies. By comparator, 1 study compared autotransplantation with implants and 6 studies compared it with the contralateral natural tooth; no studies compared it with fixed partial dentures (bridges) or tooth reimplantation. The follow-up period ranged from 13.4 to 74.4 months (6.2 years). The risk-of-bias assessment identified that many of the studies did not adequately account for confounding variables. Accordingly, the overall certainty of evidence was rated as "Low." Safety The safety of tooth autotransplantation was assessed with a focus on procedure-related complications and adverse events in patients who have or are expected to have a missing tooth and who have a healthy autologous tooth available for transplantation. According to 1 prospective cohort study comparing autotransplantation with implants, in the autotransplantation group, early osseointegration occurred in 25% (4/16), buccal oral mucosal injury in 6.3% (1/16), early splint loss in 12.5% (2/16), and separation of necrotic bone tissue in the vestibule in 6.3% (1/16). In the implant group, reduction or removal of the temporary abutment due to fistula formation and other causes occurred in 20% (3/15). In the 3 studies comparing autotransplantation with the contralateral natural tooth, procedure-related complications and adverse events were, by the nature of the procedure, reported only in the autotransplantation group. In one study, gingival inflammation was the most common finding, occurring in 88.2% (15/17), followed by osseointegration in 17.6% (3/17). Inflammatory root resorption was reported in 2 studies at 11.8% (2/17) and 2.7% (1/37), respectively. In one of these, pulp necrosis occurred in 2.7% (1/37), for which root canal treatment was performed. Transient pain on palpation was reported at 1 week after surgery but resolved thereafter. In addition, radiographic abnormalities such as periapical radiolucent lesions, pathological periodontal pockets, ankylosis, and root resorption were reported in some studies. Effectiveness The effectiveness of tooth autotransplantation was assessed based on the survival and success rates of the transplanted tooth, patient-reported outcomes, bone remodeling, periodontal healing, and masticatory function. None of the studies reported quality of life among the assessment items. The survival and success rates of the transplanted tooth were reported in 1 study, which showed a survival rate of 100% (17/17) over an average follow-up of 28 months, and a success rate of 76.5% (13/17) based on aesthetic and radiographic evaluation at 12 months after surgery. Patient-reported outcomes were reported in the studies comparing autotransplantation with implants and with the contralateral natural tooth. They were assessed using subjective pain, subjective inflammation, satisfaction with the procedure, overall treatment satisfaction, and aesthetic evaluation scores. Among these, the subjective inflammation score was higher in the autotransplantation group (p=0.015), while no significant differences were found for the other outcomes. In another study comparing with the contralateral natural tooth, the aesthetic evaluation score (maxillary canine aesthetic index, MCAI) of the autotransplantation group averaged 6 points (range 2–12), corresponding to an "excellent" level. Masticatory function was reported in 1 study comparing autotransplantation with the contralateral natural tooth, which assessed preferred chewing side, tactile threshold, and occlusal force-related indicators. The proportion of the preferred chewing side, meaning the side used more often during actual mastication, was higher in the contralateral tooth group (60%, 12/20) than in the autotransplantation group (40%, 8/20). The tactile threshold, the minimum level required to perceive a stimulus, was significantly higher in the autotransplantation group (p<0.05). Occlusal force, the force with which one can chew, including individual occlusal force and occlusal force ratio, was significantly lower in the autotransplantation group than in the contralateral natural tooth group (p<0.05). Bone remodeling was reported in 3 studies, all of which compared autotransplantation with the contralateral natural tooth. Measurements using cone-beam computed tomography or a digital caliper showed no significant differences between the two groups for most indicators. The distance between the marginal bone and the cementoenamel junction was slightly shorter in the autotransplantation group than in the contralateral natural tooth (p<0.05). However, both groups were within the normal range. Periodontal healing was compared with implants and with the contralateral natural tooth, respectively. The reduction in gingival volume from the gingival margin to a depth of 3 mm was smaller in the autotransplantation group than in the implant group (p<0.05). In the 4 studies comparing with the contralateral natural tooth, most clinical indicators, including tooth mobility, plaque index, bleeding index, and periodontal pocket depth, showed no differences between the two groups. In another study, root growth in the premolar transplantation group was at the same level as the contralateral natural tooth, while in the molar transplantation group, compared with the contralateral natural tooth, root growth was normal in 82.5% (14/17) and abnormal in 17.6% (3/17). The GRADE certainty of evidence for both safety and effectiveness was rated as "Low." Conclusion and Recommendation Decision Based on the current assessment results, the Health Technology Reassessment Subcommittee presented the following conclusions regarding "Tooth Autotransplantation" performed to restore a missing tooth site in patients who have or are expected to have a missing tooth and who have another healthy tooth available for transplantation. The Subcommittee assessed that tooth autotransplantation, performed to restore a missing tooth site in patients who have or are expected to have a missing tooth and who have a healthy tooth available for transplantation, is overall a safe technology. Regarding effectiveness, a comprehensive judgment was difficult due to the limited evidence comparing autotransplantation with implants; however, because outcomes recovered to a similar level when compared with the contralateral natural tooth, the Subcommittee assessed it as a technology that can be selectively used depending on the patient's condition. The Subcommittee further noted, as additional considerations and recommendations, that because the success of tooth autotransplantation is influenced by case selection and technique sensitivity, outcomes similar to those of implants can be expected when appropriate patients are selected, and a skilled specialist performs the procedure. In addition, considering histological characteristics such as the excellent bone regeneration capacity of stem cells on the root surface, the Subcommittee noted that this technology could serve as a clinically valid alternative for patients for whom implant treatment is difficult, and that it has high potential as a treatment option considering future technological developments and possible applications. The 4th Reassessment Expert Committee of 2025* (December 8, 2025), after comprehensively considering the evidence for clinical effectiveness and other assessment items, deliberated and decided to "weakly recommend" the use of "Tooth Autotransplantation" in the domestic clinical setting for patients who have or are expected to have a missing tooth and who have a healthy tooth available for transplantation, when used for the purpose of restoring the missing site. However, the Committee noted that, since the evidence accumulated to date is insufficient, future studies comparing long-term outcomes with implants are needed.
