경구 음식물유발시험과 경구 약물유발시험검사
초록
평가배경 ‘경구 음식물유발시험및 경구 약물유발시험’은 약물 또는 음식물 알레르기의 확진 및 원인 알레르겐을 규명하기 위하여 실시하는 검사로 신의료기술평가 도입 이전인 2004년에 보건복지부 고시(제2004-89호, 2004. 12. 29.)에 따라 비급여로 등재되었다. 동 기술은 비급여 보고제도 항목으로, 수요조사(유관기관)를 통해 제안되었으며 의료기술평가가 수행된 바가 없는 등 재평가 필요성이 인정되어 관련 선정 절차를 거쳐 최종 의료기술재평가 대상으로 선정되었다. 의료기술재평가를 통해 동 기술의 안전성 및 효과성에 대한 근거를 확인하고자, 2025년 제5차 의료기술재평가위원회(2024. 5. 16.)에서 재평가 계획서 및 소위원회 구성을 심의한 후 재평가를 수행하였다. 평가목적 본 평가는 약물 또는 음식물(식품) 과민반응 의심 환자에게서 ‘경구 음식물유발시험과 경구 약물유발시험검사’가 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고, 해당 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다. 평가방법 모든 평가방법은 평가목적에 따라 “약물 및 음식물유발시험 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였으며, 소위원회 구성은 알레르기내과 2인, 소아청소년과 2인, 감염내과 1인, 피부과 1인, 가정의학 1인, 근거기반의학과 1인, 총 8인으로 구성하였다. 현행 급여목록에서 유발시험을 항원의 종류와 투여경로(경구, 주사)에 따라 ‘주사제 약물 유발시험’과 ‘경구 음식물유발시험과 경구 약물유발시험’으로 구분하고 있으며, 해당 두 기술에 대한 재평가를 함께 진행하였다. 다만, 소위원회에서는 두 기술에 대한 급여목록의 행위 구분과 달리, 항원의 유형(약물 또는 식품)에 따라 약물유발시험과 음식물(식품) 유발시험으로 구분하여 의료기술을 평가하기로 하였다. 약물유발시험의 경우 임상진료지침 및 교과서에서 투여경로를 구분하지 않는 것을 고려하여, 투여 경로의 구분 없이 평가하였다. ‘경구 음식물유발시험과 경구 약물유발시험검사’의 특성상 진단 정확도가 높고, 참조표준검사인 점을 고려하여 임상진료지침을 중심으로 안전성과 유효성을 확인하였다. 동 의료기술들에 대한 교과서 내 기술이 많지 않고, 최신의 근거를 반영하고 있지 않다고 판단하여, 교과서는 검토대상에서 배제하였다. 임상진료지침 검색은 핵심질문을 토대로 국외 데이터베이스 2개(Ovid MEDLINE, Ovid EMBASE)과 9개의 주제 특화 공식 웹사이트에서 수행되었다. 또한 최신의 근거를 중심으로 동 의료기술들을 평가하기 위하여, 2020년 이후 발간된 문헌을 중심으로 검토하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다. 평가결과 총 26편(약물유발시험 관련 지침 16편, 음식물유발시험 관련 지침 10편)의 임상진료지침을 검토하였으며 국내외 여러 학회 및 기관에서 발간되었다. 2020년 이후 발행된 문헌은 총 12편(약물유발시험 6편, 음식물유발시험 6편)이었다. 약물유발시험과 관련된 16편의 지침 중 4편의 문헌이 약물유발시험을 중심으로 한 임상진료지침이었고, 약물 과민반응 3편, 항생제 과민반응 4편, 그 외 5편이 비스테로이드성 항염증제(Non-Steroidal Anti-Inflamatory Drugs, NSAIDs) 등 약물 과민반응에 대한 지침이었다. 음식물유발시험에 대한 임상진료지침은 총 10편으로, 식품유발시험에 대한 임상진료지침 1편, 식품 과민반응 4편, IgE(Immunoglobulin E) 매개 식품 과민반응 1편, 우유 과민반응 2편, 땅콩 과민반응 1편, 그 외 과민반응 진단 1편이었다. 약물유발시험 평가대상 의료기술인 ‘경구 음식물유발시험과 경구 약물유발시험’과 ‘주사제 약물 유발시험’ 중 약물을 항원으로 사용하는 약물유발시험(Drug provocation test, DPT)에 대한 평가 결과, 대부분의 임상진료지침에서 약물유발시험을 약물 과민반응 진단을 위한 참조표준검사(gold standard)로 정의하고 있었다. DPT는 진단이 불확실하거나 병력이 비특이적인 경우, 또는 저위험으로 판단되는 환자에서 원인 약물의 과민반응 여부를 확인하거나 배제하기 위해 다양한 약물을 대상으로 수행할 수 있다. 안전성 DPT는 환자의 과민반응에 대한 잠재적 위험성을 수반하기 때문에, 환자의 안전 확보하기 위해 검사 전 위험계층화(Risk Stratification)를 수행하여 투여 간격 및 용량 등의 검사 전략을 조정하고, 개인화된 노출 프로토콜에 따라 시행해야 한다. 아나필락시스 병력이 있는 환자의 경우 검사 수행은 권장되지 않는다. 아나필락시스 또는 중증 과민반응이 예상되는 경우에는 개별 약제와 환자 특성을 고려하여 피부검사가 가능한 경우 DPT보다 피부검사를 우선 시행하는 것이 권장된다. 검사의 위험성을 고려하여, 소생 장비와 전문 인력 등으로 구성된 응급 대응 체계를 갖춘 환경에서 숙련된 의료진의 감독하에 시행해야 한다. 임상적 유용성 약물유발시험은 약물 과민반응의 확진뿐 아니라 잘못된 과민반응 진단을 교정하여 불필요한 약물 회피를 줄이고 치료 접근성을 향상시키는 핵심 진단 절차이다. 환자의 병력 및 피부검사 결과만으로 과민반응을 진단할 경우 과다진단의 우려가 있어, DPT를 통한 정확한 진단이 중요하다. 또한 구조적으로 유사한 약물 간 교차반응을 평가하여 안전한 대체약을 확인하고, 약물요법의 연속성과 접근성 확보 및 적절한 항생제 선택에 기여할 수 있다. 사회적 가치 약물유발시험을 통해 약물 과민반응을 정확하게 진단하고 안전한 대체 약제를 확인함으로써, 불필요한 광범위 항생제 사용을 줄이고 보다 적절한 감염 치료가 가능해진다. 이에 따라 항생제 내성 발생, 감염 및 감염 후유증, 사망 위험을 감소시켜 임상적 치료 결과를 개선하고 기대수명 향상에 기여할 수 있다. 약물 과민반응에 대한 적절한 진단은 비용효율성에도 영향을 미쳐, 환자가 독성이 높고 효과성이 낮은 약제로 치료받는 것을 방지할 수 있다. 또한 의료서비스 이용 측면에서도 외래 방문, 응급실 방문 및 입원 일수가 감소한 것으로 보고되었다. 음식물유발시험 음식물유발시험(Oral Food Challenge, OFC) 또는 식품유발시험은 과민반응이 확정되었거나 의심되는 식품을 단회 또는 단계적으로 섭취시켜 유발 증상의 유무를 확인하는 검사로, 대부분의 임상진료지침에서 참조표준검사로 언급하고 있다. 임상적 안전성 OFC는 극히 적은 용량에서도 심각한 반응을 일으킬 수 있는 고유한 위험이 있어 안전성 문제가 발생할 수 있다. 따라서 검사 전에는 환자의 과거력을 통해 과민반응의 중증도, 동반질환 및 금기증 여부 등을 확인하고, 필요시 피부검사나 특이면역글로불린 E(specific Immunoglobulin E, sIgE) 등을 검사하여 개별 환자의 안전성 평가를 반드시 수행해야 한다. 검사는 아나필락시스 등의 과민반응 치료에 필요한 약물, 시설, 숙련된 인력 등이 갖추어진 환경에서 시행되어야 한다. 임상적 유용성 일반적으로 피부단자시험과 특이 IgE 검사를 1차 검사로 권고하며, OFC는 필요시 진단 과정의 어느 시점에든 시행할 수 있다. 환자 또는 보호자에 의해 자가 보고된 식품 과민반응 발생률은 실제보다 과대평가되는 경향이 있으며, 피부검사, sIgE 등의 검사에서 결과가 불명확한 사례가 있어 OFC를 이용한 확진이 필요하다. OFC를 통해 원인 식품의 정확한 규명함으로써 회피해야 할 음식과 섭취 가능한 음식을 파악할 수 있다. 이를 통해 불필요한 식이 제한을 없애 환자의 식이 다양성을 유지하고, 영양 결핍 및 음식 기피를 예방해 적절한 치료관리를 가능하게 하며, 특히 아동의 성장에 기여한다. OFC는 내성 발달 여부 평가하고 섭취 가능한 용량을 파악함으로써 장기 예후를 예측하는 임상적 가치도 지닌다. 사회적 가치 OFC는 진단 및 식품 재도입 외에 환자 및 보호자 교육과 상담에도 활용된다. 이를 통해 교육 및 상담에도 활용되며, 과민반응으로 인한 불안감을 완화시키고 사회적 제약에 따른 환자의 삶의 질도 개선시킬 수 있다. 또한 OFC는 경제적 측면에서도 정확한 진단을 기반으로 불필요한 의료 이용을 줄이고, 효과적인 치료를 가능하게 함으로써 의료비 증가를 방지할 수 있다. 결론 및 제언 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같이 결과 및 의견을 제시하였다. 약물유발시험 약물유발시험(DPT)은 대부분의 임상진료지침에서 약물 과민반응을 진단하기 위한 참조표준검사(gold standard)로 명시하고 있다. 항원으로 예상되는 약물로 인한 아나필락시스 또는 중증 과민반응이 예상되는 경우, 개별 약제와 환자의 사례를 고려하여 DPT 시행 전 피부검사를 우선 시행할 수 있으며, 환자의 위험도를 평가하여 개인화된 진단 전략을 수립해야 한다고 보고되었다. 또한 응급 대응 체계가 갖춰진 의료기관에서 숙련된 의료진의 감독하에 시행할 것을 권고하고 있다. 지침에서는 약물유발시험이 약물 과민반응의 확진, 부적절하게 부여된 약물 과민반응 진단의 해제(removal of the label), 교차반응성 평가를 통한 대체약물 탐색 등에 기여하며, 이를 통해 임상 치료 결과 향상, 항생제 내성 감소, 불필요한 의료서비스 이용 감소, 의료비 절감 및 환자의 삶의 질 개선 등 다양한 긍정적 효과를 기대할 수 있다고 보고하였다. 소위원회는 해당 검사의 기술적 특성상 검사 난이도와 위험성이 높으므로, 응급 대응 체계와 숙련된 인력을 갖춘 의료기관에서 시행되어야 하며, 환자와 보호자에게 검사 관련 위험성과 절차에 대한 충분한 정보 제공이 필요하다는 점을 강조하였다. 국내에서는 2017년 이후 임상진료지침이 새로 개발되지 않았고, 약제의 종류가 다양하여 표준화가 어렵다는 한계가 있었다. 최근 약물 과민반응 검사에 대한 교육 및 임상적 관심이 증가함에 따라 사회적 수요가 확대되고 있어, 표준화된 진료지침 개발을 위한 제도적 지원이 필요하다는 의견이 제시되었다. 약물유발시험은 검사의 난이도와 위험성이 높음에도 불구하고 임상적으로 반드시 필요한 기술로 평가되고 있어서, 향후 주요 약제 및 질환별 사용 현황을 파악하여 급여화 가능성을 검토할 필요가 있다고 제언하였다. 음식물유발시험 음식물유발시험(OFC)은 대부분의 임상진료지침에서 식품 과민반응 진단의 참조표준검사로 제시되고 있다. 음식물유발시험은 소량의 식품에서도 과민반응이 유발될 수 있으므로, 환자의 안전을 위해 숙련된 의료진이 표준화된 절차와 응급 대응 체계를 갖춘 의료기관에서 시행해야 한다고 권고되고 있다. 지침에서 음식물유발시험은 식품 과민반응의 확진과 질병 경과 평가를 위한 핵심 진단검사로, 이를 통해 불필요한 식이 제한을 줄이고 원인 식품과 내성 여부를 명확히 판단하여 적절한 치료 및 관리 계획을 수립할 수 있다고 보고하였다. 정확한 진단과 식이 재도입을 통해 환자의 영양 상태, 삶의 질, 심리적 안정을 개선하고 불필요한 식이 회피로 인한 경제적·사회적 부담을 감소시킨다고 보고하였다. 소위원회는 검사 과정에서 심각한 알레르기 반응이 유발될 수 있으므로, 검사에 수반되는 위험성과 안전수칙에 대해 환자 및 보호자에게 충분한 정보가 제공되어야 한다는 점을 강조하였다. 국내에서는 아직 관련 임상진료지침이 마련되지 않았으며, 식품의 종류가 다양하고 표준화가 어렵다는 한계가 지적되었다. 그러나 최근 식품 과민반응 검사에 대한 교육 및 임상적 관심이 증가함에 따라 사회적 수요 및 사용량도 증가하고 있다. 특히 소아 및 식품 과민반응 환자에게 필수적인 기술로 평가되고 있어, 실제 사용 현황을 파악하고 표준화된 지침을 개발하기 위한 제도적 지원이 필요하다는 의견이 제시되었다. 현재 수가체계에서는 투여경로에 따라 항목이 분리·등재되어 있다. 약물유발시험의 경우 ‘주사제 약물유발시험’과 ‘경구 약물유발시험’을 포함하고 있지만, 이 중 ‘경구 약물유발시험’은 투여경로가 동일한 ‘경구 음식물(식품) 유발시험’과 하나의 고시항목으로 통합되어 있다. 그러나 이러한 분류체계는 항원(예: 식품, 약물 등)의 특성을 충분히 반영하지 못한다는 한계가 있어, 향후에는 항원의 종류에 따라 유발시험을 구분하는 방안을 검토할 필요가 있다는 의견이 제시되었다. 2025년 제4차 재평가전문위원회*(2025.12.8.)는 ‘경구 음식물유발시험과 경구 약물유발시험’에 대해 다음과 같이 심의하였다. 재평가전문위원회는 임상적 안전성과 효과성 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 약물 및 음식물 과민반응 의심환자를 대상으로 과민반응 원인 규명 및 확진을 위해 ‘경구 음식물유발시험과 경구 약물유발시험’의 사용을 ‘권고함’으로 결정하였다.
Background The ‘Oral Food Challenge and Oral Drug Provocation Test’ is used to confirm drug or food allergy and identify the causative allergen. It was listed as a non-covered service under Ministry of Health and Welfare Notice No. 2004-89 (December 29, 2004), before the introduction of the health technology assessment system. This technology, which is subject to the non-covered service reporting system, was proposed through a demand survey of relevant organizations. The need for reassessment was recognized because no health technology assessment had previously been conducted, and the technology was ultimately selected for health technology reassessment through the relevant selection process. To examine the evidence on the safety and effectiveness of this technology through health technology reassessment, the reassessment plan and the composition of the subcommittee were reviewed at the fifth meeting of the Health Technology Reassessment Committee in 2025 (May 16, 2025), after which the reassessment was conducted. Objective This assessment aimed to provide evidence on whether the ‘Oral Food Challenge and Oral Drug Provocation Test’ is clinically safe and effective in patients with suspected drug or food hypersensitivity and to determine the recommendation grade for the use of this technology through health technology reassessment. Methods All assessment methods were finalized through discussion by the Subcommittee on Drug and Food Provocation Testing (hereinafter, the ‘Subcommittee’) in accordance with the assessment objective. The Subcommittee comprised eight members: two specialists in allergy and clinical immunology, two in pediatrics, one in infectious diseases, one in dermatology, one in family medicine, and one in evidence-based medicine. In the current benefit schedule, provocation tests are classified by antigen type and route of administration (oral or injection) as either ‘Injectable Drug Provocation Test’ or ‘Oral Food Challenge and Oral Drug Provocation Test,’ and the two technologies were reassessed together. However, unlike the service classification in the benefit schedule, the Subcommittee decided to assess the technologies according to antigen type, distinguishing drug provocation tests from food challenges. Because clinical practice guidelines and textbooks do not distinguish drug provocation tests by route of administration, drug provocation testing was assessed without differentiating the administration route. Given the high diagnostic accuracy of the ‘Oral Food Challenge and Oral Drug Provocation Test’ and its status as a reference standard, its safety and effectiveness were evaluated primarily on the basis of clinical practice guidelines. Textbooks were excluded from the review because they contained limited descriptions of these health technologies and were considered not to reflect the latest evidence. Clinical practice guidelines were searched based on the key questions in two international databases (Ovid MEDLINE and Ovid EMBASE) and nine topic-specific official websites. To assess these health technologies using the most recent evidence, the review focused on publications issued since 2020. The recommendation grade for health technology reassessment was determined after final deliberation by the Expert Committee for Reassessment, based on the Subcommittee’s review findings. Results A total of 26 clinical practice guidelines—16 related to drug provocation testing and 10 related to food challenges—published by various domestic and international societies and organizations were reviewed. Twelve publications had been issued since 2020 (six on drug provocation testing and six on food challenges). Of the 16 guidelines related to drug provocation testing, four focused specifically on drug provocation tests, three addressed drug hypersensitivity, four addressed antibiotic hypersensitivity, and the remaining five addressed hypersensitivity to drugs such as nonsteroidal anti-inflammatory drugs (NSAIDs). The 10 clinical practice guidelines on food challenges comprised one guideline on food challenges, four on food hypersensitivity, one on immunoglobulin E (IgE)-mediated food hypersensitivity, two on cow’s milk hypersensitivity, one on peanut hypersensitivity, and one on the diagnosis of other hypersensitivity reactions. Drug Provocation Testing Among the health technologies assessed—the ‘Oral Food Challenge and Oral Drug Provocation Test’ and the ‘Injectable Drug Provocation Test’—the evaluation of drug provocation tests (DPTs), in which a drug is used as the antigen, found that most clinical practice guidelines defined DPT as the reference standard (gold standard) for diagnosing drug hypersensitivity. DPT may be performed with various drugs to confirm or exclude hypersensitivity to a suspected drug when the diagnosis is uncertain, the clinical history is nonspecific, or the patient is considered to be at low risk. Safety Because DPT carries a potential risk of hypersensitivity reactions, pre-test risk stratification should be performed to ensure patient safety, the testing strategy—including dosing intervals and doses—should be adjusted accordingly, and the test should follow an individualized exposure protocol. Testing is not recommended for patients with a history of anaphylaxis. When anaphylaxis or a severe hypersensitivity reaction is anticipated, skin testing, if available, is recommended before DPT, taking the specific drug and patient characteristics into account. Given the risks, DPT should be conducted under the supervision of experienced healthcare professionals in a setting equipped with an emergency response system, including resuscitation equipment and trained personnel. Clinical Utility Drug provocation testing is a key diagnostic procedure that not only confirms drug hypersensitivity but also corrects erroneous hypersensitivity diagnoses, thereby reducing unnecessary drug avoidance and improving access to treatment. Because diagnosis based only on clinical history and skin test results may lead to overdiagnosis, accurate diagnosis through DPT is important. DPT can also assess cross-reactivity between structurally similar drugs, identify safe alternatives, maintain continuity of and access to pharmacotherapy, and support appropriate antibiotic selection. Social Value By accurately diagnosing drug hypersensitivity and identifying safe alternative drugs, DPT can reduce unnecessary use of broad-spectrum antibiotics and enable more appropriate treatment of infections. This may improve clinical outcomes and contribute to longer life expectancy by reducing antimicrobial resistance, infections and their sequelae, and the risk of death. Appropriate diagnosis of drug hypersensitivity also affects cost-effectiveness by preventing treatment with drugs that are more toxic and less effective. Reductions in outpatient visits, emergency department visits, and hospital length of stay have also been reported. Food Challenge An oral food challenge (OFC), or food challenge, is a test in which a food known or suspected to cause hypersensitivity is ingested as a single dose or in graded doses to determine whether symptoms are elicited. Most clinical practice guidelines identify it as the reference standard. Clinical Safety OFC carries an inherent risk of serious reactions even at extremely small doses and may therefore raise safety concerns. Before testing, an individualized safety assessment is essential. This should include reviewing the patient’s history to determine the severity of previous hypersensitivity reactions, comorbidities, and any contraindications, as well as performing skin testing or measuring food-specific immunoglobulin E (sIgE) when necessary. The test should be conducted in a setting with the medications, facilities, and trained personnel needed to treat hypersensitivity reactions such as anaphylaxis. Clinical Utility Skin prick testing and specific IgE testing are generally recommended as first-line tests, while OFC may be performed at any point in the diagnostic process when needed. The prevalence of food hypersensitivity self-reported by patients or caregivers tends to be overestimated, and findings from skin testing, sIgE testing, and similar tests may be inconclusive; confirmation by OFC is therefore necessary. By accurately identifying the causative food, OFC helps determine which foods should be avoided and which can be consumed. This eliminates unnecessary dietary restrictions, preserves dietary diversity, prevents nutritional deficiencies and food aversion, enables appropriate treatment and management, and supports growth, particularly in children. OFC also has clinical value in predicting long-term prognosis by assessing whether tolerance has developed and determining the amount that can be consumed. Social Value In addition to diagnosis and food reintroduction, OFC is used to educate and counsel patients and caregivers. It can alleviate anxiety related to hypersensitivity and improve patients’ quality of life by reducing social restrictions. From an economic perspective, accurate diagnosis through OFC can also prevent increases in healthcare costs by reducing unnecessary healthcare utilization and enabling effective treatment. Conclusions and Recommendations Based on the current assessment findings, the Health Technology Reassessment Subcommittee presented the following conclusions and opinions. Drug Provocation Testing Most clinical practice guidelines identify drug provocation testing (DPT) as the reference standard (gold standard) for diagnosing drug hypersensitivity. When the suspected antigenic drug is expected to cause anaphylaxis or a severe hypersensitivity reaction, skin testing may be performed before DPT based on the specific drug and the patient’s circumstances, and an individualized diagnostic strategy should be established through patient risk assessment. The guidelines also recommend that DPT be conducted under the supervision of experienced healthcare professionals at a medical institution equipped with an emergency response system. The guidelines reported that DPT contributes to confirming drug hypersensitivity, removing an inappropriately assigned drug hypersensitivity label (delabeling), and identifying alternative drugs by assessing cross-reactivity. These functions may yield various positive effects, including improved clinical outcomes, reduced antimicrobial resistance, less unnecessary healthcare utilization, lower healthcare costs, and better quality of life. The Subcommittee emphasized that, because the test is technically difficult and carries substantial risk, it should be conducted at a medical institution with an emergency response system and experienced personnel, and patients and caregivers should receive sufficient information about the test-related risks and procedures. In Korea, no new clinical practice guideline has been developed since 2017, and standardization is difficult because of the wide variety of drugs. As education and clinical interest in drug hypersensitivity testing have recently increased, social demand has expanded, prompting the view that institutional support is needed to develop standardized clinical practice guidelines. Although DPT is technically difficult and high-risk, it is considered clinically essential. It was therefore suggested that utilization by major drug class and condition should be assessed in the future to examine the feasibility of insurance coverage. Food Challenge Most clinical practice guidelines present the oral food challenge (OFC) as the reference standard for diagnosing food hypersensitivity. Because even a small amount of food can trigger a hypersensitivity reaction, the guidelines recommend that OFC be conducted by experienced healthcare professionals at a medical institution with standardized procedures and an emergency response system to ensure patient safety. The guidelines reported that OFC is a key diagnostic test for confirming food hypersensitivity and evaluating the disease course. It can reduce unnecessary dietary restrictions and clearly determine the causative food and whether tolerance has developed, enabling an appropriate treatment and management plan. Accurate diagnosis and food reintroduction can improve patients’ nutritional status, quality of life, and psychological well-being while reducing the economic and social burden caused by unnecessary food avoidance. The Subcommittee emphasized that, because serious allergic reactions may be triggered during testing, patients and caregivers should receive sufficient information about the associated risks and safety precautions. Korea does not yet have relevant clinical practice guidelines, and the diversity of foods and difficulty of standardization have been identified as limitations. However, recent growth in education and clinical interest in food hypersensitivity testing has been accompanied by increased social demand and utilization. Because OFC is considered essential, particularly for children and patients with food hypersensitivity, institutional support is needed to assess current utilization and develop standardized guidelines. Under the current fee schedule, services are separately listed according to the route of administration. Drug provocation testing includes both the ‘Injectable Drug Provocation Test’ and the ‘Oral Drug Provocation Test’; however, the ‘Oral Drug Provocation Test’ is grouped under a single notified service item with the ‘Oral Food Challenge,’ which uses the same route of administration. This classification does not adequately reflect the characteristics of the antigen (e.g., food or drug). It was therefore suggested that provocation tests should in the future be classified according to antigen type. At its fourth meeting in 2025* (December 8, 2025), the Expert Committee for Reassessment deliberated on the ‘Oral Food Challenge and Oral Drug Provocation Test’ as follows. After comprehensively considering clinical safety and effectiveness as well as the other assessment domains, the Expert Committee for Reassessment decided to ‘recommend’ the use of the ‘Oral Food Challenge and Oral Drug Provocation Test’ in the Korean clinical setting to identify the cause of and confirm hypersensitivity in patients with suspected drug or food hypersensitivity.
